Clinical and endoscopic features of Chinese reflux esophagitis patients

AIM： To analyze the clinical and endoscopic features of Chinese patients with reflux esophagitis （RE）. METHODS： A total of 1405 RE patients were analyzed retrospectively. Data on gender, age, presence/absence of H pylori infection and associated esophageal hiatal hernia were collected. Esophagitis was divided into different grades according to Los Angeles Classification. RESULTS： Of 18823 patients, 1405 were diagnosed as RE. The ratio of male to female patients was 1.75：1 （P 〈 0.01）. The mean age of male and female patients was significantly different （P = 0.01）. The peak age at onset of the disease was 40-60 years. According to Los Angeles Classification, there were significant differences in the age of patients with grades A and B compared to patients with grades C and D （P 〈 0.01）. Two hundred and seventy-seven patients were infected with H pyiorl, the infection rate was low （P 〈 0.01）. Complication of esophageal hiatal hernia was found to be significantly associated with the severity of esophagitis and age in 195 patients （P 〈 0.01）. Esophageal mucosa damages were mainly located at the right esophageal wall. CONCLUSION： The peak age of onset of RE is 40-60 years and higher in males than in females. The mean age of onset of RE is lower in males than in females. The infection rate of Hpylori is significantly decreased in patients with esophagitis. Old age and esophageal hiatal hernia are associated with more severe esophagitis. Right esophageal mucosal damage can occur more often in RE patients.

Progress on immuno-microenvironment and immune-related therapies in patients with pseudomyxoma peritonei

Pseudomyxoma peritonei(PMP) is an indolent malignant syndrome. The standard treatment for PMP is cytoreductive surgery combined with intraperitoneal hyperthermic chemotherapy(CRS + HIPEC). However, the high recurrence rate and latent clinical symptoms and signs are major obstacles to further improving clinical outcomes. Moreover, patients in advanced stages receive little benefit from CRS + HIPEC due to widespread intraperitoneal metastases. Another challenge in PMP treatment involves the progressive sclerosis of PMP cell-secreted mucus, which is often increased due to activating mutations in the gene coding for guanine nucleotide-binding protein alpha subunit(GNAS). Consequently, the development of other PMP therapies is urgently needed. Several immune-related therapies have shown promise, including the use of bacterium-derived non-specific immunogenic agents, radioimmunotherapeutic agents, and tumor cell-derived neoantigens, but a well-recognized immunotherapy has not been established. In this review the roles of GNAS mutations in the promotion of mucin secretion and disease development are discussed. In addition, the immunologic features of the PMP microenvironment and immune-associated treatments are discussed to summarize the current understanding of key features of the disease and to facilitate the development of immunotherapies.

Background Infantile epileptic spasms syndrome(IESS)is a serious disease in infants,and it usually evolves to other epilepsy types or syndromes,especially refractory or super-refractory focal *** adrenocorticotropic hormone(ACTH)is one of the first-line and effective treatment plans for IESS,it has serious side effects and is not sufficiently *** A retrospective study of the clinical outcomes of ACTH combined with magnesium sulfate(MgSO4)therapy for IESS in two hospital centers was *** major outcome of the single and combined treatment was evaluated by changes in seizure frequency and improvements in hypsarrhythmia electroencephalography(EEG).To reduce the confounding bias between the two groups,we used SPSS for the propensity score matching(PSM)*** We initially recruited 1205 IESS patients from two Chinese hospitals and treated them with ACTH combined with MgSO4 and ACTH *** 1005 patients were enrolled in the treatment(ACTH combined with MgSO4:744,ACTH:261),and both treatment plans had a more than 55%response ***,compared to patients treated with ACTH alone,those patients treated with ACTH combined with MgSO4 had better performance in terms of the seizure frequency and hypsarrhythmia *** PSM,the two groups also showed significant differences in responder rate[70.8%(95%confidence interval,CI)=66.7%–74.8%)vs.53.8%(95%CI=47.4%–60.2%),Plinical *** with less than 3 months of lead time responded to the treatment much better than those with>3 months(P<0.05).In addition,the overall incidence of adverse reactions in the ACTH combined with MgSO4 group was much lower than that in the ACTH group(31.4%vs.63.1%,P<0.001).During the treatment,only

Introduction:The purpose of this study is to examine the potential causal relationship between levels of circulating glycine and coronary artery disease(CAD)using a two-step Mendelian randomization(MR)***:We analyzed data from genome-wide association studies(GWAS)conducted on European and East Asian *** assess the causal effects of circulating glycine levels on the risk of *** used the inverse-variance weighting(IVW),weighted median(WM),MR-Egger,and Mendelian Randomization Pleiotropy RESidual Sum and Outlier(MR-PRESSO)***,we conducted mediation analysis to investigate the contribution of blood pressure and other cardiovascular disease-related ***:The two-step Mendelian randomization analysis revealed that higher levels of glycine in the blood were associated with a reduced risk of CAD in Europeans[odds ratio(OR)=0.84,95%confidence interval(CI):0.72,−0.98;P=0.029]and East Asians:(OR=0.76,95%CI:0.66,−0.89;P=3.57×10−4).Sensitivity analysis confirmed the robustness of these ***,our results suggest that about 6.06%of the observed causal effect is mediated through genetically predicted systolic blood pressure(SBP)in the European ***:Our results contribute to the current knowledge regarding the involvement of glycine in the progression of CAD,and provide valuable methodological insights for the prevention and treatment of this condition.
