The present study investigated the effect of as-built and post heat-treated microstructures of IN738LC alloy fabricated via selective laser melting process on high temperature oxidation *** as-built microstructure showed fine cell and columnar structure due to high cooling *** element segregation was observed in inter-cell/inter-columnar *** post heat-treatment,the initially-observed cell structure disappeared,instead bimodal Ni_3(Al,Ti)particles *** temperature(1273 K and 1373 K)oxidation test results showed parabolic oxidation curves regardless of temperature and initial *** as-built IN738LC fabricated via the selective laser melting process displayed oxidation resistance similar to or slightly better than that of IN738LC fabricated via wrought or cast ***-treated SLM IN738LC,although had similar oxidation weight-gain values to those of the SLM asbuilt material at 1273K,showed relatively better oxidation resistance at 1373 *** Ni_3(Al,Ti)precipitate formed in the post heat treatment changed the local chemical composition,thereby led to changes in alumina former/chromia former location and fraction on the alloy *** was concluded that in heat-treated IN738LC increased alumina former fraction was found,and this resulted in excellent oxidation resistance and relatively low weight-gain.

Systemic air embolism through a bronchovenous fistu-la(BVF) has been described in patients undergoing positive-pressure ventilation. However, no report has mentioned the potential risks of systemic air embolism through a BVF in patients undergoing extracorporeal membrane oxygenation(ECMO). Positive-pressure ventilation and ECMO support in patients with lung injury can increase the risk of systemic air embolism through a BVF. Increased alveolar pressure, decreased pulmonary venous pressure, and anticoagulation are thought to be the factors that contribute to this complication. Here, we present a case of systemic air embolism in a patient with ECMO and mechanical ventilator support.

Background: Elbow dislocations in pediatric patients are rare injuries. This is a therapeutic emergency because a delay in treatment can have disastrous consequences on the functional level of the upper limb. Objective: To present the management of elbow dislocation in children in cases of limited resources. Presentation of the Cases: These were 2 older male children aged 7 and 9 years old, admitted to the emergency room for painful functional impotence of the right elbow after a fall and landing on the right hand. Clinical and radiological examinations were in favor of posterior elbow dislocations. The reductions were carried out under sedation and immobilization in Jersey. According to Robert’s criteria, the functional result was excellent in both patients. Conclusion: Emergency reduction and immobilization whatever the means ensure an excellent functional prognosis even in cases of limited resources.

AIM To study complete dose-dependent effects of obestatin on lipolytic and glucose transport activities in human adipocyte preparations highly responsive to *** Adipocytes were prepared by liberase digestion from subcutaneous abdominal adipose tissue obtained from overweight subjects undergoing plastic surgery. The index of lipolytic activity was the glycerol released in the incubation medium, while glucose transport was assessed by [~3H]-2-deoxyglucose uptake *** When tested from 0.1 nmol/L to 1 μmol/L, obestatin did not stimulate glycerol release; it did not inhibit the lipolytic effect of isoprenaline and did not alter the insulin antilipolytic effect. Obestatin hardly activated glucose transport at 1 μmol/L only. Moreover, the obestatin stimulation effect was clearly lower than the threefold increase induced by insulin 100 nmol/*** Low doses of obestatin cannot directly influence lipolysis and glucose uptake in human fat cells.

Here, we present the case of a 53-year-old man with a hepatothorax due to a right diaphragmatic rupture related to duodenal ulcer perforation. On admission, the patient complained of severe acute abdominal pain, with physical examination findings suspicious for a perforated peptic ulcer. Of note, the patient had no history of other medical conditions or recent trauma, and the initial chest radiography and laboratory findings were not specific. A subsequent abdominal computed tomography revealed intrathoracic displacement of the liver, gallbladder, transverse colon and omentum through a right diaphragmatic defect. The patient then underwent an explorative laparotomy that confirmed duodenal ulcer perforation. A primary repair of the duodenal perforation was performed, and the diaphrag-matic defect was repaired using a polytetrafluoroeth-ylene patch after the organs were reduced and the cavity irrigated. This particular case proves interesting as right-sided spontaneous diaphragmatic ruptures are very rare and difficult to diagnose. Additionally, the best treatment for such large diaphragmatic defects is still controversial, especially in cases of intrathoracic or intra-abdominal contamination.

Recurrent Neural Networks(RNNs)have been widely applied to deal with temporal problems,such as flood forecasting and financial data *** the one hand,traditional RNNs models amplify the gradient issue due to the strict time serial dependency,making it difficult to realize a long-term memory *** the other hand,RNNs cells are highly complex,which will signifi-cantly increase computational complexity and cause waste of computational resources during model *** this paper,an improved Time Feedforward Connections Recurrent Neural Networks(TFC-RNNs)model was first proposed to address the gradient issue.A parallel branch was introduced for the hidden state at time t−2 to be directly transferred to time t without the nonlinear transforma-tion at time t−*** is effective in improving the long-term dependence of ***,a novel cell structure named Single Gate Recurrent Unit(SGRU)was *** cell structure can reduce the number of parameters for RNNs cell,consequently reducing the computational ***,applying SGRU to TFC-RNNs as a new TFC-SGRU model solves the above two ***,the performance of our proposed TFC-SGRU was verified through sev-eral experiments in terms of long-term memory and anti-interference *** results demonstrated that our proposed TFC-SGRU model can cap-ture helpful information with time step 1500 and effectively filter out the *** TFC-SGRU model accuracy is better than the LSTM and GRU models regarding language processing ability.

Neural Machine Translation(NMT)is an end-to-end learning approach for automated translation,overcoming the weaknesses of conventional phrase-based translation *** NMT based systems have gained their popularity in commercial translation applications,there is still plenty of room for *** the most popular search algorithm in NMT,beam search is vital to the translation ***,traditional beam search can produce duplicate or missing translation due to its target sequence selection *** to alleviate this problem,this paper proposed neural machine translation improvements based on a novel beam search evaluation *** we use reinforcement learning to train a translation evaluation system to select better candidate words for generating *** the experiments,we conducted extensive experiments to evaluate our *** corpus and the 1,000,000 pairs of bilingual corpora of NiuTrans are used in our *** experiment results prove that the proposed methods can effectively improve the English to Chinese translation quality.

Bile duct cancers are frequently diagnosed as advanced diseases. Over half of patients with advanced bile duct cancer present with unresectable malignancies and their prognosis has been very poor even after curative resections. Although there has been a need to diagnose bile duct cancer at its eady stage, it has been a difficult goal to achieve due to our lack of knowledge regarding this disease entity. Early bile duct cancer may be defined as a carcinoma whose invasion is confined within the fibromuscular layer of the extrahepatic bile duct or intrahepatic large bile duct without distant metastasis irrespective of lymph node involvement. Approximately 3%-10% of resected bile duct cancers have been reported to be early cancers in the literature. The clinicopathological features of patients with early bile duct cancer differ from those of patients with advanced bile duct cancer, with more frequent asymptomatic presentation, characteristic histopathological findings, and excellent prognosis. This manuscript is organized to emphasize the need for convening an international consensus to develop the concept of early bile duct cancer.
