Secondary endoscopic submucosal dissection for locally recurrent or incompletely resected gastric neoplasms

AIM To investigate the feasibility and safety of secondary endoscopic submucosal dissection(ESD) for residual or locally recurrent gastric tumors. METHODS Between 2010 and 2017, 1623 consecutive patients underwent ESD for gastric neoplasms at a single tertiary referral center. Among these, 28 patients underwent secondary ESD for a residual or locally recurrent tumor. Our analysis compared clinicopathologic factors between primary ESD and secondary ESD groups. RESULTS The en bloc resection and curative rate of resection of secondary ESD were 92.9% and 89.3%, respectively. The average procedure time of secondary ESD was significantly longer than primary ESD(78.2 min vs 55.1 min, P = 0.004), and the adverse events rate was not significantly different but trended slightly higher in the secondary ESD group compared to the primary ESD group(10.7% vs 3.8%, P = 0.095). Patients who received secondary ESD had favorable outcomes without severe adverse events. During a mean follow-up period, no local recurrence occurred in patients who received secondary ESD. CONCLUSION Secondary ESD of residual or locally recurrent gastric tumors appears to be a feasible and curative treatment though it requires greater technical efficiency and longer procedure time.

AIM:To evaluate whether weekend or nighttime admission affects prognosis of peptic ulcer bleeding despite early ***:Retrospective data collection from four referral centers,all of which had a formal out-of-hours emergency endoscopy service,even at weekends.A total of 388 patients with bleeding peptic ulcers who were admitted via the emergency room between January 2007 and December 2009 were *** parameters included time from patients' arrival until endoscopy,mortality,rebleeding,need for surgery and length of hospital ***:The weekday and weekend admission groups comprised 326 and 62 patients,*** were no significant differences in baseline characteristics between the two groups,except for younger age in the weekend *** patients (97%) had undergone early endoscopy,which resulted in a low mortality rate regardless of point of presentation (1.8% overall vs 1.6% on the weekend).The only outcome that was worse in the weekend group was a higher rate of rebleeding (12% vs 21%,P = 0.030).However,multivariate analysis revealed nighttime admission and a high Rockall score (≥ 6) as significant independent risk factors for rebleeding,rather than weekend ***:Early endoscopy for peptic ulcer bleeding can prevent the weekend effect,and nighttime admission was identified as a novel risk factor for rebleeding,namely the nighttime effect.

AIM To investigate post endoscopic submucosal dissection electrocoagulation syndrome(PEECS) of the *** We analyzed 55 consecutive cases with esophageal endoscopic submucosal dissection for superficial esophageal squamous neoplasms at a tertiary referral hospital in South Korea. Esophageal PEECS was defined as "mild" meeting one of the following criteria without any obvious perforation: fever(≥ 37.8 ℃), leukocytosis(> 10800 cells/μl), or regional chest pain more than 5/10 points as rated on a numeric pain intensity scale. The grade of PEECS was determined as "severe" when meet two or more of above *** We included 51 cases without obvious complications in the analysis. The incidence of mild and severe esophageal PEECS was 47.1% and 17.6%, respectively. Risk factor analysis revealed that resected area, procedure time, and muscle layer exposure were significantly associated with PEECS. In multivariate analysis, a resected area larger than 6.0 cm^2(OR = 4.995, 95%CI: 1.110-22.489, P = 0.036) and muscle layer exposure(OR = 5.661, 95%CI: 1.422-22.534, P = 0.014) were independent predictors of esophageal PEECS. AIM： To investigate endoscopic and clinicopathologic characteristics of early gastric cancer （EGC） according to microsatellite instability phenotype. METHODS： Data were retrospectively collected from a single tertiary referral center. Of 981 EGC patients surgically treated between December 2003 and October 2007, 73 consecutive EGC patients with two or more microsatellite instability （MSI） mutation [high MSI （MSI-H）] and 146 consecutive EGC patients with one or no MSI mutation （non-MSI-H） were selected. The endoscopic and clinicopathologic features were compared between the MSI-H and non-MSI-H EGC ***： In terms of endoscopic characteristics, MSI-H EGCs more frequently presented with elevated pattern （OR 4.38, 95% Cl： 2.40-8.01, P 〈 0.001）, moderate- to-severe atrophy in the surrounding mucosa （OR 1.91, 95% CI： 1.05-3.47, P = 0.033）, antral location （OR 3.99, 95% CI： 2.12-7.52, P 〈 0.001） and synchronous le- sions, compared to non-MSI-H EGCs （OR 2.65, 95% CI： 1.16-6.07, P = 0.021）. Other significant clinicopatholog- ic characteristics of MSI-H EGC included predominance of female sex （OR 2.77, 95% CI： 1.53-4.99, P 〈 0.001）, older age （〉 70 years）（OR 3.30, 95% CI： 1.57-6.92, P = 0.002）, better histologic differentiation （OR 2.35, 95% Cl： 1.27-4.34, P = 0.007）, intestinal type by Lau- ren classification （OR 2.34, 95% CI： 1.15-4.76, P = 0.019）, absence of a signet ring cell component （OR 2.44, 95% CI： 1.02-5.86, P = 0.046）, presence of mu- cinous component （OR 5.06, 95% Cl： 1.27-20.17, P = 0.022）, moderate-to-severe lymphoid stromal reaction （OR 3.95, 95% CI： 1.59-9.80, P = 0.003）, and co-exist- ing underlying adenoma （OR 2.66, 95% CI： 1.43-4.95, P = 0.002）. CONCLUSION： MSI-H EGC is associated with unique endoscopic and clinicopathologic characteristics includ- ing frequent presentation in protruded type, co-exist- ing underlying adenoma, and synchronous lesions.

With the rise of remote collaboration,the demand for advanced storage and collaboration tools has rapidly ***,traditional collaboration tools primarily rely on access control,leaving data stored on cloud servers vulnerable due to insufficient *** paper introduces a novel mechanism that encrypts data in‘bundle’units,designed to meet the dual requirements of efficiency and security for frequently updated collaborative *** bundle includes updated information,allowing only the updated portions to be reencrypted when changes *** encryption method proposed in this paper addresses the inefficiencies of traditional encryption modes,such as Cipher Block Chaining(CBC)and Counter(CTR),which require decrypting and re-encrypting the entire dataset whenever updates *** proposed method leverages update-specific information embedded within data bundles and metadata that maps the relationship between these bundles and the plaintext *** utilizing this information,the method accurately identifies the modified portions and applies algorithms to selectively re-encrypt only those *** approach significantly enhances the efficiency of data updates while maintaining high performance,particularly in large-scale data *** validate this approach,we conducted experiments measuring execution time as both the size of the modified data and the total dataset size *** show that the proposed method significantly outperforms CBC and CTR modes in execution speed,with greater performance gains as data size ***,our security evaluation confirms that this method provides robust protection against both passive and active attacks.

The Climate Variabilities of Air Temperature Around the Korean Peninsula

In this study, changes in climatological conditions around the KoreanPeninsula are estimated quantitatively using various types of high order statistical analyses. Thetemperature data collected from Incheon station have been analyzed for the assessment of the climatevariation. According to our analysis, the climate changes observed over the Korean Peninsula forthe last century are similar to the global observational data in many respects. First of all, thewarming trend [+1.5℃ (100 yr)^(-1)] and the overall evolving pattern throughout the century axequite similar to each other. The temperature change in the Korean Peninsula is about two to threetimes larger than that of the global scale which may partially be ascribed to the influence ofurbanization at mid and high latitudes. In this work, a new Winter Monsoon Index (WMI) is suggestedbased on the European continental scale circulation index (EU1) pattern. Our WMI is defined as thenormalized sea level pressure (SLP) difference in the winter period between the centers of the EastSea and west of Lake Baikal in Siberia, the two eastern centers of the EU1 action patterns. A strongsimilarity is found between the time series of the WMI and surface air temperature at Incheon. TheWMI has decreased gradually since the 1920s but has shifted to a rapid increasing trend in the lasttwo decades; it was in fact accompanied by a weakening of the Siberian High and a decreasing of thenortherly during winter. Our findings of the close correlations between the surface air temperatureat Incheon and the WMI strongly indicate that our newly suggested index is unique and can be used asan efficient tool to predict climate variability in Korea.

In storage service, data de-duplication is a specialized technique for eliminating duplicate copies of repeating data in storage. Especially, client-side de-duplication has more merits than server-side deduplication since they can improve both the space efficiency and the communication bandwidth. For secure client-side de-duplication, we need a way to prove the ownership of a file to be stored. In the upload step,the server should verify the ownership of a client to give the right of the file without uploading it. On the contrary, the client also want to verify the retrievability for the file since he will delete it from his storage after protocol execution. Existing proof of ownership techniques have been designed for server's need. In this paper, we first point out that we need the second property in client's view point, and give a very simple and practical solution which can support the server and the client to prove that they have the same file. We first describe a generic strategy which can help us to construction a bi-directional and concurrent proof of ownership technique from an ordinary proof of ownership technique, and then give an efficient hash-based scheme with security proof in the random oracle model.

Validation of Sea Level Data in the East Asian Marginal Seas: Comparison between TOPEX/POSEIDON Altimeter and In-Situ Tide Gauges

In an effort to assess the reliability of satellite altimeter systems, the authors conduct a comparative analysis of sea level data that were collected from the TOPEX/POSEIDON (T/P) altimeter and 10 tide gauges (TG) near the satellite passing ground tracks. The analysis is made using datasets collected from marginal sea regions surrounding the Korean Peninsula at T/P cycles of 2 to 230, which correspond to October 1992 to December 1998. Proper treatment of tidal errors is a very critical step in data processing because the study area has very strong tide. When the T/P data are processed, the procedures of Park and Gamberoni (1995) are adapted to reduce errors associated with the tide. When the T/P data are processed in this way, the alias periods of M2, S2, and K1 constituents are found to be 62.1, 58.7, and 173 days repectively. The compatibility of the T/P and TG datasets are examined at various filtering periods. The results indicate that the low-frequency signals of the T/P data can be interpreted more safely with longer filtering periods (such as up to the maximum selected value of 200 days). When RMS errors for the 200-day low-pass filter period are compared with all 10 tidal stations, the values span the range of 2.8 to 6.7 cm. The results of a correlation analysis for this filtering period also show a strong agreement between the T/P and TG datasets across all stations investigated (e.g.,p- values consistently less than 0.001). Hence according to the analysis, the conclusion is made that the analysis of surface sea level using satellite altimeter data can be made safely with reasonably extended filtering periods such as 200 days.
