Midazolam may have the cardioversion effectiveness in some patients who underwent extensive ablation for persistent atrial fibrillation

摘 要：Objective The administration of midazolam had no effect upon the electrophysiologic prope rties of cardiac tissue. Methods Radiofrequency ablation was performed in 201 consecutive persistent AF patients (150 males,mean age 58.1±10.5 years),who under sedation with a bolus of midazolam and anal gesia with a continuous infusion of *** intravenous bolus of midazolam was used for in ducing sedation for electrical cardioversion in patients whose atrial arrhythmias were not terminate d by ablation. Results After extensive ablation,atrial arrhythmia was failed to be terminated by ablation i n 106(52.7%) ***,termination of atrial arrhythmias and restoration of sinus rhy thm were observed in 5 patients(4.7%,all male,mean age 53.0±9.9 years old) 16.0±4.2 seconds after the administration of midazolam(l-2mg) without antiarrhythmics or electrical cardioversion. Two patients encountered the recurrence of atrial flutter(AFL),and one of them had a second *** 32.8±16.7 months post the last procedure,4(80%) patients were free of arrhyth mias without the use of antiarrhythmics and 1 patient remained in drug-refractory persistent AFL. Conclusions In patients who underwent extensive ablation for persistent AF,an intravenous bolus of midazolam may have the cardioversion effectiveness.
