AIM： To investigate distal small bowel motility and lipid absorption in patients following elective abdominal aortic aneurysm （AAA） repair surgery, METHODS： Nine patients （aged 35-78 years; body mass index （BMI） range： 23-36 kg/m2） post-surgery for AAA repair, and seven healthy control subjects （20-50 years; BMI range： 21-29 kg/m^2） were studied, Continuous distal small bowel manometry was performed for up to 72 h, during periods of fasting and enteral feeding （Nutrison）. Recordings were analyzed for the frequency, odgin, length of migration, and direction of small intestinal burst activity. Lipid absorption was assessed on the first day and the third day post surgery in a subset of patients using the ^13C-triolein-breath test, and compared with healthy controls. Subjects received a 20-min intraduodenal infusion of 50 mL liquid feed mixed with 200μL ^13C-triolein. End-expiratory breath samples were collected for 6 h and analyzed for ^13CO2 concentration. RESULTS： The frequency of burst activity in the proximal and distal small intestine was higher in patients than in healthy subjects, under both fasting and fed conditions （P〈 0.005）. In patients there was a higher proportion of abnormally propagated bursts （71% abnormal）, which began to normalize by d 3 （25% abnormal） *** absorption data was available for seven patients on d 1 and four patients on d 3 post surgery. In patients, absorption on d 1 post-surgery was half that of healthy control subjects （AUC ^13CO2 1323±244 vs 2646±365; P〈0.05, respectively）, and was reduced to the one-fitch that of healthy controls by d 3 （AUC ^13CO2 470±832 vs 2646±365; P〈 0.05, respectively）. CONCLUSION： Both proximal and distal small intestinal motor activity are transiently disrupted in critically ill patients immediately after major surgery, with abnormal motility patterns extending as far as the ileum. Basic infrastructures and services present an important socio-economic and territorial equity issue, because the availability of these makes it possible to improve the living conditions of populations, makes territories attractive and stimulates their development. The objective of our study has been achieved, so our results provided information on the rate of propagation of basic infrastructures and services, as well as the evolution of the forms of spatial distribution of the latter. The data used comes from the census and geolocation of basic infrastructures and services in the Niakhar area, carried out by the IRD in 2018. Centrographic measurements were used to describe the overall spatial distribution and the use of chi-square statistics confirmed the existence of a preferential direction of distribution. To verify the existence of a spatial structure of infrastructure and service seeding, the Ripley statistic is used. Our results can be useful for land use planning and spatial resource allocation policies. Indeed, the identification of different types of spatial aggregates and the highlighting of the preferred directions of the distribution of basic infrastructures and services, make it possible to correct disparities in the spatial distribution of basic resources.

Objective Patients with congenital malformations(CMs)of the gastrointestinal tract(GIT)have a very high ***,the literature on the factors associated with mortality in these patients is scarce in sub-Saharan *** aim of this study is to identify independent risk factors for mortality in patients with CMs of the GIT at our pediatric surgical *** We conducted a retrospective analysis of cases with CMs of the GIT managed at a tertiary center from 2018 to *** were subdivided into two groups based on the outcomes,and variables with a significant difference were analyzed by logistic *** Our review included 226 patients,63 of whom died(27.88%).Patient age ranged from 0 to 15 *** into account statistical significance,mortality was more frequent in neonates than in older patients(57.30%vs 6.15%),in patients coming out of the Dakar area than in those from the Dakar area(43.75%vs 19.18%),in patients with abnormal prenatal ultrasound than in those with normal ultrasound(100%vs 26.67%),in premature children than in those born at term(78.57%vs 21.87%),in patients with an additional malformation than in those with an isolated malformation(69.23%vs 25.35%),and in those with intestinal,esophageal,duodenal and colonic atresia than in those with other diagnoses(100%,89%,56.25%and 50%,respectively).Referred patients died more than those who changed hospitals or came from home(55.29%vs 25%and 9.09%,respectively).On multivariable logistic regression,two independent factors of mortality were identified:presence of associated malformation[odds ratio(OR)=13.299;95%Confidence interval(CI)1.370 to 129.137]and diagnosis of esophageal atresia(OR=46.529;95%CI 5.828 to 371.425).Conclusion The presence of an associated malformation or diagnosis of esophageal atresia increases mortality in patients with CMs of the GIT in our environment.

An Analysis of the Spring-to-Summer Transition in the West Central Plains for Application to Long Range Forecasting

The spring-to-summer transition is of special importance in long range forecasting, as the general circulation transitions to a less energetic regime. This affects the Midwestern United States in a profound way, since agriculture is very sensitive to the variability of weather and climate. Beginning at the local scale, surface temperature observations are used from a representative station in the West Central Missouri Plains region in order to identify the shift from late spring to early summer. Using upper-air re-analyses as a supplement, the 500-mb height observations are examined to find a spring-to-summer transition date by tracking the location of a representative contour. Each of these is used to identify spring-to-summer transition date and then statistical analysis is performed on this long-term data set. Finally, teleconnections, specifically the influence of El Ni?o Southern Oscillation (ENSO) and Pacific Decadal Oscillation (PDO), and blocking are examined in order to quantify interannual variability. It was found that examining these criteria, developed in an earlier study that covered a much shorter time period, produced similar statistics to this 68-year study of spring-to-summer transitions. It was also found that the onset of La Ni?a was associated with hotter summers in the region, a result first found in the earlier study, but this association was much stronger here.
