Treatment of patients with advanced gastrointestinal stromal tumor of small bowel: Implications of imatinib mesylate

AIM： To examine the impact of imatinib mesylate （Glivec） on patient survival and response and its safety, and the correlation of the response rate with the kit gene mutation status. METHODS： Thirty-three of 74 （44.6%） small bowel gastrointestinal stromal tumor （GIST） patients who developed recurrence after curative resection and not treated with Glivec were classified as group A patients. Twenty-two advanced small bowel GIST patients treated with Glivec were classified as group B patients. Clinicopathological features, post-recurrence and overall survival rates were compared. Each tumor in group B patients was investigated for mutations of kit or plateletderived growth factor alpha （PDGFRA）. The mutation type was correlated with clinical outcomes. The antitumor effect and safety of Glivec in group B patients were also assessed. RESULTS： Advanced small bowel GIST patients treated with Glivec had substatntially longer post-recurrence survival and higher overall survival rates than those not treated with Glivec. A total of 15 patients had a partial response （PR）（67.8%）. Activated mutations of c-kit were found in 16 of 19 tested patients and no PDGFRA mutant was identified. In 13 patients with GISTs harboring exon 11 kit mutations, the partial response rate （PR） was 69.3%, whereas two of three patients with tumors containing an exon 9 kit mutation had an overall response rate （ORR） of 66.7% （not significant）. CONCLUSION： Glivec significantly prolongs the postrecurrence and overall survival of Asian patients with advanced GISTs. Glivec induces a sustained objective response in more than half of Asian patients withadvanced small bowel GISTs. Activated mutations of kit exon 11 are detectable in the vast majority of GISTs. There is no difference in the PR rate for patients whose GISTs have kit exon 9 and exon 11 mutations.

BACKGROUND Anaplastic sarcoma of the kidney(ASK)is a rare and newly recognized renal *** tumor usually is extensive and cystic,characterized by pleomorphic spindle cells with marked atypia and associated with multinucleated *** date,only 27 cases have been reported in the *** authors present an additional case and summarize the relevant knowledge in the *** SUMMARY A 27-year-old previously healthy woman presented with a palpable mass over the abdomen and right flank soreness for one *** the computed tomography study,the patient underwent right radical nephrectomy obtaining a 1680-g tumor with a size of 18.4 cm×14.5 cm×11 *** tumor is chiefly composed of anaplastic spindle cells with marked nuclear atypia admixed with multinucleated *** evaluation of tumor cells exhibited diffuse positivity for CD56,p53,and vimentin,and focally positive for *** diagnosis of ASK was ***,a local tumor recurrence followed by a distant metastasis developed within *** patient died 26 months after the initial *** to the previously 27 cases of ASK,the current case had a relatively worse prognosis,which might be potentially associated with older patient age,larger tumor size,and the lack of en-bloc resection of adjacent organs during the initial radical *** This case points out the featured pathological findings for diagnosing ASK and suggests more aggressive management for patients with ASK.

Effectiveness and safety of traditional Chinese medical therapy for cancer-related fatigue: a systematic review and Meta-analysis of randomized controlled trials

OBJECTIVE:To assess the effectiveness and safety of traditional Chinese medical therapy for cancer-related ***:We systematically searched eight electronic databases up to June 2017 for randomized clinical trials of traditional Chinese medical therapy for cancer-related *** authors independently extracted data and assessed the risk bias of the included trials using the Cochrane *** were analyzed by Rev Man 5.2 ***:A total of 23 trials involving 1832 participants identified with cancer-related fatigue were *** trials reported a beneficial effect of traditional Chinese medical therapy on cancer-related *** pooling the data from Chinese herbal medicine therapy and acupuncture or moxibustion therapy,respectively,significant differences were found between experimental groups and control *** improvement rates showed significant differences between traditional Chinese medical therapy and control groups[odds ratio(OR),7.62;95%confidence interval(CI),3.75-15.49;P<0.00001;and OR,3.78;95%CI,2.29-6.23;P<0.00001).Fatigue change scores also showed significant differences between the two groups(mean difference,-0.91;95%CI,-1.16 to-0.65;P<0.00001).Eleven trials demonstrated that traditional Chinese medical therapy improved the quality of life of cancer *** severe adverse effects occurred in traditional Chinese medical therapy ***:Meta-analysis showed that Chinese medical therapy seems to be effective and safe in the treatment of cancer-related fatigue.

With increasing demand to reduce the carbon emission of buildings,it is crucial to quantify the life cycle envi-ronmental impact of new buildings,including the environmental impact due to natural hazards,such as *** study presents a novel comprehensive probabilistic framework to quantify the environmental impact of buildings,including uncertainties in the material extraction and production,transportation,construction,seis-mic exposure and aging(including deterioration),and end-of-life *** developed framework is used to quantify the environmental impact of a 3-story residential building located in Vancouver,*** results show that there is a significant variation in the environmental impact of the prototype building in each stage of the life cycle *** the prototype building is hit by the design level earthquake,it is expected that the median environmental impact of the prototype will be further increased by 42%.In addition,by accounting for the probability of occurrence of different earthquakes within a 50-year design life of the prototype building,the earthquake related damage will result in an additional 5%of the initial carbon emission of the *** shows the importance of including earthquake hazard and deterioration in whole building life cycle assessments.

Predictors of mortality and endoscopic intervention in patients with upper gastrointestinal bleeding in the intensive care unit

Background:The outcomes of patients undergoing esophagogastroduodenoscopy(EGD)in the intensive care unit(ICU)for upper gastrointestinal bleeding(UGIB)are not well *** aims were to determine predictors of 30-day mortality and endoscopic intervention,and assess the utility of existing clinical-prediction tools for UGIB in this ***:Patients hospitalized in an ICU between 2008 and 2015 who underwent EGD were identified using a validated,machine-learning *** regression was used to determine factors associated with 30-day mortality and endoscopic *** under receiver-operating characteristics(AUROC)analysis was used to evaluate established UGIB scoring systems in predicting mortality and endoscopic intervention in patients who presented to the hospital with ***:A total of 606 patients underwent EGD for UGIB while admitted to an *** median age of the cohort was 62 years and 55.9%were *** analysis revealed that predictors associated with 30-day mortality included American Society of Anesthesiologists(ASA)class(odds ratio[OR]4.1,95%confidence interval[CI]2.2-7.9),Charlson score(OR 1.2,95%CI 1.0-1.3),and duration from hospital admission to EGD(OR 1.04,95%CI 1.01-1.07).Rockall,Glasgow-Blatchford,and AIMS65 scores were poorly predictive of endoscopic intervention(AUROC:0.521,0.514,and 0.540,respectively)and in-hospital mortality(AUROC:0.510,0.568,and 0.506,respectively).Conclusions:Predictors associated with 30-day mortality include ASA classification,Charlson score,and duration in the hospital prior to *** risk tools are poorly predictive of clinical outcomes,which highlights the need for a more accurate risk-stratification tool to predict the benefit of intervention within the ICU population.

Objective: To assess the efficacy and safety of traditional Chinese medical therapy for the treatment of cancer-related fatigue. Methods : We systematically searched eight electronic databases up to June 2017 for randomized clinical trials of traditional Chinese medical therapy for cancer-related fatigue. Two authors independently extracted data and assess the risk bias of the included trials using Cochrane Handbook. Data was analyzed by RevMan 5.2 software. Results : We included 23 trials involving 1832 participants identified cancer-related fatigue in the study. 20 trials reported traditional Chinese medical therapy brings beneficialeffect on the treatment of cancer-related fatigue. Pooling the data that used Chinese herbal medicine therapy and acupuncture or moxibustion therapy respectively, it was found significant differences between the experimental groups and control groups. Fatigue improvement rates were respectively showed significant differences between traditional Chinese medical therapy groups and control groups.(OR: 7.62;95%CI: 3.75-15.49;P<0.00001;and OR : 3.78;95%CI: 2.29-6.23;P<0.00001) The data of Fatigue change scores also showed significant difference between the two groups.(MD:-0.91;95%CI:-1.16--0.65;P<0.00001). 11 trials demonstrated traditional Chinese medical therapy improves the quality of life on cancer patients. There were no severe adverse effects occurred at traditional Chinese medical therapy group. Conclusion: we found traditional Chinese medical therapy seems to be effective and safe in the treatment of cancer-related fatigue.

1972年,山西省太谷县水秀公社郭家堡大队高忠丽同志在大田发现一株不育小麦。几年来该大队与一些研究单位协作搞三系配套,主攻保持系,至今还未筛选出有完全保持能力的品种或有完全恢复力的品种。1976年,我组从郭家堡大队引进这份材料。经过三年研究,通过细胞学观察,并结合多年的数据分析,初步确定这个太谷不育小麦是“无花粉型”的,它的细胞质是正常的,不育性是由一个显性雄性不育单基因所控制的。这是小麦中第一次被发现的一个天然突变体,并给它命名为 Ta 1。不育株与任何普通小麦杂交,F1就分离出可育与不育各半,没有中间类型,但农艺性状分离却丰富多采,因而作为育种工具,其价值很大。
