Background Acute vestibular syndrome(AVS)features continuous dizziness and may result from a benign inner ear disorder or *** head impulse-nystagmus-test of skew(HINTS)bedside assessment is more sensitive than brain MRI in identifying stroke as the cause of AVS within the first 24 ***’perspectives of the test in UK secondary care remains ***,we explore front-line clinicians’perspectives of use of the HINTS for the diagnosis of *** Clinicians from two large UK hospitals who assess AVS patients completed a short online survey,newly designed with closed and open *** Almost half of 73 total responders reported limited(n=33),or no experience(n=19),reflected in low rates of use of HINTS(n=31).While recognising the potential utility of HINTS,many reported concerns about subjectivity,need for specialist skills and poor patient *** clinicians reported high levels of confidence in performing HINTS,with 98%identifying training needs.A lack of formalised training was associated with onward specialist referrals and neuroimaging(p=0.044).Conclusions Although the low sample size in this study limits the generalisability of findings to wider sites,our preliminary data identified barriers to the application of the HINTS in AVS patients and training needs to improve rapid,cost-effective and accurate clinical diagnosis of stroke presenting with vertigo.

CD4^(+)T cells can"help"or"license" conventional type 1 dendritic cells(cDC1s)to induce CD8^(+)cytotoxic T lymphocyte(CTl)anticancer responses,as proven in mouse *** recently identified cDC1s with a transcriptomic imprint of CD4^(+)T-cell help,specifically in T-cell-infiltrated human cancers,and these cells were associated with a good prognosis and response to PD-1-targeting ***,we delineate the mechanism of cDC1 licensing by CD4^(+)T cells in *** CD4^(+)T cells produce IFNβvia the STING pathway,which promotes MHC-I antigen(cross-)presentation by cDC1s and thereby improves their ability to induce CTl anticancer *** cooperation with CD40 ligand(l),IFNβalso optimizes the costimulatory and other functions of cDC1s required for CTl response ***-I-producing CD4^(+)T cells are present in diverse T-cell-infiltrated cancers and likely deliver“help”signals to CTls locally,according to their transcriptomic profile and colocalization with“helped/licensed”cDCs and tumor-reactive CD8^(+)T *** agreement with this scenario,the presence of IFN-I-producing CD4^(+)T cells in the TME is associated with overall survival and the response to PD-1 checkpoint blockade in cancer patients.

Introduction :Congenital adrenal hyperplasia (CAH) is the most common cause of primary adrenal insufficiency. It is a rare monogenic recessive disorder. In African setting in absence of neonatal screening, the diagnosis is still late, based on a clinical approach. During this clinical enquiry, information from past history or pedigree of the patient is of a huge importance and may revealed surprises. Patients and Methods: In this observational study, we retrospectively included all patients with a diagnosis of CAH. The diagnosis of CAH was retained based on a high 17 hydroxyprogesterone level in addition to clinical and morphological findings. From patients’ files, we extracted data on family history of disease, pedigree, clinical findings and genetics when available of 39 patients from two endocrinopeadiatric centers. Results: In 13 (30%) families, we found 20 reported deaths of infant less than 12 months. In these 13 families, half of the patients followed had 21 hydroxylase deficiencies and had 11 hydroxylase deficiencies. Unsurprisingly, we suspected adrenal insufficiency in these patients at verbal autopsy even in families with a patient with 11 hydroxylase deficiency. Other non DSD malformations or genetic disorders with apparently no link with CAH were reported in 3 families. The father of a patient reported to have hypospadias. Conclusion: Each diagnosis of CAH made in our context is visible part of an iceberg. Behind a diagnosis of CAH made in our setting, is a long course of care, a dramatic past history revealing access to appropriate care disparity. Neonatal screening should thus be considered as an emergency.

Alzheimer’s disease (AD) is the most common neurodegenerative disease causing an alteration of life quality in the terminal stage. The purpose was to report 14 years of experience about the early impact on the quality of life of patients with AD. Methodology: Descriptive retrospective study over 14 years in the geriatric department of Pitié Salpêtrière Hospital, using the activity of daily living, Instrumental activity of daily living, neuropsychological inventory and Hoen Yahr scale evaluated at the time of diagnosis of AD according to the National Institute of Neurological and Communicative Disorders and Stroke-Alzheimer Disease’s and Related Disorders Association diagnostic criteria. Results: A total of 214 exploitable files had been listed. At the moment of diagnosis, the mean age was 82.1 years with extremes 68 to 95 with sex ratio 1.6 in women’s favor. The mean socio-cultural level was 4.9 with extremes about 0 to 7. There was poly pathology with a mean Cumulative Illness Rate Scale = 4.6 with extremes 0 to 16. the mean cognitive status was moderate = 22.5 with extremes 0 to 30. Quality life showed moderate impairment of IADl = 9.2 with extreme 3 to 11 compared to activity of daily living. The activity of daily living was more affected in 68 - 80-year-olds, while poly pathology impacted more on IADl in men. The cognitive impairment was more deficient in IADl when the MMSE test was low. The common disorders at the NPI were psychological, behavioral and psychotic. Conclusion: At the early diagnosis of Alzheimer’s Disease cognitive deficiencies were predominant and influenced on global Instrumental activity and psychological, behavioral disorders.

liver fibrosis is a wound-healing response following chronic liver injury caused by hepatitis virus infection,obesity,or excessive *** is a dynamic and reversible process characterized by the activation of hepatic stellate cells and excess accumulation of extracellular *** fibrosis could lead to cirrhosis and even liver cancer,which has become a significant health burden *** studies have revealed that noncoding RNAs(ncRNAs),including microRNAs,long noncoding RNAs and circular RNAs,are involved in the pathogenesis and development of liver fibrosis by regulating signaling pathways including transforming growth factor-βpathway,phosphatidylinositol 3-kinase/protein kinase B pathway,and Wnt/β-catenin *** in serum or exosomes have been reported to tentatively applied in the diagnosis and staging of liver fibrosis and combined with elastography to improve the accuracy of *** mimics,ncRNAs in mesenchymal stem cell-derived exosomes,and lipid nanoparticles-encapsulated ncRNAs have become promising therapeutic approaches for the treatment of liver *** this review,we update the latest knowledge on ncRNAs in the pathogenesis and progression of liver fibrosis,and discuss the potentials and challenges to use these ncRNAs for diagnosis,staging and treatment of liver *** these will help us to develop a comprehensive understanding of the role of ncRNAs in liver fibrosis.

Introduction: Sickle cell anaemia is a hereditary disease that combines physical and psychological manifestations, including suicidal tendencies. So far, to our knowledge, no study has been conducted on suicidal behaviours among people with sickle cell disease in Cameroon. This is what justifies our study on the prevalence and factors related to suicidal behaviors in our study population. Methods: We conducted a cross-sectional, analytical study of 171 sickle cell patients aged from 12 years upwards who came to the sickle cell disease care service of the laquintinie Hospital in Douala, over a period of 6 months, that is, from 1 January to 31 June 2022. Data were collected using a structured questionnaire with questions on suicidal behaviours based on the “MINI” (Mini International Neuropsychiatric Interview). The data were processed using the SPSS 26.0 software. The related factors were studied in both a bivariate and multivariate analysis. Results: Female sickle cell patients accounted for 60.2% of the sample. The mean age was 23.36 ± 8.42 years. Suicidal ideation was prevalent in 56% of cases and 13% attempted suicide. The factors most associated with suicidal ideation were: primary level of education (OR = 0.08 (0.09 - 0.79);p = 0.03), feeling unworthy of life (OR = 0.40 (0.08 - 1.96);p = 0.02), not often being considered by those around them (OR = 2.97 (1.26 - 6.99);p = 0.01), lack of family support (OR = 0.34 (0.15 - 0.77);p = 0.01). Meanwhile, the factors associated with suicide attempts were: the fact of being rarely exposed to suicide-related media (OR = 4.17 (1.40 - 71.80);p = 0.03), and a constant feeling of sadness when returning home (OR = 18.02 (1.59 - 20.55);p = 0.01). Conclusion: More than half of sickle cell patients had had suicidal thoughts and 1/6 had made at least one suicide attempt, women and young adults being the most concerned. It is therefore necessary to ensure optimum psychological care for patients with sickle cell disease.

Disparities have emerged as an important issue in many aspects of healthcare in developed countries and may be based on race,ethnicity,sex,geographical location,and socioeconomic *** liver disease specifically,these potential disparities can affect access to care and outcome in viral hepatitis,chronic liver disease,and hepatocellular *** in hepatologists and medical providers versed in liver disease may amplify these disparities by compromising early detection of liver disease,surveillance for hepatocellular carcinoma,and prompt referral to subspecialists and transplant *** the United States,continued efforts have been made to address some of these disparities with better education of healthcare providers,use of telehealth to enhance access to specialists,reminders in electronic medical records,and modifying organ allocation systems for liver *** review will detail the current status of disparities in liver disease and describe current efforts to minimize these disparities.

Advances in the oncology field have led to improved survival ***,quality of life after remission is anticipated,which includes the possibility to conceive *** cancer treatments are potentially gonadotoxic,fertility preservation must be *** fertility preservation is mainly based on ejaculated sperm *** this is not possible,testicular sperm extraction(TESE)may be *** identify situations in which TESE has been beneficial,a systematic review was *** search was carried out on the PubMed,Scopus,Google Scholar,and CISMeF databases from 1 January 2000 to 19 March *** Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)recommendations were followed in selecting items of ***-four articles were included in the systematic review,including 15 articles on oncological testicular sperm extraction(oncoTESE),18 articles on postgonadotoxic treatment TESE and 1 article on both oncoTESE and postgonadotoxic treatment *** sperm freezing was possible for 42.9%to 57.7%of patients before gonadotoxic treatment and for 32.4%to 75.5%of patients after gonadotoxic treatment,depending on the type of malignant *** no formal conclusion could be drawn about the chances to obtain sperm in specific situations,our results suggest that TESE can be proposed before and after gonadotoxic *** treatment,TESE is more often proposed for men with testicular cancer presenting with azoospermia since TESE can be performed simultaneously with tumor removal or *** chemotherapy,TESE may be planned if the patient presents with persistent azoospermia.

The power conversion efficiencies(PCEs)of single-junction organic solar cells(OSCs)have surpassed 19%,owing to the emerging Y-series nonfullerene acceptors(NFAs).Undoubtedly,the power and flexibility of chemical design has been a strong driver for this rapid efficiency improvement in the OSC *** the course of the past 3 years,a variety of modifications have been made to the structure of the Y6 acceptor,and a large number of Y-series NFAs have been reported to further improve ***,we present our insights into the rationale behind the Y6 acceptor and discuss the design principles toward high-performance Y-series *** is clear that structural modifications through choice of heteroatom,soluble chains,πspacers,central cores,and end groups alter the material characteristics and properties,contributing to distinctive photovoltaic ***,we analyze various design strategies of Y-series-containing materials,including polymerized small-molecule acceptors(PSMA),non-fused-ring acceptors(NFRA),and all-fused-ring acceptors(AFRA).This review is expected to be of value in providing effective molecular design strategies for high-performance NFAs in future innovations.
