Background The effect of transdermal glyceryl trinitrate(GTN,a nitrovasodilator)on clinical outcome when administered before hospital admission in suspected stroke patients is ***,we assess the safety and efficacy of GTN in the prespecified subgroup of patients who had an ischaemic stroke within the Rapid Intervention with Glyceryl trinitrate in Hypertensive stroke Trial-2(RIGHT-2).Methods RIGHT-2 was an ambulance-based multicentre sham-controlled blinded-endpoint study with patients randomised within 4hours of *** primary outcome was a shift in scores on the modified Rankin scale(mRS)at day *** outcomes included death;a global analysis(Wei-Lachin test)containing Barthel Index,EuroQol-5D,mRS,telephone interview for cognitive status-modified and Zung depression scale;and neuroimaging-determined‘brain frailty’*** were reported as n(%),mean(SD),median[IQR],adjusted common OR(acOR),mean difference or Mann-Whitney difference(MWD)with 95%*** 597 of 1149(52%)patients had a final diagnosis of ischaemic stroke;age 75(12)years,premorbid mRS>2107(18%),Glasgow Coma Scale 14(2)and time from onset to randomisation 67[45,108]***‘brain frailty’was common:median score 2[2,3](range 0–3).At day 90,GTN did not influence the primary outcome(acOR for increased disability 1.15,95%CI 0.85 to 1.54),death or global analysis(MWD 0.00,95%CI-0.10 to 0.09).In subgroup analyses,there were non-significant interactions suggesting GTN may be associated with more death and dependency in participants randomised within 1hour of symptom onset and in those with more severe *** In patients who had an ischaemic stroke,ultra-acute administration of transdermal GTN in the ambulance did not improve clinical outcomes in a population with more clinical and radiological frailty than seen in previous in-hospital *** IS ALREADY KNOWN ON THIS TOPIC⇒Transdermal glyceryl trinitrate(GTN)was associat-ed with less death and dependency in those w

Purinergic signaling systems across comparative models of spinal cord injury

Within the last several decades,the scientific community has made substantial progress in elucidating the complex pathophysiology underlying spinal cord ***,despite the many advances using conventional mammalian models,both cellular and axonal regeneration following spinal cord injury have remained out of *** this sense,turning to non-mammalian,regenerative species presents a unique opportunity to identify pro-regenerative cues and chara cterize a spinal cord microenvironment permissive to *** the signaling pathways hypothesized to be dysregulated during spinal cord injury is the purinergic signaling *** addition to its well-known role as energy currency in cells,ATP and its metabolites are small molecule neurotransmitte rs that mediate many diverse cellular processes within the central nervous *** our unde rstanding of the roles of the purinergic system following spinal cord injury is limited,this signaling pathway has been implicated in all injury-induced secondary processes,including cellular death,inflammation,reactive gliosis,and neural *** that the purinergic system is also evolutionarily conserved between mammalian and non-mammalian species,comparisons of these roles may provide important insights into conditions responsible for recovery ***,we compare the secondary processes between key model species and the influence of purinergic signaling in each *** our understanding of this signaling system and pro-regenerative conditions continues to evolve,so does the potential for the development of novel therapeutic interventions for spinal cord injury.

The role of soil in structure response of a building damaged by the 26 December 2018 earthquake in Italy

Local soil conditions can significantly modify the seismic motion expected on the soil *** most cases,the indications concerning the influence of the underlying soil provided by the in-force European and Italian Building Codes underestimate the real seismic amplification *** this reason,numerical analyses of the local seismic response(LSR)have been encouraged to estimate the soil filtering *** analyses are generally performed in free-field conditions,ignoring the presence of superstructures and,therefore,the effects of dynamic soil-structure interaction(DSSI).Moreover,many studies on DSSI are characterised by a sophisticated modelling of the structure and an approximate modelling of the soil(using springs and dashpots at the foundation level);while others are characterised by a sophisticated modelling of the soil and an approximate modelling of the structure(considered as a simple linear elastic structure or a single degree of freedom system).This paper presents a set of finite element method(FEM)analyses on a fully-coupled soil-structure system for a reinforced concrete building located in Fleri(Catania,Italy).The building,designed for gravity loads only,was severely damaged during the 26 December 2018 *** soil was modelled considering an equivalent visco-elastic behaviour,while the structure was modelled assuming both the visco-elastic and visco-inelastic *** comparison made between the results of the FEM analyses and the observed damage is valuable.

Long-term monitoring data reveal effects of age,population density,and environmental aspects on hatching success of Common Cranes(Grus grus)

Facing climate and land use change,a species’ability to successfully adapt to changing environments is crucial for its *** drainage and intensification of agriculture and forestry set wetlands and associated species at risk of population *** population of Common Cranes(Grus grus)has experienced considerable fluctuations over the last *** increasing population numbers,hatching success seemed to have decreased over the last *** aim of this study was to identify factors influencing hatching success and nest survival of Common Cranes based on analyses of long-term individual-based monitoring data from northeastern Germany and evaluate the species ability to adapt to changing *** success decreased over the course of the study period from 0.75 to ***,nest survival and hatching success did not vary across different nesting habitats,whereas factors such as female age,timing of nest initiation and breeding pair density were found to have significant effects on hatching *** females showed higher hatching success,even though the proportion of unhatched eggs was highest in females aged 20 years or *** nest initiation had a positive effect on hatching *** levels are more favorable early in the nesting season,whereas increasing evaporation with time causes water levels to decrease,granting easier access for *** of female age,hatching success decreased with increasing numbers of breeding pairs within a 2-km radius around a nesting *** population densities intensify competition for resources and promote intraspecific interactions,affecting reproductive outcome *** study gives first insights into mechanisms behind population regulation in Common Cranes,highlighting the importance of population dynamics and individual *** suggest to further investigate density dependent effects including landscape and habitat features as well as reproduct

Genotyping ex situ trees of Abies nebrodensis translocated from the original Sicilian population to enrich the gene pool

As a dynamic ex situ conservation strategy,a clonal seed orchard was started in a nursery in Pomaio(POM)in central Italy in 1993 for an assisted migration experiment of Abies nebrodensis(Lojac.)*** artifi cial ex situ populations were planted with this gene pool:a seedling arboretum in Pieve Santo Stefano(PSS)and a small dendrological collection in Papiano(PAP),both originating from the Sicilian relict ***,using AFLP markers,we estimated the relatedness among the relocated genotypes of the three collections to check whether the three collections had suffi cient genetic variability to be considered as additional sources of variability to the original gene pool for the assisted migration *** individual genetic variability was found in the collections;each plant had a diff erent genotype and was confi rmed to belong to its population of *** and PSS trees were shown to be only from the original population of *** species and were derived from a limited set of maternal fertile *** on the Sicilian fi r population inventory,nursery production in Sicily,and structure clustering analysis,close genetic relationships among POM,PAP and several PSS trees(1–35)were ***,the PSS group(36–78)was genetically close to tree 1 of POM and in a lesser proportion to plants 7 and 9 of *** sampling of seedlings used to form batches in the nursery might have infl uenced the structure of the resultant *** genotypes will be useful for enriching the original gene pool.

Recently,the Andean subparamo in Colombia has experienced severe wildfires,but little is known about the functional composition of recovering or not after a ***,we examined the functional community composition subparamo affected byfire in *** documented how functional traits changed 31 months after the disturbance and compared them with an unburned *** sampled from one to two years after the fire every four months,then registered all recruits in 1655m *** individuals were classified into strategy functional groups based on the traits of persistence and *** first group was stem type and regeneration mechanism(seedling and resprout),and the second was fruit type and dispersal *** investigated the degree to which functional diversity changes plant communities over time(woody and non-woody),and we compared it with an unburned *** most relevant results showed that resprouts and seed regenerated increased post-fire time and significant differences between sampling *** anemochory is the most relevant dispersal mode that indicates the community capacity to colonize the new gaps opened by the fire *** discuss how wildfire appears to be a triggering factor for persistence and dispersal strategy groups in subparamo burned given their characteristics of tolerance to *** this reason,a greater functional divergence between the ecosystems studied post-fire recovery has been related to higher levels of biodiversity at the landscape scale due to the high degree of endemism and significant differences in species composition between paramos.

Introduction of proton pump inhibitor(PPI)therapy into clinical practice has revolutionized treatment approach to acid-related *** its clinical success came a widespread use of PPI ***,several studies found that PPIs were oftentimes overprescribed in primary care and emergency setting,likely attributed to seemingly low side-effect profile and physicians having low threshold to initiate ***,now there is a growing concern over PPI side-effect profile among both patients and *** would like to bring more awareness to the currently available guidelines on PPI use,discuss clinical indications for PPIs and the evidence behind the reported *** hope that increased awareness of proper PPI use will make the initiation or continuation of therapy a well informed and an evidence-based decision between patient and *** also hope that discussing evidence behind the reported side-effect profile will help clarify the growing concerns over PPI therapy.

Highly accurate biosensors for few or single molecule detection play a central role in numerous key fields,such as healthcare and environmental *** the last decade,laser biosensors have been investigated as proofs of concept,and several technologies have been *** here propose a demonstration of polymeric whispering gallery microlasers as biosensors for detecting small amounts of proteins,down to 400 *** have the advantage of working in free space without any need for waveguiding for input excitation or output signal *** photonic microsensors can be easily patterned on microscope slides and operate in air and *** estimate the limit of detection up to 148 nm/RIU for three different protein *** addition,the sensing ability of passive spherical resonators in the presence of dielectric nanoparticles that mimic proteins is described by massive ab initio numerical simulations.
