Objective:To explore the effect and mechanism of action of Celastrus paniculatus oil on the treatment of perinatal rats with attention deficit hyperactivity ***:In the perinatal stage,the rats were either isolated or administered with lead acetate to establish an animal model of attention deficit hyperactivity *** served as the reference ***’behaviours were assessed through Y-maze,novel object preference,fear conditioning and residentintruder aggression *** stress parameters,bioamine concentration(dopamine,noradrenaline and 5-hydroxytryptamine),nerve growth factor,interleukin-6,nuclear factor-κB,and tumour necrosis factor(TNF)-αwere *** immunohistochemical assay was ***:Celastrus paniculatus oil significantly improved behavioural parameters in Y maze,novel object preference,discrimination index,fear conditioning and resident intruder aggressive *** treatment groups showed a decrease in malondialdehyde *** in the levels of dopamine,noradrenaline,and serotonin were restored by Celastrus paniculatus *** paniculatus oil increased nerve growth factor and decreased interleukin-6,nuclear factor-κB,and TNF-α.synaptophysin immunoreactivity was also improved by Celastrus paniculatus oil with alleviated reactive gliosis,degeneration,and vascular ***:This research shows the therapeutic potential of Celastrus paniculatus oil for the treatment of attention deficit hyperactivity disorder.

Objective:To assess the therapeutic potential of ethanolic extract of Azadirachta(A.)indica in rats with polycystic ovary syndrome(PCOs).Methods:Thirty-five prepubertal female sprague Dawley rats were randomly divided into five groups with 7 animals in each *** 1 received 0.5%carboxy methyl cellulose *** 2 to 5 received testosterone propionate(0.2 mg/kg,s.c.)dissolved in olive oil daily for 42 days to induce *** addition,group 3 was administered with *** extract(100 mg/kg,0.5%carboxy methyl cellulose orally)from the 7th to 12th week,group 4 received quercetin(100 mg/kg,0.5%carboxy methyl cellulose orally)and group 5 received wartmannin(100 mg/kg,0.5%carboxy methyl cellulose orally).At the end of treatment,blood was collected for biochemical *** follicular count and uterus corpus luteum count followed by PI3K gene expression in the ovary and uterus were ***:The ethanolic extracts of *** significantly reduced body weight,ovary weight and uterus weight of *** of *** also significantly increased the levels of serum glucose,total cholesterol,triglyceride,low-density lipoprotein,very low-density lipoprotein,insulin,testosterone,and luteinizing *** also reduced lipid peroxidation and increased antioxidant parameters in the liver homogenates of PCOs-induced *** examination of the ovary and uterus confirmed PCOs occurrence and remission state in the PCOs-induced and treated groups,***,*** and quercetin significantly downregulated PI3K gene *** results of the ovary and uterus also proved the protective role of ***:*** leaf extract has beneficial effects in the treatment of PCOs by downregulation of PI3K gene expression.

Despite a record setting number of heart transplants performed annually,the national donor shortage continues to plague transplant teams across the United *** we describe the barriers to adaptation of numerous“non-traditional”orthotopic heart transplant donor characteristics including donors with hepatitis C virus,those meeting criteria for donation after cardiac death,donors with coronavirus disease 19 infection,donors with the human immunodeficiency virus,and grafts with left ventricular systolic *** center’s objective was to increase our transplant volume by expanding our donor pool from“traditional”donors to these“non-traditional”*** detail how medical advances such as certain laboratory studies,pharmacologic interventions,and organ care systems have allowed our center to expand the donor pool thereby increasing transplantation volume without adverse effects on outcomes.

Diagnostic performance of C-reactive protein level and its role as a potential biomarker of severe dengue in adults

Objective: To determine the relationship between C-reactive protein(CRP) level and the severity of dengue and the potential use of CRP in predicting acute dengue ***: A prospective observational study was performed on dengue patients admitted to a tertiary care hospital in southern India. All patients of age above 18 years, diagnosed with dengue were included in the study. The detailed laboratory parameters pertaining to dengue were recorded. CRP levels were estimated and compared between groups i.e. severe and non-severe dengue. CRP cut-off value was detected using the receiver-operator ***: Totally 98 patients with a mean age of 40 years were included. Among them, 11.2% of the patients suffered from severe dengue, 54.1% of the cases had non-severe dengue without warning signs, and 34.7% had non-severe dengue with warning signs. The median CRP was significantly higher in patients with severe dengue compared to patients with non-severe dengue(96.2 mg/d L vs. 5.3 mg/d L). Univariate logistic regression analysis showed that the odd’s ratio(OR) of CRP was 1.053(P≤0.001, 95% CI=1.029-1.078). CRP at a cutoff value of 21.6 mg/L(0.929 AUC) had excellent sensitivity(100%) and specificity(81.6%) in predicting severe dengue infection. Multivariate logistic regression analysis showed that CRP(OR=1.089, P=0.013) and ALT(OR=1.010, P=0.034) were statistically significant independent predictors of dengue ***: CRP level could be used as a potential biomarker to predict severity of dengue in adults.

Let s and z be complex variables, Γ(s) be the Gamma function, and for any complex v be the generalized Pochhammer symbol. Wright Type Hypergeometric Function is defined (Virchenko et al. [1]), as: where which is a direct generalization of classical Gauss Hypergeometric Function 2F1(a,b;c;z). The principal aim of this paper is to study the various properties of this Wright type hypergeometric function 2R1(a,b;c;τ;z);which includes differentiation and integration, representation in terms of pFq and in terms of Mellin-Barnes type integral. Euler (Beta) transforms, Laplace transform, Mellin transform, Whittaker transform have also been obtained;along with its relationship with Fox H-function and Wright hypergeometric function.

Objective: To ascertain the role of serum potassium levels in predicting clinical outcomes in diarrhea-associated hemolytic uremic syndrome (HUs D+). Methods: We reviewed clinical and laboratory data from HUs D+ patients at our tertiary care institution from 2001 to 2008. serum potassium concentration at presentation and during the acute phase of acute renal failure were recorded and related to laboratory parameters and clinical outcomes. Results: 15 HUs D+ cases were identified. E. coli 0157:H7 was found in 9/15 cases (70%). Potassium levels were not predictive of clinical outcomes. Normal serum potassium levels were found in the majority of patients. Potassium levels sentation in 3/15 patients (23%), and no patient manifested hyperkalemia even when creatinine levels were concurrently increase. Conclusions: This study suggests the presence of vigorous compensatory mechanisms in the homoestasis of serum potassium levels during HUs D+ disease since neither the increase stool volumes associated with diarrhea nor the presence of renal failure resulted in clinically significant changes in serum potassium levels.

This paper mainly concentrates on design of improved controller and its implementation based on single phase synchronous reference frame theory (sRFT) for Dynamic Voltage Restorer (DVR) compensating voltage sag particularly for nonlinear load. In case of single phase distribution line with nonlinear load, the complexity of controller’s design becomes more serious issue. The present single phase and/or three phase theories applicable to DVR shows poor response to restore voltage sag in case of nonlinear load due to presence of harmonics. Hence restoration of voltage sag in single phase nonlinear load connected system has been a serious concern. Therefore, new controller for DVR has been proposed incorporating effective design concept for fundamental component extraction in case of nonlinear load. The single phase sRFT based main controller for DVR works on two separate closed path viz. feed forward path for quick transient response and feedback path for reducing the steady state error. Moreover, pre-sag mitigation strategy of DVR has been adapted through these two aforementioned paths. Complete design of proposed controller is based on phasor analysis. It also consist of proportional integral (PI) controller to reduce the error in the DC-link voltage during compensation time. The controller performance has been verified in MATLAB simulink for both types (linear and nonlinear) of load. The results obtained indicates that the proposed controller is effective in its performance.

总结一组持续性部分性癫痫(Epilepsia partialis continua,EPC)患儿的特点,并提出一种整合关键鉴别诊断的诊断算法。对2002年—2019年在三级儿科神经病学中心就诊的EPC患儿进行分析。54例患儿符合EPC诊断标准。中位发病年龄为7岁(0.6~15岁),中位随访时间为4.3年(0.2~16年)。其中,诊断为Rasmussen脑炎(Rasmussen encephalitis,RE)30例(56%),线粒体疾病12例(22.2%),磁共振成像(MRI)阳性病灶相关局灶性癫痫6例(11.1%),诊断不明5例(9%)。线粒体疾病患儿更早出现EPC,EPC发病年龄每增加一年,诊断线粒体疾病的几率降低26%(P=0.02)。EPC发作前存在发育问题(OR=22,PsF)蛋白水平升高(OR=16)均预示着线粒体疾病。30例RE患儿中有18例(60%)出现EPC发作时,MRI提示明显不对称或一侧半球萎缩,其余患儿出现的中位时间则在EPC发作后6个月(3~15个月)。首选诊断检查为头部MRI。一侧半球萎缩结合临床表现及脑电图改变的偏侧性,提示诊断为RE。对于初次扫描时不能诊断为RE,但临床和影像学表现提示RE的儿童,建议每6个月重复影像学检查以排查进行性一侧半球皮质萎缩,并应考虑脑活检,鞘内炎症的证据(寡克隆带和新蝶呤升高)可为佐证。对于存在双侧半球源性EPC的患儿,建议进行快速DNA聚合酶γ(POLG)基因检测,如为阴性,则应提取外周血DNA进行mtDNA测序及全外显子组测序。线粒体病所致EPC患儿表现出与RE和结构性癫痫不同的临床特征。这一针对EPC患儿的诊断算法有助于进行针对性检查并及时诊断。

BACKGROUND Itraconazole is a broad-spectrum triazole antifungal inhibiting fungal growth by inhibiting ergosterol synthesis and exhibits a nonlinear pharmacokinetic *** absorption pattern with wide fluctuations in blood levels causes inconsistent and unpredictable clinical behaviour of this drug despite its low minimum inhibitory concentration(MIC)as compared to other antifungal *** To compare the oral bioavailability and bioequivalence of Fixtral sB(supra bioavailable itraconazole)with reference product R2(supra bioavailable 2×50 mg itraconazole).METHODs The study population consisted of 54 healthy volunteers,aged between 18-45 years and randomized to receive a single oral dose of either test[T;Fixtral sB(supra bioavailable itraconazole)100 mg]or reference product(R1;sporanox 100 mg×2 capsules and R2;Lozanoc capsules 50 mg×2 capsules).Blood samples were taken pre-dose and post-dose up to 96 *** study evaluated bioequivalence by comparing the oral bioavailability of the test product with reference product *** pharmacodynamic characteristics of the drug were evaluated by comparing the test product with reference product ***(PK)-PD comparative analysis[area under the concentration-time curve(AUC)/minimum inhibitory concentration(MIC)>25]was performed for conventional itraconazole 100 mg and supra bioavailable itraconazole 50 *** events(AEs)assessments were performed in each study period and post-study *** statistical analysis of primary PK variables revealed bioequivalence,with confidence intervals being completely inside the acceptance criteria of 80%-125%.The peak concentration levels of itraconazole were achieved at 10 h(T)and 8.5 h(R2),*** parameter assessment showed that AUC/MIC for R1 are comparable to Fixtral sB 100mg for MIC levels up to 16mcg/mL(P>0.05 and observed P=0.3196).six AEs were observed that were mild to moderate in severity and *** severe AE was reported.
