Heterotopic gastric mucosa in the anus and rectum:first case report of endoscopic submucosal dissection and systematic review

Background:Heterotopic gastric mucosa(HGM)is the most reported epithelial heterotopia,but it is very rare in the rectum and ***:The first case of an asymptomatic adult male with a large nonpolypoid HGM in the low rectum underwent complete resection by endoscopic submucosal dissection(EsD)is *** systematic review was based on a comprehensive search of MEDLINE,EMBAsE and Google *** on humans were identified with the term‘heterotopic gastric mucosa in the rectum and/or anus.’Results:The search identified 79 citations,and 72 cases were evaluated comprising the present *** malformations were observed in 17(24%)patients;rectal duplication accounted for most of the *** HGM was located in the anus and perineal rectum in 25 cases(41%)and low,middle and proximal pelvic rectum in 20(33%),five(8%)and 11 cases(18%),*** was nonpolypoid in 37 cases(51%),polypoid in 26 cases(36%)and ulcerated in nine cases(13%).specific anorectal symptoms were reported by 50(69%)patients of the whole study population,and by 33(97%)of 34 patients≤18 *** were observed in 23 cases(32%).The HGM was excised in 50 cases(83%).Endoscopic resection was performed in 17 cases(34%);resection was piecemeal in five of 12 lesions≥5mm,required argon plasma coagulation in two cases and was associated with residual tissue in two(17%).Intestinal metaplasia and an adenoma with lowgrade dysplasia were described in three adults(4%).Discussion:This systematic review shows that the HGM in the rectum and anus may be associated with specific rectal symptoms and serious complications,mainly in the pediatric population,and a risk of malignancy in *** complete excision should be recommended,and the EsD can overcome the technical limits of conventional endoscopic snare resection and avoid unnecessary surgery.

Evaluation of the sperm DNA fragmentation index in infertile Japanese men by in-house flow cytometric analysis

semen analysis has long been used to evaluate male ***,several sperm function tests have been *** those,the sperm DNA fragmentation index(DFI),which describes the status of the sperm DNA,is thought to be a suitable parameter for evaluating male ***,there have been no large-scale studies on the sperm DFI of Japanese ***,we investigated the feasibility of using an in-house flow cytometry-based sperm DFI analysis based on the sperm DNA fragmentation test of sperm chromatin structure assay(sCsA)to assess male fertility in *** study enrolled 743 infertile and 20 fertile Japanese *** evaluate reproducibility,inter-and intraobserver precision was analyzed.A receiver operating characteristic curve analysis was used to set a cutoff value for the sperm DFI to identify men who could father children by timed intercourse or intrauterine *** variability of the sperm DFI among fertile volunteers was *** relationship between semen parameters and the sperm DFI was assessed by spearman’s rho test.A precision analysis revealed good reproducibility of the sperm *** cutoff value of sperm DNA fragmentation in infertile men was 24.0%.semen volume had no relationship with the sperm *** concentration,sperm motility,total motile sperm count,and percentage of normal-shaped sperm were significantly and negatively correlated with the sperm *** median sperm DFI was smaller in fertile volunteers(7.7%)than that in infertile men(19.4%).sperm DNA fragmentation analysis can be used to assess sperm functions that cannot be evaluated by ordinary semen analysis.

On the Asymptotic stability of Discrete Crocodilians Model

The sex ratio of crocodiles is strongly biased towards females, often as high as 10 females to 1 male. In crocodilians, the temperature of egg incubation is the environmental factor determining sex. If the temperature is low, around 30˚C, the hatchlings are all females. Higher temperature, around 34˚C, hatch all males. This study was made to consider the asymptotic stability of a positive equilibrium point in a nonlinear discrete model of the basic nesting population model, which is described in three-region depending on the temperature of egg incubation. This model is based on key life-historical data and Murray’s research. To study above, we have applied the classical linearization method and P. Cull’s method and moreover, we employ non-standard discretization methods for later our Equations (6)-(8) and (15).

Enhanced thermoelectric performance in MnTe due to doping and insitu nanocompositing effects by Ag2s addition

Extremely low lattice thermal conductivity is always the pursuit of thermoelectric materials *** this work,we reported an exceptional effect of Ag2s addition in MnTe,an emerging promising midtemperature thermoelectric material,to enable the realization of minimum lattice thermal conductivity,namely-0.4 Wm^(-1) K^(-1).such a low lattice thermal conductivity is guaranteed by the incorporation of in-situ formed Ag rich phase(Ag2Te)with ultralow lattice thermal conductivity and further scattering of phonons from the partial doping effects induced point defects and boundaries between various *** from the dramatically decreased lattice thermal conductivity,the partial doping of Ag and s simultaneously enhance the electrical conductivity,further contributing to enhanced thermoelectric ***,an inverse sign of seebeck and Hall coefficient was observed and rationalized by the influence of highly electron-conductive Ag_(2)Te *** to the synergetic modulation of electrical and thermal transport properties by in-situ formed composite,a high ZT value of 1.1 was achieved in MnTe based thermoelectric materials,which also demonstrates the importance of compositing approaches to design state-of-the-art thermoelectric materials.

Magnetic anchor guidance for endoscopic submucosal dissection and other endoscopic procedures

Endoscopic submucosal dissection(EsD) is a wellestablished, minimally invasive treatment for superficial neoplasms of the gastrointestinal tract. The universal adoption of EsD has been limited by its slow learning curve, long procedure times, and high risk of complications. One technical challenge is the lack of a second hand that can provide traction, as in conventional surgery. Reliable tissue retraction that exposes the submucosal plane of dissection would allow for safer and more efficient dissection. Magnetic anchor guided endoscopic submucosal dissection(MAGEsD) has potential benefits compared to other current traction methods. MAG-EsD offers dynamic tissue retraction independent of the endoscope mimicking a surgeon's "second hand". Two types of magnets can be used: electromagnets and permanent magnets. In this article we review the MAG-EsD technology, published work and studies of magnets in EsD. We also review the use of magnetic anchor guidance systems in natural orifice transluminal endoscopic surgery and the idea of magnetic non-contact retraction using surface ferromagentization. We discuss the current limitations, the future potential of MAG-EsD and the developments needed for adoption of this technology.

Acute?leukemia?is?often?associated?with?oral?manifestations.?We?report?an?acute?promyelocytic?leukemia?(APL)?case?with?oral?manifestations?leading?to?the?diagnosis. A?21-year-old?female visited?our?hospital?with?complaints?of?gingival?bleeding?and?swelling?of?the?left?lower?wisdom?tooth.?The?patient’s?complete?blood?count?revealed?a?marked?increase?in?white?blood?cells?and?a?decrease?in?red?blood?cells?and?an?abnormal?leukocyte?differential,?and APL?was?diagnosed?on?the?basis?of?bone?marrow?samples in?the internal?medicine?department.?The?patient?was?treated?with?all-trans-retinoic?acid combined?with?chemotherapy?and?has?maintained?clinical?and?molecular?complete?remission?at?12?months?of?follow-up. Bone metastasis from early gastric cancer following non-curative endoscopic submucosal dissection

A 67-year-old male underwent endoscopic submucosal dissection(EsD)to treat early gastric cancer(EGC)in *** lesion(50 mm × 25 mm diameter)was histologically diagnosed as poorly differentiated adenocarcinoma,with an ulcer *** the tumor was confined to the mucosa with no evidence of lymphovascular involvement,the EsD was regarded as a noncurative resection due to the histological type,tumor size,and existence of an ulcer finding(as indicated by the 2010 Japanese gastric cancer treatment guidelines,ver.3).Despite strong recommendation for subsequent gastrectomy,the patient refused *** alternative follow-up routine was designed,which included five years of biannual clinical examinations to detect and measure serum tumor markers and perform visual assessment of recurrence by endoscopy and computed tomography scan after which the examinations were performed *** patient's condition remained stable for eight years,until a complaint of back pain in 2010 prompted further clinical *** scintigraphy indicated increased *** examination of biopsy specimens taken from the lumbar spine revealed adenocarcinoma resembling the carcinoma cells from the EGC that had been treated previously by EsD,and which was consistent with immunohistochemical findings of gastrointestinal tract ***,the diagnosis of bone metastasis from EGC was *** reported rates of EGC recurrence in surgically resected cases range 1.4%-3.4%,but among these bone metastasis is very *** our knowledge,this is the first reported case of bone metastasis from EGC following a non-curative EsD and occurring after an eight-year disease-free interval.
