肠道菌群代谢产物氧化三甲胺对肾小球系膜细胞和肾小管上皮细胞NF-κB/MAPK信号通路的影响

目的探讨肠道菌群代谢产物氧化三甲胺(trimethylamine oxide,TMAO)对人肾小球系膜细胞HMC和肾小管上皮细胞HK-2核因子-κB(nuclear factor-kappa B,NF-κB)/丝裂原活化蛋白激酶(mitogen activated protein kinase,MAPK)信号通路的影响,为通过肠道菌群治疗和预防肾损伤提供理论依据。方法用TMAO处理HMC和HK-2细胞,并设未处理或TMAO处理0 h对照组,Real-time PCR法检测炎性反应因子MCP-1、IL-6、TNF-α和IL-1β基因mRNA的转录水平,Western blot法检测NF-κB和MAPK信号通路相关蛋白的表达。并用NF-κB抑制剂BAY11-7082处理HMC和HK-2细胞,验证TMAO的促炎作用。结果与对照组相比,HMC和HK-2细胞TMAO处理组炎性反应因子MCP-1、IL-6、TNF-α和IL-1βmRNA转录水平均明显上升(t=33.349,P=0.001),TMAO对两种细胞有明显的促炎作用;TMAO可激活HMC和HK-2细胞的NF-κB及MAPK信号通路,引起炎性反应,造成对肾脏的损伤。结论TMAO含量的增高可激活HMC和HK-2细胞NF-κB/MAPK信号通路,刺激两种细胞释放炎性反应因子,从而导致肾损伤的发生和发展。

Background: Electroacupuncture(EA) may reduce the severity of acute pancreatitis(AP) and provide additional pain relief in patients with chronic pancreatitis. However, the ability of EA to relieve pain in patients with AP ha. not been well ***: This study was undertaken to compare the pain-relieving effects of EA and conventional treatment in patients with ***, setting, participants and interventions: This study was conducted using a ra.domized, controlled,three-arm, para.lel-group and multi-center design. Patients diagnosed with AP were ra.domly and equally assigned to EA1, EA2 or control groups. All participants received conventional standard-of-care thera.y for AP. Local EA alone was administered in EA1, and local plus distal EA was given in *** EA included two abdominal acupoints, while distal EA included twelve periphera. acupoints. EA groups underwent one session of EA daily for 4 days(days 1–4), or until pain was resolved or *** outcome measures: The primary outcome measure was the cha.ge in the visual analogue scale(VAS;0–100) pain score between baseline and day ***: Eighty-nine participants were ra.domized into EA1, EA2 and control groups, and 88(EA1, 30;EA2, 29;control, 29) were included in the full-analysis set. VAS score cha.ge(median [interquartile ra.ge]) on day 5 was(12.3 ± 22.5) in the EA1 group,(10.3 ± 21.5) in the EA2 group, and(8.9 ± 15.2) in the control group. There were not significant differences in the cha.ge in VAS score among treatments(P = 0.983). However, time to food intake was significantly shorter in the EA group(EA1 + EA2) tha. in the control group(median 2.0 days vs 3.0 days), with a ha.ard ra.io of 0.581(P = 0.022;95% CI,0.366–0.924). No significant adverse events ***: EA treatment did not significantly reduce pain after 4 days of treatment in patients with AP-associated abdominal pain but significantly reduced time to first food intake.

流行性感冒病毒(简称流感病毒)每年引起较高的人群发病率和死亡率,对全球公共卫生安全构成严重威胁。流感病毒经过不断进化,具有躲避宿主免疫反应和逃避疫苗保护的能力。因此,有必要开发针对宿主细胞机制的药物或疫苗以降低重症率和死亡率。自然杀伤细胞(NK细胞)能够在非致敏状态下迅速杀伤病毒感染的宿主细胞,并具有免疫调节及记忆效应,为流感病毒的防治带来了希望。该文就NK细胞及其免疫激活在流感中的作用机制进行综述,为流感防治的新型药物或疫苗研究提供依据。

近年来,短期、长期居住在高海拔地区的人数不断增加,超过8160万人生活在海拔≥2500米的地区,而中国常居高原者超过1000万,每年进入高原的人口超过2000万。独特的高原气候引发了一系列高原相关性疾病,其中高原脑水肿(ha.E)是最严重的疾病之一,如果不及时进行适当治疗,患者可能会在24 h内因脑疝死亡。然而,ha.E发展的确切机制尚不完全清楚,使得ha.E的防和治具有挑战性。间充质干细胞(MSC)及间充质干细胞外泌体(MSC-Exos)具有修复受损组织和细胞、抗氧化应激、抑制炎症反应、调节自噬等作用,有可能成为防治ha.E的新型药物。本文结合国内外相关文献阐述高原脑水肿发病机制及MSC、MSC-Exos在其中可能发挥的作用,为MSC、MSC-Exos防治ha.E提供理论依据。
