Endovascular treatment of wide-necked intracranial aneurysms using of ＂remodeling technique＂ with the HyperForm balloon

Background Aneurysms with wide-necked or a large neck/fundus ratio, especially located on an arterial bifurcation or a small artery, are challenges for interventional neuroradiologist because of the risk of coil migration or coil protrusion into the parent vessels. Our study was designed to improve the efficacy and safety of the ＂remodeling technique＂ with the HyperForm balloon for these difficult aneurysms and was confirmed by a follow-up result. Methods From June 2004 to September 2006, forty-two patients （20 men, 22 women） with wide-necked or large neck/fundus ratio aneurysms were treated by using the ＂remodeling technique＂ with the HyperForm balloon. Results Forty wide-necked aneurysms were successfully treated with the HyperForm balloon remodeling technique with only two failed cases. Final results consisted of total occlusion in 34 cases （80.9%）, subtotal in 4 （9.5%） and incomplete in 2 （4.8%）. One aneurysmal rupture occurred, but no clinical consequence was shown. No thromboembolic events were observed during treatment. Final angiographic follow-up time ranged from 3 to 18 months. Conclusions The ＂remodeling technique＂ with the HyperForm balloon is a very useful tool in the treatment of wide-necked or unfavorable neck/fundus ratio intracranial aneurysms - located on an arterial bifurcation or a small artery and, especially, located on the bifurcation of a large artery and a small one. In our experience, this technique provided a safe and efficient treatment for difficult aneurvsms when the standard remodelina techniaue miaht have failed.

Embolization of brain arteriovenous malformations with ethylene vinyl alcohol copolymer： technical aspects

Background Endovascular therapy plays an important role in the treatment of brain arteriovenous malformations （BAVMs）. Ethylene vinyl alcohol copolymer （Onyx） is a novel liquid embolic material. This study aimed to summarize our experience of using Onyx for embolization of BAVMs with the focus on embolization technique. Methods From September 2003 to November 2007, 115 patients （43 women and 72 men, with a mean age of 29 years） with BAVMs were endovascularly treated with Onyx in our department. The following features of all AVMs were evaluated prior to treatment： type of nidus and shunt, draining veins, and feeding arteries. A total of 196 endovascular procedures were performed. Results The course of endovascular treatment was completed in 88 patients. Additional sessions were planned in 27 patients. Of the 88 patients, total occlusion was obtained in 23 patients （26.1%）, near-total （〉80% of the original volume） occlusion was obtained in 35 patients （39.8%） and partial occlusion （〈80% of the original volume） was obtained in 30 patients （34.1%） using embolization as the sole therapeutic technique. Mean volume reduction was 72% （range 30%-100%） in 115 patients. Thirty four patients （38.6%, 34/88） underwent radiosurgical treatment. Additional embolization sessions were planned in 27 patients. Complications occurred in 19 patients （16.5%, 19/115）, leading to death in one patient （mortality 0.9%） and permanent disabling in 3 patients （morbidity 2.6%）. Conclusions Onyx was shown to be feasible and safe for embolization of BAVMs. Proper use of the Onyx injection technique largely improved the endovascular treatment of BAVMs. Large AVMs can be adequately reduced in size through the use of additional treatment.

Noninvasive assessment of liver fibrosis with combined serum aminotransferase/platelet ratio index and hyaluronic acid in patients with chronic hepatitis B

AIM： To construct a noninvasive assessment model consisting of routine laboratory data to predict significant fibrosis and cirrhosis in patients with chronic hepatitis B （CHB）. METHODS： A total of 137 consecutive patients with CriB who underwent percutaneous liver biopsy were retrospectively analyzed. These patients were divided into two groups according to their aminotransferase （ALT） level. The sensitivity, specificity, positive predictive value （PPV）, negative predictive value （NPV）, the likelihood ratio （LR） of aminotransferase/platelet ratio index （APRI） ≥ 1.5 or 〈 1.5 in combination with different hyaluronic acid （HA） cut-off points were calculated for the presence of moderate to severe fibrosis/cirrhosis （fibrosis stages 2 and 4） and no to mild fibrosis/cirrhosis （fibrosis stages 0 and 1）. RESULTS： The APRI correlated with fibrosis stage in CriB patients. The APRI ≥1.5 in combination with a cut-off HA cut-off point 〉 300 ng/mL could detect moderate to severe fibrosis （stages 2-4） in Crib patients. The PPV was 93.7%, the specificity was 98.9%. The APRI 〈 1.5 in combination with different HA cut-off points could not detect no to mild fibrosis in CHB patients. CONCLUSION： The APRI ≥ 1.5 in combination with a HA cut-off point 〉 300 ng/mL can detect moderate to severe fibrosis （stages 2-4） in Crib patients.

The coordination of enrgy transition,fixed cost recovery,and sufficient generation supply leads to a new challenge for a traditional capacity market ***,in order to better match network expansion at the same time,it is crucial to redesign the capacity market mechanism considering system *** this paper,a novel capacity market mechanism is proposed considering spot market operations,network expansion,and energy transition,which can minimize the total cost of capacity investment,network expansion,and generation operations,while satisfying the energy transition constraints and topology ***,the capacity market mechanism co-ordinated with spot market operations is illustrated,in which the energy transition and network constraints are ***,a bi-level optimization model is established where the trade organizers minimize the total cost of both investment and operations,subject to the spot power market simultaneously minimizing the local dispatching *** numerical results of a test system show that more economical capacity portfolios can be obtained by constructing reasonable transmission lines,thereby obtaining a more optimal market cost.A detailed multi-scenario simulation is further analyzed to verify the effectiveness of the proposed market mechanism.

1960—2007年山东省克山病病情变化及发病特点的动态观察

目的观察1960—2007年山东省克山病病情及发病特点的动态变化，为克山病防治及科研提供依据。方法采用资料回顾方法，收集山东省各市（地）、县（市、区）、乡（镇）、村多层次防治网报告的每年克山病发病及死亡情况，收集1962、1969、1... 详细信息

目的观察1960—2007年山东省克山病病情及发病特点的动态变化，为克山病防治及科研提供依据。方法采用资料回顾方法，收集山东省各市（地）、县（市、区）、乡（镇）、村多层次防治网报告的每年克山病发病及死亡情况，收集1962、1969、1973、1982年4个年度进行的5次全省或大范围克山病流行病学调查及各病区县（市、区）流行病学调查结果，分析克山病发病特点。结果1960—2007年48年间全省共发现克山病患者4228例，年最高发病率为2．636／万（1587599368）。亚急型克山病当年病死率为88．0l％（492／559），慢型克山病5年病死率为67．91％（491／723）。全省发病经历了高发年、低发年、基本控制年3个阶段。1960—1979年为高发年，发现病人3969例，年最高发病率为2．636／万（158／599368）；1980—1989年为低发年，发现病人200例，年最高发病率为0．058／万（57／9908013）；1990—2007年为基本控制年，发现病人59例，年最高发病率为O．016／万（16／9720832）。克山病发病的基本变化特点是发病类型由急型、亚急型为主逐渐转变为慢型、潜在型为主；发病年龄由4—10岁为主逐渐转变为13～25岁及20岁以上为主；发病季节由3、4月份为高峰期逐渐转变为全年散在发生。结论山东省48年来克山病发病经历了3个阶段，大流行过后，发病呈逐年下降趋势，发病特点随病情的变化而演变。

Dual-ion carrier storage through Mg^(2+) addition for high-energy and long-life zinc-ion hybrid capacitor

Cation additives can efficiently enhance the total electrochemical capabilities of zinc-ion hybrid capacitors (ZHCs).However their energy storage mechanisms in zinc-based systems are still under ***,we modulate the electrolyte and achieve dual-ion storage by adding magnesium *** we assemble several Zn//activated carbon devices with different electrolyte concentrations and investigate their electrochemical reaction dynamic *** zinc-ion capacitor with Mg^(2+)mixed solution delivers 82 mAh·g^(-1)capacity at 1 A·g^(-1) and maintains 91%of the original capacitance after 10000 *** is superior to the other assembled zinc-ion devices in single-component *** finding demonstrates that the double-ion storage mechanism enables the superior rate performance and long cycle lifetime of ZHCs.

As an essential concept to understand the world,whether the real values(or physical realities)of observables are suitable to physical systems beyond the classic has been debated for many *** standard nogo results based on Bell inequalities have ruled out the joint reality of incompatible quantum observables,the possibility of giving simple yet strong arguments to rule out joint reality in any physical system(not necessarily quantum)with weaker assumptions and less observables has been explored and proposed ***,we perform a device-independent experiment on a two-qubit superconducting system to show that the joint reality of two observables is incompatible with locality under the weaker assumption of the reality of observables in a single space-time region(or single qubit).Our results clearly show the violation of certain inequalities derived from both linear and nonlinear *** addition,we study the robustness of the linear and nonlinear criterion against the effect of systematic *** demonstration opens up the possibility of delineating classical and non-classical boundaries with simpler nontrivial quantum system.

Pre-combustion mercury removal with co-production of hydrogen via coal electrolysis

Pre-combustion mercury removal via coal electrolysis was performed and investigated on a bench-scale coal electrolytic cell(CEC)systemically,and factorial design was used to determine the effect of different operating conditions(coal particle size,operating temperature,operating cell voltage,and flow rate of slurry)on the percentage of mercury removal,percentage of ash removal,and dry heating value *** results showed that the operating cell voltage,as well as the interaction between operating cell voltage and coal particle size,are significant factors in the percentage of mercury *** is no significant factor in the percentage of ash removal and the dry heating value change,but the coal could be purified while keeping the dry heating value almost constant after electrolysis.A co-product of hydrogen could be produced during coal electrolysis with 50%lower energy consumption compared with water ***,a mechanism for mercury removal in coal was *** facts indicate that coal electrolysis is a promising method for precombustion mercury removal.
