Radical Prostatectomy by Laparotomy: Retrospective Study Comparing Separate and Continuous Stitches in the Performance of Vesicourethral Anastomosis at a Single Center in the Public Health Network

Background: Prostate cancer ranks as the second most prevalent malignancy in men, with treatment strategies ranging from observation to various interventions. Radical prostatectomy is a common approach, aiming for a cure, and the technique for vesico-urethral anastomosis varies, employing separate or continuous stitches. However, a consensus on the optimal technique is lacking. This study aims to compare vesico-urethral anastomosis techniques (separate vs. continuous stitches) in laparotomy-based radical prostatectomies, evaluating peri and postoperative outcomes. Materials and Method: A retrospective analysis of 140 patients’ medical records yielded 86 eligible cases, divided into two groups based on anastomosis technique. Both groups underwent surgery at the same urology center between 2016 and 2019, with a follow-up period exceeding 12 months. Pre-operative characteristics were statistically similar between groups. Results: Perioperative complication rates did not significantly differ between the two techniques (p > 0.99). However, the continuous stitch group experienced a 20-minute longer procedure time (p 0.05), patients with continuous stitches had 2.5 times higher diaper usage at twelve months (p Conclusion: This research suggests that vesico-urethral anastomosis with separate stitches yields superior results in laparotomy-based radical prostatectomies compared to continuous stitches, demonstrating benefits in surgical time, hospital stay, sentinel drain duration, stenosis rates, and long-term urinary incontinence outcomes.

Candidiasis, also known as candidiasis vulvovaginitis, is an infection caused by different types of Candida fungi, the most frequent being Candida albicans. The present study reports an effective strategy, which opens new avenues for the treatment of this public health problem. The MAC® Methodology, conventional laser light-emitting (LLLT)/LED) methods are based on the biphasic response demonstrated many times in LLLT research and as with other forms of drugs, a “drug” (irradiation parameters) and a “dose” (irradiation times) and the “Arndt-Schulz Law” is often cited as a suitable model to describe the dose-dependent effects of LLLT. This method uses photopharmaceuticals, cell markers and the use of correct parameters for each case to induce the acceleration of tissue repair. The present study shows a case of a 32-year-old patient diagnosed with recurrent candidiasis 4 years ago. Eighteen sessions were performed (every other day) using a photoactivated component (Methylene blue 1% + Clotrimazole 1%) and LED phototherapy (red, blue and violet) with emission times of 60 - 260 seconds for each applicator, according to the dose recommendations of the scar acceleration method (MAC®). At the sixth treatment session there was a noticeable decrease in the itching sensation reported by the patient. In session 11 she reported feeling a great improvement, indicating that she no longer felt itching in any area after 18 sessions. The present case demonstrates new methodologies to treat common problems in the population that have a positive impact on the quality of life. This methodology has a promising future because it is non-invasive and requires a great biological transformation for inflammatory, fungal and viral control.

Background: Elevated anxiety levels are common in patients on mechanical ventilation (MV) and may challenge recovery. Research suggests music-based interventions may reduce anxiety during MV. However, studies investigating specific music therapy techniques, addressing psychological and physiological well-being in patients on MV, are scarce. Methods: This three-arm randomized clinical pilot study was conducted with MV patients admitted to the intensive care unit (ICU) of Hospital San José in Bogotá, Colombia between March 7, 2022, and July 11, 2022. Patients were divided into three groups: intervention group 1 (IG1), music-assisted relaxation; intervention group 2 (IG2), patient-preferred therapeutic music listening; and control group (CG), standard care. The main outcome measure was the 6-item State-Anxiety Inventory. Secondary outcomes were: pain, measured with a visual analog scale; resilience, measured with the Brief Resilience Scale; agitation/sedation, measured with the Richmond Agitation–Sedation Scale; vital signs, including heart rate, blood pressure, oxygen saturation, and respiratory rate; days of MV; extubation success; and days in the ICU. Additionally, three patients underwent electroencephalography during the interventions. Results: Data from 23 patients were analyzed in this study. The age range of the patients was 24–84 years, with a median age of 66 years (interquartile range: 57–74). Of the 23 patients, 19 were female (82.6%). No statistically significant differences between the groups were observed for anxiety (P=0.330), pain (P=0.624), resilience (P=0.916), agitation/sedation (P=0.273), length of ICU stay (P=0.785), or vital signs. A statistically significant difference between the groups was found for days of MV (P=0.019). Automated image systems to characterize aquatic organisms improve research and enable fast response to environmental risk situations. In November 2015, a dam in Mariana City-MG (Brazil) collapsed and led to the disposal of mud tailings from the mining process to the Doce River. The accident resulted in several casualties and incalculable damage to surrounding communities and the environment. The mud increased water turbidity, an essential condition to the functioning of the image analysis systems, and directly affected the characterization of the organisms, making it impossible to distinguish copepods in the mud, due to the blurred outline. To get a quick response evaluating environmental situations, this work aimed to develop and test different algorithms characterizing and classifying copepods by their size (length and area) using in situ images acquired by the Lightframe On-Sight Keyspecies Investigation device. Field tests were carried out under different turbidity levels throughout the gradient observed in the coastal zone adjacent to the Doce River. The best algorithm reduced nearly 50% of the noise in some images when compared with manual treatment and led to 96% accuracy in measurement and counting. Semi-automated devices that perform post-processing corrections are suitable for fast environmental evaluation under high turbidity scenarios.

Background: We currently have international and national guidelines regarding the assessment and monitoring of clinical signs and humane endpoints in animals used in teaching and research, which make the performance of these activities mandatory for any experiment and professional working in this area. Assigning the severity of a research experiment is the result of an analysis of records of observations of the animal’s behavior, and clinical signs. The aim of this study was to describe the importance of carrying out a severity assessment associated with clinical and behavioral monitoring of rodents and rabbits during experimentation to maintain the welfare of these animals undergoing scientific research. Methods: The literature search was carried out using the following terms: “Monitoring”;“Humane endpoints”;“Animal welfare”, “Rodents”;“Rabbits”, and as connectors “and”;“or”, in the following databases: PubMed;LILACS/BIREME and SciELO. Results: A total of 987 articles were identified in the databases, and 20 of these studies were included in this review. Conclusions: Humane endpoint protocols and procedure severity tables are of the utmost importance, both from an ethical point and to refine the results of research conducted on laboratory animals. They should be drawn up jointly by the teams responsible for the project and the maintenance of the animals during the research period, and the data obtained should be published so that the scientific community can have access to it, helping to disseminate these practices, as well as helping to draw up new procedures. Monitoring and evaluating the welfare and clinical condition of animals undergoing scientific research procedures is the responsibility of the professors, researchers, veterinarians, and animal facility coordinators. Ground penetrating radar (GPR) surveys have being applied to investigate very near-surface stratification of sedimentary units in coastal plains and to define their depositional conditions. This paper presents, however, low-frequency GPR survey to investigate fault-related depositional systems at greater depths. The Quinta-Cassino area in the Rio Grande do Sul Coastal Plain (RGSCP, Brazil) shows a wide strandplain that is made off by very long, continuous, and linear geomorphic features (beach ridges). This strandplain extends for ~70 km southward. The beach ridges show low-angle truncations against the Quinta escarpment, and also truncations in the strandplain. The traditional approach points that RGSCP was developed by juxtaposition of four lagoons/barrier systems as consequence of sea level changes;previous model assumes that no deformational episode occurred in RGSCP. The geophysical and geological surveys carried out in this area showed the existence of listric fault controlling the beach ridges in the escarpments and hanging-wall blocks. The radargrams could distinguish Pleistocene basement unit anticlockwise rotation, thickening of beach ridges radarfacies close to listric normal faults, and horst structures. These deformational features indicate that the extensional zone of a large-scale gravity-driven structure controlled the mechanical subsidence, the Holocene sedimentation and its stratigraphic and geomorphic features in the Quinta-Cassino area to build up an asymmetric delta. The results point to a new approach in dealing with RGSCP Holocene evolution.
