Introduction: Current echocardiographic parameters have a limitation in assessing mitral valve (MV)apparatus in rheumatic mitral stenosis (MS) patient. In the current study, we use 2 dimensional (2D)longitudinal strain (S) and strain rate (Sr) imaging in evaluating the papillary muscle longitudinal strain (LS) as an objective and quantitative echocardiographic parameter with high reproducibility in the assessment of MV apparatus in patients with mild to moderate rheumatic MS with preserved ejection fraction (EF%).?Patients and Method: The study included 40 patients with established diagnosis of MS subdivided in to 31 patients with moderate MS (mean age: 32 ± 5) and 9 patients with mild MS (mean age: 31 ± 6). 20 healthy individuals (mean age 31 ± 6) as a control for cases. The mitral valve area (MVA) was estimated using planimetry and pressure half time (PHT) methods. 2D longitudinal systolic S and Sr imaging was carried out for all participants from the apical long axis (LAX), 4 chamber (4C), 2 chamber (2C) views. Global longitudinal systolic S and Sr were estimated by averaging the 3 apical views. Longitudinal myocardial strain of papillary muscle PMs was assessed by the use of the free strain method from apical 4 chamber view for the antrolateral papillary muscle (APM) and apical long axis view for postromedial papillary muscle (PPM).?Results: Patients with MS had significantly decreased longitudinal LV systolic S and Sr in comparison with control group (p Conclusion: Patients with MS and preserved EF% had decreased APM-LS & PPM-LS in comparison with control group, and had decreased longitudinal LV systolic S and Sr when compared with control group. 2D strain as well as Sr imaging might be a useful method for assessment of mitral valve apparatus in patients with MS & preserved EF%.

AIM To directly visualize Helicobacter pylori(H. pylori) by the highly sensitive and specific technique of immunohistochemical staining in colonic tissue from patients newly diagnosed with ulcerative colitis(UC).METHODS Colonoscopic biopsies from thirty patients with newly diagnosed UC and thirty controls were stained with Giemsa stain and immunohistochemical stain for detection of H. pylori in the colonic tissue. Results were confirmed by testing H. pylori Ag in the stool then infected patients were randomized to receive either anti H. pylori treatment or *** Twelve/30(40%) of the UC patients were positive for H. pylori by Giemsa, and 17/30(56.6%) by immunohistochemistry stain. Among the control group 4/30(13.3%) and 6/30(20 %) were positive for H. pylori by Giemsa and immunohistochemistry staining respectively. H. pylori was significantly higher in UC than in controls(P = 0.04 and 0.007). All Giemsa positive patients and controls were positive by immunohistochemical stain. Four cases of the control group positive for H. pylori also showed microscopic features consistent with early *** H. pylori can be detected in colonic mucosa of patients with UC and patients with histological superficial ulcerations and mild infiltration consistent with early UC. There seems to be an association between UC and presence of H. pylori in the colonic tissue. Whether this is a causal relationship or not remains to be discovered.

Background: Acute myocardial infarction is a leading cause of death worldwide nowadays and treatment of choice is primary percutaneous coronary intervention (PCI). No reflow is a complication that increases mortality and morbidity post intervention and one of its predictors is platelet lymphocyte ratio. Aim of Study: To assess relation between admission platelet to lymphocyte ratio (PLR) and angiographic reflow after primary PCI in acute ST elevation myocardial infarction (STEMI). Patients and Methods: This is a prospective study that was conducted from May 2017 to May 2018 at Cardiology Department, Menoufia University Hospital. Sixty patients presented with ST-elevation myocardial infarction who were eligible for primary PCI were enrolled in the study. According to TIMI flow post intervention, patients were arranged into 2 groups: Group 1 (Normal Reflow) included thirty patients with post intervention TIMI flow III and Group 2 (NO Reflow) included thirty patients with post intervention TIMI flow (0, I, II). Comparison between both groups was done regarding platelet lymphocyte ratio (PLR). Result: PLR was significantly higher in patients with coronary no reflow than in patients with normal reflow with a P-value of , timing interval between onset of chest pain to time of intervention and thrombus grading was significantly higher in patients with no reflow than in patients with normal reflow. Conclusion: Pre-intervention PLR is an independent predictor of slow flow/no reflow following PPCI in patient with acute STEMI.

Bi-function Compact graphene lens antenna in terahertz (THz) band has been investigated. The array function is switched between two status, reflectarray and/or transmitarray. The tunability of graphene conductivity introduces the bi-function characteristics of a single array structure in the THz band. The design depends on changing the graphene DC biasing voltage to transform the transmitting antenna to reflecting antenna. The compact structure of the antenna array saves the cost and the allocation area for the terahertz communication applications. A 13 × 13 reflectarray/ transmitarray antenna covering an area of 364 × 364 μm2 is proposed. A dual-beams reflectarray/transmitarray antenna is achieved by rearranging the cell elements of the array successively. Finally, a single structure is used to work as reflectarray and transmitarray antenna at the same time by rearranging the applied voltages between the different pieces of the graphene sheet using chess board arrangement. The phases of the successive unit-cells are kept the same of their locations in the original full array. The radiation characteristics of the array are investigated using the CST Microwave Studio for the bi-function operation.

金黄色葡萄球菌肠毒素(SEs)是金黄色葡萄球菌在生长的对数期或从指数期向稳定期过渡期间合成的有效胃肠外毒素。金黄色葡萄球菌可产生多种胃肠毒素,包括金黄色葡萄球菌肠毒素A、B、C、D、E、F、G、H、I、J、K、L、M、N、O、P等多种肠毒素。金黄色葡萄球菌肠毒素P(SEP)是导致食物中毒的主要因素之一。食物尤其是肉制品和乳制品可因处理不当或在高温下储存而被金黄色葡萄球菌污染。SEP中毒症状发病迅速,病人恶心和剧烈呕吐,伴有或不伴有腹泻。鉴于SEP检测在食品安全方面的重要性,本研究亚克隆了SEP基因至大肠杆菌表达载体、表达、纯化、免疫产蛋鸡并制备了鸡卵黄抗体。以期建立基于免疫学方法检测SEP的相关试剂。本论文的主要研究成果如下:(1)以pUC18-SEP为模板DNA,以Primer F(含EcoRI位点)和Primer R(含SalI位点)为引物扩增了SEP基因。扩增产物经EcoRI和SalI酶切、琼脂糖凝胶电泳分离、回收后与相应限制性内切酶处理的pET28a载体连接,获得pET28a-SEP重组载体。经核苷酸序列分析,亚克隆的SEP基因序列与模板和NCBI数据库中的SEP序列一致。(2)含重组质粒pET28a-SEP的大肠杆菌(*** Rossita)活化后在37℃、220rpm下培养至OD≥0.6,之后在16℃、220rpm下以1mmol/L IPTG诱导过夜。离心收集菌体,超声破碎后进行SDS-PAGE分析。结果表明,SEP蛋白主要以可溶性蛋白形式存在于菌体破碎后的上清中,沉淀中仅有少量SEP蛋白。(3)以商品化SEP抗体检测上述大肠杆菌诱导表达后的蛋白,发现诱导表达量最大的蛋白带(32.6k Da)呈阳性。其与含有标签肽(6x His-Thrombin-T7 Tag)的SEP蛋白的理论分子量(29.77k Da)基本一致。(4)上述细胞裂解液上清以Ni-NTA亲和层析法进行了纯化。纯化后的样品进行SDS-PAGE分析,显示分子量在32.6k Da的蛋白被纯化保留。(5)根据SDS-PAGE结果,Ni-NTA纯化的重组SEP蛋白中仍然存在一些微量的杂蛋白。据此,我们以Sephadex G-100进行了进一步纯化,随后以50%的饱和硫酸铵进行了选择性沉淀,进一步提高了重组蛋白SEP的纯度。(6)经Ni-NTA亲和层析纯化后,从SDS-PAGE凝胶中切下分子量为32.6k Da蛋白的条带做串联质谱鉴定。串联质谱共测得8条肽段的氨基酸残基序列,均与重组SEP蛋白的序列一致。(7)每毫克纯化的重组SEP蛋白与完全弗氏佐剂混合后免疫产蛋鸡三次,每次间隔15天。Dot Blotting分析初步表明,在第二次注射抗原后,IgY活性最高。

Objectives: To evaluate the association between magnesium levels, morbidity and mortality in critically ill elderly patients admitted in ICU. Methods: A cross-sectional study was done on patients admitted to the intensive care unit (ICU) of the Geriatrics department at a University Hospital over 1.5 years period. Data collected included patients demographics and medical history, length of stay in the ICU. Lab tests included serum Magnesium on admission, serum sodium and potassium. In addition, the Acute Physiology and Chronic Health Evaluation (APACHE) II score and the Sequential Organ Failure Assessment (SOFA) score were determined at the day of admission. Results: A single center, cross sectional study was done on 100 patients who were admitted to the Geriatrics medical ICU at Ain Shams university Hospital in Cairo. At admission 23% of patients had low magnesium (Mg) level and 59% had normal magnesium level and 18% had high Mg level. The mean age of patients with hypomagnesaemia was 71.69 years old while those with normal magnesium level were 69.5 years old and that with hypermagnesaemia were 69.7 years old. The length of ICU stay was longer among patients with high Mg level (11.7 ± 14.4 days) and among those with normal magnesium level (7.6 ± 5.8 days) versus (7.5 ± 3.8 days) those with low Mg level, among the studied groups (55.6%) of patients with Hypermagnesaemia died versus (44.1%) of patients with normal Mg level. Among the studied groups, those with high Mg level were found to have higher APACHE II score (20.7 ± 7.4) and SOFA score (5.5 ± 3.6) than the other subjects. Significant positive correlation was found between mean Mg level and SOFA score. Conclusion: Development of hypermagnesaemia during an ICU stay is associated with bad prognosis. Monitoring of serum magnesium levels may have prognostic, and perhaps therapeutic, implications.
