morphological changes of cervical squamous premalignant lesions have been well defined in the past. These changes include the gradual progression from HPV - associated koilocytie atypia,, to low grade dysplasia, to high grade dysplasia, to carcinoma in situ, and eventually invasive carcinoma. This review summarizes the evidence about telemonitoring in patients with inflammatory bowel disease(IBD). To give an overview of the advances performed, as well as the enablers and barriers which favoured/hindered telemonitoring implementation. We performed a literature search in Pub med, EmBASE, medLINE, Cochrane Database, Web of Science and Conference Proceedings. Titles and abstracts published up to September 2022 were screened for a set of inclusion criteria: telemonitoring intervention, IBD as the main disease, and a primary study performed. Ninety-seven reports were selected for full review. Finally, 20 were included for data extraction and critical appraisal. most studies used telemonitoring combined with tele-education, and programs evolved from home telemanagement systems towards web portals through m Health applications. Web systems demonstrated patients’ acceptance, improvement in quality of life, disease activity and knowledge, with a good cost-effectiveness profile in the short-term. Initially, telemonitoring was almost restricted to ulcerative colitis, but new patient reported outcome measures, home-based tests and mobile devices favoured its expansion to different patients’ categories. However, technological and knowledge advances led to legal, ethical, economical and logistic issues. Standardization of remote healthcare is necessary, to improve the interoperability of systems as well as to address liability concerns and users’ preferences. Telemonitoring IBD is well accepted and improves clinical outcomes at a lower cost in the short-term. Funders, policymakers, providers, and patients need to align their interests to overcome the emerging barriers for its full implementation.

背景和目的：特发性肺纤维化（IPF）是一种进行性发展的、病死率很高的肺部疾病。研究结果显示，被数种酪氨酸激酶受体激活的信号通路参与了肺纤维化的过程，因此有人提出抑制这些受体有可能减慢IPF的进展。方法：在一项为期12个月的Ⅱ期临床试验中，我们在IPF患者中评价了4种不同口服剂量的酪氨酸激酶抑制剂BIBFll20与安慰剂相比的有效性和安全性。主要研究终点为用力肺活量（FVC）的年下降率，次要终点包括急性加重的次数、生活质量[用圣乔治呼吸问卷（SGRQ）测定]和肺总量等。结果：共432例IPF患者随机分组后，分别接受BIBFll20的4种剂量之一（50mg1次／d，50mg2次／d，100mg2次／d，或150mg2次／d）或安慰剂。在接受150mg2次／d组中，FVC每年下降0．06L，而安慰剂组每年下降0．19L（多重校正的封闭检验P=0．06，分层检验P=0．01）。与安慰剂相比，该剂量还使急性加重的发生率降低（每100人中分别为15．7次／年、2．4次／年，P=0．02），使SGRQ评分（根据0-100评分量表进行评价，分数越低表明生活质量越好）小幅下降，而安慰剂组SGRQ评分增加（分别为-0．66、5．46，P=0．007）。150mg2次／d组与安慰剂组比较，胃肠道症状（导致150mg20：／d组停药比安慰剂组多）和肝脏转氨酶升高更为常见。结论：与安慰剂相比，酪氨酸酶抑制剂B1BFll20（150mg，2次／d）可降低IPF患者肺功能的年下降率，减少急性加重的次数以及改善生活质量。

背景包括镇静评分和常规镇静暂停在内的镇静方案，有助干减少机械通气时间和重症监护病房（ICU）入住时间。因临床上对于镇静深度的评估工作量大、仅能间断进行且干扰镇静和睡眠，所以通过脑电生理信号评估镇静深度的替代方法已经开始倍受青睐：我们想明确听觉诱发相关电位（ERPs）、脑电双频指数（BIS）和墒（Entropy）是否也能够评估临床镇静深度。方法选择10例择期全麻下行胸部或腹部手术的患者。手术后进入ICU，逐渐减少丙泊酚和瑞芬太尼靶控镇静程度[采用Rickmond Agitation镇静评分（RASS），-5分极深镇静，-4分深度镇静，-3～-1分中度镇静，0分清醒]期间，记录脑电图、BIS、状态熵（SE）、反应墒（RE）和ERPs。手术前或手术后数天测量指标的相关基础水平。结果基线、PASS-5分、RASS-4分、RASS-3至-1分和PASS-0分，对应的BIS值分别是94[4]（中位数，四分位数间距）、47[15]、68[9]、75[10]和88[6]；对应的SE值分别是87[3]、46[10]、60[22]、74[21]和87[5]；对应的RE值分别是97[4]、48[9]、71[25]、81[8]和96[3]（P〈0．05，Friedman检验）。BIS和墒都有高变异度。当单独考虑ERP振幅100时，各个镇静水平的ERPs值差异无显著性：但是、复括双参数主要变量分析的ERP辨别分析显示区别深度镇静、中度镇静和清醒状态的推算概率PK大约是0．89。与PK对应的RE，SE和BIS值分别是0．88、0．89和0．85。结论ERPs、BIS和熵均不能替代基于标准评分系统的临床镇静评估。全麻后对于极深镇静、深度到中度镇静和清醒的辨别，ERPs和脑电图仅能提供相似的PK值。BIS和熵存在较高的个体间和个体自身变异度，使其难以确定预测镇静水平的目标值范围，因而在危重患者中的使用受到限制。ERPs的变异度还不清楚。

There is overwhelming evidence that hypertension is an important risk factor for both macrovascular and microvascular complications in patients with diabetes,but the problem remains to identify appropriate goals for preventive therapies.A number of guidelines (the European Society of Cardiology (ESC)/European Association for the Study of Diabetes (EASD) 2007,the Joint National Committee (JNC)-Ⅶ 2003,the American Diabetes Association (ADA) 2011) have for example advocated a blood pressure goal of less than 130/80 mmHg,but this suggestion has been challenged by findings in recent trials and meta-analyses (2011).The European Society of Hypertension (ESH) therefore recommends a systolic blood pressure goal of "well below" 140 *** on evidence from both randomized controlled trials (hypertension optimal treatment (HOT),action in diabetes and vascular disease:preterax and diamicron mR controlled evaluation (ADVANCE),action to control cardiovascular risk in diabetes (ACCORD)) and observational studies (ongoing telmisartan alone and in combination with ramipril global endpoint trial (ONTARGET),international verapamil-trandolapril study (INVEST),treat to new targets (TNT),and the National Diabetes Register (NDR)),it has been shown that the benefit for stroke reduction remains even at lower achieved blood pressure levels,but the risk of coronary events may be uninfluenced or even increased at lower systolic blood pressure *** a recent meta-analysis,it was therefore concluded that the new recommended goal should be 130-135 mmHg systolic blood pressure for most patients with type 2 *** risk factors should also be controlled with a more ambitious strategy applied in the younger patients with shorter diabetes duration,but a more cautious approach in the elderly and frail patients with a number of vascular or non-vascular *** patients from East Asia,such as China,the stroke risk is relatively higher than the risk of coronary *** must al

This study conducted clinical observation of the treatment of noninsulin dependent dia-betes mellitus (type ) with Semen Persical decoction for purgation with addition (SPDPA) in compari-son with a control group treated with glyburide. The results of the clinical observations show the total effective rate of SPDPA on 106 cases was 79%. which was equivalent to levels in the control group, butSPDPA was more effective in relieving symptoms. The animal experimental study showed SPDPA couldreduce tasting blood sugar in diabetic patients and rats. Its mechanism may be due to improved secretionof insulin, inhibited excretion by the endocrine cell, increased endocrine pellet inhibition ot insular cellsand improved composition of hepatic glycogen. In traditional Chinese medical theory, the mechanism oftherapeutic action of SPDPA in diabetes mellftus is based on synergistic regulation benefiting the vital en-ergy and nourishing Yin, the activation of blood circulation to dissipate blood stasis and the loosening ofthe bowel to relieve constipation.
