Association between serum albumin levels and disease severity in adult patients with dengue: A 7-year retrospective cohort study in China's Mainland

Objective:To identify the association between serum albumin levels and disease severity among adult patients with dengue in China's Mainland.Methods:This retrospective cohort study analyzed the data of adult patients(aged≥18 years)hospitalized with laboratory-confirmed dengue in a tertiary center for infectious diseases from 2013 to 2019 in China's Mainland.Serum albumin levels were estimated and compared between patients with severe dengue and non-severe dengue.Additionally,the association between serum albumin levels and severe dengue was evaluated using a generalized linear model[relative risks(RR)].Multivariate logistic regression analysis was performed to identify the potential predictors of severe dengue.Results:Overall,1568 patients were included in this study.Of these patients,34(2.17%)developed severe dengue.The median serum albumin levels were significantly lower in patients with severe dengue than that in those with non-severe dengue(33 g/L vs.37 g/L,P<0.001).After adjustment for age,sex,and comorbidities,hypoalbuminemia(RR 4.877,95%CI 2.193-11.461,P<0.001)was found to be a predictor of severe dengue.Serum albumin levels(OR 1.303,95%CI 1.161-1.462,P<0.001)and age(OR 1.038,95%CI 1.017-1.061,P<0.001)were significant risk factors for severe dengue.The area under the curve for serum albumin levels to distinguish severe dengue was 0.787.Conclusions:Lower serum albumin levels were significantly associated with disease severity in adult patients with dengue.Hypoalbuminemia on admission resulted in at least a four-fold increased risk of severe dengue.

Dear Editor,Dengue infections are caused by all four serotypes of dengue virus(DENV1–4).In recent years it has been confirmed that DENV-1 and DENV-2 are co-circulated in Guangzhou,Guangdong,China(Lai et al.2015;Luo et al.2017).DENV-2 strains were imported from Thailand and Indonesia(Luo et al.2017;Zhao et al.2014,2016),and had become the second local circulated serotype in Guangzhou.

Dengue fever is a mosquito-borne viral disease spread in tropical and subtropical regions caused by the dengue virus(DENV).DENV causes a febrile illness,severe forms including hemorrhagic fevers and shock with fatalities in humans.DENV-2 is frequently associated with severe dengue infections and epidemics.DENV-2 strains from Guangdong,China,have not been characterized to compare the phylogenetics and pathogenicity of different DENV-2 subgenotype strains in both vitro and vivo.A total of 22 patients tested to be DENV-2 positive and were enrolled in this study,22 complete genomes were obtained by virus isolation and high-throughput sequencing.Phylogenetic and single amino polymorphism(SAP)analysis indicated that two major subgenotypes(A and C)of DENV-2 Cosmopolitan were prevalent in Guangdong in 2018.The apparent change of major subgenotypes of DENV-2 circulating in Guangdong indicated the diversity of DENV-2 strains,including endemic genotype and imported genotype.It alerted the risk of cross-border transmission of DENV.A significant difference in replication rate was observed in C6/36 between different DENV-2 strains,although growth kinetics comparison of different DENV-2 Cosmopolitan subgenotypes showed similar profiles.DENV-2 subgenotypes(A and C)replicated efficiently in IFNAR−/−C57BL/6 mice,and subgenotype A of Cosmopolitan infected mice showed increased weight loss and delayed viral clearance compared with the subgenotype C group.DENV-2 prevalent in Guangdong in 2018 showed apparent genetic and pathogenicity diversity in both vitro and vivo,indicating the necessity of molecular surveillance and exploration of the relationship between its pathogenicity and clinical characteristics.

Abnormal glucose and lipid metabolism in COVID-19 patients were recently reported with unclear mechanism.In this study,we retrospectively investigated a cohort of COVID-19 patients without pre-existing metabolic-related diseases,and found new-onset in suli n resista nee,hyperglycemia,and decreased HDL-C in these patie nts.Mecha nistically,SARS-CoV-2 infecti on in creased the expression of RE1-silencing transcription factor(REST),which modulated the expression of secreted metabolic factors including myeloperoxidase,apelin,and myostatin at the transcriptional level,resulting in the perturbation of glucose and lipid metabolism.Furthermore,several lipids,including(±)5-HETE,(±)12-HETE,propionic acid,and isobutyric acid were identified as the potential biomarkers of COVID-19-induced metabolic dysregulation,especially in insulin resistance.Taken together,our study revealed insulin resistance as the direct cause of hyperglycemia upon COVID-19,and further illustrated the underlying mechanisms,providing potential therapeutic targets for COVID-19-induced metabolic complications.

Effective therapies are urgently needed for the SARS-CoV-2 pandemic. Chloroquine has been proved to have antiviral effect against coronavirus in vitro. In this study, we aimed to assess the efficacy and safety of chloroquine with different doses in COVID-19. In this multicenter prospective observational study, we enrolled patients older than 18 years old with confirmed SARS-CoV-2 infection excluding critical cases from 12 hospitals in Guangdong and Hubei Provinces. Eligible patients received chloroquine phosphate500 mg, orally, once(half dose) or twice(full dose) daily. Patients treated with non-chloroquine therapy were included as historical controls. The primary endpoint is the time to undetectable viral RNA. Secondary outcomes include the proportion of patients with undetectable viral RNA by day 10 and 14, hospitalization time, duration of fever, and adverse events. A total of 197 patients completed chloroquine treatment, and 176 patients were included as historical controls. The median time to achieve an undetectable viral RNA was shorter in chloroquine than in non-chloroquine(absolute difference in medians-6.0 days; 95% CI-6.0 to-4.0). The duration of fever is shorter in chloroquine(geometric mean ratio 0.6; 95% CI 0.5 to 0.8). No serious adverse events were observed in the chloroquine group. Patients treated with half dose experienced lower rate of adverse events than with full dose. Although randomized trials are needed for further evaluation, this study provides evidence for safety and efficacy of chloroquine in COVID-19 and suggests that chloroquine can be a cost-effective therapy for combating the COVID-19 pandemic.

Biliary stenting is an important interventional method for the prevention and treatment of biliary tract diseases.However,complications,such as postoperative biliary infection and re-stenosis,frequently occur due to the extensive scope of the biliary system and the complex composition of bile.The com-bination of coating technology and biliary stents is expected to bring new approaches to the solution of these problems.The cutting-edge advance on functional coatings on biliary stents is reviewed from seven perspectives:anticorrosion,-bacterial,-tumor,stone-dissolving,X-ray visibility,anti-stent migration and functional composite coatings.The de-velopment trend is also discussed.Overall.the performance of the numerous functional coatings for various purposes is generally up to expectations,but the balance between the medica tions'effectiveness and their safety needs to be further adjusted.Many contemporary investigations have advanced to the leve of animal experiments,offering crucial fundamental assurance for broader human studies.The combination of biliary stent and functional coatings is an innovative idea with great potential for future development.

T cell-redirecting bispecific antibodies are specifically designed to bind to tumor-associated antigens,thereby engaging with CD3 on the T cell receptor.This linkage between tumor cells and T cells actively triggers T cell activation and initiates targeted killing of the identified tumor cells.These antibodies have emerged as one of the most promising avenues within tumor immunotherapy.However,despite success in treating hematological malignancies,significant advancements in solid tumors have yet to be explored.In this review,we aim to address the critical challenges associated with T cellredirecting bispecific antibodies and explore novel strategies to overcome these obstacles,with the ultimate goal of expanding the application of this therapy to include solid tumors.

Temperature dependence of photoluminescence from self-organized Ge quantum dots with large size and low density

Photoluminescence(PL) from self-organized Ge quantum dots(QDs) with large size and low density has been investigated over a temperature range from 10 to 300 K using continuous-wave(CW) optical excitation.The integrated PL intensity of QDs observed is negligible at about 10 K and rapidly increases with raising temperature up to 100 K.Through analyzing the PL experimental data of the QDs and wetting layer(WL),we provide direct evidence that there exists a potential barrier,arising from the greater compressive strain surrounding large QDs,which could trap carriers in WL at low temperatures and could be overcome via increasing temperature.

Market Rule or Equal Opportunity Rule:An Empirical Analysis Based on Acquisitions of Chinese Listed Companies

This paper examines the data of A-share listed companies in China from 2002 to 2017,drawing on the theory of equal opportunity and market rules in M&A transactions.This paper investigates the correlation between changes in tender offer policy and M&A tendencies and performance.The findings suggest that following the policy shift and the adoption of market rules,companies that secure an exemption from the mandatory tender offer obligation not only exhibit stronger M&A tendencies but also improved long-term M&A performance.This indicates that market rules are more suitable for China and contribute to enhancing the efficiency of the M&A market.The paper also presents evidence of a moderating effect,demonstrating that exemptions from the mandatory tender offer obligation positively influence the relationship between policy change and M&A performance.Lastly,this paper finds that state-owned and large-scale firms tend to exhibit a higher degree of M&A tendencies.

Light-triggered fluorochromic cholesteric liquid crystal elastomer with hydrogen-bonded fluorescent switch: dualmodal-switchable circularly polarized luminescence

Solid materials with dynamically tunable circularly polarized luminescence（CPL） feature higher security levels and devicefriendly characteristics, showing great superiority in the field of information technology and anti-counterfeiting. To address the limited photoisomerization of fluorescent photoswitch in CPL-active solid materials, here a cholesteric liquid crystal elastomer（CLCE） containing a hydrogen-bonded（H-bonded） polymerizable fluorescent switch AHBA-PSD is prepared. Owing to the good flexibility and low glass-transition temperature of CLCE, AHBA-PSD shows a fast and fully reversible photocyclization/cycloreversion with fluorochromic behavior in CLCE. Further, by controlling the spectral overlapping area between the emission and reflection bands, the CLCE exhibits a strong CPL with a glumvalue of up to 0.76 accompanied by a reversible phototunable CPL signal. Meanwhile, the H-bond helps to stabilize the mechanical property, and the force-induced switching-off of CPL signal could be achieved due to the destroyed helical structures by external force. Finally, an automatically-recognized identification card with abundant chiroptical information is demonstrated.
