目的了解四川省基层医疗机构医院感染(简称“院感”)预防与控制管理的现状,为后续精准制定院感防控政策提供科学依据。方法于2023年7至12月对四川省88所基层医疗机构院感防控管理情况进行调查,采用问卷调查的方式进行,数据通过Excel 2013软件进行收集与整理,采用描述性分析的方法分析不同医疗机构的院感管理体系建设情况、院感管理专职人员构成及履职情况。采用SPSS 27.0软件对构成比的比较行χ2检验,检验水准为α=0.05。结果调查的88家医疗单位中,72家机构设置了院感管理委员会,58家机构独立设置了院感管理部门,74家医院临床科室设置了院感管理小组。78.41%医疗机构均配置了院感管理专职人员,专职人员以护理学专业(75.70%)、大专和本科学历(89.72%)、中级和初级及其他技术职称(85.98%)、30~49岁的中青年(57.94%)、女性(85.98%)和从事院感工作0~3年(39.25%)为主。高达96.59%医疗机构开展院感管理质量控制检查工作,其中98.82%的将结果反馈给临床科室。结论四川省的基层医疗机构在院感控制管理方面已经取得了一定进步,但仍面临从业人员的结构不均衡、人才来源的局限性以及相关人员的经验不足等问题。未来有必要对院感管理工作进行更深入的研究与探索,并持续优化相关的制度和机制建设。

Targeting the PD-1/PD-L1 axis with small-molecular inhibitors is a promising approach for immunotherapy. Here, we identify a natural pentacyclic triterpenoid, Pygenic Acid A(PA), as a PD-1 signaling inhibitor. PA exerts anti-tumor activity in hPD-1 knock-in C57BL/6 mice and enhances effector functions of T cells to promote immune responses by disrupting the PD-1 signaling transduction. Furthermore, we identify SHP-2 as the direct molecular target of PA for inhibiting the PD-1 signaling transduction. Subsequently, mechanistic studies suggest that PA binds to a new druggable site in the phosphorylated PD-1 ITSM recognition site of SHP-2, inhibiting the recruitment of SHP-2 by PD-1. Taken together, our findings demonstrate that PA has a potential application in cancer immunotherapy and occupying the phosphorylated ITSM recognition site of SHP-2 may serve as an alternative strategy to develop PD-1 signaling inhibitors. In addition, our success in target recognition provides a paradigm of target identification and confirmation for natural products.

Synthesis of spherical carbon beads with effective CO_2 capture capability is highly desirable for large scale application of CO2 sorption, but remains challenging. Herein, a facile and efficient strategy to prepare nitrogen-doped hierarchically porous carbon spheres was developed via co-pyrolyzation of poly(vinylidene chloride) and melamine in alginate gel beads. In this approach, melamine not only serves as the nitrogen precursor, but also acts as a template for the macropores structures. The nitrogen contents in the hierarchically porous carbon spheres reach a high level, ranging from 11.8 wt% to 14.7 wt%, as the melamine amount increases. Owing to the enriched nitrogen functionalities and the special hierarchical porous structure, the carbon spheres exhibit an outstanding CO_2 capture performance, with the dynamic capacity of as much as about 7 wt% and a separation factor about 49 at 25 °C in a gas mixture of CO_2/N_2(0.5:99.5, v/v).

Target detection circuits have been previously designed,which are propelled by conventional PCR,isothermal amplification and strand-displacement *** detection circuits obtain the target signal via the replication of the target strand,the aggregation of the signal particles or the branch *** we constructed a tri-ode-like enzyme-free catalyst strand-displacement circuit for target DNA *** target strand triggered die reaction and released the fluorescence signal strand circularly through branch ***,the main challenge of strand-displacement reaction is the signal ***,we designed a double strand structure"junction fuel",which was used to increase the binding energy across the displacement ***,the leakage of the system obtained stable inhabitation due to the junction fuel *** limit of detection of the system was as low as 0.11 nmol/L and the gain of the system was as high as 28-fold(the concentration of target was 50 nmol/L).Furthermore,the process of the system was visualized vividly in the reaction curve through the kinetic simulation implemented,which suggests that the combination of the kinetic simulation and the experiment exhibits a promising prospect towards the use of strand-displacement circuit in analytical,diagnostic application and synthetic biology.

Background:Whether functional gastrointestinal disorders(FGIDs)are associated with the long-term risk of chronic kidney disease(CKD)remains *** aimed to investigate the prospective association of FGIDs with CKD and examine whether mental health mediated the ***:About 416,258 participants without a prior CKD diagnosis enrolled in the UK Biobank between 2006 and 2010 were *** with FGIDs(including irritable bowel syndrome[IBS],dyspepsia,and other functional intestinal disorders[FIDs;mainly composed of constipation])were the exposure group,and non-FGID participants were the non-exposure *** primary outcome was incident CKD,ascertained from hospital admission and death registry records.A Cox proportional hazard regression model was used to investigate the association between FGIDs and CKD,and the mediation analysis was performed to investigate the mediation proportions of mental ***:At baseline,33,156(8.0%)participants were diagnosed with FGIDs,including 21,060(5.1%),8262(2.0%),and 6437(1.6%)cases of IBS,dyspepsia,and other FIDs,*** a mean follow-up period of 12.1 years,11,001(2.6%)participants developed *** were significantly associated with a higher risk of incident CKD compared to the absence of FGIDs(hazard ratio[HR],1.36;95%confidence interval[CI],1.28-1.44).Similar results were observed for IBS(HR,1.27;95%CI,1.17-1.38),dyspepsia(HR,1.30;95%CI,1.17-1.44),and other FIDs(HR,1.60;95%CI,1.43-1.79).Mediation analyses suggested that the mental health score significantly mediated 9.05%of the association of FGIDs with incident CKD and 5.63-13.97%of the associations of FGID subtypes with ***,the positive associations of FGIDs and FGID subtypes with CKD were more pronounced in participants with a high genetic risk of ***:Participants with FGIDs had a higher risk of incident CKD,which was partly explained by mental health scores and was more pronounced in those with high gen

In this study,we introduce our newly developed measurement-fed-perception self-adaption Low-cost UAV Coordinated Carbon Observation Network(LUCCN)*** LUCCN primarily consists of two categories of instruments,including ground-based and UAV-based in-situ *** use the GMP343,a low-cost non-dispersive infrared sensor,in both ground-based and UAV-based *** first integrated measurement campaign took place in Shenzhen,China,4 May *** the campaign,we found that LUCCN’s UAV component presented significant data-collecting advantages over its ground-based counterpart owing to the relatively high altitudes of the point emission sources,which was especially obvious at a gas power plant in *** emission flux was calculated by a crosssectional flux(CSF)method,the results of which differed from the Open-Data Inventory for Anthropogenic Carbon dioxide(ODIAC).The CSF result was slightly larger than others because of the low sampling rate of the whole emission cross *** LUCCN system will be applied in future carbon monitoring campaigns to increase the spatiotemporal coverage of carbon emission information,especially in scenarios involving the detection of smaller-scale,rapidly varying sources and sinks.

远距高动态对空精确打击是当前国外制空作战领域的重要发展方向。本文针对当前国外对超远程对空打击作战的装备、技术和应用概念等方面的发展情况进行了研究,分析了美国等国家在该领域的主要研究工作现状,从中梳理了作战概念、武器平台和杀伤链等方面的发展趋势,从制导信息如何供给和保障的层面提出了协同目标探测制导远距攻击、协同目标指示制导远距攻击、网络化接力制导远距攻击3种典型远距对空打击应用样式,结合国外发展情况总结论述了远距对空打击的关键性技术。
