Protocol for the development and validation of a clinical measurement tool for fear of disease progression and recurrence in cardiac patients

Introduction: One in two cardiac patients fear having another heart event or their heart condition getting worse. research in other chronic illnesses demonstrates that screening for fear of progression and recurrence is vital for adequately addressing such concerns in clinical care. The current project aims to develop and validate a measure for fear of progression and recurrence in cardiac ***: The Fear of Cardiac recurrence and Progression Scale (FCrP) will be developed through a multistep process. An initial item pool will be generated through a review of the literature and existing measures and consultation with and feedback from key informants. The item pool will be tested in a sample of over 250 adults who have ever had an acute coronary event, undergone cardiac surgery, or a chronic cardiac condition. Exploratory factor analysis will be used to identify the underlying factors, and rasch analysis will be used to reduce the number of items. A short form version of the FCrP will be developed for use as a brief screening tool, informed by clinical relevance and rasch psychometric ***: while many cardiac patients experience fears related to the progression or recurrence of their illness, there remains the need for a validated tool with which these concerns can be identified and measured. It is expected that the design and validation of the FCrP will aid identification of cardiac patients suffering from clinically significant levels of fear of progression and recurrence and facilitate the design of tailored psychological interventions to target these fears.

Soil security and global food security

Over the course of the postglacial period has managed to add degrade a substantial portion of the world's potential agricultural land. The soil loss and degradation that has repeatedly impacted regional societies around the world resulted from agricultural practices that increased the physical loss of soil(erosion), reduced soil organic matter, changed p H(acidification) or salinity,and disrupted or altered communities of soil life. In the coming century, as continued soil degradation threatens global food security while the global population keeps rising it is imperative that farmers develop and adopt soilhealth building(regenerative) practices to solve a problem that has plagued societies throughout history. Growing evidence suggests that agricultural systems that combine cover crops, reduced tillage, and diverse crop rotations can reduce erosion, enhance soil health and rebuild soil organic matter to cultivate beneficial soil life and harvest both economic and environmental benefits. In the coming post-oil world, global food security would benefit from a global effort to promote soil restoration to help addresses the challenge of sustainably feeding the world, increase soil-based carbon sequestration,protect on-farm biodiversity and reduce off-farm water pollution. Because soil security sets a solid foundation for global food security, agricultural policies and subsidies should be reformed to encourage farmers to adopt regenerative,soil-building practices.

Towards a lightweight security framework using blockchain and machine learning

Cyber-attacks pose a significant challenge to the security of Internet of Things (IoT) sensor networks, necessitating the development of robust countermeasures tailored to their unique characteristics and limitations. Various prevention and detection techniques have been proposed to mitigate these attacks. In this paper, we propose an integrated security framework using blockchain and Machine Learning (ML) to protect IoT sensor networks. The framework consists of two modules: a blockchain prevention module and an ML detection module. The blockchain prevention module has two lightweight mechanisms: identity management and trust management. Identity management employs a lightweight Smart Contract (SC) to manage node registration and authentication, ensuring that unauthorized entities are prohibited from engaging in any tasks, while trust management uses a lightweight SC that is responsible for maintaining trust and credibility between sensor nodes throughout the network's lifetime and tracking historical node behaviors. Consensus and transaction validation are achieved through a Verifiable Byzantine Fault Tolerance (VBFT) mechanism to ensure network reliability and integrity. The ML detection module utilizes the Light Gradient Boosting Machine (LightGBM) algorithm to classify malicious nodes and notify the blockchain network if it must make decisions to mitigate their impacts. we investigate the performance of several off -the -shelf ML algorithms, including Logistic regression, Complement Naive Bayes, Nearest Centroid, and Stacking, using the wSN-DS dataset. LightGBM is selected following a detailed comparative analysis conducted using accuracy, precision, recall, F1 -score, processing time, training time, prediction time, computational complexity, and Matthews Correlation Coefficient (MCC) evaluation metrics.

A time-space network-based model for transportation service optimization of China railway Express

This paper describes the development and optimization plans for the China railway Express (Cr Express). As a new type of international land transport organization, Cr Express has emerged with the continuous expansion of China toward European investment and trade, and in particular, has expanded with the continuous progress of the One Belt and One road (OBOr) initiative. In addition to improving the service quality of Cr Express, the operating costs must be reduced for developing “smart railways” that serve “smart cities”. we propose a dual-objective-based function mathematical optimization model; the satisfaction of the cargo owner is considered, and the timeliness, transportation capacity, and goods category constraints of Cr Express transportation are designed. Moreover, we present the normalized equivalent method of the two-objective function of the model. Finally, a case study is conducted against the background of certain trains in the western corridor of Cr Express to validate the effectiveness of the model and research methods proposed in this study. © 2024 Beijing Jiaotong University

Basis for the diversity and extent in loss of digestible nutrients created by dietary phytin: Emphasis on fowl and swine

Phytin is the Ca 2+ -Mg 2+ -K + salt of phytic acid that is created and deposited in the aleurone layer and/or germ of grains and legumes. Its high presence in feedstuffs for fowl and swine diets results in it being a universal and significant impediment to optimum performance. Phytin impairs gastrointestinal recovery of a wide array of nutrients, the effect varying with the nutrient concerned. On exposure to low pH during gastric digestion, phytin dissociates into phytic acid and solubilized Ca 2+ . Even at low gastric pH, phytic acid is negatively charged which forms the basis of its anti-nutritive behavior. Pepsinogen has extensive basic amino acids on its activation peptide that are presented as cations at low pH which are targeted by pepsin for activation. Partially crystalized Ca 2+ near the enzyme's active site further stabilizes its newly formed structure. Thus, phytic acid appears to interfere with gastric digestion by several mechanisms; interfering with pepsinogen activation by binding to the polypeptide's basic amino acids; coordinating free Ca 2+ , destabilizing pepsin; binding some dietary proteins directly, further compromising gastric proteolysis. Upon digesta attaining neutrality in the duodenum, Ca 2+ and other cations re-bind with accessible anions, phytic acid being a significant contender. Phytate not only binds free cations but can also strip them from enzymes (e.g. Ca 2+ , Zn 2+ ) which reduces their structural resistance to autolysis and ability as co-factors (e.g. Zn 2+ ) to increase enzyme activity. Goblet cells initially employ Ca 2+ as an electronic shield between mucin layers enabling granule formation and cell storage. After mucin granule release, Ca 2+ is progressively displaced by Na + to free the viscous mucins enabling its translocation. Mucin entangles with the glycocalyx of adjacent enterocytes thereby constructing the unstirred water layer (USwL). Excessive removal of Ca 2+ from mucin by phytic acid increases its fluidity facilit
