This study aimed to examine the clinical and radiographic outcomes of primary total knee arthroplasy（TKA） with use of Nex Gen#174; Legacy#174; Constrained Condylar Knee（CCK） prosthesis for severe knee deformity. Clinical data of 46 patients（48 knees in total, aged 61 years on average） with severe knee deformity who underwent TKA with Nex Gen#174; Legacy#174; CCK prosthesis between December 2007 and February 2012 were retrospectively analyzed. There were 34 knees with severe valgus with incompetent medial collateral ligament, 11 knees with severe flexion contracture with inability to achieve knee balancing in flexion and extension by posterior soft tissue release, 2 knees with Charcot arthritis with severe varus and bone loss, and 1 with traumatic osteoarthritis with severe varus and ligamentous instability. The mean duration of follow-up was 71 months（range 40–90 months）. The New Knee Society scoring（NKSS） system and the Hospital for Special Surgery（HSS） score were used to evaluate the functional and clinical outcomes. Visual Analogue Scale（VAS） was used for pain measurement and Knee Society criteria for evaluation of radiological images. The results showed that, in the total 48 knees, 1 case of loosening due to short-stem tibial component at 3 months post-operatively underwent revision. The 6-year prosthesis survival rate in this cohort was 97.9%. There was no component infection occurring within 6 years. Significant post-operative improvements were found in NKSS and HSS scores. Patient satisfaction was significantly increased. Pain score was decreased significantly. Total functional score was improved from 31.46±11.43 to 86.42±8.87, range of motion（ROM） from 42.42°±23.57° to 95.31°±23.45° and the flexion contracture from 5.31°±7.87° to 0.92°±1.80°. Preoperative radiographic study showed excessive valgus（≥7°） in 37 knees, and varus deformity in 3 knees. Post-operative femorotibial alignment was valgus 3.88°±1.76° in 48 knees. 背景:腹腔镜阑尾切除术和开腹阑尾切除术都是治疗急性阑尾炎的有效手段。目前腹腔镜阑尾切除术已广泛普及,但人们对腹腔镜阑尾切除术和开腹阑尾切除术孰优孰劣仍存在争议。本研究主要从手术时间、住院时间和术后并发症发生率这几方面对腹腔镜阑尾切除术和开腹阑尾切除术进行对比研究。方法:对2016年1月至2016年12月期间的226例(111例男性和115例女性)急性阑尾炎手术患者进行回顾性研究。将226例患者分成两组,腹腔镜阑尾切除术组54例,开腹阑尾切除术组172例。两组间计数资料比较采用卡方检验,计量资料比较采用t检验。结果:腹腔镜阑尾切除术组患者平均手术时间为61.52分钟,而开腹阑尾切除术组患者为39.61分钟(p<0.001);前者平均住院时间为2.69天,而后者平均住院时间为4.03天(p<0.001);前者术后恢复到正常生活和工作状态的平均时间为12天,而后者为21天(p<0.001);前者术后有8例患者(14.8%)出现中度以上疼痛,而后者为58例(33.7%)(p<0.005)。腹腔镜阑尾切除术组有1例患者(1.8%)出现术后切口感染,开腹阑尾切除术组有17例患者(9.8%)出现术后切口感染。结论:腹腔镜阑尾切除术和开腹阑尾切除术一样安全有效,其优势是更微创,术后疼痛减少且恢复更快,能缩短患者住院时间,缺点是手术时间相比开腹手术更长。相比较开腹阑尾切除术,腹腔镜阑尾切除术术后感染率明显下降。

Yarsagumba(Ophiocordyceps sinensis), an endemic species to the Himalayas and Tibetan Plateau, is one of the most valuable medicinal mushrooms in the world. In Nepal, it is distributed largely in isolated patches of alpine grasslands of 3,000-5,000 m elevation. Although it is reported from 27 northernmost districts of Nepal, the local distribution pattern of this species is largely ***, the collection system and local management regime of this species are not well documented. We conducted a field survey at Dhorpatan Hunting Reserve(DHR) among the different stakeholders in January-June 2012 to assess collection sites, patterns and trends and to understand the management regime. We estimated that about 75 kg of Yarsagumba is collected every year from DHR and the amount has been declining since 2008. To manage the resource, locals have initiated regulating the collection by issuing permits, taxing to the collectors, and monitoring the activities of harvesters with the help of park authorities. The revenue generated at local level from the permits has been used for community developmental activities.

The Moth Flame Optimization(MFO)algorithm shows decent performance results compared to other meta-heuristic algorithms for tackling non-linear constrained global optimization ***,it still suffers from obtaining quality solution and slow convergence *** the other hand,the Butterfly Optimization Algorithm(BOA)is a comparatively new algorithm which is gaining its popularity due to its simplicity,but it also suffers from poor exploitation *** this study,a novel hybrid algorithm,h-MFOBOA,is introduced,which integrates BOA with the MFO algorithm to overcome the shortcomings of both the algorithms and at the same time inherit their *** performance evaluation,the proposed h-MFOBOA algorithm is applied on 23 classical benchmark functions with varied *** tested results of the proposed algorithm are compared with some well-known traditional meta-heuristic algorithms as well as MFO *** rank test and Wilcoxon signed rank test are employed to measure the performance of the newly introduced algorithm *** computational complexity has been ***,the proposed algorithm has been applied to solve one constrained and one unconstrained real-life problems to examine its problem-solving capability of both type of *** comparison results of benchmark functions,statistical analysis,real-world problems confirm that the proposed h-MFOBOA algorithm provides superior results compared to the other conventional optimization algorithms.

Livestock rearing and agriculture are the main sources of community-based livelihoods in western *** the rural mid-hills region of Gandaki Province,leopards are the top predator and frequently depredate livestock and attack *** patterns of human-leopard conflicts(HLC) in Nepal are poorly known at the provincial and national scales,which are essential to formulating effective conflict mitigation strategies and implementing them in the *** study aims to analyze the spatiotemporal pattern of HLC by applying Maxent modeling to covariates relating to known and registered conflict cases(n=842) collected from Nepalese government *** found that cases of HLC have been increasing significantly over the past five *** also concluded that mid-elevation,south-facing slopes were more susceptible to HLC,but that mean annual temperature was by far the most important predictor of HLC;overall livestock density and proximity to roads were also important,but secondarily *** we found the increase in human fatalities to 2.16/year was significant(p0.05).However,we also found an increasing trend in livestock depredation rates for this same five-year period(p<0.05),which averaged 159.6 head/year among incidents *** also found that winter was the main season when depredations occurred,and that goats were the most depredated of all livestock.A total US $86,892.25($17,378.45/year) of economic losses were incurred by communities during this time,with 78.57% of the total value reimbursed as compensatory relief through the government’s relief *** recommend that the use of predator-proof livestock corrals,greater awareness in local communities about wildlife behavior,better animal husbandry and security practices,and a more efficient compensation program,can improve coexistence between leopard populations and human communities in western Nepal.

Effects of delay in immunological response of HIV infection

In this paper, a human immunodeficiency virus （HIV） infection model with both the types of immune responses, the antibody and the killer cell immune responses has been introduced. The model has been made more logical by including two delays in the acti-vation of both the immune responses, along with the combination drug therapy. The inclusion of both the delayed immune responses provides a greater understanding of long-term dynamics of the disease. The dependence of the stability of the steady states of the model on the reproduction number R0 has been explored through stability theory. Moreover, the global stability analysis of the infection-free steady state and the infected steady state has been proved with respect to R0. The bifurcation analysis of the infected steady state with respect to both delays has been performed. Numerical simulations have been carried out to justify the results proved. This model is capable of explaining the long-term dynamics of HIV infection to a greater extent than that of the existing model as it captures some basic parameters involved in the system such as immunological delay and immune response. Similarly, the model also explains the basic understanding of the disease dynamics as a result of activation of the immune response toward the virus.

Moth Flame Optimization(MFO)is a nature-inspired optimization algorithm,based on the principle of navigation technique of moth toward *** to less parameter and easy implementation,MFO is used in various field to solve optimization ***,for the complex higher dimensional problems,MFO is unable to make a good trade-off between global and local *** overcome these drawbacks of MFO,in this work,an enhanced MFO,namely WF-MFO,is introduced to solve higher dimensional optimization *** a more optimal balance between global and local search,the original MFO’s exploration ability is improved by an exploration operator,namely,Weibull flight *** addition,the local optimal solutions have been avoided and the convergence speed has been increased using a Fibonacci search process-based technique that improves the quality of the solutions ***-nine benchmark functions of varying complexity with 1000 and 2000 dimensions have been utilized to verify the projected *** popular algorithms and MFO versions have been compared to the achieved *** addition,the robustness of the proposed WF-MFO method has been evaluated using the Friedman rank test,the Wilcoxon rank test,and convergence *** to other methods,the proposed WF-MFO algorithm provides higher quality solutions and converges more quickly,as shown by the ***,the proposed WF-MFO has been used to the solution of two engineering design issues,with striking *** improved performance of the proposed WF-MFO algorithm for addressing larger dimensional optimization problems is guaranteed by analyses of numerical data,statistical tests,and convergence performance.
