Investigation and Improvement of the Staggered Labyrinth Seal

Recent studies on staggered labyrinth seals have focused on the effects of different parameters,such as the pressure ratio and rotational speed on the leakage flow ***,few investigations pay sufficient attention to flow details and the sealing mechanism,which would be of practical importance in designing seals having higher *** paper establishes a theoretical model to study the seal mechanism,thus revealing that leakage is determined by the pressure ratio and geometric *** simulation is implemented to illustrate details of the flow field within the seal *** dissipation is used to quantitatively investigate the contribution that each location makes to the seal performance,revealing that orifices and stagnation points are the most important positions in the seal structure,generating the most *** orifice is carefully studied by using the theoretical *** for different pressure ratios are conducted and the results match well with those of the theoretical model and numerical simulation,verifying the theoretical model and analysis of the seal *** new designs,based on a good understanding of the seal mechanism,are presented,with one reducing leakage by 24.5%.

Development of a Method for Assessing Country-Based Flood Risk at the Global Scale

International frameworks such as the Sustainable Development Goals and the Sendai Framework for Disaster Risk Reduction 2015-2030 require the quantification of country-based flood ***,few approaches at the global scale include the three necessary components(hazard,exposure,and vulnerability)for determining disaster risk and are country-based assessments,owing to major challenges such as limited data availability and vulnerability proxy ***,in this study,a method was developed with the following features:Incorporating the hazard,exposure,and vulnerability components;Applicable to the vast majority of countries in the world;Visualizing priority countries and illustrating effective measures and strategies;Clear and easy to understand by leaders and decision makers of international organizations,governments,and other stakeholders;Identifying each country’s challenges and providing guidance on specific issues for more detailed investigation and policy creation;Including more extensive factors compared with past *** Asia and the Pacific,the Flood Risk Index computed by the developed method is compared with the fatality ratio,and the results show that improving flood resilience secures people and society regardless of the magnitude and frequency of *** at the global scale visualizes regional tendencies and indicates that countries closer to the equator have higher flood *** of country-based flood risk based on five indicators demonstrates that the developed method can assist international organizations,governments,and other stakeholders to further examine country-specific conditions and establish and implement policies and strategies toward building a resilient society and achieving international targets.

Active surveillance as a practical strategy to differentiate lethal and non-lethal prostate cancer subtypes

Differentiation between lethal and non-lethal prostate cancer subtypes has become a very important issue in avoiding excessive treatment in an era when prostate-specific antigen （PSA） screening has reduced the rate of prostate cancer deaths by more than 20%. However, it is difficult to determine the patients who may or may not benefit from immediate treatment interventions at the time of the initial diagnosis. The selection of candidate patients who can postpone immediate treatment and undergo follow-ups with a specific surveillance program, or ＇active surveillance,＇ is a practical way to minimize overtreatment. In this review, the benefits and risks of active surveillance are discussed. Future perspectives, including imaging and new biomarkers for improving the outcomes of active surveillance programs, are also discussed.

Gastric xanthoma is a predictive marker for metachronous and synchronous gastric cancer

To investigate predictive markers for metachronous and synchronous gastric cancer (GC), which can develop after endoscopic submucosal dissection (ESD). METHODSA total of 352 patients underwent ESD for early GC at NTT West Osaka Hospital between June 2006 and February 2016. Exclusion criteria were as follows: Remnant stomach, unknown Helicobacter pylori status, and endoscopic observation of the whole stomach outside our hospital. We analyzed data from 192 patients comprising 109 patients with solitary GC (Group A) and 83 with metachronous and synchronous GC (Group B). We retrospectively investigated the clinicopathological and endoscopic characteristics, and endoscopic risk score as predictive markers for GC. RESULTSThe median age of Group B [72 years (interquartile range 63-78)] was significantly higher than that of Group A [66 years (interquartile range 61-74), respectively, P = 0.0009]. The prevalence of intestinal metaplasia in Group B tended to be higher than that in Group A (57.8% vs 45.0%, P = 0.08). The prevalence of gastric xanthoma (GX) in Group B was significantly higher than that in Group A (54.2% vs 32.1%, P = 0.003). The atrophy score in Group B was significantly higher than that in Group A (P = 0.005). Multivariate analysis revealed that higher age and the presence of GX were independently related to metachronous and synchronous GC [OR = 1.04 (1.01-1.08), P = 0.02; and OR = 2.11 (1.14-3.99), P = 0.02, respectively]. CONCLUSIONThe presence of GX is a useful predictive marker for metachronous and synchronous GC.

Making continental crust:The sanukitoid connection

The average continental crust possesses intermediate compositions that typify arc magmatism and as a result it is believed to have been created at ancient convergent plate boundaries. One possible mechanism for intermediate continental crust formation is the direct production of andesitic melts in the upper mantle. Sanukitoids, which characterize the Setouchi volcanic belt, SW Japan, include un-usually high-Mg andesites (HMA). They were generated by slab melting and subsequent melt-mantle interactions under unusual tectonic settings such as where warm lithosphere subducts into hot upper mantle. Such conditions would have existed in the Archean. Hydrous HMA magmas are likely to have solidified within the crust to form HMA plutons, which were then remelted to produce differentiated sanukitoids. At present, generation and differentiation of HMA magmas may be taking place in the Izu-Bonin-Mariana arc-trench system (IBM), because (1) HMA magmatism characterizes the initial stages of the IBM evolution and (2) the IBM middle crust exhibits Vp identical to that of the bulk conti-nental crust. Vp estimates for plutonic rocks with HMA compositions support this. However tonalitic composition for middle-crust-forming rocks cannot be ruled out, suggesting an alternative possibility that the continental crust has been created by differentiation of mantle-derived basaltic magmas.

Bioinformatic approach for understanding the heterogeneity of cholangiocytes

It is remarkable that microarray technologies have nearly reached a pinnacle. Establishment of further analysis and management of enormous data derived from microarray technology is currently the highest priority. The heterogeneous functions of cholangiocytes regulate the pathophysiology of the biliary epithelium in relation to secretory, proliferative and apoptotic activities. Distinct expression profiles of two murine cholangiocyte lines, termed small and large have been revealed by microarray analysis. The features of the two cholangiocyte cell lines, categorized partly according to gene ontology, indicate the specific physiological role of each cell line. The large cholangiocytes are characterized as “transport” and “immune/ inflammatory responses” In contrast, small cholangiocytes are associated with properties of limited physiological functional ability and proliferating/migrating potential with specific molecules like Eph receptors, comparable to mesenchymal cells. ＇Omic study will be of great help in understanding the heterogeniety of cholangiocytes.

An updated result of European Randomized Screening Trial for Prostate Cancer encourages Asian countries to proceed prostate-specific antigen （PSA） screening for early detection. On the other hand, the issue of overtreatment is emerging as a serious problem. Active surveillance （AS） should be more prevalent than now to avoid overtreatment for patients with favourablerisk prostate cancer （PCa） detected by PSA test.

Percutaneous endoscopic necrosectomy for walled-off necrosis in the retroperitoneal space of the elderly:A case report

BACKGROUND Walled-off necrosis(WON)is a late complication of acute pancreatitis possibly with a fatal *** for WON spreading to the retroperitoneal space,percutaneous endoscopic necrosectomy(PEN)can be an alternate approach to surgical necrosectomy,particularly for the older individuals or patients with poor condition because of *** SUMMARY An 88-year-old man was admitted to our hospital with a *** retrograde cholangiopancreatography(ERCP)was performed to improve jaundice;however,post-ERCP pancreatitis *** inflammation of pancreatitis spread widely from the right retroperitoneal cavity to the pelvis,and WON was formed 4 wk later.A percutaneous drainage tube was placed into the WON under computed tomography ***,the drainage did not ameliorate clinical symptoms including fever,which assured less invasive necrosectomy.A metallic stent for the upper gastrointestinal(GI)tract was placed from the percutaneous drainage *** upper GI endoscope was inserted into the inside of the WON through the metallic stent,and the necrotic tissues were bluntly removed with a snare *** times of these necrosectomies resulted in the near-complete removal of necrotic *** procedures consequently abated his fever and remarkable improvement in blood *** PEN for WON occurring in the retroperitoneal space was safe and effective for very old individuals.

Transmission of hepatitis C virus: Self-limiting hepatitis or chronic hepatitis?

It has been suggested that hepatitis C virus(HCV)is selectively transmitted to a new host as an infectious clone from multiple HCV variants(quasispecies)in the *** individuals with HCV infection develop chronic hepatitis,but approximately 15%-40%of them clear the virus spontaneously and the hepatitis is resolved in a self-limiting manner in the acute phase of *** difference in the outcome of acute hepatitis C is attributable to both viral characteristics and genetic regulation of *** particular,the evolutionary dynamics of the infecting virus and host genetic polymorphisms pertaining mainly to the immune system,including polymorphisms in the region of the Interleukin 28B gene encoding interferon-λ-3,are associated with susceptibility to HCV infection.
