Pediatric interfacility transport effects on mortality and length of stay

Background We aimed to evaluate the effects of interfacility pediatric critical care transport response time,physician presence during transport,and mode of transport on mortality and length of stay(LOS)among pediatric *** hypothesized that a shorter response time and helicopter transports,but not physician presence,are associated with lower mortality and a shorter *** Retrospective,single-center,cohort study of 841 patients(mode of transport,response time,stabilization time,return duration,mortality risk(pediatric index of mortality-2 and pediatric risk of mortality-3),and inotrope,vasopressor,or mechanical ventilation presence on *** Four hundred and twenty-eight(50.9%)patients were transported by helicopter,and 413(49.1%)were transported by *** accompanied 239(28.4%)*** median response time was 2.0(interquartile range 1.4–2.9)*** physician presence increased the median response time by 0.26 hours(P=0.020),neither physician presence nor response time significantly affected mortality,ICU length of stay(ILOS)or hospital length of stay(HLOS).Helicopter transports were not significantly associated with mortality or ILOS,but were associated with a longer HLOS(3.24 days,95%confidence interval 0.59–5.90)than ambulance transports(P=0.017).Conclusions These results suggest response time and physician presence do not significantly affect mortality or *** may reflect the quality of pre-transport care and medical control *** transports were only associated with a longer *** analysis provides a framework for examining transport workforce needs and associated costs.

New guidance for using t-SNE:Alternative defaults,hyperparameter selection automation,and comparative evaluation

We present new guidelines for choosing hyperparameters for t-SNE and an evaluation comparing these guidelines to current *** guidelines include a proposed empirically optimum guideline derived from a t-SNE hyperparameter grid search over a large collection of data *** also introduce a new method to featurize data sets using graph-based metrics called scagnostics;we use these features to train a neural network that predicts optimal t-SNE hyperparameters for the respective data *** neural network has the potential to simplify the use of t-SNE by removing guesswork about which hyperparameters will produce the best *** evaluate and compare our neural network-derived and empirically optimum hyperparameters to several other t-SNE hyperparameter guidelines from the literature on 68 data *** hyperparameters predicted by our neural network yield embeddings with similar accuracy as the best current t-SNE *** our empirically optimum hyperparameters is simpler than following previously published guidelines but yields more accurate embeddings,in some cases by a statistically significant *** find that the useful ranges for t-SNE hyperparameters are narrower and include smaller values than previously reported in the ***,we also quantify the potential for future improvements in this area:using data from a grid search of t-SNE hyperparameters we find that an optimal selection method could improve embedding accuracy by up to two percentage points over the methods examined in this paper.

Towards real-time detection of wastewater in surface waters using fluorescence spectroscopy

The presence of municipal wastewater at the intake of a major drinking water treatment facility located on Lake Ontario was examined using fluorescence data collected during a period of continuous *** addition,controlled mixing of lake water and wastewater sampled from a local treatment facility were conducted using a bench-scale fluorescence system to quantify observed changes in natural organic *** linear regression was applied to components derived from parallel factors *** resulting mean absolute error for predicted wastewater level was 0.22%(V/V,wastewater/lake water),indicating that wastewater detection at below 1.0%(V/V)was *** of sucralose,a wastewater indicator,were conducted for treated wastewater as well as surface water collected at two intake locations on Lake *** suggested minimal wastewater contribution at the drinking water intake.A wastewater detection model using a moving baseline was developed and applied to continuous fluorescence data collected at one of the drinking water intakes,which agreed well with sucralose ***,the simulated addition of 1.0%(V/V)of wastewater/wastewater was detectable in 89%of samples analyzed,demonstrating the utility of fluorescence-based wastewater monitoring.

A novel crystal characterization instrument has been built up in which a combination of X-ray multiple diffraction and X-ray topography is applied to enabling the cross-correlation between micro-crystallographic symmetry and its spatial dependence in relation to lattice defects. This facility is used to examine, in a self-consistent manner, growth sector-dependant changes to both the crystallographic structure and the lattice defects associated with the action of habit-modifying additives in a number of representative crystal growth systems. In addition, the new instrument can be used to probe micro-crystallographic aspects(such as distortion to crystal symmetry) and relate these in a spatially resolved manner to the crystal defect structure in crystals doped with known habit modifiers.

Surfactant proteins A(SP-A)and D(SP-D),both members of the collectin family,play a well established role in apoptotic cell recognition and *** in vitro data show that SP-A and SP-D interact with apoptotic neutrophils in a distinct ***-A and SP-D bind in a Ca^(2+)-dependent manner to viable and early apoptotic neutrophils whereas the much greater interaction with late apoptotic neutrophils is Ca^(2+)-*** surface molecules on the apoptotic target cells responsible for these interactions had not been identified and this study was done to find candidate target ***(mPO),a specific intracellular defense molecule of neutrophils that becomes exposed on the outside of the cell upon apoptosis,was identified by affinity purification,mass-spectrometry and western blotting as a novel binding molecule for SP-A and *** confirm its role in recognition,it was shown that purified immobilised mPO binds SP-A and SP-D,and that mPO is surface-exposed on late apoptotic ***-A and SP-D inhibit binding of an anti-mPO monoclonal Ab to late apoptotic *** microscopy confirmed that anti-mPO mAb and SP-A/SP-D colocalise on late apoptotic *** was identified as a further potential ligand for SP-A,and neutrophil defensin as a target for both proteins.

On-chip detection of a single nucleotide polymorphism without polymerase amplification

A nanoparticle-assembled photonic crystal （PC） array was used to detect single nucleotide polymorphism （SNP）. The assay platform with PC nanostructure enhanced the fluorescent signal from nanoparticle-hybridized DNA complexes due to phase matching of excitation and emission. Nanoparticles coupled with probe DNA were trapped into nanowells in an array by using an electrophoretic particle entrapment system. The PC/DNA assay platform was able to identify a 1 base pair （bp） difference in synthesized nucleotide sequences that mimicked the mutation seen in a feline model of human autosomal dominant polycystic kidney disease （PKD） with a sensitivity of 0.9 fg/mL （50 am）-sensitivity, which corresponds to 30 oligos/array. The reliability of the PC/DNA assay platform to detect SNP in a real sample was demonstrated by using genomic DNA （gDNA） extracted from the urine and blood of two PKD-wild type and three PKD positive cats. The standard curves for PKD positive （PKD＋） and negative （PKD-） DNA were created using two feline-urine samples. An additional three urine samples were analyzed in a similar fashion and showed satisfactory agreement with the standard curve, confirming the presence of the mutation in affected urine. The limit of detection （LOD） was 0.005 ng/mL which corresponds to 6 fg per array for gDNA in urine and blood. The PC system demonstrated the ability to detect a number of genome equivalents for the PKD SNP that was very similar to the results reported with real time polymerase chain reaction （PCR）. The favorable comparison with quantitative PCR suggests that the PC technology may find application well beyond the detection of the PKD SNP, into areas where a simple, cheap and portable nucleic acid analvsis is desirable.

Hepatitis B and C infection and liver disease trends among human immunodeficiency virus-infected individuals

AIm:To examine trends in and correlates of liver disease and viral hepatitis in an human immunodeficiency virus (HIV)-infected cohort. mETHODS:The multi-site adult/adolescent spectrum of HIV-related diseases (ASD) followed 29 490 HIVinfected individuals receiving medical care in 11 U.S. metropolitan areas for an average of 2.4 years,and a total of 69 487 person-years,between 1998 and 2004. ASD collected data on the presentation,treatment,and outcomes of HIV,including liver disease,hepatitis screening,and hepatitis diagnoses. RESULTS:Incident liver disease,chronic hepatitis B virus (HBV),and hepatitis C virus (HCV) were diagnosed in 0.9,1.8,and 4.7 per 100 person-years. HBV and HCV screening increased from fewer than 20% to over 60% during this period of observation (P mong individuals with a diagnosis of liver disease. Despite care guidelines promoting screening and vaccination for HBV and screening for HCV,screening and vaccination were not universally conducted or,if conducted,not documented. CONCLUSION:Due to high rates of incident liver disease,viral hepatitis screening,vaccination,and treatment among HIV-infected individuals should be a priority.

On the Use of Adjoint-Based Sensitivity Estimates to Control Local mesh Refinement

The goal of efficient and robust error control, through local mesh adaptationin the computational solution of partial differential equations, is predicated on theability to identify in an a posteriori way those localized regions whose refinement willlead to the most significant reductions in the error. The development of a posteriori errorestimation schemes and of a refinement infrastructure both facilitate this goal, howeverthey are incomplete in the sense that they do not provide an answer as to where themaximal impact of refinement may be gained or what type of refinement — elementalpartitioning (h-refinement) or polynomial enrichment (p-refinement) — will best leadto that gain. In essence, one also requires knowledge of the sensitivity of the error toboth the location and the type of refinement. In this communication we propose theuse of adjoint-based sensitivity analysis to discriminate both where and how to *** present both an adjoint-based and an algebraic perspective on defining and usingsensitivities, and then demonstrate through several one-dimensional model problemexperiments the feasibility and benefits of our approach.

Optical imaging of nociception in primary somatosensory cortex of nonhuman primates

While the activation of primary somatosensory(SI) cortex during pain perception is consistently reported in functional imaging studies on normal subjects and chronic pain patients,the specific roles of SI,particularly the subregions within SI,in the processing of sensory aspects of pain are still largely *** optical imaging of intrinsic signal(OIS) and single unit electrophysiology,we studied cortical activation patterns within SI cortex(among Brodmann areas 3a,3b and 1) and signal amplitude changes to various intensities of non-nociceptive,thermal nociceptive and mechanical nociceptive stimulation of individual distal finerpads in anesthetized squirrel *** have demonstrated that areas 3a and 1 are preferentially involved in the processing of nociceptive information while areas 3b and 1 are preferentially activated in the processing of non-nociceptive(touch) *** activations of individual fingerpad were organized topographically suggesting that nociceptive topographic map exits in areas 3a and *** amplitude was enhanced to increasing intensity of mechanical nociceptive stimuli in areas 3a,3b and *** area 1,nociceptive response co-localizes with the non-nociceptive ***,we hypothesize that nocicepitve information is area-specifically represented within SI cortex,in which nociceptive inputs are preferentially represented in areas 3a and 1 while non-nociceptive inputs are preferentially represented in areas 3b and 1.
