BACKGROUND Transcatheter aortic valve replacement(TAVR)is a revolutionary procedure for severe aortic *** coexistence of chronic kidney disease(CKD)and TAVR introduces a challenge that significantly impacts patient *** To define readmission rates,predictors,and causes after TAVR procedure in CKD stage 1-4 *** We used the national readmission database 2018 and 2020 to look into readmission rates,causes and predictors after TAVR procedure in patients with CKD stage *** Out of 24758 who underwent TAVR and had CKD,7892(32.4%)patients were readmitted within 90 days,and had higher adjusted odds of being females(adjusted odds ratio:1.17,95%CI:1.02-1.31,P=0.02)with longer length of hospital stay>6 days,and more comorbidities including but not limited to diabetes mellitus,anemia,and congestive heart failure(CHF).CONCLUSION Most common causes of readmission included CHF(18.0%),sepsis,and complete atrioventricular *** readmission predictors with very close followup is warranted to prevent such high rate of readmission.

背景术后谵妄（postoperativedelirium，POD）在老年患者中很常见，且与不良预后相关。在行择期手术的患者中，POD的认知和功能性后遗症尚且未知。我们试图研究：①在老年手术患者中，敏感性神经认知测试的较低评分能否作为POD的独立风险因子；@POD能否预测手术3个月后的认知和功能下降。方法我们对年龄I〉65岁的全髋关节或全膝关节置换术患者进行了一项前瞻性队列研究。参与者接受术前的神经认知和功能测试。使用混乱评估法诊断POD。出现POD的患者组与对照组在手术后3个月接受重复的神经认知和功能测试。结果418例患者符合入选标准，有42例出现POD。各组之间在基础细微精神状态检查评分、酗酒、抑郁和语言智力等方面没有差别。POD的独立预测因子包括年龄、精神疾病史、功能状态的下降和语言记忆的下降。对于所有测试，POD患者组和匹配的对照组之／＇^-j在术前和术后3个月的变化相似。结论术前神经认知和功能状态的轻微下降可以预测POD。然而在出现POD的小群体中，没有手术后3个月出现认知和功能下降的证据。POD与术前认知功能的降低有关，但在择期手术的老年患者中，可能不存在手术后3个月有害的认知或功能后遗症。

Atherosclerosis is a chronic inflammatory disease that is characterized by the build-up of lipid-rich plaques in the arterial walls. The standard treatment for patients with atherosclerosis is statin therapy aimed to lower serum lipid levels. Despite its widespread use, many patients taking statins continue to experience acute events. Thus, to develop improved and alternative therapies, we previously reported on microRNA-145 (miR-145 micelles) and its ability to inhibit atherosclerosis by targeting vascular smooth muscle cells (VSMCs). Importantly, one dose of miR-145 micelles significantly abrogated disease progression when evaluated two weeks post-administration. Thus, in this study, to evaluate how long the sustained effects of miR-145 micelles can be maintained and towards identifying a dosing regimen that is practical for patients with chronic disease, the therapeutic effects of a single dose of miR-145 micelles were evaluated for up to two months in vivo. After one and two months post-treatment, miR-145 micelles were found to reduce plaque size and overall lesion area compared to all other controls including statins without causing adverse effects. Furthermore, a single dose of miR-145 micelle treatment inhibited VSMC transdifferentiation into pathogenic macrophage-like and osteogenic cells in plaques. Together, our data shows the long-term efficacy and sustained effects of miR-145 micelles that is amenable using a dosing frequency relevant to chronic disease patients.

在300~5000℃、应变速率为0.1~0.0001 s−1的条件下,在万能试验机上对Al-5.6Zn-2Mg铝合金进行热压缩试验,以确定动态再结晶开始时的加工硬化速率曲线σ_(c)(ε_(c))l以及关键特征σ_(c)(ε_(c))l、σ_(p)(ε_(p))和σ_(ss)与Z系数之间的相关性。使用了四个本构模型,Arrhenius模型、改进的johnson-cook模型(MjC)、改进的Zerilli-Armstrong模型(MZA)和开发的人工神经网络(ANN)。结果表明,ANN型模型和Arrhenius模型的相对误差的绝对平均值最低,分别为0.486%和3.36%,MZA型模型和MjC型模型的相对误差的绝对平均值较高,分别为8.84%和3.93%。由于Arrhenius模型能够处理各种因素之间的非线性关系,因此它被认为是最合适的预测模型,但在材料性质未知或实验数据有限的情况下,MjC模型可能是一种更简单的替代方法。MZA模型不适合估计热压缩时的流变应力。此外,训练最好的神经网络模型的预测性能最好,相对误差的绝对平均值为0.486%,R值为0.99。

Background & Aims: The pathophysiology of constipation in the syndrome of obstructed defecation is unknown. Using 24-hour pancolonic manometric recordings of the unprepared colon to record basal pressures and spontaneous defecation episodes,we tested the hypothesis that the frequency, timing, or spatial distribution of propagating colonic pressure waves is abnormal in patients with obstructed defecation. Methods: In 11 patients with obstructed defecation and 16 healthy controls,pressures were recorded using a nasocolonic catheter that was positioned such that 16 recording sites spanned the unprepared colon at 7.5cm intervals. Results: The overall frequency of propagating sequences (PS) in the colon did not differ between patients and controls. When compared with controls, patients had a significant increase in the frequency of retrograde and antegrade PS (P< 0.05) in the left colon and a significant reduction in the amplitude of propagating pressure waves throughout the entire colon (P < 0.03). Defecation occurred in 6 of 11 patients and 9 of 16 controls. In the 15 minutes before defecation,controls showed a highly significant increase in frequency (P= 0.001) and amplitude (P = 0.01) of PS. In contrast, patients did not demonstrate this or the typical spatiotemporal organization of PS normally observed before expulsion of ***: Patients with obstructed defecation lack the normal predefecatory augmentation in frequency and amplitude of propagating pressure waves and lack the normal stereotypic spatiotemporal patterning of colonic pressure waves that would normally culminate in effective expulsion of stool.

Oral antibody to interleukin-10(anti-IL-10)enhances the intestinal immune defense against *** sulfur amino acids methionine and cysteine(MtC)play essential roles in inducing and maintaining protective immune responses during intestinal ***,increased dietary MtC may support the anti-IL-10-induced intestinal immunity to ***(n=640)were arranged in a 2×2×2 factorial design with 2 levels of each of the 3 main factors:dietary standardized ileal digestible(SID)MtC levels(0.6%or 0.8%),dietary anti-IL-10 supplementation(with or without),and coccidiosis challenge(control or challenge).Briefly,the broilers were supplied with either 0.6%or 0.8%SID MtC,each with or without anti-IL-10(300 mg/kg),from d 10 to *** d 14,broilers from each diet were gavaged with either PBS or *** resulting Eimeria infection induced fecal oocyst shedding and intestinal *** fed 0.8%SID MtC(main effects,P≤0.05)had decreased feed-to-gain ratio,increased duodenum and cecum luminal anti-Eimeria IgA titers,and decreased fecal oocyst counts,when compared to 0.6%SID *** supplementation of anti-IL-10(main effects,P≤0.05)increased cecum luminal total IgA concentration and decreased cecum ***(P≤0.05)were detected for growth performance and cecum luminal IFN-γ.Briefly,the highest body weight gain and feed intake were reached in PBS-gavaged broilers fed 0.8%SID MtC with no anti-IL-10 and in Eimeria-challenged broilers fed 0.8%SID MtC with *** Eimeria-infected broilers,anti-IL-10 increased intestinal luminal IFN-γ and body weight gain only at 0.8%SID ***,anti-IL-10 increased intestinal luminal IFN-γ levels,decreased cecum lesions and restored growth only when fed with adequate amounts of sulfur amino *** findings underscore the importance of providing sufficient essential nutrients to support the anti-IL-10 induced immunity against coccidiosis.
