Role of TNFSF15 in the intestinal inflammatory response

Gastrointestinal diseases, specifically Crohn's disease, ulcerative colitis, diverticular disease, and primary biliary cirrhosis are all characterized by complicated inflammation of the digestive tract. Their pathology is multifactorial, and risk factors encompass both genetic and environmental factors. Recent advances in the genetic component of inflammatory bowel diseases(IBDs) have revealed that the tumor necrosis factor superfamily member 15(TNFSF15) contains a number of risk alleles associated not only with IBD but also with other diseases such as diverticular disease and primary biliary cirrhosis. These risk alleles in TNFSF15 and the altered expression of its gene product can serve as the common ground between these disorders by explaining at least some of the underlying processes that lead to a dysregulated immune response and subsequent chronic inflammation. Here, we aim to outline how the TNFSF15 gene is involved in the proliferation and cell fate of different populations of T cells and subsequently in the control of both pro-and anti-inflammatory cytokines. Furthermore, we summarize what is currently known of TNFSF15 control region variants, how they are associated with each mentioned disease, and how these variants can explain the autoimmune pathology of said diseases through altered TNFSF15 expression.

Background:Surgery in Crohn’s disease(CD)may be associated with poor prognosis and clinical and surgical *** aim of this study was to describe and compare the post-operative management and outcomes of patients with CD who underwent first vs recurrent ***:Observational study that included adult CD patients from 26 Spanish hospitals who underwent ileocolonic resection with ileocolonic anastomosis between January 2007 and December *** were retrospectively collected from the medical ***:Data from 314 patients were analysed,of whom 262(83%)underwent first surgery and 52(17%)referred to previous CD *** characteristics were similar between the two groups except for a higher rate of stricturing behavior at diagnosis among re-operated patients(P=0.03).After surgery,a higher proportion of re-operated patients received prophylactic treatment with immunomodulators compared with patients with first surgery(P=0.04).In re-operated patients,time to clinical recurrence was not associated with the fact of receiving or not prophylaxis,whereas,in patients with first surgery,recurrence-free survival was greater when prophylaxis was received(P=0.03).Conclusions:After surgery,a higher proportion of patients with previous surgeries received prophylactic treatment with immunomodulators compared with patients with first *** prophylactic treatment was beneficial for preventing clinical recurrence in patients operated on for the first time,it did not significantly reduce the risk of further recurrence in patients with previous *** suggests that effective prophylactic therapies are still needed in this subset of patients.

Background:This study is aimed at describing the prevalence of and risk factors associated with early post-operative complications after Crohn’s disease-related intestinal ***:This was a retrospective analysis of data from the PRACTICROHN *** Crohn’s disease patients who underwent ileocolonic resection with ileocolonic anastomosis between January 2007 and December 2010 were *** complications evaluated included death,ileus,anastomotic leak,abscess,wound infection,catheter-related infection,digestive bleeding and other extra-abdominal infections that occurred in the 30 days after ***:A total of 364 patients(median age at surgery 38 years and 50%men)were *** for surgery was:stricturing disease(46.4%),penetrating disease(31.3%),penetrating and stricturing disease(14.0%)or resistance to medical treatment(5.8%).Early complications were recorded in 100(27.5%)patients,with wound infection,intra-abdominal abscess and anastomotic leakage being the most frequent *** hospitalization duration was 16 days for patients with complications vs.9 days without complications(P<0.001).Complications were more common among patients with penetrating disease(36/114,31.6%)and those refractory to treatment(9/21,42.9%)compared with stricturing disease(45/169,26.6%)or stricturingtpenetrating disease(6/51,11.8%)(P=0.040).The rate of complications was higher among patients with diagnosis made at the time of surgery(15/31,48.4%)compared with the rest(85/331,25.7%)(P=0.013).Medication received at the time of surgery did not affect the rate of ***:Almost a quarter of patients developed early complications after intestinal *** disease and urgent surgery were associated with an increased risk of complications.

Myasthenia gravis as a form of clinical presentation of thymic carcinoma

Thymic carcinomas are rare tumors of the thymus arising in the thymic epithelium. They represent less than 1% of thymic malignancies. They often present with an advanced disease and metastasize to regional lymph nodes and distant sites. They have a worse prognosis with a 5-year survival rate of 30%-50%, while thymomas are much less invasive and have a 5-year survival of approximately 78%. We report a rare form of clinical presentation of a thymic carcinoma in which the diagnosis of myasthenia gravis was the cornerstone of the diagnosis of cancer. Surgery is considered the salvage treatment when possible. Radiotherapy is a second choice of salvage treatment, when possible depending on its localization and relation to nearby structures such as vascular structures. Molecular target therapy is a more directed, more expensive but less toxic treatment. Further studies need to be carried out for its approval worldwide, outside clinical trials.

The slippery slope framework explains tax compliance along two main dimensions,trust in authorities and power of authorities,which influence taxpayers’compliance *** frequentist and Bayesian analyses,we investigated the framework’s assumptions on a sample of 2786 self-employed taxpayers from eleven post-communist and non-post-communist countries doing business in five economic *** using scenarios that experimentally manipulated trust and power,our results confirmed the framework’s assumptions regarding the attitudes of the self-employed taxpayers;trust and power fostered intended tax compliance and diminished tax evasion,trust boosted voluntary tax compliance,whereas power increased enforced tax ***,self-employed taxpayers from post-communist countries reported higher intended tax compliance and lower tax evasion than those from non-post-communist *** results offer tax authorities insights into how trust and power may contribute to obtaining and maintaining high tax compliance levels amid global economic challenges,downturns,and increasing tax compliance costs.

AIM:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic C virus,especially in patients with advanced fibrosis. METHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOC) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOC and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOC(52%)(P analysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOC and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,IL28 B CC and non-F4; the type of treatment(BOC or TVR) was not significant. Telaprevir and Boceprevir are the first direct acting antivirals approved for chronic hepatitis C in combination with peg-interferon alfa and *** due to myelotoxicity caused by these drugs may occur,but severe hematological abnormalities or aplastic anemia(AA) have not been *** collected all cases of severe pancytopenia observed during triple therapy with telaprevir in four Spanish centers since approval of the drug in *** 142 cirrhotic patients receiving treatment,7 cases of severe pancytopenia(5%) were identified and three were consistent with the diagnosis of *** age was 59 years,five patients had compensated cirrhosis and two patients had severe hepatitis C recurrence after liver *** pancytopenia was diagnosed a median of 10 wk after the initiation of *** patients had pre-treatment hematological abnormalities related to *** six patients,antiviral treatment was interrupted at the onset of hematological *** patients died due to septic complications and one patient due to acute alveolar *** remaining patients *** pancytopenia and especially AA,are not rare during triple therapy with telaprevir in patients with advanced liver *** monitoring is imperative in this setting to promptly detect serious hematological disorders and to prevent further complications.

土壤修复主要通过将受污染土壤进行挖掘和填埋,这种方法在进行大量位点修复时不太切合实际,因此要选择其它棕地(brownfield)修复方法。本研究介绍了评估受铜铬砷(CCA)污染土壤采用含铁(Fe)爆破砂(BS)或嵌氧颗粒(OSG)处理后的化学稳定过程。就工业尺度下混合改良物的可行性和在不同氧化还原条件下的稳定效率对这种稳定技术进行了评估。在野外自然条件下,采用1m^3测渗计调查了这种稳定性并在实验室(10L)对氧化还原条件的效应进行了评估。结果表明改良物的添加率最低时(0.1%和1%)化学计量中还有足够的铁,但是不管在野外还是实验室都是采用高添加率的改良物(8%和17%)效果较好。铁和混合物的颗粒大小会影响稳定效率。厌氧条件的发展(模拟自水饱和度)会提高砷(As^(3+))的份额,因此增强了砷的迁移能力。在有氧条件下,采用高浓度OSG提高了孔隙水中镍(Ni)和铜(Cu)的浓度,但是在厌氧条件下,与未作处理的土壤相比,砷的淋溶率降低了,而镍和铜的淋溶并不是关键的。所处理土壤的最终目的决定了改良方案,也就是说,对于在厌氧条件下填埋的土壤,OSG浓度为10%较合适。也可以在有氧条件下将1%的BS与土壤混合,置于填埋区上层或者棕色地块(brownfield site)原处。另外从长期来看,采用BS处理似乎比用OSG处理效果要好。
