Pediatric brain tumors are a type of tumors that are commonly present in children and young adults.With the improvement of treatment,the quality of life,especially the cognitive functioning,is gaining increasingly more attention.Apart from cognitive evaluations,neuroimaging studies begin to play an important part in neurocognitive functioning investigation.In this way,the brain tissue changes caused by tumor variables(including tumor location and tumor size)and treatment variables(including surgery,chemotherapy and radiotherapy)can be detected by neuroimaging.Recent advancement of neuroimaging techniques,such as functionalMRI(f MRI)and diffusion tensor imaging(DTI),made great contributions to understanding cognitive dysfunction and quantifying the effects of tumor variables and treatment variables.In recent years,laminar-f MRI provided a potentially valuable tool for examining the exact origins of neural activity and cognitive function.On the other hand,molecular f MRI might guide diagnosis and treatment of brain disease in the future by using new biomarkers,and DTI can detect white matter changes and obtain some anatomically specific information.

BACKGROUND Biliary obstruction is a relatively common condition that affects approximately 5 in 1000 people annually.Malnutrition is very common in patients with biliary obstruction and since it is associated with significant morbidity and mortality,it is important to identify factors and mechanisms involved in its development.AIM To determine the influence of obstructive jaundice on the hormones controlling appetite and nutritive status.METHODS This was a prospective case control study performed in a tertiary center in Zagreb,Croatia.Patients with biliary obstruction undergoing internal biliary drainage from September 2012 until August 2013 were enrolled.After excluding patients who developed procedure related complications or were lost in the follow-up,out of initial 73 patients,55 patients were included in the analysis,including 34 with benign and 21 with malignant disease.Meanwhile,40 non-jaundiced controls were also included.Appetite,nutritional status,and serum ghrelin,cholecystokinin(CCK),interleukin 6(IL-6),and tumor necrosis factorα(TNF-α)were determined at admission,48 h and 28 d after internal biliary drainage.Chi square test was used for categorical variables.Continuous variables were analysed for normality by Kolmogorov-Smirnov test and relevant non-parametric(Mann-Whitney,Kruskal-Wallis,and Friedman)or parametric(t-test and analysis of variance)tests were used.RESULTS Patients with obstructive jaundice were significantly malnourished compared to controls,regardless of disease etiology.Plasma ghrelin and CCK levels were significantly higher in patients with obstructive jaundice.Serum bilirubin concentrations were negatively correlated with ghrelin levels and positively correlated with TNF-α,but had no correlation with CCK concentrations.After internal biliary drainage,a significant improvement of nutritional status was observed although serum concentrations of ghrelin,IL-6,and TNF-α remained significantly elevated even 28 d after the procedure.CCK levels in patients without malnutrition remained elev

The number of people diagnosed with diabetes continues to increase,especially among younger populations.Apart from genetic predisposition and lifestyle,there is increasing scientific and public concern that environmental agents may also contribute to diabetes.Food contamination by chemical substances that originate from packaging materials,or are the result of chemical reactions during food processing,is generally recognized as a worldwide problem with potential health hazards.Phthalates,bisphenol A(BPA)and acrylamide(AA)have been the focus of attention in recent years,due to the numerous adverse health effects associated with their exposure.This paper summarizes the available data about the association between phthalates,BPA and AA exposure and diabetes.Although their mechanism of action has not been fully clarified,in vitro,in vivo and epidemiological studies have made significant progress toward identifying the potential roles of phthalates,BPA and AA in diabetes development and progression.These chemicals interfere with multiple signaling pathways involved in glucose and lipid homeostasis and can aggravate the symptoms of diabetes.Especially concerning are the effects of exposure during early stages and the gestational period.Well-designed prospective studies are needed in order to better establish prevention strategies against the harmful effects of these food contaminants.

BACKGROUND The vein of Galen aneurismal malformations(VGAM) are rare arteriovenous malformations of the embryonic choroid plexus. They represent about 30% of paediatric neurovascular disorders and show diverse characteristics. The VGAM is constituted by a midline dilated venous structure that receives blood from abnormal macroscopic or microscopic arteriovenous shunting vessels. Two types of VGAM exist, the choroidal and the mural. The treatment represents a challenge with the therapeutic objective to preserve the normal brain development without creating new neurological deficits.CASE SUMMARY We present three cases of VGAM in the early postnatal period and their treatment. All patents were treated with the endovascular technique, which was successful. According to our experience, the endovascular technique is a safe and efficient mode of VGAM treatment.CONCLUSION The objective of treatment aims to child's normal neurological development. A proper selection of patients and a thorough diagnostic workup is of vital importance. When the endovascular treatment is performed, the primary aim is not a complete VGAM exclusion at one time, which could produce a sudden reversal of blood flow with consequent venous infarction and ischemia. The aim is therefore to occlude as much of the VGAM as needed to relieve the congestive cardiac failure, to gain time and to create the conditions for a normal maturation of the neurovascular system. With the use of endovascular techniques, which represent not only the first choice of treatment but also the only safe therapeutic modality, the natural history of VGAM and their risks may be avoided safely.

BACKGROUND Hepatitis E virus(HEV)is an emerging virus of global health concern.The seroprevalence rates differ greatly according to geographic region and population group.AIM To analyze the seroprevalence of HEV in exposed(animal-related professions)and nonexposed populations,as well as solid organ and hematopoietic stem cell transplant patients.METHODS Forestry workers(n=93),hunters(n=74),and veterinarians(n=151)represented the exposed population.The general population(n=126)and pregnant women(n=118)constituted the control group.Transplant patients included liver transplant recipients(LTRs)(n=83),kidney transplant recipients(KTRs)(n=43),and hematopoietic stem cell transplant recipients(HSCRs)(n=39).HEV immunoglobulin G antibodies were detected using the enzyme-linked immunosorbent assay and confirmed by the immunoblot test.RESULTS The HEV seroprevalence significantly differed between groups:Veterinarians 15.2%,hunters 14.9%,forestry workers 6.5%,general population 7.1%,and pregnant women 1.7%.In transplant patients,the seropositivity was highest in LTRs(19.3%),while in KTRs and HSCRs,the seroprevalence was similar to the general population(6.9%and 5.1%,respectively).A significant increase in seropositivity with age was observed from 2.9%in individuals less than 30 years to 23.5%in those older than 60 years.Sociodemographic characteristics(sex,educational level,area of residence,and number of household members),eating habits(game meat,offal,and pork products consumption),and environmental and housing conditions(drinking water supply,type of water drainage/sewer,waste disposal,domestic animals)were not associated with HEV seropositivity.However,individuals who reported a pet ownership were more often seropositive compared to those who did not have pet animals(12.5%vs 7.0%).CONCLUSION The results of this study showed that individuals in professional contact with animals and LTRs are at higher risk for HEV infection.In addition,age is a significant risk factor for HEV seropositivity.

Changes in gut microbiota influence both the gut and liver,which are strictly connected by the so-called“gut-liver axis”.The gut microbiota acts as a major determinant of this relationship in the onset and clinical course of liver diseases.According to the results of several studies,gut dysbiosis is linked to viral hepatitis,mainly hepatitis C virus and hepatitis B virus infection.Gut bacteriaderived metabolites and cellular components are key molecules that affect liver function and modulate the pathology of viral hepatitis.Recent studies showed that the gut microbiota produces various molecules,such as peptidoglycans,lipopolysaccharides,DNA,lipoteichoic acid,indole-derivatives,bile acids,and trimethylamine,which are translocated to the liver and interact with liver immune cells causing pathological effects.Therefore,the existence of crosstalk between the gut microbiota and the liver and its implications on host health and pathologic status are essential factors impacting the etiology and therapeutic approach.Concrete mechanisms behind the pathogenic role of gut-derived components on the pathogenesis of viral hepatitis remain unclear and not understood.In this review,we discuss the current findings of research on the bidirectional relationship of the components of gut microbiota and the progression of liver diseases and viral hepatitis and vice versa.Moreover,this paper highlights the current therapeutic and preventive strategies,such as fecal transplantation,used to restore the gut microbiota composition and so improve host health.

Objective The aim of this study is to investigate the effects of oral cadmium(Cd) ingestion on the pulmonary immune response. Methods Determination of Cd content in lungs and histopathological evaluation of the tissue was performed in rats following 30-day oral Cd administration(5 and 50 mg/L). Antioxidant enzyme defense(superoxide dismutase and catalase), cell infiltration, and production of tumor necrosis factor(TNF) and interferon(IFN)-γ, as well as the activity of myeloperoxidase(MPO), nitric oxide(NO), and various cytokines [interleukin(IL)-1β, IL-6, IL-10, and IL-17] were investigated. Results Cd caused tissue damage and cell infiltration in the lungs, and this damage was more pronounced at higher doses. Cd deposition resulted in lung inflammation characterized by a dose-dependent IL-1β increase in lung homogenates, increased TNF levels at both doses, and IL-6 stimulation at low doses with inhibition observed at higher doses. Cd exerted differential effects on lung leukocytes isolated by enzyme digestion, and these effects were characterized by a lack of change in the production of reactive oxygen and nitrogen species, an inhibition of IL-1β and TNF, and stimulation of MPO and IFN-γ. The higher capacity of Cd-exposed lung cells to respond to the opportunistic pathogen Staphylococcus epidermidis was demonstrated in vitro. Conclusion The potential of ingested Cd to exert both proinflammatory and immunosuppressive effects on pulmonary tissue inflammation and immune reactivity highlights the complex immunomodulatory actions of this metal.

Chronic pancreatitis is a long-standing inflammation of the pancreas,characterized by progressive inflammatory and fibrotic changes,resulting in permanent structural damage of pancreatic parenchyma[1].Pancreatitis(chronic or acute)is the primary risk factor for pseudo-splenic artery aneurysms,along with pancreatic pseudocysts and trauma.As the disease progresses,patients with chronic pancreatitis may develop complications due to exocrine and endocrine pancreatic functional loss,such as fat malabsorption with steatorrhea,glucose intolerance,and ultimately diabetes mellitus[1,2].Furthermore,severe pancreatic inflammation can cause weakening of vessel walls,with subsequent formation of arterial pseudoaneurysms.Splenic artery is the most commonly affected visceral artery[2,3].Liver cirrhosis,portal hypertension,liver transplantation,atherosclerosis,hypertension,pregnancy and multiparity are main risk factors for true splenic artery aneurysms[2].Rupture and intraperitoneal bleeding is the cause of death in 30%−50%of patients.Thus,prompt diagnosis and appropriate treatment are of great clinical importance[4,5].Herein,we present a case of a giant splenic artery pseudoaneurysm within a walled of necrosis involving pancreatic parenchyma,as a complication of long standing chronic pancreatitis.

AIM:To evaluate the clinical characteristics of splenic marginal-zone lymphoma (SMZL) following antigen expression and the influence of therapeutic approaches on clinical outcome and overall survival (OS).METHODS: A total of 30 patients with typical histological and immunohistochemical SMZL patterns were examined. Splenectomy plus chemotherapy was applied in 20 patients, while splenectomy as a single treatment-option was performed in 10 patients. Prognostic factor and overall survival rate were analyzed.RESULTS: Complete remission (CR) was achieved in 20 (66.7%), partial remission (PR) in seven (23.3%), and lethal outcome due to disease progression occurred in three (10.0%) patients. Median survival of patients with a splenectomy was 93.0 mo and for patients with splenectomy plus chemotherapy it was 107.5 mo (Log rank=0.056, P>0.05). Time from onset of first symptoms to the beginning of the treatment (mean 9.4 mo) was influenced by spleen dimensions, as measured by computerized tomography and ultra-sound (t=2.558, P=0.018). Strong positivity (+++) of CD20 antigen expression in splenic tissue had a positive influence on OS (Log rank=5.244, P0.05) effects on the OS. The expression of other antigens (immunohistochemistry) also had no effect on survival-rate, as measured by a χ2 test (P>0.05).CONCLUSION: Initial splenectomy combined with chemotherapy has been shown to be beneficial due to its advanced remission rate/duration; however, a larger controlled clinical study is required to conf irm our findings.
