Anthrax is a zoonotic disease caused by Bacillus anthracis.It is potentially fatal and highly contagious disease.Herbivores arc the natural host.Hum.n acquire the disease incidentally by contact with infected anim.l or anim.l products.In the 18th century an epidem.c destroyed approxim.tely half of the sheep in Europe.In 1900 hum.n inhalational anthrax occured sporadically in the United Stales.In 1979 an outbreak of hum.n anthrax occured in Sverdlovsk of Soviet Union.Anthrax continued to represent a world wide presence.the incidence of the disease has decreased in developed countries as a result of vaccination and im.roved industrial hygiene.Hum.n anthrax clinically presents in three form.,i.***.cutaneous,gastrointestinal and inhalational.About 95%of hum.n anthrax is cutaneous and 5%is inhalational.Gastrointestinal anthrax is very rare(less than 1%).Inhalational form.is used as a biological warefare agent. Penicillin,ciprofloxacin(and other quinolones),doxicyclin,am.icillin,im.penem.clindam.cin, clarithrom.cin,vancom.cin,chloram.henicol,rifam.icin are effective antim.crobials. Antim.crobial therapy for 60 days is recom.ended.Hum.n anthrax vaccine is available. Adm.nistration of anti-protective antigen(PA) antibody in com.ination with ciprofloxacin produced 90%-100%survival.the com.ination of CPG-adjuvanted anthrax vaccine adsorbed(AVA) plus dalbavancin significantly im.roved survival.

the m.soproterozoic(1.11 Ga)Um.ondo large igneous province(LIP)in southern Africa and Antarctica was em.laced inm.r and is dom.nated by low-Ti tholeiitic doleritic-gabbroic sills.It is of particular interest because it is the least studied LIP in southern Africa with both sublithospheric and lithospheric m.ntle sources proposed and it coincides with the early assem.ly of Rodinia,so it has im.ortance in understanding the nature of m.gm.tism.and tectonics in and around the Kalahari craton during the m.soproterozoic.In this study,we com.iled a large database of existing(~750)and new(~100)m.jor and trace elem.nt data for the Um.ondo province,as well as 42 new Sr-Nd isotopic m.asurem.nts,to provide constraints on its m.gm. sources and geochem.cal evolution.m.jor elem.nt com.ositional variations in the low-Ti tholeiites are explained by low-pressure(1 kbar)three-phase fractional crystallisation(olivine,clinopyroxene and plagioclase)of a parent m.gm. with~10 wt.%m.O in oxidising conditions(QFm.1).Inverse m.dels show that the low-Ti tholeiitic m.gm.s were derived as residual m.lts after the crystallization of 12%-33%olivine from.prim.ry kom.tiitic-basaltic m.gm.s(up to~20 wt.%m.O)in equilibrium.with m.ntle olivine(Fo90).Low Sm.Yb and TiO2/Yb-Nb/Yb indicate that the prim.ry m.gm.s were derived by 2%-20%shallow(40-50 km.partial m.lting of spinel lherzolite.High Sm.Yb is restricted to dyke swarm. and m.y im.ly lim.ted m.gm. production from.deeper(up to~70 km.garnet lherzolite-like sources.the low-Ti tholeiites of the Um.ondo province are enriched in large ion lithophile elem.nts(Rb-Sr-Cs-K)and depleted in high-field strength elem.nts(Zr-Hf-Nb-Ta),indicating the involvem.nt of crustal m.terial and/or the subcontinental lithospheric m.ntle.this is supported by covariations in th/Nb,Nb/Yb,Nb/La and Ce/Sm.with generally negativeΔNb.Sr-Nd isotopes lend support to the notion that the Um.ondo m.gm.s were derived from.depleted and/or enriched sublithospheric m.ntle sources and subsequently contam.nated by enriched lithospheric m

Gastrointestinal infections are a m.jor cause for serious clinical com.lications in infants.the induction of antibody responses by B cells is critical for protective im.unity against infections and requires CXCR5+PD-1++CD4+T cells(TFH cells).We investigated the ontogeny of CXCR5+PD-1++CD4+T cells in hum.n intestines.While CXCR5+PD-1++CD4+T cells were absent in fetal intestines,CXCR5+PD-1++CD4+T cells increased after birth and were abundant in infant intestines,resulting in significant higher num.ers com.ared to adults.these findings were supported by scRNAseq analyses,showing increased frequencies of CD4+T cells with a TFH gene signature in infant intestines com.ared to blood.Co-cultures of autologous infant intestinal CXCR5+PD-1+/−CD4+T cells with B cells further dem.nstrated that infant intestinal TFH cells were able to effectively prom.te class switching and antibody production by B cells.Taken together,we dem.nstrate that functional TFH cells are num.rous in infant intestines,m.king them.a prom.sing target for oral pediatric vaccine strategies.

BACKGROUND Postoperative delirium.POD)is one of the m.st com.on com.lications in older adult patients undergoing elective surgery.Few studies have com.ared,within the sam. institution,the type of surgery,risk factors and type of anesthesia and analgesia associated with the developm.nt of POD.AIm.To investigate the following three questions:(1)What is the incidence of POD after non-am.ulatory orthopedic surgery at a high-volum. orthopedic specialty hospital?(2)Does surgical procedure influence incidence of POD after nonam.ulatory orthopedic surgery?And(3)For POD after non-am.ulatory orthopedic surgery,what are m.difiable risk factors?m.thODS A retrospective cohort study was conducted of all non-am.ulatory orthopedic surgeries at a single orthopedic specialty hospital between 2009 and 2014.Patients under 18 years were excluded from.the cohort.Patient characteristics and m.dical history were obtained from.electronic m.dical records.Patients with POD were identified using International Classification of Diseases,9^th Revision(ICD-9)codes that were not present on adm.ssion.For incidence analyses,the cohort was grouped into total hip arthroplasty(thA),bilateral thA,total knee arthroplasty(TKA),bilateral TKA,spine fusion,other spine procedures,fem.r/pelvic fracture,and other procedures using ICD-9 codes.For descriptive and regression analyses,the cohort was grouped,using ICD-9 codes,into thA,TKA,spinal fusions,and all procedures.RESULTS Of 78492 surgical inpatient surgeries,the incidence from.2009 to 2014 was 1.2%with 959 diagnosed with POD.the incidence of POD was higher in patients undergoing spinal fusions(3.3%)than for patients undergoing thA(0.8%);thA patients had the lowest incidence.Also,urgent and/or em.rgent procedures,defined by fem.ral and pelvic fractures,had the highest incidence of POD(7.2%)than all other procedures.General anesthesia was not seen as a significant risk factor for POD for any procedure type;however,IV patient-controlled analgesia was a significant risk factor for patients undergoing thA[Odd

　以多孔块状硅体为模板,通过一步浸渍、炭化、酸处理工艺,制备出结构可控、孔隙联通且具有多重孔隙的铸型炭体。分别采用氮吸附技术、高分辨透射电镜和扫描电镜对模板硅、硅碳复合物及相应炭体进行了结构表征。从微米尺度分析,该炭体是由高度联接的枝状结构单元为骨架构筑而成。与模板硅相比较,此炭体是硅体的正复本。从纳米尺度分析,这种炭体枝状骨架是由分布均匀且相互连接的中孔构成,此中孔及其孔壁对应于模板硅的孔壁及中孔,是多孔硅的负复本。此外构成中孔的炭骨架本体又含有大量炭化过程产生的微孔。因此该法合成的铸型炭具有孔隙高度发达和结构层层嵌套的特点。

Background: Pediatric casualties account for a notable proportion of encounters in the deployed setting based on the hum.nitarian m.dical care m.ssion. Previously published data dem.nstrates that an age-adjust shock index m.y be a useful tool in predicting m.ssive transfusion and death in children. We seek to determ.ne if those previous findings are applicable to the deployed, com.at traum. setting.m.thods: We queried the Departm.nt of Defense Traum. Registry(DODTR) for all pediatric subjects adm.tted to US and Coalition fixed-facility hospitals in Iraq and Afghanistan from.January 2007 to January 2016. this was a secondary analysis of casualties seeking to validate previously published data using the shock index, pediatric age adjusted. We then used previously published thresholds to determ.ne patients outcom. for validation by age grouping, 1–3 years(1.2), 4–6 years(1.2), 7–12 years(1.0), 13–17 years(0.9).Results: From.January 2007 through January 2016 there were 3439 pediatric casualties of which 3145 had a docum.nted heart rate and systolic pressure. Of those 502(16.0%) underwent m.ssive transfusion and 226(7.2%) died prior to hospital discharge. Receiver operating characteristic(ROC) thresholds were inconsistent across age groups ranging from.1.0 to 1.9 with generally lim.ted area under the curve(AUC) values for both m.ssive transfusion and death prediction characteristics. Using the previously defined thresholds for validation, we reported sensitivity and specificity for the m.ssive transfusion by age-group: 1–3(0.73, 0.35), 4–6(0.63, 0.60), 7–12(0.80, 0.57), 13–17(0.77, 0.62). For death, 1–3(0.75, 0.34), 4–6(0.66–0.59), 7–12(0.64, 0.52), 13–17(0.70, 0.57). However, negative predictive values(NPV) were generally high with all greater than 0.87.Conclusions: Within the com.at setting, the age-adjusted pediatric shock index had m.derate sensitivity and relatively poor specificity for predicting m.ssive transfusion and death. Esophageal cancer is the 7th leading cause of cancer deaths worldwide.While squam.us cell carcinom. is the m.st prevalent histology internationally,adenocarcinom. of the distal esophagus accounts for nearly 50% of cases in developed countries due to the differences in the etiologic factors such as gastroesophageal reflux disease(GERD) and obesity that predom.nate.While surgery is the m.instay of treatm.nt of this disease,the utilization of chem.radiation,eitherused postoperatively or neoadjuvantly,has becom. a standard practice in the United States.What is the optim.l m.nagem.nt approach is still an area of contention,however,and m.y be different in different regions around the world.this article reviews som. of these controversies,including the role for surgery in patients treated with definitive chem.radiation.At the end,we will also outline recom.endations regarding radiotherapy procedures and techniques.
