Influence of comorbidity on outcomes of older patients with acute pancreatitis based on a national administrative database

Little information is available on the influ- ence of comorbidities on outcomes of older patients with acute pancreatitis. This study aimed to investigate the influence of co- morbidities on outcomes of older patients with acute pancreati- tis using data from a national Iapanese administrative database. mETHODS： A total of 14 322 older patients （〉70 years） with acute pancreatitis were referred to 1090 hospitals between 2010 and 2012 in Japan. We collected patients＇ data from the administrative database to compare the in-hospital mortal- ity and length of stay of older patients with acute pancreatitis. The patients were categorized into four groups according to comorbidity level using the Charlson Comorbidity Index （CCI）： none （CCI score=0; n=6890）; mild （1; n=3874）; moder- ate （2; n=2192） and severe （〉3; n=1366）. RESULTS： multiple logistic and linear regression analyses revealed that severe comorbidity was significantly associated with higher in-hospital mortality and longer length of stay [odds ratio （OR）=2.26; 95% confidence interval （CI）： 1.75-2.92, P〈0.001 and coefficient 4.37 days; 95% CI： 2.89-5.85, P〈0.001, respectively]. In addition, cardiovascular and renal diseases were the most significant comorbidities affecting outcomes of the older patients. ORs of cardiovascular and renal diseases for mortality were 1.44 （95% CI： 1.13-1.85, P=0.003） and 2.69 （95% CI： 1.88-3.85, P〈0.001）, respectively, and coefficients forlength of stay were 3.01 days （95% CI： 1.34-4.67, P〈0.001） and 3.72 days （95% CI： 1.01-6.42, P=0.007）, respectively. CONCLUSION： This study demonstrated that comorbidities significantly influenced outcomes of older patients with acute pancreatitis and cardiovascular and renal comorbidities were significant factors affecting outcomes.

A new approach to reduce the reverse current of Ge pin photodiodes on Si is presented, in which an i-Si layer is inserted between Ge and top Si layers to reduce the electric field in the Ge layer. Without post- growth annealing, the reverse current density is reduced to -10 mA/cm^2 at -1 V, i.e., over one order of magnitude lower than that of the reference photodiode without i-Si layer. However, the responsivity of the photodiodes is not severely compromised. This lowered-reverse-current is explained by band-pinning at the i-Si/i-Ge interface. Barrier lowering mechanism induced by E-field is also discussed. The presented ＂non-thermal＂ approach to reduce reverse current should accelerate electronics-photonics convergence by using Oe on the Si complementary metal oxide semiconductor （CmOS） platform.

BACKGROUND Primary sclerosing cholangitis(PSC)is an extraintestinal manifestation of ulcerative colitis(UC).PSC is a well-known risk factor for intrahepatic cholangiocarcinoma(ICC),and ICC is known to have a poor *** SUmmARY We present two cases of ICC in patients with PSC associated with *** the first case,a tumor was found by magnetic resonance imaging(mRI)in the liver of a patient with PSC and UC who presented to our hospital with right-sided rib *** second patient was asymptomatic,but we unexpectedly detected two liver tumors in an mRI performed to evaluate bile duct stenosis associated with *** was strongly suspected by computed tomography and mRI in both cases,and surgery was performed,but unfortunately,the first patient died of ICC recurrence 16 mo postoperatively,and the second patient died of liver failure 14 mo *** Careful follow-up of patients with UC and PSC with imaging and blood tests is necessary for early detection of ICC.

We reported that layered polymer crystals are obtained by the topochemical polymerization of 1,3-diene monomers and provided as host material for organic intercalation[1]. For intercalation using various long-alkyl amines as the guest species, its reaction behavior, mechanism, characteristics, and potential to application have been clarified[2]. We also succeeded in the synthesis of several host layered polymer crystals with different tacticities and layer structures[3]. We describe here intercalation using various stereoregular poly(muconic acid)s (PmA) and n-alkylamines as the host and guest compounds, respectively. The reaction behavior and the layered structure of the obtained ammonium polymers are discussed from the viewpoint of stereochemical structure of the host polymers.

Acquiring accurate scattering properties is important for rendering translucent materials. In particular, the phase function, which determines the distribution of scattering directions, plays a significant role in the appearance of a material. We propose a distinctive scattering theory that approximates the effect of single scattering to acquire the non-parametric phase function from a single image. Furthermore, in various experiments, we measured the phase functions from several real diluted media and rendered images of these materials to evaluate the effectiveness of our theory.

The detection of acyl homoserine lactones（AHLs） in activated sludge is essential for clarifying their function in wastewater treatment processes. An LC–mS/mS method was developed for the detection of AHLs in both the aqueous and solid phases of activated sludge. In addition, the effects of proteases and extracellular polymeric substances（EPS） on the detection of AHLs were evaluated by adding protease inhibitors and extracting EPS,respectively. Recoveries of each AHL were improved by adding 50 μL of protease inhibitor,and recoveries were also improved from 0 to 56.9% to 24.2%–105.8% by EPS *** the developed method to determine the type and concentration of AHLs showed that C4-HSL, C6-HSL, C8-HSL and 3-oxo-C8-HSL were widely detected in a suspended activated sludge system. The dominant AHL was C8-HSL, with a highest concentration of304.3 ng/L. C4-HSL was mainly distributed in the aqueous phase, whereas C6-HSL, C8-HSL and 3-oxo-C8-HSL were preferentially distributed in the sludge phase.
