Crohn’s disease (CD) and ulcerative colitis (UC) are the two main forms of inflammatory bowel disease (IBD) with both overlapping and distinct clinical, pathological and biomolecular features. It has been suggested that pediatric IBD is a distinct disease entity, with probably different disease *** aim of this study is to review and summarize the evolution of the current concept of pediatric IBD. The results of this review reinforce the idea that pediatric CD and UC may be further classified in various clinicopathologic entities. For clinicians and pathologists convenience, practical algorithms for the distinction of the various subphenotypes of pediatric IBD are also provided.

AIM:To identify those with a micropapillary pattern,ascertain relative frequency and document clinicopathological characteristics by reviewing gastric ***:One hundred and fifty-one patients diagnosed with gastric cancer who underwent gastrectomy were retrospectively studied and the presence of a regional invasive micropapillary component was evaluated by light *** available hematoxylin-eosin(HE)-stained slides were histologically reviewed and 5 tumors were selected as putative micropapillary carcinoma when cancer cell clusters without a vascular core within empty lymphatic-like space comprised at least5%of the *** tissues from these 5 invasive gastric carcinomas were immunostained using an antimucin 1(MUC1)antibody(clone MA695)to detect the characteristic inside-out pattern and with D2-40antibody to determine the presence of intratumoral lymph *** of intraepithelial neutrophil apoptosis was evaluated in consecutive histological tissue sections by three independent methods,namely light microscopy with HE staining,the conventional terminal deoxynucleotidyl transferase-mediated d UTP-biotin nick end-labeling(TUNEL)method and immunohistochemistry for activated caspase-3(clone C92-605).RESULTS:Among 151 gastric cancers resected for cure,5(3.3%)were adenocarcinomas with a micropapillary *** of the patients died of disease from 6 to 23 mo and one patient was alive with metastases at 9 *** patients had advanced-stage cancer(≥p T2)and lymph node *** MUC1 immunostaining on the stroma-facing surface(inside-out pattern)of the carcinomatous cluster cells,together with negative immunostaining for D2-40 in the cells limiting lymphatic-like spaces,confirmed the true micropapillary pattern in these gastric *** all five cases,several micropapillae were infiltrated by *** staining,TUNEL assay and immunostaining for caspase-3 demonstrated apoptoticneutrophils within cytoplasmic vacuoles of

In the present review we have analyzed the clinical applications of endoscopic ultrasound-guided-fineneedle-aspiration(EUS-FNA) and the methodological aspects obtained by cell-block procedure(CBP) in the diagnostic approach to the gastrointestinal neoplastic pathology. CBP showed numerous advantages in comparison to the cytologic routine smears; in particular, better preservation of cell architecture, achievement of routine haematoxylin-eosin staining equivalent to histological slides and possibility to perform immunohistochemistry or molecular analyses represented the most evident reasons to choose this method. Moreover, by this approach, the differential diagnosis of solid gastrointestinal neoplasias may be more easily achieved and the background of contaminant nonneoplastic gastrointestinal avoided. Finally, biological samples collected by EUS-FNA CBP-assisted should be investigated in order to identify and quantify further potential molecular markers.

Changes in human epidermal growth factor receptor 2 status between primary breast/gastric carcinomas and synchronous metastatic lymph nodes: how can we explain them?

Aim:Previous studies demonstrated discordant expression of human epidermal growth-factor receptor 2(HER2)between primary cancer and their recurrence/*** study further evaluated HER2 status between primary gastric and breast invasive carcinomas and paired metastatic disease to lymph ***:This study collected formalin-fi xed paraffi n-embedded representative tissue blocks from 62 gastric and 65 breast primary carcinomas as well as synchronous metastatic lymph nodes(male:female=39:88;age ranged between 44 and 95 years with mean age of 69.32 years)for immunohistochemical staining of HER2 expression(DAKO HercepTest^(TM)kit).If immunohistochemical HER2 score reached to 2+,HER2 amplifi cation was then assessed using fl uorescence in situ hybridization(PharmDx^(TM)kit DAKO).Results:The discordant HER2 pooled rate,regardless either negative or positive conversion,was 9.67%in primary gastric carcinoma and corresponding nodal metastasis,while the changes in HER2 expression were revealed in 4.61%of mammary and lymph node neoplastic samples.A high-level concordance in HER2 expression between primary carcinoma and synchronous metastatic lymph nodes was confi rmed in both types of cancer;the observed event of discordant HER2 status should be ascribed to intra-tumor heterogeneity,mostly appreciable in gastric ***:In any case,the shift from positive to negative HER2 expression suggests that trastuzumab could be the targeted treatment choice whereas the opposite shift should be evaluated by a simultaneous HER2 determination in both primary and metastatic lymph nodes.

AIM： To evaluate the role of genetic factors in the pathogenesis of Crohn＇s disease （CD） and ulcerative colitis （UC）, we investigated the single nucleotide polymorphisms （SNPs） of NOD2/CARD15 （R702W, Gg08R and L1007finsC）, and Toll-like receptor 4 （TLR4） genes （D299G and T399I） in a selected inflammatory bowel disease （IBD） population coming from Southern Italy. METHODS： Allele and genotype frequencies of NOD2/ CARD15 （R702W, Gg08R and L1007finsC） and TLR4 （D299G and T399I） SNPs were examined in 133 CD patients, in 45 UC patients, and in 103 healthy controls. A genotype-phenotype correlation was performed. RESULTS： NOD2/CARD15 R702W mutation was significantly more frequent in CD （9.8%） than in controls （2.4%, P = 0.001） and in UC （2.3%, P = 0.03）. No significant difference was found between UC patients and control group （P 〉 0.05）. In CD and UC patients, no significant association with G908R variant was found. L1007finsC SNP showed an association with CD （9.8%） compared with controls （2.9%, P = 0.002） and UC patients （2.3%, P = 0.01）. Moreover, in CD patients, G908R and L1007finsC mutations were significantly associated with different phenotypes compared to CD wild-type patients. No association of IBD with the TLR4 SNPs was found in either cohort （allele frequencies： D299G-controls 3.9%, CD 3.7%, UC 3.4%, P 〉 0.05; T399I-controls 2.9%, CD 3.0%, UC 3.4%, P 〉 0.05）. CONCLUSION： These findings confirm that, in our IBD patients selected from Southern Italy, the NOD2/ CARD15, but not TLR4 SNPs, are associated with increased risk of CD.

Background:Hypertrophic scars and keloids,common complications following median sternotomy for cardiac surgery,signiﬁcantly impact patient quality of life due to their aesthetic and symptomatic *** advancements in laser therapy have made it a prominent option for managing these complex scars,yet a com-prehensive understanding of its efﬁcacy is *** aim of this scoping review is to explore the effects of laser therapy in managing hypertrophic scars and keloids after median ***:This scoping review ana-lyzed studies up to February 2024 from databases including PubMed,EMBASE,CINAHL,Scopus,Web of Science,and the Cochrane *** included any study that assessed laser therapy’s effects on hypertrophic scars and keloids following median *** were selected based on predeﬁned inclusion criteria with-out publication year,design,or origin ***:Six studies met the inclusion criteria,involving a total ofﬁve RCTs and one *** studies primarily tested 585 and 595-nm pulsed dye laser(PDL)treatments,focusing on scar appearance,patient symptoms,and treatment *** studies reported signiﬁcant improvements in scar height reduction and patient symptom relief after treatment,with mixed results for scar erythema and *** events were generally mild and ***:Laser therapy offers a beneﬁcial approach for improving the appearance and symptoms of hypertrophic scars and keloids post-median ***,further research is necessary to optimize treatment parameters and explore the long-term psychosocial impacts of this *** review highlights the need for more comprehensive studies to establish standardized treatment protocols and evaluate their effectiveness.

目的旨在翻译Chase护理管理者胜任力测评工具(the Chase Nurse Manager Competency Instrument,CNMCI)为意大利语,并对其进行信度和效度检验。方法对英文版CNMCI进行翻译,并通过横断面研究验证其信效度。采用预翻译(建立等效)、初始翻译、预测试、审查和管理5个步骤的问卷翻译方法,将英文版工具翻译为意大利语版。于2022年8月到2023年6月,选取意大利3个地区31所医疗机构349名护理管理者进行调查。采用内容效度、结构效度评估工具的效度;采用Cronbach'sα系数、重测信度评估工具的信度。结果意大利语版CNMCI,包括“知识理解”和“实施和/或使用能力”两个分量表,每个分量表包括53个条目。工具的条目内容效度指数(I-CVI)为0.83~1.00,两个分量表的内容效度指数(S-CVI)均为0.97。经验证性因子分析显示该工具各项拟合指数表现良好。工具中“知识理解”分量表的Cronbach'sα系数为0.978,“实施和/或使用能力”分量表的Cronbach'sα系数为0.976。各个维度之间的相关系数为0.494~0.908重测信度为0.820,表明工具的一致性较好。结论意大利语版CNMCI在测量护理管理者胜任力方面显示出良好的信度和效度,并支持该工具的多维度结构。此外,研究结果支持了这些因素的层次性质,进一步支持了该工具的有效性。
