Endoscopic biliary treatment of unresectable cholangiocarcinoma: A meta-analysis of survival outcomes and systematic review

BACKGROUNd Endoscopic radiofrequency ablation(ERFA),percutaneous radiofrequency ablation(PRFA),and photodynamic therapy(PdT),when used in conjunction with conventional biliary stenting,have demonstrated a survival benefit in patients with unresectable *** To compare pooled survival outcomes,adverse event rates,and mean stent patency for those undergoing these *** A comprehensive literature review of published studies and abstracts from January 2011 to december 2020 was performed comparing survival outcomes in patients undergoing ERFA with stenting,biliary stenting alone,PRFA with stenting,and PdT with stenting for unresectable cholangiocarcinoma(CCA).RESULTS data from four studies demonstrated a pooled mean survival favoring ERFA as compared to biliary stenting alone(12.0±0.9 mo vs 6.8±0.3 mo,Pd median survival time(13 mo vs 8 mo,Pd PRFA with stenting groups demonstrated statistical superiority to biliary stenting alone(Pd P=0.004,respectively).However,when comparing ERFA to PRFA,pooled data demonstrated overall higher mean survival in the ERFA with stenting cohort as compared to PRFA with stent cohort(12.0+0.9 mo vs 8.1+2.1 mo,Pdata from two studies demonstrated a pooled median survival favoring ERFA with stenting as compared to PdT with stenting(11.3 mo vs 8.5 mo,P=0.02).CONCLUSION While further prospective,randomized studies are needed to assess efficacy of ERFA,our metaanalysis demonstrated that this technique offers endoscopists a reasonable palliative method by which to treat patients with unresectable CCA that results in longer survival as compared to biliary stenting alone,percutaneous radiofrequency ablation with biliary stenting,and PdT with biliary stenting as well as an acceptable adverse event profile based on available published data.

AIM: To ascertain fine needle aspiration(FNA) tech-niques by endosonographers with varying levels of ex-perience and ***: A survey study was performed on United States based endosonographers. The subjects complet-ed an anonymous online electronic survey. The main outcome measurements were differences in needle choice, FNA technique, and clinical decision making among endosonographers and how this relates to years in practice, volume of EUS-FNA procedures, and prac-tice ***: A total of 210(30.8%) endosonographers completed the survey. Just over half(51.4%) identified themselves as academic/university-based practitioners. The vast majority of respondents(77.1%) identified themselves as high-volume endoscopic ultrasound(EUS)(> 150 EUS/year) and high-volume FNA(> 75 FNA/year) performers(73.3). If final cytology is non-diagnostic, high-volume EUS physicians were more likely than low volume physicians to repeat FNA with a core needle(60.5% vs 31.2%; P = 0.0004), and low volume physicians were more likely to refer patients for either surgical or percutaneous biopsy,(33.4% vs 4.9%, P demic physicians were more likely to repeat FNA with a core needle(66.7%) compared to community physicians(40.2%, P d States endosonographers. differences appear to be related to EUS volume and practice environment.

Good nutrition plays a crucial role in maintaining a balanced lifestyle. The beneficial effects of nutrition have been found to counteract nutritional disturbances with the expanded use of nutraceuticals to treat and manage cardiovascular diseases, cancer, and other developmental defects over the last decade. Flavonoids are found abundantly in plant-derived foods such as fruits, vegetables, tea, cocoa, and wine. Fruits and vegetables contain phytochemicals like flavonoids, phenolics, alkaloids, saponins, and terpenoids. Flavonoids can act as anti-inflammatory, anti-allergic, anti-microbial(antibacterial, antifungal, and antiviral) antioxidant, anti-cancer, and anti-diarrheal agents. Flavonoids are also reported to upregulate apoptotic activity in several cancers such as hepatic, pancreatic, breast, esophageal, and colon. Myricetin is a flavonol which is naturally present in fruits and vegetables and has shown possible nutraceutical value. Myricetin has been portrayed as a potent nutraceutical that may protect against cancer. The focus of the present review is to present an updated account of studies demonstrating the anticancer potential of myricetin and the molecular mechanisms involved therein. A better understanding of the molecular mechanism(s) underlying its anticancer activity would eventually help in its development as a novel anticancer nutraceutical having minimal side effects.

3d object recognition is a challenging task for intelligent and robot systems in industrial and home indoor *** is critical for such systems to recognize and segment the 3d object instances that they encounter on a frequent *** computer vision,graphics,and machine learning fields have all given it a lot of ***,3d segmentation was done with hand-crafted features and designed approaches that didn’t achieve acceptable performance and couldn’t be generalized to large-scale *** learning approaches have lately become the preferred method for 3d segmentation challenges by their great success in 2d computer ***,the task of instance segmentation is currently less *** this paper,we propose a novel approach for efficient 3d instance segmentation using red green blue and depth(RGB-d)data based on deep *** 2d region based convolutional neural networks(Mask R-CNN)deep learning model with point based rending module is adapted to integrate with depth information to recognize and segment 3d instances of *** order to generate 3d point cloud coordinates(x,y,z),segmented 2d pixels(u,v)of recognized object regions in the RGB image are merged into(u,v)points of the depth ***,we conducted an experiment and analysis to compare our proposed method from various points of view and *** experimentation shows the proposed 3d object recognition and instance segmentation are sufficiently beneficial to support object handling in robotic and intelligent systems.

Petrogenesis of the Late Cretaceous Tholeiitic Volcanism and Oceanic Island Arc Affinity of the Chagai Arc, Western Pakistan

The Late Cretaceous Chagai arc outcrops in western Pakistan, southern Afghanistan and eastern Iran. It is in the Tethyan convergence zone, formed by northward subduction of the Arabian oceanic plate beneath the Afghan block. The oldest unit of the Chagai arc is the Late Cretaceous Sinjrani Volcanic Group. This is composed of porphyritic lava flows and volcaniclastic rocks, and subordinate shale, sandstone, limestone and chert. The flows are fractionated low-K tholeiitic basalts, basaltic- andesites, and andesites. Relative enrichment in their LILE and depletion in HFSE, and negative Nb and Ta and positive K, Ba and Sr anomalies point to a subduction-related origin. Compared to MORB, the least fractionated Chagai basalts have low Na2O, Fe2O3^T, CaO, Ti, Zr, Y and ^87Sr/^86Sr. Rather than an Andean setting, these results suggest derivation from a highly depleted mantle in an intraoceanic arc formed by Late Cretaceous convergence in the Ceno-Tethys. The segmented subduction zone formed between Gondwana and a collage of small continental blocks （Iran, Afghan, Karakoram, Lhasa and Burma） was accompanied by a chain of oceanic island arcs and suprasubduction ophiolites including Semail, Zagros, Chagai-Raskoh, Kandahar, Muslim Bagh, Waziristan and Kohistan-Ladakh, Nidar, Nagaland and Manipur. These complexes accreted to the southern margin of Eurasia in the Late Cretaceous.

due to their complex structure,2-d models are challenging to work with;additionally,simulation,analysis,design,and control get increasingly difficult as the order of the model ***,in particular time intervals,Gawronski and Juang’s time-limited model reduction schemes produce an unstable reduced-order model for the 2-d and 1-d *** revealed some stability preservation solutions to address this key flaw which ensure the stability of 1-d reduced-order systems;nevertheless,these strategies result in large approximation ***,to the best of the authors’knowledge,there is no literature available for the stability preserving time-limited-interval Gramian-based model reduction framework for the 2-d discrete-time *** this article,2-d models are decomposed into two separate sub-models(i.e.,two cascaded 1-d models)using the condition of minimal *** reduction procedures are conducted on these obtained two 1-d sub-models using limited-time *** suggested methodology works for both 2-d and 1-d ***,the suggested methodology gives the stability of the reduced model as well as a priori error-bound expressions for the 2-d and 1-d *** results and comparisons between existing and suggested methodologies are provided to demonstrate the effectiveness of the suggested methodology.

Objective:To delineate immunomodulatory role of histamine on antibody generation pr of ile in rabbit in the present dose-dependent histamine ***:The cohort comprised of three groups(Ⅲ,ⅣandⅤ),containing six rabbits each,and received subcutaneous histamine 50μg/kg×bis in die(b.i.d.),100μg/kg×*** 200μg/kg X b.i.d.,respectively for 10 days (starting from the 1st day).They were subsequently immunized on the 3rd day with intravenous injection of sheep blood cell(SRBC)(1×10 cells/mL).GroupⅡ(positive control)(n=6) received vehicle(sterile distilled water) and immunized at day 3 similarly while groupⅠ(negative control) (n=6) remained non-immunized and received only *** experimentations were performed in *** samples were collected on pre-immunization(pre-I)(day 0),as well as on days 7-,14-,21-,28- and 58- post-immunization(post-I).Immunological parameters[total immunoglobulins(Igs),IgM and IgG]were analyzed by enzyme linked immunosorbent assay (ELISA) ***:Histamine could influence a detectable antibody response to SRBC as early as day 7-post-I,which lasted until day 58- *** results were found statistically significant(Pde evidence that histamine has a short-term effect on antibody generation(until its presence in the body),and the antibody generation titer in vivo were affected by the concentration of histamine.

Objectives:To determine the learning curve(LC)of total operative time and the discrete components of the robotic-assisted radical prostatectomy(RARP)for a recent robotic fellowship-trained urologic *** and methods:We performed a retrospective analysis of RARP procedures performed by a single new attending surgeon from August 2015 to April ***'demographics and operative details were *** operative time was divided and prospectively recorded in 7 parts:(a)docking robot,(b)dissecting seminal vesicles(SVs)(c)dissecting endopelvic fascia(EPF),(d)incising bladder neck(BN),(e)completing the dissection,(f)lymph node dissection,and(g)urethrovesical anastomosis(UVA)and robot *** sum analysis was used to ascertain the LC for total operative time and the 7 parts of the ***:One hundred twenty consecutive RARPs were *** LC was overcome at 25 cases for total operative time,13 cases for docking the robot,33 cases for dissecting SVs,31 cases for dissecting EPF,46 cases for incising BN,38 cases for prostate dissection,25 cases for lymph node dissection,and 52 cases for *** operative time was decreased 22.8%(pd time for robot docking,dissecting SVs,dissecting EPF,incising BN,completing prostate dissection,lymph node dissection,and UVA were decreased 16.7%,30.5%,29.5%,36.2%,37.3%,32.2%,and 26.9%,respectively(all pd a 25-case LC for a fellowship-trained urologist to achieve stable operative performance of RARP *** components demonstrated variable LCs including the UVA that required upward of 52 cases.

Neuroprotective Effect of Chrysophanol as a PI3K/AKT/mTOR Signaling Inhibitor in an Experimental Model of Autologous Blood-induced Intracerebral Hemorrhage

Objective Intracerebral hemorrhage(ICH)refers to predominant,sporadic,and non-traumatic bleeding in the brain *** PI3K/AKT/mTOR signaling pathway is an important signal transduction pathway regulated by enzyme-linked receptors and has many biological functions in *** plays a key role in neuronal metabolism,gene expression regulation,and tissue homeostasis in the healthy and diseased *** In the present study,the role of the PI3K/AKT/mTOR pathway inhibitor chrysophanol(CPH)(10 mg/kg and 20 mg/kg,orally)in the improvement of ICH-associated neurological defects in rats was *** blood(20µL/5 min/unilateral/intracerebroventricular)mimics ICH-like defects involving cellular and molecular dysfunction and neurotransmitter *** current study also included various behavioral assessments to examine cognition,memory,and motor and neuromuscular *** protein expression levels of PI3K,AKT,and mTOR as well as myelin basic protein and apoptotic markers,such as Bax,Bcl-2,and caspase-3,were examined using ELISA ***,the levels of various neuroinflammatory cytokines and oxidative stress markers were ***,the neurological severity score,brain water content,gross brain pathology,and hematoma size were used to indicate neurological function and brain *** CPH was found to be neuroprotective by restoring neurobehavioral alterations and significantly reducing the elevated PI3K,AKT,and mTOR protein levels,and modulating the apoptotic markers such as Bax,Bcl-2,and caspase-3 in rat brain *** substantially reduced the inflammatory cytokines like interleukin(IL)-1β,IL-6,and tumor necrosis factor-α.CPH administration restored the neurotransmitters GABA,glutamate,acetylcholine,dopamine,and various oxidative stress *** Our results show that CPH may be a promising therapeutic approach for overcoming neuronal damage caused by the overexpression of the PI3K/AKT/mTOR signal
