Cardiac Radiosurgery for the Treatment of Atrial Fibrillation

An estimated 4 million Americans currently have atrial fibrillation (AF), the most common form of cardiac arrhythmia. The annual growth rate of AF is approximately 9.2% and advanced statistical analyses project that the prevalence of AF will be 12.1 million in the United States by 2030. The current therapeutic options to treat AF put certain patient groups at a high risk for a serious bleeding event, stroke or thromboembolism, myocardial infarction, and death. The most effective treatment for AF—catheter ablation—has been widely reported to require repeat operations and only leaves 59.4% of patients AF-free after five years. A majority of the reported complications and increased risks for catheter ablation occur because the procedure is invasive. Thus, there is a need for a novel, non-invasive therapeutic option to treat AF that can potentially expand the treatable patient population, increase patient safety, and improve clinical outcomes. CyberHeart Inc. has been exploring the efficacy and potential of cardiac radiosurgery as an option to treat heart conditions such as AF. With its proprietary CardioPlanTM software, the CyberHeart System utilizes the Cyberknife stereotactic radiosurgery platform to non-invasively ablate the heart. Early studies have displayed that the CyberHeart System is highly accurate at delivering radiation to cardiac targets and effective at minimizing radiation exposure to healthy tissue that is in close proximity to the targeted anatomic region. The focus of this review is to present the current standard of treatment for AF, identify the specific patient population that is at a high risk for the current treatment modalities, and address the potential for the CyberHeart Stereotactic Radiosurgery System to noninvasively ablate cardiac tissue and return sinus rhythm to patients with AF.

Variations in Typhoon Landfalls over China

The interannual variability in typhoon landfalls over China is investigated using historical and modern records. For the purpose of substantiating and elaborating upon the claim of north to south variation in tropical cyclone activity over China, a north-to-south anti-correlation in yearly activity is confirmed in the historical cyclone records. When cyclone activity over the province of Guangdong is high （low）, it tends to be low （high） over the province of Fujian. A similar spatial variation is identified in the modern records using a factor analysis model, which delineates typhoon activity over the southern provinces of Guangdong and Hainan from the activity over the northern provinces of Fujian, Taiwan, Zhejiang, Shanghai, Jiangsu, and Shandong. A landfall index of typhoon activity representing the degree to which each year follows this pattern of activity is used to identify correlated climate variables. A useful statistical regression model that includes sea-level pressure differences between Mongolia and western China and sea-surface temperature （SST） over the northwestern Pacific Ocean during the summer explains 26% of the interannual variability of the landfall index. It is suggested that a stronger than normal north-south pressure gradient increases the surface easterly wind flow over northern China： this, coupled with lower SST over the Pacific, favors typhoons taking a more westerly track toward southern China.

This article describes recent advances in the capability of new satellite sensors for observing Tropical Cyclones(TC)fine structure,wind field,and temporal *** article is based on a World Meteorological Organization(WMO)report prepared for the 10th International Workshop on Tropical Cyclones(IWTC),held in Bali in December 2022,and its objective is to present updates in TC research and operation every four *** we focus on updates regarding the most recent space-based TC observations,and we cover new methodologies and techniques using polar orbiting sensors,such as C-band synthetic aperture radars(SARs),L-band and combined C/X-band radiometers,scatterometers,and microwave imagers/*** additionally address progress made with the new generation of geostationary and small satellites,and discuss future sensors planned to be launched in the next *** then briefly describe some examples on how the newest sensors are used in operations and data assimilation for TC forecasting and research,and conclude the article with a discussion on the remaining challenges of TC space-based observations and possible ways to address them in the near future.

The standard methods for regression analyses of clustered riverine larval habitat data of Simulium damnosum s.l.a major black-fly vector of onchoceriasis,postulate models relating observational ecological-sampled parameter estimators to prolific habitats without accounting for residual intra-cluster error correlation ***,this correlation comes from two sources:(1)the design of the random effects and their assumed covariance from the multiple levels within the regression model and(2)the correlation structure of the ***,inconspicuous errors in residual intracluster correlation estimates can overstate precision in forecasted *** *** larval habitat explanatory attributes regardless how they are treated(***,autoregressive,Toeplitz,etc.).In this research,the geographical locations for multiple riverine-based *** *** ecosystem habitats sampled from two preestablished epidemiological sites in Togo were identified and recorded from July 2009 to June ***,the data were aggregated into PROC *** agglomerative hierarchical residual cluster-based analysis was then *** sampled clustered study site data was then analyzed for statistical correlations using monthly biting rates(MBR).Euclidean distance measurements and terrain-related geomorphological statistics were then generated in ArcGIS.A digital overlay was then performed also in ArcGIS using the georeferenced ground coordinates of high and low density clusters stratified by annual biting rates(ABR).The data was overlain onto multitemporal sub-meter pixel resolution satellite data(*** 0.61m wavbands).Orthogonal spatial filter eigenvectors were then generated in SAS/Geographic Information Systems(GIS).Univariate and nonlinear regression-based models(***,Poisson,and negative binomial)were also employed to determine probability distributions and to identify statistically significant parameter estimators from the

In July 2023,the Center of Excellence in Respiratory Pathogens organized a two-day workshop on infectious diseases modelling and the lessons learnt from the Covid-19 *** report summarizes the rich discussions that occurred during the *** workshop participants discussed multisource data integration and highlighted the benefits of combining traditional surveillance with more novel data sources like mobility data,social media,and wastewater *** advancements were noted in the development of predictive models,with examples from various countries showcasing the use of machine learning and artificial intelligence in detecting and monitoring disease *** role of open collaboration between various stakeholders in modelling was stressed,advocating for the continuation of such partnerships beyond the pandemic.A major gap identified was the absence of a common international framework for data sharing,which is crucial for global pandemic ***,the workshop underscored the need for robust,adaptable modelling frameworks and the integration of different data sources and collaboration across sectors,as key elements in enhancing future pandemic response and preparedness.

背景:全球均有对广泛耐药结核病的文献报道,但对儿童广泛耐药结核病的文献报道非常罕见。目的:报告南非农村地区广泛耐药结核病患儿的特征。设计:在南非共和国图盖拉渡口地区,对2006年1月—2007年12月间登记的经痰培养确诊为广泛耐药结核病的患儿进行了回顾性评估。从患儿病案中获得其人口统计学、临床和微生物学信息。结果:4例6～8岁的儿童为广泛耐药结核病患者。其中2例患儿先前有结核病史。4例患儿均为感染了人类免疫缺陷病毒的孤儿,其中3例在诊断患有广泛耐药结核病之前已接受了高效抗逆转录病毒治疗。在接受标准化抗广泛耐药结核病和高效抗逆转录病毒治疗后,所有患儿的临床表现、影像学均改善,痰培养阴转。其中2例患儿较好地耐受了抗广泛耐药结核病和高效抗逆转录病毒同时治疗。另外2例出现了环丝氨酸相关的神经精神系统不良反应。4例患儿的存活时间均超过24个月,广泛耐药结核病均得到治愈。在诊断为广泛耐药结核病之前,这些患儿在儿科病房的平均住院时间为8个月(5～17个月),包括3个月的住院重叠期。结论:广泛耐药结核病依靠微生物学来确诊,合并艾滋病的广泛耐药结核病患者也能被成功诊断。尽管需要对同时接受两种治疗的潜在交叉毒性开展更多的研究,但本研究显示患儿同时接受抗广泛耐药结核病和高效抗逆转录病毒治疗是可行且有效的。
