Multi-gradient drilling is a new offshore drilling method.The accurate calculation of the related wellbore temperature is of great significance for the prediction of the gas hydrate formation area and the precise control of the wellbore pressure.In this study,a new heat transfer model is proposed by which the variable mass flow is properly taken into account.Using this model,the effects of the main factors influencing the wellbore temperature are analyzed.The results indicate that at the position where the separation injection device is installed,the temperature increase of the fluid in the drill pipe is mitigated due to the inflow/outflow of hollow spheres,and the temperature drop of the fluid in the annulus also decreases.In addition,a lower separation efficiency of the device,a shallower installation depth and a smaller circulating displacement tend to increase the temperature near the bottom of the annulus,thereby helping to reduce the hydrate generation area and playing a positive role in the prevention and control of hydrates in deepwater drilling.

OBJECTIVE:To formulate the first clinical practice guideline for the treatment of cough using Chinese medicine based on the grading of recommendations assessment,development,and evaluation(GRADE)systematic approach,including clinical evidence,evaluation of ancient literature,and expert consensus.METHODS:In the process of development,the regulation of"evidence-based,consensus-assisted,and empirical"was followed,and a comprehensive systematic approach of recommendation assessment,GRADE,evidence-based evaluation,expert consensus,and the Delphi method was used.In the process of guideline development,evidence-based evaluation of ancient literature was included for the first time,and clinical evidence was fully integrated with clinical expert consensus.RESULTS:The clinical practice guidelines for the treatment of cough with Chinese herbal medicine were developed after a comprehensive consideration of evidence-based evaluation and expert opinions.The guideline recommendations focused on recommending herbal compound decoctions and Chinese patent medicines for cough in different conditions.Based on the GRADE systematic approach,we conducted an evidence-based evaluation of the recommended Chinese patent medicines one by one;meanwhile,the expert consensus method was used to unify the recommendations of both.CONCLUSION:Based on clinical evidence,ancient literature evaluation,and expert consensus,a clinical practice guideline for Traditional Chinese Medicine(TCM)in the treatment of cough was developed,providing the first current clinical practice guideline for domestic and foreign TCM and Western medicine practitioners,especially respiratory professionals at home and abroad.

Solid-state zinc-ion capacitors are emerging as promising candidates for large-scale energy storage owing to improved safety,mechanical and thermal stability and easy-to-direct stacking.Hydrogel electrolytes are appealing solid-state electrolytes because of eco-friendliness,high conductivity and intrinsic flexibility.However,the electrolyte/electrode interfacial contact and anti-freezing properties of current hydrogel electrolytes are still challenging for practical applications of zinc-ion capacitors.Here,we report a class of hydrogel electrolytes that couple high interfacial adhesion and anti-freezing performance.The synergy of tough hydrogel matrix and chemical anchorage enables a well-adhered interface between hydrogel electrolyte and electrode.Meanwhile,the cooperative solvation of ZnCl2 and LiCl hybrid salts renders the hydrogel electrolyte high ionic conductivity and mechanical elasticity simultaneously at low temperatures.More significantly,the Zn||carbon nanotubes hybrid capacitor based on this hydrogel electrolyte exhibits low-temperature capacitive performance,delivering high-energy density of 39 Wh kg^(-1)at-60°C with capacity retention of 98.7%over 10,000 cycles.With the benefits of the well-adhered electrolyte/electrode interface and the anti-freezing hydrogel electrolyte,the Zn/Li hybrid capacitor is able to accommodate dynamic deformations and function well under 1000 tension cycles even at-60°C.This work provides a powerful strategy for enabling stable operation of low-temperature zinc-ion capacitors.

Draft tube vortex rope is considered a special cavitation flow phenomenon in tubular turbine units.Cavitation vortex rope is one of the most detrimental factors affecting the safety of hydraulic turbines.In this study,ANSYS CFX software was utilized to numerically simulate the internal cavitation flow of a hydraulic turbine draft tube.The evolution of the cavitation vortex core was characterized by vortex line distribution and vorticity transport equation.The shape and number of blades influenced the revolving direction and distribution characteristics of the vortex close to the runner cone,which formed a counterclockwise-clockwise-counterclockwise distribution pattern.Simultaneously,there were many secondary flows in the draft tube.Mutual cancellation and dissipation between the flows was one of the reasons for reduction in vorticity.When the cross-sectional shape of the draft tube was changed,the vorticity was distributed from the center of the vortex rope to all parts of the cross-sectional draft tube,with extreme values at the center and at the walls.The vortex stretching and dilatation terms played a major role in the change in vorticity,with the baroclinic torque having an effect at the center of the vortex rope,this study is helpful to understand the flow of water in the draft tube and guide the design and optimization of the draft tube in engineering application.

Targeted delivery of hyaluronic acid nanomicelles to hepatic stellate cells in hepatic fibrosis rats

Hepatic fibrosis is one kind of liver diseases with a high mortality rate and incidence.The activation and proliferation of hepatic stellate cells(HSCs)is the most fundamental reason of hepatic fibrosis.There are no specific and effective drug delivery carriers for the treatment of hepatic fibrosis at present.We found that when hepatic fibrosis occurs,the expression of CD44 receptors on the surface of HSCs is significantly increased.Based on this finding,we designed silibinin-loaded hyaluronic acid(SLB-HA)micelles to achieve the treatment of hepatic fibrosis.Meanwhile,we constructed liver fibrosis rat model using Sprague-Dawley rats.We demonstrated that HA micelles had specific uptake to HSCs in vitro while avoiding the distribution in normal liver cells and the phagocytosis of macrophages.Importantly,HA micelles showed a significant liver targeting effect in vivo,especially in fibrotic liver which highly expressed CD44 receptors.In addition,SLB-HA micelles could selectively kill activated HSCs,having an excellent anti-hepatic fibrosis effect in vivo and a significant sustained release effect,and also had a good biological safety and biocompatibility.Overall,HA micelles represented a novel nanomicelle system which showed great potentiality in anti-hepatic fibrosis drugs delivery.

新生儿期起病的癫痫综合征是一组在新生儿期起病、具有典型临床和脑电图特征的遗传异质性癫痫,但仅依靠临床表现和脑电图进行遗传病因诊断存在局限性。近年来,二代测序技术的发展促进了癫痫的遗传诊断,实现了精准化治疗。国际抗癫痫联盟将癫痫综合征分为自限性癫痫、发育性癫痫性脑病及病因特异性癫痫综合征三类,本文将对这3个新生儿期起病的癫痫综合征的遗传病因、遗传诊断和基于基因变异机制的精准治疗展开综述,以期为新生儿期起病的癫痫综合征的早期遗传学诊治提供帮助。

目的探讨基于入院24 h内全脑CT灌注(CTP)及临床数据预测动脉瘤性蛛网膜下腔出血(aSAH)患者发生分流依赖性脑积水(SDHC)的价值。方法回顾性收集2018年3月至2022年8月在皖南医学院第一附属医院接受介入栓塞治疗的aSAH患者入院时的临床和影像学数据。所有患者均在症状发生后24 h内完成一站式全脑CTP检查,并经后处理得到CTP的定性和定量参数。通过查阅电子病历或进行电话随访,每2个月随访1次,共随访6个月。根据是否发生SDHC,将患者分为SDHC组及非SDHC组,比较两组间的差异,采用logistic回归模型分析确定SDHC发生的影响因素,建立SDHC预测模型,通过绘制受试者工作特征(ROC)曲线评估预测模型效能。结果共纳入414例患者,其中男132例,女282例,年龄为(59±11)岁。17.6%(73/414)患者发生SDHC。是否发生急性脑积水、世界神经外科联盟分级(WFNS)、Hunt-Hess分级、改良Fisher评分(mFS)、CTP定性及定量参数在两组间的差异有统计学意义(均P<0.001)。多因素logisitc回归分析显示急性脑积水(OR=8.621,95%CI:4.237~17.542)、高龄(OR=1.107,95%CI:1.068~1.148)、高mFS及高Hunt-Hess分级(OR=3.740,95%CI:1.352~10.342)是aSAH患者发生SDHC的危险因素,高平均脑血流量(mCBF)(OR=0.931,95%CI:0.885~0.980)是发生SDHC的保护因素,由这5个变量构建的预测模型的ROC曲线下面积(AUC)=0.923(95%CI:0.89~0.95),其灵敏度为84.5%、特异度为87.7%。结论aSAH患者入院24 h内的mCBF和急性脑积水、年龄、mFS、Hunt-Hess分级可用于预测其SDHC的发生。
