Gastrointestinal disorders associated with migraine: A comprehensive review

Migraine is a recurrent and commonly disabling primary headache disorder that affects over 17% of women and 5%-8% of men. Migraine susceptibility is multifactorial with genetic, hormonal and environmental factors all playing an important role. The physiopathology of migraine is complex and still not fully understood. Many different neuropeptides, neurotransmitters and brain pathways have been implicated. In connection with the myriad mechanisms and pathways implicated in migraine, a variety of multisystemic comorbidities(e.g., cardiovascular, psychiatric and other neurological conditions) have been found to be closely associated with migraine. recent reports demonstrate an increased frequency of gastrointestinal(GI) disorders in patients with migraine compared with the general population. Helicobacter pylori infection, irritable bowel syndrome, gastroparesis, hepatobiliary disorders, celiac disease and alterations in the microbiota have been linked to the occurrence of migraine. Several mechanisms involving the gut-brain axis, such as a chronic inflammatory response with inflammatory and vasoactive mediators passing to the circulatory system, intestinal microbiota modulation of the enteric immunological milieu and dysfunction of the autonomic and enteric nervous system, have been postulated to explain these associations. However, the precise mechanisms and pathways related to the gut-brain axis in migraine need to be fully elucidated. In this review, we survey the available literature linking migraine with GI disorders. We discuss the possible physiopathological mechanisms, and clinical implications as well as several future areas of interest for research.

Focus on the gut-brain axis: multiple sclerosis, the intestinal barrier and the microbiome

The brain-gut axis serves as the bidirectional connection between the gut microbiome, the intestinal barrier and the immune system that might be relevant for the pathophysiology of inflammatory demyelinating diseases. People with multiple sclerosis have been shown to have an altered microbiome, increased intestinal permeability and changes in bile acid metabolism. Experimental evidence suggests that these changes can lead to profound alterations of peripheral and central nervous system immune regulation. Besides being of pathophysiological interest, the brain-gut axis could also open new avenues of therapeutic targets. Modification of the microbiome, the use of probiotics, fecal microbiota transplantation, supplementation with bile acids and intestinal barrier enhancers are all promising candidates. Hopefully, pre-clinical studies and clinical trials will soon yield significant results.

Different patterns of intestinal response to injury after arterial,venous or arteriovenous occlusion in rats

AIM： To investigate the differences in injury patterns caused by arterial, venous or arteriovenous mesenteric occlusion. METHODS： Male Wistar rats were separated equally into four groups. Occlusion was performed by clamping the superior mesenteric artery （A）, the mesenteric vein （V） or both （AV） for 30 min, followed by 60 min of reperfusion. A control group received sham surgery only. Intestinal sections were examined for histological damage and serum tumor necrosis factor-α （TNF-α）, endothelin-1 （ET-1）, P-selectin, antithrombin Ⅲ （AT Ⅲ） and soluble intracellular adhesion molecule-1 （IcAM-1） concentrations were measured. rESULTS： All groups showed significant mucosal injury compared to controls. Furthermore, mucosal injury was significantly more severe in the V and AV groups compared to the A group （3.6 ± 0.55, 3.4 ± 0.55 and 2± 0.71, respectively, P = 0.01）. IcAM-1 was similarly elevated in all groups, with no significant differences between the groups. P-selectin levels were significantly elevated in the V and AV groups but not the A group （1.4 ± 0.5 ng/mL, 2.52 ± 0.9 ng/mL and 0.02± 0.01 ng/mL, respectively, P = 0.01） and ET-1 was significantly elevated in the A and V groups but not the AV group （0.32 ± 0.04 pg/mL, 0.36 ± 0.05 pg/mL and 0.29 ± 0.03 pg/mL, respectively, P = 0.01） compared to sham controls. ATⅢ levels were markedly depleted in the V and AV groups, but not in the A group （29.1 ± 5.2 pg/mL, 31.4 ±21.8 pg/mL and 55.8± 35.6 pg/mL, respectively, P = 0.01）, compared to controls. Serum TNF-α was significantly increased in all groups compared to sham controls （1.32 ± 0.87 ng/mL, 1.79 ± 0.20 ng/mL and 4.4 ± 0.69 ng/mL, for groups A, V and AV, respectively, P = 0.01）, with higher values in the AV group. cONcLUSION： Different patterns of response to ischemia/reperfusion are associated with venous, arterial or arteriovenous occlusion. Efficacy of antimicrobials in preventing resistance in solid organ transplant recipients:A systematic review of clinical trials

Projections of the impacts of climate change on the water deficit and on the precipitation erosive indexes in Mantaro river Basin, Peru

Projections of climate change are essential to guide sustainable development plans in the tropical Andean countries such as Peru. This study assessed the projections of precipitation and potential evaporation, rain erosive potential, and precipitation concentration in the Mantaro river Basin, in the Peruvian Andes, which is important for agriculture and energy production in Peru. We assumed the Intergovernmental Panel on climate change （IPcc） AIB greenhouse gas emission scenario and simulated the global climate change by the HadcM3 global climate model. Due to the steepness of the mountain slopes and the narrowness of the river valley, this study uses the downscaling of the global model simulations by the regional Eta model down to 2o-km resolution. The downscaling projections show decrease in the monthly precipitation with respect to the baseline period, especially during the rainy season, between February and April, until the end of the 21st century. Meanwhile, a progressive increase in the monthly evaporation from the baseline period is projected. The Modified Fournier Index （MFI） shows a statistically significant downward trend in the Mantaro river Basin, whieh suggests a possible reduction in the rain erosive potential. The Precipitation concentration Index （PcI） shows a statistically significant increasing trend, which indicates increasingly more irregular temporal distribution of precipitation towards the end of the century. The results of this study allow us to conclude that there should be a gradual increase in water deficit and precipitation concentration. Both changes can be negative for agriculture, power generation, and water supply in the Mantaro river Basin in Peru.
