Objective Catheter-based pulmonary vein isolation(PVI) is an established therapy to treat patients with paroxysmal atrial *** high-density mesh mapper(HDMM),a novel multi-electrode circumferential mapping catheter,guides the circumferential atrium isolation without help of 3D electro-anatomic *** compared the difference of the circumferential pulmonary vein anatomy mapping guided between by Carto system(one 3D electro-anatomic mapping system) and by HDMM. Methods From August 2010 to February 2012,forty-four consecutive patients with paroxysmal atrial fibrillation were scheduled for a first procedure of PVI.A circumferential PV ostial anatomy map was set up in Carto system around the HDMM(HDMM,Bard Electrophysiology) firstly,while the operator was blinded to the Carto ***-guided PVI was performed in every patient by irrigated radiofrequency application,which established another circumferential PV(CPV) ostial anatomy *** study compared the difference between the two images,including the point-to-point straight distance,the area,the perimeter and so on. Results All 176 PVs in 44 patients could be mapped by the HDMM and *** 43.2%of the PV ostial anatomies were matched very well between the two different map *** point-to-point straight distance between the HDMM-guided map and Carto-guided map is(6.20±1.44)*** distance in some PV segments between the two images is *** area of circumferential right PV(CRPV) in the two views is 8.41±2.88mm in Carto map,9.37±1.84 mm in HDMM map respectively(P=0.013). The area of circumferential left PV(CLPV) is(8.12±2.48)mm in Carto,(8.98±2.33) mm in HDMM respectively(P=0.071).The CRPV perimeter in Carto and HDMM is(11.63±1.95)mm vs (11.87±1.72)mm(P=0.471),the CLPV(11.29±1.59)mm vs(11.12±1.71)mm(P=0.573). Conclusion Compared to the Carto-guided CPV anatomy image,the HDMM-guided one still has some discrepancies,which maybe explain a part of reasons why the high-density mesh ablator(

Objective To investigate the clinical significance of AT termination in patients with persistent AF undergoing ablation. Methods A total of 200 consecutive patients with persistent AF were enrolled in this study,and underwent radiofrequency catheter ablation for the first time in individualized ablation strategy navigated by Carto(Biosense Webster,Diamond Bar,CA,UsA).If AF organized into AT,we tried to define the mechanism of every AT,then optimal ablation was performed to terminate these *** we failed to define or terminate the ATs,cardioversion was used to restore sinus rhythm(sR).A 3-month blanking period was resumed. Results AF organized into AT during procedure in 93 patients(mean age 57.1±11.3 years,73 male),AT was terminated to sR successfully by further ablation in 62 patients and by cardioversion in 31 *** a mean follow-up of 38.5±15.9 months,sR was maintained in 51 patients(54.8%).There was a lower recurrent incidence(32.3%vs.71.0%,P = 0.001) in patients who had AT termination by ablation alone compared with those who did not. Conclusions AT termination by ablation alone is associated with an improved clinical outcome in patients with persistent AF.

Objective The administration of midazolam had no effect upon the electrophysiologic prope rties of cardiac tissue. Methods Radiofrequency ablation was performed in 201 consecutive persistent AF patients (150 males,mean age 58.1±10.5 years),who under sedation with a bolus of midazolam and anal gesia with a continuous infusion of *** intravenous bolus of midazolam was used for in ducing sedation for electrical cardioversion in patients whose atrial arrhythmias were not terminate d by ablation. Results After extensive ablation,atrial arrhythmia was failed to be terminated by ablation i n 106(52.7%) ***,termination of atrial arrhythmias and restoration of sinus rhy thm were observed in 5 patients(4.7%,all male,mean age 53.0±9.9 years old) 16.0±4.2 seconds after the administration of midazolam(l-2mg) without antiarrhythmics or electrical cardioversion. Two patients encountered the recurrence of atrial flutter(AFL),and one of them had a second *** 32.8±16.7 months post the last procedure,4(80%) patients were free of arrhyth mias without the use of antiarrhythmics and 1 patient remained in drug-refractory persistent AFL. Conclusions In patients who underwent extensive ablation for persistent AF,an intravenous bolus of midazolam may have the cardioversion effectiveness.

我国每年因终末期肝病死亡的病人超过20万人,由于缺乏有效的治疗手段,这些病人大多因肝衰竭而死亡,而肝移植已成为治疗终末期肝病,尤其是救治濒临死亡的暴发性肝衰竭病人唯一的有效和可靠手段,而且移植技术日臻成熟,与手术技术相关的术后并发症明显减少.目前,免疫抑制治疗和康复治疗已蜒成为影响移植手术的关键.在实际工作中发现＞80%的移植术后病人存在过度焦虑等负性情绪.

目的探讨早发痛风的临床特征及其影响因素。方法选取2015年至2018年于内分泌代谢科就诊的痛风男性患者,按发病年龄分为早发组(发病年龄≤30岁)和晚发组(30岁stic回归分析表明,含糖饮料的饮用(P=0.012)、糖尿病家族史(P=0.037)为早发痛风的独立影响因素。结论痛风的早发与糖尿病家族史相关。有糖尿病家族史的患者更易早发痛风,这可能与共同的遗传基础有关。

The coexistence of coronary artery disease(CAD)and atrial fibrillation(AF)is common in clinical *** with CAD require antiplatelet therapy to reduce the occurrence of myocardial ischemic ***,patients with AF at high risk of thromboembolism require oral anticoagulants to reduce the occurrence of thromboembolic events such as *** cases where CAD coexists with AF,the combined use of antiplatelet and anticoagulation therapy can effectively reduce the risk of ischemic and thromboembolic events but increase the risk of bleeding at the same *** optimal antithrombotic regimen in patients with both CAD and AF has been controversial in clinical *** recent years,the expert consensuses on antithrombotic therapy in patients with AF presenting with acute coronary syndrome and/or undergoing percutaneous coronary intervention have been successively released in Europe and North America,and have been updated in a timely *** contrast,the guidelines on antithrombotic therapies in China are *** on published clinical evidence,this consensus incorporated relevant international and Chinese guidelines,consensuses,and expert recommendations,and addressed the issues encountered in the clinical practice of antithrombotic therapy in patients with AF and different types of *** current guideline is of great significance to guide treatment in patients with both CAD and AF in China.

目的探讨血浆透明质酸(HA)与外周血M2型单核细胞/M1型单核细胞比值(简称M2/M1比值)联合检测在乳腺癌中的临床应用价值。方法选取98例乳腺癌患者(乳腺癌组)、37例乳腺良性疾病患者(乳腺良性疾病组)和41名体检健康者(正常对照组)。采用化学发光法检测血浆HA和糖类抗原(CA)15-3水平;采用流式细胞术检测外周血M1型(CD14+CD163-CD86+)和M2型(CD14+CD163+CD204+)单核细胞的表达,并计算M2/M1比值。各项指标之间的相关性分析采用spearman相关分析。采用受试者工作特征(ROC)曲线分析各项指标的诊断效能。结果乳腺癌组血浆HA、CA15-3水平及M2型单核细胞比例、M2/M1比值均明显高于乳腺良性疾病组及正常对照组(P0.05)。HA与M2/M1比值、M2型单核细胞比例、M1型单核细胞比例均相关(rs值分别为0.401、0.393、-0.189,P0.05)。结论 HA与M2/M1比值联合检测对辅助诊断乳腺癌具有一定的临床价值。

目的比较外周血M2/M1单核细胞比例在乳腺癌患者和体检健康者间的差异,初步探讨外周血M2/M1单核细胞比例作为循环肿瘤标志物的临床意义。方法采用流式细胞术检测89例乳腺癌患者、53名体检健康者外周血M2/M1单核细胞比例,并与癌胚抗原(CEA)和糖类抗原15-3(CA15-3)进行比较分析。结果乳腺癌患者外周血M2/M1单核细胞比例明显高于体检健康者(P<0.001)。外周血M2/M1单核细胞比例诊断乳腺癌的受试者工作特征(ROC)曲线的曲线下面积大于CEA和CA15-3。结论应用流式细胞术检测的外周血M2/M1单核细胞比例具有较高的乳腺癌诊断价值,其可能成为新的用于乳腺癌诊断的循环肿瘤标志物。

目的 :评估产次、胎位、分娩方式对婴儿发育性髋关节发育异常(DDH)发病的影响。方法 :回顾性分析于我院行DDH超声筛查婴儿5 227例(10 454个髋关节),记录其性别、年龄、胎次、胎位、分娩方式,依据Graf分类法将超声筛查结果分为正常和异常髋关节(发育不良、半脱位、脱位),并行统计分析。结果:5 227例,共10 454个髋关节,其中第1胎DDH的检出率为4.47%(284/6 350)高于第2胎(3.05%,125/4 104)(χ~2=15.08,P<0.01);臀位DDH检出率9.05%(42/464)较头位(3.67%,366/9 968)明显高(χ~2=34.15,P<0.01);剖宫产DDH检出率5.33%(196/3 678)高于自然分娩(3.14%,213/6 776)(χ~2=27.51,P<0.01)。结论 :产次、胎位、分娩方式均是影响DDH的因素,而臀位的影响更显著,可提示DDH的超声筛查。

目的探究血清透明质酸(HA)及透明质酸合成酶(HAs)2在骨肿瘤辅助诊断中的应用价值。方法检测35例骨肿瘤患者(骨肿瘤组)手术前、后以及40例骨感染性炎症患者(骨感染性炎症组)、60名体检健康者(正常对照组)的血清HA、HAs2水平和血清HAs2 mRNA相对表达量,采用受试者工作特征(ROC)曲线评估各项指标诊断骨肿瘤的效能。结果骨肿瘤组术前、术后血清HA、HAs2水平及HAs2 mRNA相对表达量均明显高于正常对照组(Ps2水平和HAs2 mRNA相对表达量均明显高于骨感染性炎症组(P0.05)。骨肿瘤组术后血清HA、HAs2水平及HAs2 mRNA相对表达量均低于术前(Ps2及HAs2 mRNA单项检测及三者联合检测诊断骨肿瘤的曲线下面积(AUC)分别为0.888、0.651、0.747、0.956;以骨感染性炎症组为对照,HA、HAs2及HAs2 mRNA单项检测及三者联合检测诊断骨肿瘤的AUC分别为0.485、0.782、0.630、0.798。结论血清HA、HAs2及HAs2 mRNA在骨肿瘤的辅助诊断及疗效监测中有一定价值。
