Sleep Fragmentation and Risk of Automobile Accidents in Patients with Obstructive Sleep Apnea—Sleep Fragmentation and Automobile Accidents in OSA

Objectives: Automobile accidents related to obstructive sleep apnea (OSA) represent an important public health concern. Frequent arousal response during sleep is responsible for the symptoms of daytime sleepiness and an increased risk of automobile accidents in patients with OSA. We therefore investigated the potential relationships between the intensity of arousal and occurrence of automobile accidents in OSA. Methods: We determined the incidence of automobile accidents in the past 5 years among 51 men with OSA (age, 58.5 ± 10.4 years) using a questionnaire. Daytime sleepiness was rated with the Epworth Sleepiness Scale (ESS). We calculated an automobile accident score, with two points per automobile accident causing damage and one point per near miss. Standard polysomnography was performed for all patients, and arousals were classified as electroencephalographic (EEG) arousal of an abrupt shift in EEG frequency alone and Movement arousal, which was defined as EEG arousal with increased electromyographic activity persisting for ≥3 s. The number of EEG or Movement arousals per hour (arousal index) was determined. Results: The Movement arousal index was significantly greater in the 27 patients who experienced at least one automobile accident causing damage than in the resting 24 patients who had no history of automobile accidents. Multiple regression analysis including age, body mass index, and sleep parameters revealed that the Movement arousal index was the most significant factor of risk for automobile accidents (β = 0.563, P = 0.017) as well as the ESS score (β = 0.417, P = 0.022). Conclusion: Movement arousal during sleep contributes to daytime sleepiness and the resulting frequent involvement of OSA patients in automobile accidents.

Molecular Functions of Oxygen-Evolving Complex Family Proteins in Photosynthetic Electron Flow

Oxygen-evolving complex （OEC） protein is the original name for membrane-peripheral subunits of photosystem （PS） II. Recently, multiple isoforms and homologs for OEC proteins have been iden- tified in the chloroplast thylakoid lumen, indicating that functional diversification has occurred in the OEC family. Gene expression profiles suggest that the Arabidopsis OEC proteins are roughly categorized into three groups： the authentic OEC group, the stressresponsive group, and the group including proteins related to the chloroplast NAD（P）H dehydrogenase （NDH） complex involved in cyclic electron transport around PSI. Based on the above gene expression profiles, molecular functions of the OEC family proteins are discussed together with our current knowledge about their functions.

Autism spectrum disorder and maternal immune activation: Environmental factors during pregnancy, such as infections, maternal stress or autoimmune disorders, are closely associated with the prevalence of neurodevelopmental disorders including autism spectrum disorder(ASD), bipolar disorder and *** has been shown that severe infections during pregnancy cause maternal immune activation(MIA) and significantly increase the risk of ASD in the offspring although the mechanisms are poorly *** rodent MIA studies support this causal link by showing that offspring of dams administered with polyinosinic:polycytidylic acid(poly I:C), a viral mimetic Toll-like receptor 3 agonist, exhibit longlasting ASD-like behavioral abnormalities such as increased repetitive behavior, impaired social interaction and ***, MIA alters inflammatory cytokine expressions persisting through development and adulthood in the brain of the offspring, suggesting that chronic neuroimmune dysfunction plays a role in mediating the deleterious effects of MIA on neurodevelopment(Garay et al., 2013).

Cardiac biomarkers in children with congenital heart disease

Survey of Bereavement Care Provided by Home-Visit Nurses for Bereaved Families in Japan

Purpose: This study aims to investigate the implementation conditions of bereavement care for bereaved families and the relationship between demographics of home-visit nurses and rates of provision of bereavement care. Methods: A self-rating anonymous questionnaire survey was conducted with 2200 facilities randomly selected from among the members of the Home-visit Nursing Stations of the National Nursing Business Association. Calculating the rates of provision of each pre- and post-bereavement care item of the survey, we performed a multiple logistic regression analysis to investigate the relationship between the demographics of home-visit nurses and rates of provision of care. Statistical processing employed SPSS ver. 21.0 with the significance level set at 5%. Results: A total of 688 responses were collected (collection rate, 31.3%), and 649 were included in the analysis. “Approval of the care that the family has provided” and “Involvement in the care together with the family and listening to their thoughts” were frequently provided in both pre- and post-bereavement care;however, the rate of provision of “Provided continued support/life planning until the family fully recovers social life” was low. More care services were provided by home-visit nurses working in facilities with 24-hour services in pre-bereavement, and by those with longer home-visit nursing experience in post-bereavement. Conclusions: Home-visit nurses provide many post-bereavement care services. However, continued care that helps bereaved families fully recover their social life, including life planning, is less frequently provided. Bereavement care provision rates vary depending on the facility type, whether the facility offers 24-hour services, and the length of home-visit nursing experience.

BACKGROUND:Stent migration in the hepatopancreatic duct might arise as one of the rare complications associated with biliary or pancreatic *** there are some procedures to retrieve the migrated stent,including surgical,percutaneous,and endoscopic approaches,endoscopy should be attempted first because it is least *** study set out to evaluate the usefulness of endoscopic retrieval of migrated biliary and pancreatic ***:Plastic stents that migrated in the bile duct(35 patients)or pancreatic duct(2)were retrieved with endoscopic retrograde *** used were snare forceps,a basket catheter,grasping forceps,biopsy forceps,a balloon catheter,and the Soehendra stent ***:Endoscopic retrieval of migrated stents was performed successfully in 36(97.0%)of the 37 *** devices utilized for successful treatment were basket catheter(13 patients),grasping forceps(10),snare forceps(8),balloon catheter(3),biopsy forceps(1),and the Soehendra stent retriever(1).The unsuccessfully treated patient with chronic pancreatitis underwent surgery since the guide wire did not move forward due to bile duct stenosis,and there was also duodenal *** patient developed mild pancreatitis after withdrawal of the stent;the pancreatitis was relieved with conservative ***:Endoscopic retrieval of migrated biliary and pancreatic stents appears to be useful because of its safety and low ***,various forceps should be prepared for the retrieval of a migrated stent.

Effects of Propofol on Glutamate-Induced Calcium Mobilization in Presynaptic Boutons of Rat Hippocampal Neurons

Recent reports have suggested that various general anesthetics affect presynaptic processes in the central nervous system. However, characterizations of the influence of intravenous anesthetics on neurotransmitter release from presynaptic nerve terminals (boutons) are insufficient. Because the presynaptic calcium concentration ([Ca2+]pre) regulates neurotransmitter release, we investigate the effects of the intravenous anesthetic propofol on neurotransmitter release by measuring [Ca2+]pre in the presynaptic boutons of individual dissociated hippocampal neurons. Brain slices were prepared from Sprague–Dawley rats (10 - 14 days of age). The hippocampal CA1 area was isolated with a fire-polished glass pipette, which vibrated horizontally to dissociate hippocampal CA1 neurons along with their attached presynaptic boutons. Presynaptic boutons were visualized under a confocal laser scanning microscope after staining with FM1-43 dye, and [Ca2+]pre was measured using fluo-3 AM dye. Glutamate (3 – 100 μM) administration increased [Ca2+]pre in Ca2+- containing external solution in a concentration-dependent manner. Propofol (3 – 30 μM) dose-dependently suppressed this glutamate (30 μM)-induced increase in [Ca2+]pre in boutons attached to dendrites, but not to the soma or base of the dendritic tree. The large majority of excitatory synapses on CA1 neurons are located on dendritic spines;therefore, propofol may affect glutamate-induced Ca2+ mobilization in excitatory, but not inhibitory, presynaptic boutons. Propofol may possibly have some effect on glutamate-regulated neurotransmitter release from excitatory presynaptic nerve terminals through inhibiting the increase in [Ca2+]pre induced by glutamate.
