Hepatitis c virus (HcV) is a major health problem that leads to chronic hepatitis, cirrhosis and hepatocellular carcinoma, being the most frequent indication for liver transplantation in several countries. Unfortunately, HcV re-infects the liver graft almost invariably following reperfusion, with an accelerated history of recurrence, leading to 10%-30% of patients progressing to cirrhosis within 5 years of transplantation. In this sense, some groups have even advocated for not retransplanting this patients, as lower patient and graftoutcomes have been reported. However, the management of HcV recurrence is being optimized and several strategies to reduce post-transplant recurrence could improve outcomes, decrease the rate of re-transplantation and optimize the use of available grafts. Three moments may be the focus of potential actions in order to decrease the impact of viral recurrence: the pretransplant moment, the transplant environment and the post-transplant management. In the pre-transplant setting, it is not well established if reducing the pre transplant viral load affects the risk for HcV progression after transplant. Obviously, antiviral treatment can render the patient HcV RNA negative post transplant but the long-term benefit has not yet been fully established to justify the cost and clinical risk. In the transplant moment, factors as donor age, cold ischemia time, graft steatosis and ischemia/reperfusion injury may lead to a higher and more aggressive viral recurrence. After the transplant, discussion about immunosuppression and the moment to start the treatment (prophylactic, pre-emptive or once-confirmed) together with new antiviral drugs are of interest. This review aims to help clinicians have a global overview of posttransplant HcV recurrence and strategies to reduce its impact on our patients.

This study investigated the acute effects of green tea extract （GTE） and its polyphenol constituents, （-）-epigallocatechin-3-gallate （EGcG） and （-）-epicatechin （Ec）, on basal and stimulated testosterone production by rat Leydig cells in vitro. Leydig cells purified in a Percoll gradient were incubated for 3 h with GTE, EGcG or Ec and the testosterone precursor androstenedione, in the presence or absence of either protein kinase A （PKA） or protein kinase c （PKc） activators. The reversibility of the effect was studied by pretreating cells for 15 min with GTE or EGcG, allowing them to recover for 1 h and challenging them for 2 h with human chorionic gonadotropin （hcG）, luteinizing hormone releasing hormone （LHRH）, 22（R）-hydroxycholesterol or androstenedione. GTE and EGcG, but not Ec, inhibited both basal and kinase-stimulated testosterone production. Under the pretreatment conditions, the inhibitory effect of the higher concentration of GTE/EGcG on hcG/LHRH-stimulated or 22（R）- hydroxycholesterol-induced testosterone production was maintained, whereas androstenedione-supported testosterone production returned to control levels. At the lower concentration of GTE/EGcG, the inhibitory effect of these polyphenols on 22（R）-hydroxycholesterol-supported testosterone production was reversed. The inhibitory effects of GTE may be explained by the action of its principal component, EGcG, and the presence of a gallate group in its structure seems important for its high efficacy in inhibiting testosterone production. The mechanisms underlying the effects of GTE and EGcG involve the inhibition of the PKA/PKc signalling pathways, as well as the inhibition of P450 side-chain cleavage enzyme and 17β-hydroxysteroid dehydrogenase function.

Phytochemicals (PHT) are biologically active chemicals produced by plants, non-essential nutrients, with medicinal properties. In this short communication we report the dramatic improvement in the physical and clinical well-being of 9 terminal AIDS patients that received Phyto V7, a PHT mix, for a period of 3 months. All patients living in the Dr. cruz Jiminian Foundation (hospice) in Santo Domingo, Dominican Republic, were in an emaciated condition—most could not eat, sit down, shower, stand up or dress alone;all had high viremia (from ~50,000 to above 500,000;243,816 ± 176,724 HIV-1 RNA copies/ml) and very low cD4+ T-cells counts (142 ± 51 counts/mm3). The clinical status of all patients was c3 according to the United States centers for Disease control (cDc) status index. As antiretroviral treatment was not available to the Foundation at the time of the study, the only treatment that they received was Phyto V7 supplementation. Each individual received 5 tablets 3 times a day, each tablet containing 750 mg of Phyto V7. At the end of the 3 months, Phyto V7 supplementation radically improved the well-being of all 9 patients. All patients could eat, sit down, shower, stand up and dress alone. This study supports the notion that PHT supplementation can improve significantly the well-being of terminally ill AIDS patients and is the foundation to conducting further control studies to substantiate this notion.

Trading and Stock Behavioral Analysis Systems require efficient Artificial Intelligence techniques for analyzing large financial datasets and have become in the current economic landscape a significant challenge for multidisciplinary ***,Trading-oriented Decision Support Systems based on the chartist or Technical Analysis Relative Strength Indicator（RSI） have been published and used ***,its combination with Neural Networks as a branch of evolutionary computing which can outperform previous results remain a relevant approach which has not deserved enough *** this paper,we present the chartist Analysis Platform for Trading（cAST,in short） platform,a proof-of-concept architecture and implementation of a Trading Decision Support System based on the RSI N value calculation and Feed-Forward Neural Networks（FFNN）.cAST provides a set of relatively more accurate financial decisions yielded by the combination of Artificial Intelligence techniques to the N calculation for RSI and a more precise and improved upshot obtained from feed-forward algorithms application to stock value datasets.

AIM:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic c virus,especially in patients with advanced fibrosis. METHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis c genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOc) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOc and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOc(52%)(P cant differences. By degree of fibrosis,56% of patients were F4 and the highest SVR rates were recorded in the non-F4 patients,both TN and TE. In the analysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOc and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,IL28 B cc and non-F4; the type of treatment(BOc or TVR) was not significant. The lowest SVR values were presented by the F4-NR patients,treated with BOc(46%) or with TVR(45%). 28% of the patients interrupted the treatment,mainly by non-viral response(51%):this outcome was
