Pseudocirrhosis in a pancreatic cancer patient with liver metastases: A case report of complete resolution of pseudocirrhosis with an early recognition and management

We report a case of pseudocirrhosis arising in the setting of regression of liver metastases from pancreatic cancer. A 55-year-old asymptomatic woman presented to our clinic with newly diagnosed metastatic pancreatic cancer with extensive liver metastases. She underwent systemic chemotherapy with gemcitabine and oxaliplatin (GEMOX). After 8 cycles of therapy, she had a remarkable response to the therapy evidenced by decline of carcinoembryonic antigen (CEA) and CA19 by > 50% and nearly complete resolution of hepatic metastases in computed tomography (CT) scan. Shortly after, she developed increasing bilateral ankle edema and ascites, associated with dyspnea, progressive weight gain, and declining performance status. Gemcitabine and oxaliplatin were discontinued as other causes of her symptoms such as congestive heart disease or venous thrombosis were ruled out. CT scan 6 mo after the initiation of GEMOX revealed worsening ascites with a stable pancreatic mass. However, it also revealed a lobular hepatic contour, segmental atrophy, and capsular retraction mimicking the appearance of cirrhosis. She was managed with aggressive diuresis and albumin infusions which eventually resulted in a resolution of the above- mentioned symptoms as well as complete resolution of pseudocirrhotic appearance of the liver and ascites in CT scan. This case demonstrates that pancreatic cancer patients can develop pseudocirrhosis. Clinicians and radiologist should be well aware of this entity asearly recognition and management can lead to a near complete recovery of liver function and much improved quality of life as illustrated in this case.

Carbon dioxide fluxes of tundra vegetation communities on an esker top in the low-Arctic

Previous studies have shown that carbon dioxide fluxes vary considerably among Arctic environments and it is important to assess these differences in order to develop our understanding of the role of Arctic tundra in the global carbon cycle. Although many previous studies have examined tundra carbon dioxide fluxes, few have concentrated on elevated terrain(hills and ridge tops) that is exposed to harsh environmental conditions resulting in sparse vegetation cover and seemingly low productivity. In this study we measured carbon dioxide(CO2) exchange of four common tundra communities on the crest of an esker located in the central Canadian low-Arctic. The objectives were to quantify and compare CO2 fluxes from these communities, investigate responses to environmental variables and qualitatively compare fluxes with those from similar communities growing in less harsh lowland tundra environments. Measurements made during July and August 2010 show there was little difference in net ecosystem exchange(NEE) and gross ecosystem production(GEP) among the three deciduous shrub communities, Arctous alpina, Betula glandulosa and Vaccinium uliginosum, with means ranging from -4.09 to -6.57 μmol·m^-2·s^-1 and -7.92 to -9.24 μmol·m^-2·s^-1, respectively. Empetrum nigrum communities had significantly smaller mean NEE and GEP(-1.74 and -4.08 μmol·m^-2·s^-1, respectively). Ecosystem respiration(ER) was similar for all communities(2.56 to 3.03 μmol·m^-2·s^-1), except the B. glandulosa community which had a larger mean flux(4.66 μmol·m^-2·s^-1). Overall, fluxes for these esker-top communities were near the upper range of fluxes reported for other tundra communities. ER was related to soil temperature in all of the communities. Only B. glandulosa GEP and ER showed sensitivity to a persistent decline in soil moisture throughout the study. Schizandra arisanensis extract attenuates cytokine-mediated cytotoxicity in insulin-secreting cells

AIM: To explore the bioactivity of an ethanolic extract of Schizandra arisanensis (SA-Et) and isolated constituents against interleukin-1 β and interferon-γ-mediated β cell death and abolition of insulin secretion. METHODS: By employing BRIN-BD11 cells, the effects of SA-Et administration on cytokine-mediated cell death and abolition of insulin secretion were evaluated by a viability assay, cell cycle analysis, and insulin assay. The associated gene and protein expressions were also measured. In addition, the bioactivities of several peak compounds collected from the SA-Et were tested against cytokine-mediated β cell ***: Our Results revealed that SA-Et dose-dependently ameliorated cytokine-mediated β cell death and apoptosis. Instead of suppressing inducible nitric oxide synthase/nitric oxide cascade or p38MAPK activity, suppression of stress-activated protein kinase/c-Jun NH2-terminal kinase activity appeared to be the target for SA-Et against the cytokine mix. In addition, SA-Et provided some insulinotropic effects which re-activated the abolished insulin exocytosis in cytokine-treated BRIN-BD11 cells. Finally, schiarisanrin A and B isolated from the SA-Et showed a dose-dependent protective effect against cytokine-mediated β cell death. CONCLUSION: This is the first report on SA-Et ameliorating cytokine-mediated β cell death and dysfunction via anti-apoptotic and insulinotropic actions.

Generalized Anxiety and Major Depressive Symptoms of General Public in South Korea during the Early COVID-19 Pandemic

The extent of viral spread and strategies in dealing with the COVID-19 pandemic have been different in each *** are overall increased mental health concerns in many countries but it is unclear what the general public individuals who do not have heighten vulnerability to stressors for existing mental diseases or significant physical illnesses were experiencing during the *** evaluated the stressors and mental health of general public in South Korea that has a relatively low confirmed cases and *** on the on-line survey questions were used to assess the mental and physical symptoms in association with individuals’reported *** with the symptoms of the generalized anxiety disorder(GAD)were reported in 58.9%,and major depression(MD)in 23.2%,mostly in minimal to mild degrees,cut-off scores of 5 and 10,respectively by the validated screening tools,GAD-7 and *** GAD and MD symptoms were in 21.5%of the *** total number of stress had significant association with the scores of GAD-7,PHQ-9,physical symptoms,sleep difficulties and resilience(p<0.01).GAD scores were also associated with sleep difficulties(p<0.01)and raising young children(p<0.05).MD scores were associated with sleep difficulties,job-dissatisfaction,and educational level(p<0.05).The limitations of the study include small sample size,usage of smartphone or email,potential under-reporting by stigma in the socio-cultural context and evolving nature of *** conclude that keeping careful watch for mental symptoms,stressors,sleep difficulties and other physical symptoms are important even for the individuals without previous mental illnesses during the pandemic era.

Food waste is recognized as a valuable source for potential agricultural applications to supply organic matter and nutrients to arable ***,the information on the combined application of food waste and the plant growth-promoting bacterial strain,Chlorella,related to plant metabolic features and sodium chloride content in arable soil is *** present study was conducted to investigate the exogenous application of food waste along with Chlorella,which improved the physio-morphological features of red *** results revealed that this combination enhanced the organic matter in the soil,ultimately improving the fertility rate of the soil,and the physio-morphological features,such as chlorophyll a content(24.5±0.7),root(7.8±0.7)cm and shoot length(12.1±0.7)cm,fresh weight(2.1±0.05)g,dry weight(0.19±0.05)g,mineral contents,and hormonal concentration(ABA by up to 2 folds).The combined treatment also minimized free radicals via the activation of the intrinsic antioxidant series cascade and electrolyte *** findings showed that adding Chlorella and food wastes improved growth characteristics and can be used as a green bio-fertilizer for sustainable agriculture.

为了探讨地下水位较高条件下根区湿润方式对梨树根和茎液流及水分平衡的影响 ,开展了较系统的试验研究。试验共设三种处理 ,即传统畦灌 (CFI) ,部分根干燥灌溉 (PRD)和分根区交替灌溉 (ARDI) ,分别使用土壤水分监测系统 (Enviro SCAN)和热脉冲探针监测土壤水分动态和梨树根和茎液流。结果表明在 PRD和 ARDI情况下湿润根区的根液流不仅大于干燥根区 ,而且也大于 CFI情况下的相同区域。复水后 ,ARDI干燥区的根液流比 PRD的恢复和提高快得多 ,并且比 CFI的大 ;ARDI的茎液流比 CFI的小 ,但比 PRD保持一侧根干燥时的大。在只有一侧根区灌溉时 ,ARDI和 PRD的日耗水量比 CFI的小。 ARDI和 PRD中湿润侧的根系具有水分吸收补偿效应 ,当干燥根区复水后能够提高水分的吸收能力 ,其程度与根区持续干燥的时间长短有关。逐日根液流与参考作物蒸发蒸腾量关系密切 ,但随着表面灌溉方式和湿润根区的不同这些关系明显的不一样。逐日茎液流与参考作物蒸发蒸腾量有关 ,而且在不同的表面灌溉方式下 ,土壤含水率相同时茎液流和参考作物蒸发蒸腾量的比率不同。与 CFI相比 ,ARDI和 PRD大约少用 5 0 %的灌溉水量 ,但是梨树的水分消耗量和茎液流并没减少相同的比例。表面湿润方式对水量平衡和液流的作用明显受到地下水位的影响。
