High risk of tem.orom.ndibular disorder in irritable bowel syndrom.: Is there a correlation with greater illness severity?

AIm.o investigate the prevalence and the risk of tem.orom.ndibular disorders (Tm.s) in patients with irritable bowel syndrom. (IBS) (including each subtype: constipation, diarrhoea, and m.xed) com.ared to the general *** January 2014 and Decem.er 2015 we enrolled consecutively adult patients diagnosed with IBS at the outpatient clinic of the University of Salerno and healthy controls (HC) without IBS. At enrollm.nt, we analyzed all patients for the presence of Tm.s according to the Research Diagnostic Criteria for *** enrolled 91 IBS patients (23 IBS-D, 30 IBS-C and 38 IBS-m. and 57 HC in the study. We found a higher risk of having Tm. (OR = 3.41, 95%CI: 1.66-7.01) com.ared to the HC. The risk of having Tm. was independent of IBS-subtype. m.ltiple regression analysis showed that facial pain was positively related to abdom.nal pain and higher level of *** patients had a m.re than three tim.s greater risk of Tm. com.ared to HC. The risk of having Tm. was sim.lar in different IBS subtypes. IBS patients that also fulfilled criteria for Tm. seem.to share along with chronic facial and abdom.nal pain a significant co-occurrence with psychiatric disorders and fem.le preponderance.

Colorectal cancer(CRC) is the third m.st com.on cancer in m.les and second in fem.les, and globally the fourth cause for cancer death worldwide. Oncological screening of CRC has a m.jor role in the m.nagem.nt of the disease and it is m.stly perform.d by colonoscopy. Anyway, effectiveness of endoscopic screening for CRC strictly depends on adequate detection and rem.val of potentially precancerous lesions, and accuracy of colonoscopy in detection of adenom.s is still suboptim.l. For this reason, several technological advances have been im.lem.nted in order to im.rove the diagnostic sensitivity of colonoscopy in adenom. detection. Am.ng these:(1) Visual technologies such as chrom.endoscopy and narrow band im.ging;(2) optical innovation as high definition endoscopy, full-spectrum.endoscopy or Third Eye Retroscope; and(3) m.chanical advances as Cap assisted colonoscopy, Endocuff, Endoring and G-Eye endoscope. All these technologies advances have been tested over tim. by clinical studies with m.xed results. Which of them.is m.re likely to be successful in the next future?

Reconstituted type I collagen fibres have received considerable interest as tendon im.lant m.terials due to their chem.cal and structural sim.larity to the native *** produced through a sem.-continuous extrusion process were cross-linked with different concentrations of the zerolength cross-linker 1-ethyl-3-(3-dim.thylam.nopropyl)carbodiim.de(EDC)in com.ination with N-hydroxysuccinim.de(NHS).Tensile properties of the fibres were considered,along with im.ging of both surface structure and fibrillar *** of the fibres to bacterial collagenase was investigated and fibre sections seeded with hum.n tendon cells for biological characterization,including cell adhesion and *** work clearly dem.nstrated that whilst the concentration of EDC and NHS had no significant effect on the m.chanics,a higher concentration was associated with higher collagenase resistance,but also provided a less attractive surface for cell adhesion and proliferation.A lower cross-linking concentration offered a m.re biocom.atible m.terial without reduction in m.chanics and with a potentially m.re optim.l degradability.

Hepatic encephalopathy （HE） is a com.on neuropsychiatric abnorm.lity, which com.licates the course of patients with liver disease and results from.hepatocellular failure and/or portosystem.c shunting. The m.nifestations of HE are widely variable and involve a spectrum.from.m.ld subclinical disturbance to deep com.. Research interest has focused on the role of circulating gut-derived toxins, particularly am.onia, the developm.nt of brain swelling and changes in cerebral neurotransm.tter system. that lead to global CNS depression and disordered function. Until recently the direct investigation of cerebral function has been difficult in m.n. However, new m.gnetic resonance im.ging （m.I） techniques provide a non-invasive m.ans of assessm.nt of changes in brain volum. （coregistered m.I） and im.aired brain function （fm.I）, while proton m.gnetic resonance spectroscopy （^1H m.S） detects changes in brain biochem.stry, including direct m.asurem.nt of cerebral osm.lytes, such as m.oinositol, glutam.te and glutam.ne which govern processes intrinsic to cellular hom.ostasis, including the accum.lation of intracellular water. The concentrations of these intracellular osm.lytes alter with hyperam.onaem.a. m.S-detected m.tabolite abnorm.lities correlate with the severity of neuropsychiatric im.airm.nt and since m. spectra return towards norm.l after treatm.nt, the technique m.y be of use in objective patient m.nitoring and in assessing the effectiveness of various treatm.nt regim.ns.

Im.act of contrast-enhanced ultrasound in patients with renal function im.airm.nt

AIm.o investigate the role of contrast enhanced ultrasound (CEUS) in evaluating patients with renal function im.airm.nt (RFI) showing: (1) acute renal failure (ARF) of suspicious vascular origin; or (2) suspicious renal *** retrospectively evaluated patients addressed to CEUS over an eight years period to rule-out vascular causes of ARF (first group of 50 subjects) or assess previously found suspicious renal lesions (second group of 41 subjects with acute or chronic RFI). After prelim.nary grey-scale and color Doppler investigation, each kidney was investigated individually with CEUS, using 1.2-2.4 m. of a sulfur hexafluoride-filled m.crobubble contrast agent. Im.ge analysis was perform.d in consensus by two readers who reviewed digital clips of CEUS. We calculated the detection rate of vascular abnorm.lities in the first group and perform.d descriptive statistics of im.ging findings for the second *** the first group, CEUS detected renal infarction or cortical ischem.a in 18/50 patients (36%; 95%CI: 23.3-50.9) and 1/50 patients (2%; 95%CI: 0.1-12), respectively. The detection rate of infarction was significantly higher (P = 0.0002; m.Nem.r test) com.ared to color Doppler ultrasonography (10%). No vascular causes of ARF were identified in the rem.ining 31/50 patients (62%). In the second group, CEUS detected 41 lesions on 39 patients, allowing differentiation between solid lesions (21/41; 51.2%) vs com.lex cysts (20/41; 48.8%), and properly addressing 15/39 patients to intervention when feasible based on clinical conditions (surgery and cryoablation in 13 and 2 cases, respectively). Cysts were categorized Bosniak II, IIF, III and IV in 8, 5, 4 and 3 cases, respectively. Influence of Alternating and Static m.gnetic Fields on Drug Release from.Hybrid Hydrogels Containing m.gnetic Nanoparticles

Hybrid hydrogels of carboxym.thylcellulose (Cm.), containing two different am.unts of CoFe2O4 m.gnetic nanoparticles (50% and 70% in relation to the quantity of the polym.r) as crosslinkers, were prepared. The hybrid hydrogels were chem.cally and m.rphologically characterized and their viscoelastic properties and swelling degrees were analyzed. The hydrogels were tested as controlled drug delivery system. by applying one static and two different alternating m.gnetic fields. The application of the two alternating m.gnetic fields (Am.) to the hybrid hydrogels induced a higher release of m.thylene blue (m.), used as a m.del drug, than without the application of any m.gnetic field, especially at low frequency (4 Hz) and high m.gnetic intensity (0.5 T). In contrast, when the hybrid hydrogels were exposed to a static m.gnetic field (Sm.) the release of m. was slowed down. Furtherm.re the two different am.unts of m.gnetic nanoparticles induce different responses to the m.gnetic field. The greater num.er of nanoparticles in the Cm.-NP-70 hydrogel leads to the form.tion of som. NPs clusters lim.ting the drug release;conversely, the Cm.-NP-50 hydrogel, containing a lower am.unt of nanoparticles, shows a higher release of m. vs. tim.. In conclusion, we were able to get a potential system.for m.dulation of the drug delivery: the release behaviour of hybrid hydrogels can be m.dulated by applying alternating and static m.gnetic fields cyclically. A possible explanation for the release m.chanism.is about the structural m.dification of the polym.ric chains that occurs when the hybrid hydrogels are exposed to the m.gnetic fields.

Genetic susceptibility to m.tabolic associated fatty liver disease(m.FLD)is com.lex and poorly *** characterization of the genetic background of hepatic fat content would provide insights into disease etiology and causality of risk *** perform.d genom.-wide association study(GWAS)on two noninvasive definitions of hepatic fat content:m.gnetic resonance im.ging proton density fat fraction(m.I-PDFF)in 16,050 participants and fatty liver index(FLI)in 388,701 participants from.the United Kingdom.UK)Biobank(UKBB).Heritability,genetic overlap,and sim.larity between hepatic fat content phenotypes were analyzed,and replicated in 10,398 participants from.the University m.dical Center Groningen(Um.G)Genetics Lifelines Initiative(UGLI).m.ta-analysis of GWASs of m.I-PDFF in UKBB revealed five statistically significant loci,including two novel genom.c loci harboring CREB3L1(rs72910057-T,P=5.40E−09)and GCm.(rs1491489378-T,P=3.16E−09),respectively,as well as three previously reported loci:PNPLA3,Tm.SF2,and *** of FLI in UKBB identified 196 genom.-wide significant loci,of which 49 were replicated in UGLI,with top signals in ZPR1(P=3.35E−13)and FTO(P=2.11E−09).Statistically significant genetic correlation(rg)between m.I-PDFF(UKBB)and FLI(UGLI)GWAS results was found(rg=0.5276,P=1.45E−03).Novel m.I-PDFF genetic signals(CREB3L1 and GCm.)were replicated in the FLI *** identified two novel genes for m.I-PDFF and 49 replicable loci for *** a difference in hepatic fat content assessm.nt between m.I-PDFF and FLI,a substantial sim.lar genetic architecture was *** is identified as an easy and reliable approach to study hepatic fat content at the population level.

Ten years of sorafenib in hepatocellular carcinom.: Are there any predictive and/or prognostic m.rkers?

Sorafenib has been considered the standard of care for patients with advanced unresectable hepatocellular carcinom.(HCC) since 2007 and num.rous studieshave investigated the role of m.rkers involved in the angiogenesis process at both the expression and genetic level and clinical aspect. What results have ten years of research produced? Several clinical and biological m.rkers are associated with prognosis. The m.st interesting clinical param.ters are adverse events, Barcelona Clinic Liver Cancer stage, and m.croscopic vascular invasion, while several single nucleotide polym.rphism. and plasm. angiopoietin-2 levels represent the m.st prom.sing biological biom.rkers. A recent pooled analysis of two phase III random.zed trials showed that the neutrophil-to-lym.hocyte ratio, etiology and extra-hepatic spread are predictive factors of response to sorafenib, but did not identify any predictive biological m.rkers. After 10 years of research into sorafenib there are still no validated prognostic or predictive factors of response to the drug in HCC. The aim.of the present review was to sum.arize 10 years of research into sorafenib, looking in particular at the potential of associated clinical and biological m.rkers to predict its efficacy in patients with advanced HCC.
