Sweet’s syndrome is a neutrophilic dermatosis characterized by tender, erythematous, pseudovesicular plaques that can be associated with hematologic malignancy. We report a patient with recalcitrant Sweet’s syndrome that preceded the development of myelodysplastic syndrome by 30 months. The delay between the onset of Sweet’s syndrome and the subsequent diagnosis of myelodysplasia highlights the need for thorough and repeated evaluation for underlying malignancy in patients with such a course. Although corticosteroids are the initial treatment of choice, this patient’s eruption was only partially responsive to high-dose prednisone andwas refractory tometronidazole, dapsone, and methotrexate. Treatmentwith thalidomide resulted in complete resolution of the cutaneous lesions within one month of therapy.

Factor V Leiden is the most common genetic thrombophilia in people of Europea n descent, and is important to recognize as it can have significant implications in dermatology. We report a case of a 30- year-old man who presented for eva luation and treatment of a chronic ulceration on the site of his stump following a below the knee amputation which had been performed for non-healing ulcerati ons. Despite a variety of treatments, his ulcer persisted. He was referred to a dermatologist who performed a biopsy that was interpreted ass non-specific, an d treatment was started for pyoderma gangrenosum. Further investigation revealed a homozygous factor V Leiden mutation and cryofibrinogenemia. He was tapered of f of the methylprednisolone and was improving on stanozolol. He healed well afte r surgery and no new ulcerations have developed. This case highlights the import ance of considering this mutation in a non-healing leg ulcer.

Background: Livedoid vasculopathy is an idiopathic, chronic disorder manifested by painful, purpuric macules on the lower extremities that superficially ulcerate, resulting in atrophic, stellate scars with peripheral telangiectasias and hyperpigmentation. Observations: A 50-year-old man presented with recurrent, painful ulcerations on the medial aspect of his malleoli and calves. The clinical presentation, histologic findings, and results of laboratory evaluation confirmed the diagnosis of livedoid vasculopathy in this case. Despite being refractory to treatment with multiple other medications, the lesions responded dramatically to oral warfarin sodium therapy. Conclusion: Treatment with warfarin may be a beneficial therapy for patients with livedoid vasculopathy.

Background: The development of granulomatous lesions within tattoos is a well-recognized occurrence in individuals with sarcoidosis. The characteristic histopathological finding of sarcoidosis is the presence of noncaseating granulomas; however, similar histopathogical findings may be seen in foreign body granulomas. Several reports have challenged the assertion that the presence of foreign material within sarcoidal granulomas is incompatible with a diagnosis of sarcoidosis. Observations: We describe a patient who had multiple linearly arranged papules along her eyebrows and the vermillion border of her upper lip. She had had cosmetic tattooing performed on these areas 3 year prior to presentation. Histopathologic examination revealed sarcoidal granulomas, polarizable foreign material, and pigment granules. Hilar adenopathywas noted on a chest radiograph. After 4 months of treatment with a midpotency topical steroid and doxycycline, she experienced complete clearance of her cutaneous lesions and normalization of chest x-ray film findings. Conclusions: This case demonstrates a unique adverse result after cosmetic tattooing and highlights the concept that granulomatous histopathologic findings containing foreign material should not be an exclusionary criterion for the diagnosis of sarcoidosis. In this setting, further investigation for the presence of systemic disease is indicated.

patients with systemic lupus erythematosus (SLE)often develop leg ulceration, particularly those with antiphospholipid antibodies or with vasculitis. pyoderma gangrenosum (pG)is an idiopathic ulcerative neutrophilic dermatosis that is com monly associated with inflammatory bowel disease or seronegative polyarthritis. Although pG-like lesions have been commonly described in patients with the anti phospholipid antibody syndrome, the occurrence of pG as a preceding manifestatio n of SLE has only rarely been reported. We present a patient who developed pG ro ughly 8 years prior to developing SLE.

Linear IgA bullous dermatosis (LABD) is a rare autoimmune vesiculobullous disorder characterized by variable clinical presentations that may mimic bullous pemphigoid, dermatitis herpetiformis, cicatricial pemphigoid and erythema multi- forme. A few cases of drug-induced LABD that clinically resembled toxic epidermal necrolysis (TEN) have been reported. A subset of patients with LABD have been found to be drug-induced: the most common drug being vancomycin. The diagnosisofLABDisconfirmedbythepresenceofalinearband of IgA along the basement membrane zone on direct immunofluorescence microscopy. We report a case of a 77- year-old man who presented to us with vancomycin-induced LABD that presented clinically as TEN, He had a complete recovery over a 3- week period following discontinuation of the vancomycin and the addition of oral dapsone therapy. It is important to be aware that drug-induced LABD can mimic TEN.

Background: Dermatomyositis (DM) is a multisystem idiopathic inflammatory disorder that most commonly affects the muscles and skin. Systemic corticosteroids are the mainstay of therapy but are limited by their long-term adverse effects. Observations: We sought to evaluate the effectiveness of oral mycophenolate mofetil in patients with cutaneous lesions of DM recalcitrant to other therapies through an open-label retrospective medical chart review of patients in a university affiliated private practice setting. Twelve patients with DM who had skin lesions recalcitrant to traditional therapies or who developed toxic effects from traditional therapies began mycophenolate mofetil treatment at doses ranging from 500 mg to 1 g twice a day. Response was based on improvement in skin disease as judged clinically, an increase in strength, and/or an ability to decrease or discontinue concomitant therapies. Improvement was seen in 10 of the 12 patients, most within 4 to 8 weeks. Most patients tolerated mycophenolate mofetil treatment without problem; however, 1 patient developed a B-cell lymphoma of the central nervous system, and another developed abnormal levels of hepatic enzymes along with urinary symptoms. Resolution of these toxic reactions occurred with cessation of mycophenolate mofetil treatment in each patient. Conclusion: Mycophenolate mofetil may be an effective corticosteroid-sparing therapy for the treatment of some patients with DM.

癫痫是儿童期常见的神经系统疾病,为了明确癫痫性脑病的基因检出率,对一个癫痫遗传诊所进行了一项回顾性队列研究.2012年1月-2014年6月,在一个癫痫遗传诊所纳入了所有兼有难治性癫痫、整体性发育迟滞和认知功能障碍的患者.通过回顾电子病历,获得了患者的临床特征、神经影像、生化检验及分子遗传学资料,包括对癫痫性脑病基因的第二代目的基因测序.在110例患者中,发现28%是有基因异常的:7%有遗传性代谢障碍,包括由ALDH7A1变异引起的吡哆醇依赖性癫痫、Menkes病、吡哆醇-5-磷酸氧化酶缺乏、钴胺素G缺乏、亚甲基四氢叶酸还原酶缺乏、Ⅰ型葡萄糖载体缺陷、甘氨酸脑病和丙酮酸脱氢酶复合体缺陷;21%有其他的基因病因,包括遗传性综合征、在阵列比较基因组杂交上的致病性拷贝数变异,以及与SCN1A、SCN2A、SCN8A、KCNQ2、STXBp1、pCDH19和SLC9A6基因变异相关的癫痫性脑病.4.5%的患者得到了由第二代目的基因测序的癫痫性脑病面板作出的基因诊断.值得注意的是,4.5%的患者有可治的遗传性代谢性疾病.该研究为首个将遗传性代谢障碍和癫痫性脑病的其他遗传病因结合起来的研究.第二代目的基因测序面板将癫痫性脑病患者的基因诊断检出率从25%.
