Longitudial observation of dynamic changes in cortical function and white matter fibrous structure of patients with visual pathway lesions by blood oxygenation level dependent-functional magnetic resonance imaging combined with diffusion tensor imaging

BACKGROUND： Functional magnetic resonance imaging （fMRI） is initially used for visual cortex location. However, the application of fMRI in investigating the development of visual pathway lesions needs to be further observed. OBJECTIVE： This study is to longitudially observe the dynamic changes in cortical function and white matter fibrous structure of patients with visual pathway lesions by blood oxygenation level dependent-functional magnetic resonance imaging （BOLD-fMRI） combined with diffusion tensor imaging （DTI）, and to analyze the characteristics of brain function and structural recombination at convalescent period of lesions. DESIGN： Randomized controlled observation. SETTING： Department of Radiology, the General Hospital of Nanjing Military Area Command of Chinese PLA. PARTICIPANTS： Eight patients with unilateral or bilateral visual disorder caused by visual pathway lesions, who admitted to Department of Radiology, the General Hospital of Nanjing Military Area Command of Chinese PLA from January to September 2006 were involved, and served as experimental subjects. The patients, 6 males and 2 females, were aged 16 - 67 years. They had visual disorder confirmed by clinical examination, i.e. visual pathway lesion, which was further diagnosed by MR or CT. Another 12 subjects generally matching to those patients of experimental group in gender, age and sight, who received health examination in synchronization were involved and served as controls. The subjects had no history of eye diseases. Their binocular visual acuity （or corrected visual acuity） was over 1.0. Both routine examination of ophthalmology and examination of fundus were normal. Informed consents of detected items were obtained from all the subjects. Laparoscopy-assisted D2 radical distal gastrectomy for advanced gastric cancer： initial experience

Background Laparoscopy-assisted radical gastrectomy is gaining acceptance for treating early gastric cancer. However, few reports concerning the effectiveness of laparoscopy-assisted D2 radical distal gastrectomy （LADG） for advanced gastric cancer or data comparing the results obtained after open distal gastrectomy （ODG） are yet available. The aim of this study was to evaluate the method, feasibility and clinical result of LADG for advanced gastric cancer. Methods A retrospective study was performed comparing LADG and ODG for advanced gastric cancer. Seventy-eight patients who underwent LADG were compared with 90 patients who underwent ODG in terms of pathologic findings, operative outcome, and complications. Results There was no conversion to open surgery in the LADG group and no postoperative mortality of any patients. There were no significant differences between LADG and ODG in operative time （（245±35） vs （220±620） minutes）, complication rate （7.7% vs 10.0%）, and number of lymph nodes （23.5±6.0 vs 21.0±7.5）, while the blood loss was less after LADG （（110±25） vs （196±30） ml, P 〈0.05）. The time to postoperative flatus and postoperative hospital stay were shorter after LADG （（73.0±8.5） vs （102.0±10.5） hours, and （8.6±1.2） vs （12.1 ±2.5） days, P 〈0.05, respectively）. Conclusion LADG for advanced gastric cancer is feasible, safe, and minimally invasive.

Risk factors for bronchiolitis obliterans syndrome in allogeneic hematopoietic stem cell transplantation

Background The occurrence of bronchiolitis obliterans syndrome （BOS） after allogeneic hematopoietic stem cell transplantation （alIo-HSCT） is rare but severe. We examine the role of pre-HSCT chemotherapeutic exposure, pre-HSCT comorbidities, and transplant-related complications in the development of BOS after alIo-HSCT. Methods A nested case-control study was designed. Cases with BOS and controls matched for the year of alIo-HSCT and length of the follow-up were identified from a cohort of 1646 patients who underwent alIo-HSCT for treatment of hematologic malignancies between 2006 and 2011. Antithymocyte globulin was used in the partial matched related and unrelated matched donor HSCT, or patients with severe aplastic anemia. Results Thirty-six patients suffered from BOS; the mean age at the time of presentation was （32.7±2.4） years, and the mean time to presentation was （474±350） days post-HSCT. A pre-HSCT cyclophosphamide dose of 〉3.2 g/m2 （OR=8.74, P=0.025）, chronic graft-versus-host disease （moderate to severe）（OR=12.02, P=0.000）, and conditioning regimens without antithymocyte globulin （OR=2.79, P=0.031） were independently associated with BOS. Conclusions We found that higher pre-HSCT cyclophosphamide exposure, a conditioning regimen without antithymocyte globulin, and moderate to severe chronic graft-versus-host disease are significantly and independently associated with BOS. Based on these results, we can identify patients who are at a higher risk of developing BOS after alIo-HSCT, select a more appropriate therapeutic strategy, and improve the outcome of HSCT recipients.

Increase of β1-adrenergic receptor gene expression induced by nicotine in hippocampal slice of rat

AIM: To investigate the effect of nicotine on β1-adrenergic receptor (β1-AR) in the hippocampal slice of rat. METHODS: Hippocampal slices (400 μm thick) were incubated in artificial cerebrospinal fluid (ACSF) previously saturated with 95％ O2and 5％ CO2at 28℃ for 120 min, and then incubated with nicotine 10 μmol/L for 30, 60, 90, and 120 min. mRNA of the β1-adrenergic receptor was examined with semiquantitative reverse transcription-polymerase chain reaction (RT-PCR), and the protein level was measured by Western blot and RIA. RESULTS: The mRNA gene expression and the protein level of β1-adrenergic receptor in hippocampal slices were increased after nicotine treatment. The peak of protein occurred later but higher than that of mRNA level. CONCLUSION: Both expression of β1-adrenergic receptor gene transcription and post-transcriptional protein level in rat hippocampus were altered by nicotine.

Authentication and distinction of Shenmai injection with HPLC fingerprint analysis assisted by pattern recognition techniques

In this paper, the feasibility and advantages of employing high performance liquid chromatographic （HPLC） fingerprints combined with pattern recognition techniques for quality control of Shenmai injection were investigated and demonstrated. The Similarity Evaluation System was employed to evaluate the similarities of samples of Shenmai injection, and the HPLC generated chromatographic data were analyzed using hierarchical clustering analysis （HCA） and soft independent modeling of class analogy （SIMCA）. Consistent results were obtained to show that the authentic samples and the blended samples were successfully classified by SIMCA, which could be applied to accurate discrimination and quality control of Shenmai injection. Furthermore, samples could also be grouped in accordance with manufacturers. Our results revealed that the developed method has potential perspective for the original discrimination and quality control of Shenmai injection.

Risk Factors, Clinical Features and Prognosis for Subtypes of Ischemic Stroke in a Chinese Population

It is unanimously accepted that stroke is a highly heterogeneous disorder. Different subtypes of ischemic stroke may have different risk factors, clinical features, and prognoses. The aim of this study was to evaluate the risk factors, clinical characteristics, and prognoses of different subtypes of ischemic stroke defined by the Trial of ORG10172 in Acute Stroke Treatment （TOAST） criteria. We prospectively analyzed the data from 530 consecutive patients who were admitted to our hospital with acute ischemic stroke within 7 days of stroke onset during the study period. Standardized data assessment was used and the cause of ischemic stroke was classified according to the TOAST criteria. Patients were followed up till 30 and 90 days after stroke onset. It was found that large-artery atherosclerosis was the most frequent etiology of stroke （37.4%）, and showed the highest male preponderance, the highest prevalence of previous transient ischemic attack, and the longest hospital stay among all subtypes. Small artery disease （36.4%） was associated with higher body mass index, higher plasma triglycerides, and lower plasma high-density lipoprotein cholesterol than cardioembolism. Cardioembolism （7.7%）, which was particularly common in the elderly （i.e., individuals aged 65 years and older）, showed the highest female preponderance, the highest prevalence of atrial fibrillation, the earliest presentation to hospital after stroke onset, the most severe symptoms on admission, the maximum complications associated with an adverse outcome, and the highest rate of stroke recurrence and mortality. Our results suggest that ischemic stroke should be regarded as a highly heterogeneous disorder. Studies involving risk factors, clinical features, and prognoses of ischemic stroke should differentiate between etiologic stroke subtypes.

Stromal thrombospondin 1 suppresses angiogenesis in oral submucous fibrosis

A decline in mucosal vascularity is a histological hallmark of oral submucous fibrosis (OSF), a premalignant disease that is largely induced by betel quid chewing. However, the lack of available models has challenged studies of angiogenesis in OSF. Here, we found that the expression of thrombospondin 1 (THBS1), an endogenous angiostatic protein, was elevated in the stroma of tissues with OSF. Using a fibroblast-attached organoid (FAO) model, the overexpression of THBS1 in OSF was stably recapitulated in vitro. In the FAO model,treatment with arecoline, a major pathogenic component in areca nuts, enhanced the secretion of transforming growth factor (TGF)-β1 by epithelial cells, which then promoted the expression of THBS1 in fibroblasts. Furthermore, human umbilical vein endothelial cells (HUVECs)were incorporated into the FAO to mimic the vascularized component. Overexpression of THBS1 in fibroblasts drastically suppressed the sprouting ability of endothelial cells in vascularized FAOs (v FAOs). Consistently, treatment with arecoline reduced the expression of CD31in v FAOs, and this effect was attenuated when the endothelial cells were preincubated with neutralizing antibody of CD36, a receptor of THBS1. Finally, in an arecoline-induced rat OSF model, THBS1 inhibition alleviated collagen deposition and the decline in vascularity in vivo. Overall, we exploited an assembled organoid model to study OSF pathogenesis and provide a rationale for targeting THBS1.
