BACKGROUND Acute carpal tunnel syndrome(ACTS)is commonly caused by repetitive strain,trauma,or inflammatory ***,ACTS due to tophaceous gout is a clinical event that remains poorly understood and *** rare manifestation necessitates prompt diagnosis and intervention to prevent irreversible *** SUMMARY A 51-year-old man who had poorly controlled hyperuricemia presented with ACTS secondary to tophaceous *** of rapid symptom progression symptoms and severe median nerve compression within 3 mo,the patient underwent emergency decompression surgery for both wrists at different time ***,he exhibited complete recovery of sensory and motor functions,with no recurrence at long-term *** outcomes were achieved through immediate decompression surgery,anti-inflammatory medications,postoperative active and passive range-of-motion exercises,and intermittent wrist *** diagnosis and surgical intervention,when necessary,are crucial for preventing long-term complications and obtaining favorable outcomes in patients with *** optimal gout management strategy involving pharmacologic therapy and lifestyle modifications may help minimize ACTS recurrence and improve clinical *** Prompt surgical intervention and optimal gout management are crucial for preventing irreversible nerve damage and ACTS recurrence.

BACKGROUND This case report contributes to the medical literature by highlighting the successful management of a neglected femoral neck fracture in a patient with renal osteodystrophy and secondary hyperparathyroidism(SHPTH)who was on dialysis due to end-stage renal disease(ESRD).It underscores the efficacy of parathyroidectomy(PTX)in restoring bone mineral density(BMD)and promoting fracture healing,addressing a significant complication in ESRD *** SUMMARY A 36-year-old female with renal osteodystrophy and on dialysis due to ESRD presented with a history of left patellar tendon rupture and later,a right achilles tendon avulsion *** right hip pain led to the discovery of a neglected right femoral neck fracture,which was initially overlooked due to the patient’s complex medical *** months post-achilles tendon repair,the patient underwent PTX to manage the refractory *** postoperative course included rehabilitation and weight-bearing ***,2 years after osteosynthesis,radiographic assessments indicated a solid union of the periprothesis fracture and significant improvement in BMD,showcasing the efficacy of the treatment *** PTX,combined with appropriate rehabilitation,is crucial for improving BMD and fracture healing in ESRD patients with SHPTH.

Oxidation mechanism of aluminum nitride revisited

Different from the oxidation kinetics of other nitrides, the oxide layer on AlN can easily reach tens of micrometers at a temperature above 1200 °C. In the present study, the oxidation mechanism of AlN is investigated through microstructure observation. The analysis indicates that the oxide layer is full of small pores. The formation of pores generates additional surface area to induce further reaction. The reaction thus controls the oxidation in the temperature range from 1050 to 1350 °C. The oxidation rate becomes slow as the oxide layer reaches a critical thickness.

A high-fidelity design methodology using LES-based simulation and POD-based emulation:A case study of swirl injectors

Engineering design is undergoing a paradigm shift from design for performance to design for affordability, operability, and durability, seeking multi-objective optimization. To facilitate this transformation, significantly extended design freedom and knowledge must be available in the early design stages. This paper presents a high-fidelity framework for design and optimization of the liquid swirl injectors that are widely used in aerospace propulsion and power-generation systems. The framework assembles a set of techniques, including Design Of Experiment(DOE), high-fidelity Large Eddy Simulations(LES), machine learning, Proper Orthogonal Decomposition(POD)-based Kriging surrogate modeling(emulation), inverse problem optimization, and uncertainty quantification. LES-based simulations can reveal detailed spatiotemporal evolution of flow structures and flame dynamics in a high-fidelity manner, and identify important injector design parameters according to their effects on propellant mixing, flame stabilization, and thermal *** a given a space of design parameters, DOE determines the number of design points to perform LES-based simulations. POD-based emulations, trained by the LES database, can effectively explore the design space and deduce an optimal group of design parameters in a turn-around time that is reduced by three orders of magnitude. The accuracy of the emulated results is validated, and the uncertainty of prediction is quantified. The proposed design methodology is expected to profoundly extend the knowledge base and reduce the cost for initial design stages.

Comparison of murine cirrhosis models induced by hepatotoxin administration and common bile duct ligation

AIM: To build up the research models of hepatic fibrosis in ***: Inbred wild-type FVB/N mice were either treated with alpha-naphthyl-isothiocyanate (ANIT), allyl alcohol (AA),carbon tetrachloride (CCl4), 3,5-diethoxycarbonyl-1,4-dihydrocollidine (DDC), and silica, or subjected to common bile duct ligation (CBDL) to induce hepatic injury. Liver biopsies were performed every 4 wk to evaluate hepatic fibrosis over a period of 6 mo. Cumulative cirrhosis and survival curves were constructed by life table method and compared with Wilcoxon ***: Under the dosages used, there was neither mortality nor cirrhosis in AA and silica-treated groups. DDC and ANIT caused cirrhosis within 4-12 and 12-24 wk, *** showed significantly faster cirrhosis induction at high dosages without significant alteration of survival. The duration for cirrhosis induction by CCl4 ranged from 4 to 20 wk, mainly dependent upon the dosage. However, the increase in CCl4dosage significantly worsened survival. Intraperitoneal CCl4administration resulted in better survival in comparison with gavage administration at high dosage, but not at medium and low dosages. After CBDL, all the mice developed liver cirrhosis within 4-8 wk and then died by the end of 16 ***: CBDL and administrations of ANIT, CCl4, and DDC ensured liver cirrhosis. CBDL required the least amount of time in cirrhosis induction, but caused shortened lives of mice. It was followed by DDC and ANIT administration with favorable survival. As for CCl4, the speed of cirrhosis induction and the mouse survival depended upon the dosages and the administration route.

全直肠系膜切除(TME)理念及微创技术(如腹腔镜结直肠手术和机器人手术)的出现大大提高直肠癌的外科疗效。然而,术后排尿功能和性功能障碍率仍然较高,从而给患者造成巨大的痛苦,同时也会给手术医生带来困扰。新辅助和辅助放疗应用的增多也带来了更多的功能问题。在本文中,我们对盆腔神经解剖的目前认识进行了评价,将盆腔神经分为肠系膜下动脉根部、侧盆壁及泌尿生殖器官周围等区域,探讨了在这些区域如何进行手术操作。同时,我们也讨论了过去30年不同手术方式(包括开腹、腹腔镜和机器人)的功能状况。

Effects of sex and generation on hepatitis B viral load in families with hepatocellular carcinoma

AIM To explore factors associated with persistent hepatitis B virus(HBV) infection in a cohort of hepatocellular carcinoma(HCC)-affected families and then investigate factors that correlate with individual viral load among hepatitis B surface antigen(HBsA g)-positive relatives. METHODS We evaluated non-genetic factors associated with HBV replication in relatives of patients with HCC. Relatives of 355 HCC cases were interviewed using a structuredquestionnaire. Demographics, relationship to index case, HBs Ag status of mothers and index cases were evaluated for association with the HBV persistent infection or viral load by generalized estimating equation analysis. RESULTS Among 729 relatives enrolled, parent generation(P = 0.0076), index generation(P = 0.0044), mothers positive for HBs Ag(P = 0.0007), and HBs Ag-positive index cases(P = 5.98 × 10-8) were associated with persistent HBV infection. Factors associated with HBV viral load were evaluated among 303 HBs Ag-positive relatives. Parent generation(P = 0.0359) and sex(P = 0.0007) were independent factors associated with HBV viral load. The intra-family HBV viral load was evaluated in families clustered with HBs Ag-positive siblings. An intra-family trend of similar HBV viral load was found for 27 of 46(58.7%) families. Male offspring of HBsA gpositive mothers(P = 0.024) and older siblings were associated with high viral *** Sex and generation play important roles on HBV viral load. Maternal birth age and nutritional changes could be the reasons of viral load difference between generations.
