目的筛选和分析2型糖尿病(T2DM)合并脑小血管病变(CSVD)患者病情严重程度的影响因素。方法选取南京市中医院2018年6月至2023年5月收治的519例T2DM合并CSVD患者。根据CSVD影像学评分将患者分为轻度组(n=214)和重度组(n=305)。收集相关人口学、实验室、影像学指标资料。通过LASSO和logistic回归分析筛选出T2DM合并CSVD患者病情严重程度的影响因素,建立预测模型。绘制受试者工作特征(ROC)曲线及拟合优度评估,限制性立方样条(RCS)拟合曲线分析胱抑素C(Cys C)、白蛋白/球蛋白(A/G)与患者病情严重程度的量效关系。结果重度组男性占比、年龄大于轻度组,中性粒细胞、系统免疫炎症指数(SII)、肌酐(Crea)、尿酸(UA)、尿素(Urea)、D-二聚体(D-D)、乳酸脱氢酶(LDH)、腺苷脱氨酶(ADA)、球蛋白(GLB)、Cys C水平高于轻度组,淋巴细胞、ALT、高密度脂蛋白胆固醇(HDL-C)、血清胆碱酯酶(CHE)、前白蛋白(PAB)、A/G水平低于轻度组,差异有统计学意义(P<0.05)。LASSO和logistic回归分析显示,性别、年龄、A/G、Cys C是T2DM合并CSVD患者病情严重程度的独立影响因素。该模型曲线下面积(AUC)为0.658(95%CI:0.610~0.706),拟合良好(P=0.520)。RCS拟合曲线显示,Cys C≥0.618 mg/L与患者CSVD影像学评分呈线性关系(P=0.035),A/G≥1.268与患者CSVD影像学评分呈非线性关系(P=0.007)。结论高龄、男性、Cys C升高、A/G下降是T2DM合并CSVD患者病情严重程度的独立危险因素。

The disruption and reconstruction of the TREM2^(+) tissue resident macrophage(TRM)barrier on the surface of synovial lining play a key role in the activation and"remission"of rheumatoid arthritis(RA),which engender the prediction of this immunologic barrier as a potential driver for the achievement of"cure"in ***,strategies to promote the reconstruction of this barrier have not been reported,and the effect of patching this barrier remains *** the other hand,appropriate piezoelectric stimulation can reprogram macrophages,which has never been exerted on this barrier TRM ***,we design piezoelectric tetragonal BaTiO_(3)(BTO)ultrasound-driven nanorobots(USNRs)by the solvothermal synthesis method,which demonstrates satisfactory electro-mechanical conversion effects,paving the way to generate controllable electrical stimulation under ultrasound to reprogram the barrier TRM by minimally invasive injection into joint *** is demonstrated that the immunologic barrier could be patched by this USNR effectively,thereby eliminating the hyperplasia of vessels and nerves(HVN)and ***,TREM2 deficiency serum-transfected arthritis(STA)mice models are applied and proved the indispensable role of TREM2 in RA curing mediated by *** all,our work is an interesting and important exploration to expand the classical tetragonal BTO nanoparticles in the treatment of autoimmune diseases,providing a new idea and direction for the biomedical application of piezoelectric ceramics.

The repair of osteochondral defects is one of the major clinical challenges in ***-established osteochondral tissue engineering methods have shown promising results for the early treatment of small ***,less success has been achieved for the regeneration of large defects,which is mainly due to the mechanical environment of the joint and the heterogeneous nature of the *** this study,we developed a multi-layered osteochondral scaffold to match the heterogeneous nature of osteochondral tissue by harnessing additive manufacturing technologies and combining the established art laser sintering and material extrusion *** developed scaffold is based on a titanium and polylactic acid matrix-reinforced collagen“sandwich”composite *** microstructure and mechanical properties of the scaffold were examined,and its safety and efficacy in the repair of large osteochondral defects were tested in an ovine condyle *** 12-week in vivo evaluation period revealed extensive and significantly higher bone in-growth in the multi-layered scaffold compared with the collagen–HAp scaffold,and the achieved stable mechanical fixation provided strong support to the healing of the overlying cartilage,as demonstrated by hyaline-like cartilage *** histological examination showed that the regenerated cartilage in the multi-layer scaffold group was superior to that formed in the control *** genes such as aggrecan and collagen-II were upregulated in the scaffold and were higher than those in the control *** findings showed the safety and efficacy of the cell-free“translation-ready”osteochondral scaffold,which has the potential to be used in a one-step surgical procedure for the treatment of large osteochondral defects.

Exceeding probability analysis for rail of high-speed railway under seismic excitations

Purpose–The smoothness of the high-speed railway(HSR)on the bridge may exceed the allowable standard when an earthquake causes vibrations for HSR bridges,which may threaten the safety of running ***,few studies have evaluated the exceeding probability of rail displacement exceeding the allowable *** purposes of this article are to provide a method for investigating the exceeding probability of the rail displacement of HSRs under seismic excitation and to calculate the exceeding ***/methodology/approach–In order to investigate the exceeding probability of the rail displacement under different seismic excitations,the workflow of analyzing the smoothness of the rail based on incremental dynamic analysis(IDA)is proposed,and the intensity measure and limit state for the exceeding probability analysis of HSRs are *** a finite element model(FEM)of an assumed HSR track-bridge system is constructed,which comprises a five-span simply-supported girder bridge supporting a finite length CRTS II ballastless *** different seismic excitations,the seismic displacement response of the rail is calculated;the character of the rail displacement is analyzed;and the exceeding probability of the rail vertical displacement exceeding the allowable standard(2mm)is ***–The results show that:(1)The bridge-abutment joint position may form a step-like under seismic excitation,threatening the running safety of high-speed trains under seismic excitations,and the rail displacements at mid-span positions are bigger than that at other positions on the bridge.(2)The exceeding probability of rail displacement is up to about 44%when PGA 50.01g,which is the level-five risk probability and can be described as’very likely to happen’.(3)The exceeding probability of the rail at the mid-span positions is bigger than that above other positions of the bridge,and the mid-span positions of the track-bridge system above the bridge may be the most

Composite gel electrolytes containing several kinds of particles used as the quasi-solid-state electrolytes in dye-sensitized solar cells (DSSCs) were reported. Mesoporous particles (MCM-41) with unique structures composed of ordered nanochannels were served as a new kind of gelator for quasi-solid-state electrolytes. MCM-41, hydrophobic fumed silica Aerosil R972 and TiO2 nanopatricles P25 were dispersed into gel electrolytes respectively. The solar energy-to-electricity conversion efficiency of these cells is 4.65%, 6.85% and 5.05% respectively under 30 mW·cm-2 illumination. The preparation methods and the particles sizes exert an influence on the performance of corresponding solar cells. Owing to unique pore structures and high specific BET surface area, mesoporous silica MCM-41 was expected to have the potential to afford conducting nanochannels for redox couple diffusion.

Recycling graphite anode from spent lithium-ion batteries(SLIBs)is regarded as a crucial approach to promoting sustainable energy storage ***,the recycled graphite(RG)generally presents degraded structure and ***,the residual fluoride self-activated effect is proposed for the upgraded utilization of *** and low-energy water immersion treatment not only widens the interlayer spacing,but also retains appropriate fluoride on the surface of *** analysis and experiments demonstrate that the residual fluoride can optimize Li~+migration and deposition kinetics,resulting in better Li~+intercalation/deintercalation in the interlayer and more stable Li metal plating/stripping on the surface of RG,As a result,the designed LFP||RG full cells achieve ultrahigh reversibility(~100%Coulombic efficiency),high capacity retention(67%after 200 cycles,0.85 N/P ratio),and commendable adaptability(stable cycling without short-circuiting,0.15 N/P ratio).The energy density is improved from 334 Wh kg^(-1)of 1.1 N/P ratio to 367 Wh kg^(-1)of 0.85 N/P ratio(total mass based on cathode and anode).The exploration of RG by residual fluoride self-activated effect achieves upgraded utilization beyond fresh commercial graphite and highlights a new strategy for efficient reuse of SLIBs.

In this study, silk scaffolds with appropriate porous structures were prepared by adjusting solution concentrations and providing treatment with methanol solutions in the way of freeze drying. The effects of the preparation conditions on the microstructures and properties of the scaffolds were discussed. Fibroin solutions with different concentrations of 4, 6, 8, 10 wt% were used respectively to prepare the scaffolds. The effects of the addition of 20 vol% methanol before or after freeze drying to the 4 wt% fibroin solution were investigated. As demonstrated by Scanning Electron Microscope (SEM), the fib-roin scaffolds prepared without methanol had porous microstructures composed of thin sheets, and the sizes of the pores decreased with the increase of the fibroin solution concentrations, while the scaffolds prepared in the presence of methanol showed porous microstructures formed by fine-particle aggregates. The porosities and mechanical properties of the prepared fibroin scaffolds under different conditions were tested. The crystalline structures and conformations of the fibroin scaffolds were detected by Fourier transform infrared spectroscopy (FT-IR) and X-ray diffraction (XRD).
