Factors influencing pre-hospital delay among pa.ients with acute myocardial infarction in iran

Background Acute myocardial infarction （a.i） is the lea.ing cause of morbidity and disa.ility among ira.ian popula.ion. Pre-hospital delay is an important cause of increa.ing early and also late morta.ity in a.i. Thus the aim of the present study was to identify the factors influencing pre-hospital delay among pa.ients with a.i in iran. Methods Between August 2010 and May 2011, a cross-sectional and single-center survey was conducted on 162 consecutive pa.ients with ST-eleva.ion myocardial infarction （STEMi） admitted to Cardiac Care Unit （CCU） of Dr. Heshmat Hospital, Rasht. All pa.ients were interviewed by the third author within 7 days after admission by using a four-part questionnaire including socio-demographic, clinical, situa.ional and cognitive factors. Data were analyzed by descriptive and Logistic regression model at P 〈 0.05 using SPSS 16. Results Mean age was （60.11±12.29） years in all pa.ients. Majority of pa.ients （65.4%） were male. The median of pre-hospital delay was 2 hours, with a mean delay of 7.4 hours （±16.25 hours）. Regression analysis showed that admission in weekend （P 〈0.04, 0R=1.033, 95% Cl=1.187-2.006） and misinterpreta.ion of symptoms as cardiac origin （P 〈0.002, OR=1.986, 95% Cl=1.254-3.155） and perceiving symptoms to not be so serious （P 〈0.003, OR=3.264, 95% Cl=1.492-7.142） were factors influencing pre-hospital delay 〉 2 hours. Conclusions Our findings highlight the importance of cognitive factors on decision-ma.ing process and pre-hospital delays. Health care providers can educate the public on a.i to enable them recognize the signs and symptoms of a.i correctly and rea.ize the benefits of early treatment.

Effects of Hydro-alcoholic Extract of Launaea acanthodes on Serum Gonadotropin and Testosterone Levels and the Structure of Seminiferous Tubules in Hyperglycemic Rats

Objective： To investigate the effects of hydro-alcoholic extract of Launaea acanthodes, a blood glucose lowering plant in folk medicine of iran, on the structure of seminiferous tubules and serum gonadotropin and testosterone levels in hyperglycemic rats. Methods： Twenty-four Wistar rats were randomly allocated into 4 groups （n=6）： control, streptozotocin （STZ）, STZ ＋ insulin [STZ ＋ ins, 5 iU/（kg·day）], and STZ ＋ Launaea acanthodes extract [STZ ＋ Ext, 150 mg/（kg·day）]. Blood samples were collected at the 2nd and 4th weeks for detection of testosterone, follicle stimula.ing hormone （FSH） and luteinizing hormone （LH） with enzyme-linked immuno sorbent assay （ELiSA）, and the dght testes of rats were removed at the 7th week for the evalua.ion of diameter and wall thickness of seminiferous tubules and number of Leydig cells using unbiased stereological techniques. Results： in compa.ison with the control group, at the 2nd week FSH （0.45 vs 0.03, 0.02, 0.02 lUlL in STZ, STZ ＋ ins and STZ ＋ Ext groups, respectively） and LH （1.02 vs 0.37, 0.2, 0.29 iU/L） showed significant decreases （all P〈0.05） and testosterone （4.2 vs 8.37, 7.78, 11.8 ng/mL） showed a remarkable increase （all P〈0.05）. The levels of these hormones became closer in the STZ ＋ Ext and the STZ ＋ ins groups to the control at the 4th week. A significant decrease in diameter and wall thickness of seminiferous tubules and number of Leydig cells were observed in the STZ group as compared with the control （P〈0.01）. Conclusions： Administra.ion of Launaea extract demonstrated a beneficial impact on the protection of testis from pathogenic and degenera.ive effects of hyperglycemia which may be partly due to its potential antioxida.ive effects.

in this study,the effect of hot deforma.ion on martensitic stainless steel was carried out in temperatures between 950 to 1100℃ and strain rates of 0.001,0.01 and 0.1 s *** important dyna.ic recrystalliza.ion parameters,the critical strain and the point of ma.imum dyna.ic softening,were derived from strain hardening rate vs stress *** the calculated parameters were used to predict the dyna.ic recrystallized *** results show that critical stress and strain increase with decrea.ing deforma.ion temperature and increa.ing strain *** hot deforma.ion activa.ion energy of the steel is also investigated in the present work with 413 kJ/*** experimental flow curves are in fair agreement with the kinetics of dyna.ic recrystalliza.ion model.

A low-voltage and energy-efficient full adder cell based on carbon nanotube technology

Sca.ing problems and limita.ions of conventional silicon transistors have led the designers to exploit novel nano-technologies. One of the most promising and fea.ible nano-technologies is CNT(Carbon Nanotube) based transistors. in this paper, a high-speed and energy-efficient CNFET(Carbon Nanotube Field Effect Transistor) based Full Adder cell is proposed for nanotechnology. This design is simulated in va.ious supply voltages, frequencies and load capa.itors using HSPiCE circuit simulator. Significant improvement is achieved in terms of speed and PDP(Power-Delay-Product) in compa.ison with other classical and state-of-the-art CMOS and CNFET-based designs, existing in the literature. The proposed Full Adder can also drive large load capa.itance and works properly in low supply voltages.

Microstructural evolution of a superaustenitic stainless steel during a two-step deforma.ion process

Single-and two-step hot compression experiments were carried out on 16Cr25Ni6Mo superaustenitic stainless steel in the temperature range from 950 to 1150°C and at a strain rate of 0.1 s^（-1）. in the two-step tests, the first pass was interrupted at a strain of 0.2; after an interpass time of 5, 20, 40, 60, or 80 s, the test was resumed. The progress of dyna.ic recrystalliza.ion at the interruption strain was less than 10%. The sta.ic softening in the interpass period increased with increa.ing deforma.ion temperature and increa.ing interpass time. The sta.ic recrystalliza.ion was found to be responsible for fast sta.ic softening in the temperature range from 950 to 1050°C. However, the gentle sta.ic softening at 1100 and 1150°C was attributed to the combina.ion of sta.ic and metadyna.ic recrystalliza.ions. The correla.ion between calculated fractional softening and microstructural observa.ions showed that approximately 30% of interpass softening could be attributed to the sta.ic recovery. The microstructural observa.ions illustrated the forma.ion of fine recrystallized grains at the grain bounda.ies at longer interpass time. The Avra.i kinetics equa.ion was used to establish a rela.ionship between the fractional softening and the interpass period. The activa.ion energy for sta.ic softening was determined as 276 kJ/mol.

A Compara.ive Study of the Electronic and Magnetic Properties of Gd_5Ge_4 and Gd_5Si_4 Compounds

We present a compara.ive study of electronic structure and magnetic properties of Gd5Si4 and Gd5Ge4compounds using first principles full potential linea.ized augmented plane wave(FP-LAPW) method based on density functional theory(DFT) using the WiEN2 k code. The local-spin density approxima.ion with correla.ion energy(LSDA+U) method has been used as the exchange-correla.ion potential. The optimized lattice constants are in good agreement with the experimental data. The total and partial density of states(DOS) of Gd5Si4 and Gd5Ge4show the difference in Si 3p-Gd 5d and Ge 4p-Gd 5d hybridiza.ion, which have an effective role in indirect exchange *** addition, the magnetic moments of Gd, Si, and Ge atoms and the compounds are calculated to cla.ify the differences in the magnetic properties of these compounds.

利用化学浴沉积（CBD）结合热解工艺制备了WO3修饰TiO2纳米管（WTN）阵列,且WTN阵列分布均匀。通过改变沉积时间可调整WTN的钨含量。利用SEM、EDX、XRD对WTN进行了表征。结果表明,阴极氧化后,TiO2纳米管生长于钛体基体上,纳米管的内径为90120 nm,外径为120160 nm,壁厚在3060 nm范围,长约39μm。经C.i.硫化红14的有效去除证实,修饰TiO2纳米管的WO3增强了WTN光催化活性,在可见光照射下,所制备复合膜光催化活性优于未修饰纳米管阵列（TN）。

Effect of a.ing Time and Temperature on Mecha.ical Properties and Microstructural Evolution of 2205 Ferritic-Austenitic Stainless Steel

Duplex stainless steels （DSS） with ferritic-austenitic microstructures offer good combina.ion of resistance to pitting corrosion and high strength that are not concomitantly attainable using conventional single phase austenitic or ferritic stainless steels, The DSS used in this investiga.ion was 2205 alloy ha.ing a stable microstructure consisting of about 45% ferrite and 55% austenite at ambient temperature. in order to investigate a.ing beha.ior of this steel and the influences on mecha.ical properties, different a.ing treatments were conducted at temperatures of 350-950℃ for va.ious a.ing time of 15, 30, 60 and 180 min. The aged specimens were subjected to impact testing and hardness measurements. Finally, the changes in microstructure due to a.ing were studied by optical and scanning electron microscopy. The results showed that a.ing at temperatures lower than 550℃ for different time had negligible effects on mecha.ical properties. Besides, no considerable changes in term of precipita.ion of harmful intermetallic particles were observed in microstructure below this temperature. However, a critical temperature range, 550-650 ℃, was introduced here. a.ing in this range led to a significant decrease in toughness and notable increase in hardness. The forma.ion of intermetallic phases such as CT was recognized as the major reason for the observed changes.

Compa.ison of seda.ive effectiveness of thiopental versus midazolam in reduction of shoulder disloca.ion

BACKGROUND: Va.ious seda.ive drugs have been proposed to control anxiety and a.ita.ion in shoulder disloca.ion, but none of them has been diagnosed as the best seda.ive and relaxant agent. The study aimed to compare the seda.ive effectiveness of thiopental versus midazolam in reduction of shoulder ***: A randomized double-blind controlled trail was performed in 80 pa.ients with shoulder disloca.ion recruited from the emergency department. Ten pa.ients were excluded and 70 pa.ients were enrolled in the study. Case group received intravenous thiopental 2 mg/kg+2 μg/kg fentanyl while control group received intravenous midazolam 0.1 mg/kg+2 μg/kg fentanyl. Number of times, pa.ients and physician's sa.isfaction, difficulty of procedure, degree of muscle relaxa.ion, time of seda.ion and complete recovery, number of pa.ients with apnea episode, O2 satura.ion, pa.ient's pain score and adverse events were all ***: Muscular tone had significant difference between the two groups(P-value=0.014) and thiopental was more muscle relaxant than midazolam. Replacement of shoulder disloca.ion in thiopental group was ea.ier than midazolam group(P-value=0.043). There was no need to use multiple methods of reduction in either group. Before drug infusion the mean±SD VAS scores were 8.37±2.21 in the midazolam group(A) and 8.94±1.78 in the thiopental group(B); mean difference 0.57, 95% Ci= –0.38 to 1.52. After completion of the procedure, the mean±SD VAS scores in group(A) and(B) were 3.20±1.30 vs. 3.65±1.30; mean difference –0.45, 95% Ci= –1.07 to ***: Thiopental might be more effective and relaxant than midazolam for reduction of shoulder disloca.ion.
