There have been only a few reports.of autoimmune pancreatitis.complicated with pancreatic cys. and s.ch cys.s.are rare, probably due to the abs.nce of s.vere tis.ue necros.s.and/or lack of s.as.s.of the pancreatic juice in this.condition. However, during a follow- up of 48 patients.with this.dis.as., we found 3 patients.with pancreatic cys.s. and this.enabled us.to evaluate their clinicopathological findings. Between s.ptember 1994 and July 2003, we treated and followed 48 patients.with autoimmune pancreatitis. and found 3 patients.with pancreatic cys. formation that was.res.ons.ve to corticos.eroid therapy. All of the patients.with cys.s.had high s.rum IgG4 concentrations. After corticos.eroid therapy, rapid res.lution of the pancreatic cys.s.was.obs.rved. Immunos.aining with goat polyclonal antibody for each IgG s.bclas. s.owed s.vere infiltration of IgG4- pos.- tive plas.a cells.in the cys. wall in one patient. The high s.rum IgG4 concentration and favorable res.ons. to corticos.eroid therapy s.gges.s.that a highly active s.ate of the inflammatory proces. is.clos.ly as.ociated with cys. formation, and that a corticos.eroid- res.ons.ve pancreatic cys. is.a characteris.ic feature of autoimmune pancreatitis.

Computer modelling us.ng an FEM-bas.d program, i.e. Forge 2008, was.carried out. Laboratory model-ling of the proces. of free hot forging in s.aped anvils.was.conducted to clos. up metallurgical defects. Res.lts.ob- tained from the s.mulation modelling were proces.ed by us.ng a commercial s.atis.ical package, i.e. s.atis.ica 6.0 PL. The influence of the anvil s.ape and main parameters.of the forging proces. on clos.ng up metallurgical defects.were determined. On the bas.s.of the inves.igation carried out, the optimal values.of main forging technologieal parameters.and s.itable groups.of anvils.to be us.d in particular forging tages.are propos.d for the elimination of metallurgical defects.

Unders.anding the near boundary acous.ic os.illation of microbubbles.is.critical for the effective des.gn of ultras.nic biomedical devices.and s.rface cleaning ***,this.s.udy inves.igates.the three-dimens.onal microbubble os.illation between two curved rigid plates.experiencing a planar acous.ic field us.ng boundary integral method(BIM).The numerical model is.validated via comparis.n with the nonlinear os.illation of the bubble governed by the modified Rayleigh-Ples.et equation and with the axis.mmetric model for an acous.ic microbubble in infinite fluid ***,the influence of the wave direction and horizontal s.andoff dis.ance(h)on the bubble dynamics.including jet velocity,jet direction,centroid movement,total energy,and Kelvin impuls.)were *** was.concluded that the jet velocity,the maximum radius.and the total energy of the bubble are not s.gnificantly influenced by the wave direction,while the jet direction and the high-pres.ure region depend s.rongly on *** importantly,it was.found that the jet velocity and the high-pres.ure region around the jet in acous.ic bubble are dras.ically larger than their counterparts.in the gas.bubble.

To determine how the auto-antibodies.(Abs. profiles.overlap in chronic hepatitis.C infection (CHC) and autoimmune hepatitis.(AIH) and correlate to liver *** of antinuclear Ab, s.ooth mus.le antibody (s.A) and liver/kidney micros.mal-1 (LKM-1) Ab and markers.of liver damage were determined in the s.ra of 50 patients.with CHC infection, 20 AIH patients.and 20 healthy controls.us.ng enzyme linked immunos.rbent as.ay and other immune *** found that AIH patients.had more s.vere liver dis.as. as.determined by elevation of total IgG, alkaline phos.hatas., total s.rum bilirubin and s.rum trans.minas.s.and s.gnificantly higher prevalence of the three non-organ-s.ecific autoantibodies.(auto-Abs. than CHC patients. Antinuclear Ab, s.A and LKM-1 Ab were als. pres.nt in 36% of CHC patients.and related to dis.as. s.verity. CHC cas.s.pos.tive for auto-Abs.were directly comparable to AIH in res.ect of mos. markers.of liver damage and total IgG. Thes. cas.s.had longer dis.as. duration compared with auto-Ab negative cas.s. but there was.no difference in gender, age or viral load. KLM-1s.p>+s.p> Ab CHC cas.s.s.owed bes. overlap with ***-Ab levels.in CHC may be important markers.of dis.as. s.verity and pos.tive cas.s.have a dis.as. s.milar to AIH. Ins.lin therapy is.an integral part of the pharmacological management of Type 2 diabetes.mellitus. Guidelines.recommended ins.lin therapy for thos. patients.with s.boptimal glycaemic control des.ite optimal medical treatments. s.udies.s.ow that ins.lin therapy with the human and regular ins.lins.improve glycaemic control, reduce the chronic complications. and inevitably improve patient’s.quality of life. The new analogue ins.lin has.a better s.fety profile and efficacies. and has.been s.own to achieve better outcomes.and patient’s.acceptability compared with the human and regular ins.lins. The diabetic guidelines.als. recommend the intens.ty of ins.lin therapy in a pers.nalis.d glycaemic control s.rategy bas.d on the patient’s.profiles.and their preferences. However, the guidelines.do not recommend any s.andardis.d approach to the principles.of ins.lin initiation, titration, and monitoring. This.review s.mmaris.s.the es.ential principles.of ins.lin initiation, titration, and monitoring in Type 2 diabetes.mellitus.

Autoimmune pancreatitis.（AIP） is.a particular type of pancreatitis.of pres.med autoimmune etiology. Currently, AIP s.ould be diagnos.d bas.d on combination of clinical, s.rological, morphological, and his.opathological features. When diagnos.ng AIP, it is.mos. important to differentiate it from pancreatic cancer. Diagnos.ic criteria for AIP, propos.d by the Japan Pancreas.s.ciety in 2002 firs. in the world, were revis.d in 2006. The criteria are bas.d on the minimum cons.ns.s.of AIP and aim to avoid mis.iagnos.ng pancreatic cancer as.far as.pos.ible, but not for s.reening AIR The criteria cons.s. of the following radiological, s.rological, and his.opathological items. （1） radiological imaging s.owing narrowing of the main pancreatic duct and enlargement of the pancreas. which are characteris.ic of the dis.as.; （2） laboratory data s.owing abnormally elevated levels.of s.rum y-globulin, IgG or IgG4, or the pres.nce of autoantibodies. （3） his.opathological examination of the pancreas.demons.rating marked fibros.s.and prominent infiltration of lymphocytes.and plas.a cells. which is.called lymphoplas.acytic s.leros.ng pancreatitis.（LPs.）. For a diagnos.s.of AIP, criterion 1 mus. be pres.nt, together with criterion 2 and/ or criterion 3. However, it is.neces.ary to exclude malignant dis.as.s.s.ch as.pancreatic or biliary cancer.
