Epidemiological studies have been investigating the relationship between chlorination byproducts exposure and cancer. Studies showed the incidence of colon, rectum and bladder cancer in laboratory animals when halogenated byproducts were administered to them, such as trihalomethanes. Based on this fact, in this work, two data sets of water quality parameters were analyzed with focus on total trihalomethanes (THMt). These two data sets are from two different time periods (one in 2014 and other in 2017). All the samples were collected in the same months, in both data sets. The samples were taken from its same corresponding sampling points in both periods of time. Trihalomethanes (THMs) are undesired byproducts of chlorination and its formation occurs when the chlorine used in water treatment reacts with natural organic matter, which is present in natural waters, during the disinfection process. The aim of this research was to investigate the THMs levels in storage water from the chlorination performed by the Water Treatment Station (WTS) of the Maringá-Parana-Brazil;also, to compare the results obtained with the maximum allowable values (MAVs) established by the Consolidation Resolution n.05/2017, current law of water quality in Brazil. Water samples were collected in eight high and fundamental schools of Maringá-Paraná-Brazil and analyzed through the gas chromatography method by the use of mass spectrometry detector with purge-and-trap concentrator (GC-MS) for THM. Furthermore, parameters such as pH and residual chlorine were analyzed following the methodology proposed by the Standard Methods for the Examination of Water and Wastewater. The study results to THMt show that the maximum value of 21.5 μg/L obtained is within the MAV of 100 μg/L. Chloroform was the compound with higher concentrations in all samples in THMt analysis. Microbial fuel cells (MFCs) are bioelectrochemical systems that convert chemical energy contained in organic matter into electrical energy by using the catalytic (metabolic) activity of living microorganisms. Mediator-less two chamber H-type MFCs were constructed in the current study, using dairy digester microbial population as anode inocula to convert finely ground pine tree (Avicel) at 2% (w/v) to electricity. MFCs were placed at 37°C and after the circuit voltage was stabilized on d9, bovine rumen microorganisms cultured anaerobically for 48 hrs in cellulose broth media were added to treatment group of MFC at 1% v/v dosage. MFC power and current across an external resistor were measured daily for 10 d. At the end of incubation on d19 head space gas and anode chamber liquid solutions were collected and analyzed for total gas volume and composition, and volatile fatty acids, respectively. Addition of enriched rumen microorganisms to anaerobic anode chamber increased cellulose digestibility and increased both CO2 and methane production;however, it decreased the methane to CO2 ratio. Over the experimental period, electricity generation was increased with rumen microorganism addition, and power density normalized to anode surface area was 17.6 to 67.2 mW/m2 with average of 36.0 mW/m2 in treatment, while control group had 3.6 to 21.6 (AVE 12.0) mW/m2. These observations imply that biocatalysis in MFCs requires additional cellulolytic activities to utilize structural biomass in bioenergy production.

Obturator dislocation of the hip is caused by high-velocity accidents as evidenced by its frequent association with other traumatic injuries and, seldom found. Its main complication remains femoral head avascular necrosis. We report on four cases of obturator dislocation of the hip. The mean age of patients was 30 years, and all their injuries followed a road traffic accident. Associated lesions were a contralateral femur fracture in two cases and an osteochondral fracture in one case. Reduction of dislocations was achieved orthopedically under general anaesthesia and the average waiting time before reduction was 20 hours. One patient had an intra-articular incarcerated fragment visible on X-ray, and another patient showed signs of early coxarthrosis 15 months later. The average follow-up time was 24 months.

Background: Inflammatory responses are implicated as crucial patho-mechanisms of vascular brain malformations. Inflammation is suggested to be a key contributor to aneurysm rupture; however it is unclear whether inflammation contributes similarly to bleeding of cerebral cavernous malformations (CCMs). Black blood MRI is a sequence which identifies inflammation in blood vessel walls and in the present study is used to detect inflammatory response in CCMs. Methods: Fifteen patients with 17 CCMs treated in our department in 2017 were retrospectively analysed. All patients received black blood MRIs and the results were analysed in correlation with, size and bleeding of CCMs. Results: Size and bleeding status of CCMs did not correlate with contrast enhancement in the CCM wall. One of 3 patients with bleeding displayed contrast enhancement in black blood MRI, whereas the others had non enhancing lesions. Because of the small number of cases a statistical analysis was not performed. Conclusion: In this limited cohort, inflammatory reactions in CCMs could not be detected by black blood MRI suggesting that the level of inflammation is minimal in these lesions and those different patho-mechanisms play a more important role in the rupture of CCMs.

Background: There are controversies concerning the natural history of arteriovenous malformations (AVMs) in literature and it is not clear which AVMs should be treated and which should be just observed. Objective criteria beyond growth in serial MRIs or angiographies are needed. The use of black blood MRI is currently under investigation for evaluating the rupture risk of cerebral aneurysms, however its use for assessment of AVMs has yet to be evaluated. We therefore conducted a feasibility study on the application of black blood MRI (bbMRI) in AVMs to assess rupture risk. Methods: Retrospective study of 10 patients with intracranial AVMs and 4 patients with arteriovenous fistulas who received a black blood MRI before treatment. Results: AVM niduses (9/10) show contrast enhancement irrespective of rupture or size. All arteriovenous fistulas (4/4) were contrast enhancing irrespective of rupture. Conclusion: High flow malformations are in a permanent stage of inflammation which does not seem to allow conclusions on their rupture risk at the current stage. BbMRI is a feasible method of identifying inflammation in AVMs and arteriovenous fistulas. However, future prospective studies are needed to evaluate whether bbMRI contrast enhancement correlates with rupture risk.

The wall shear stress （WSS） that a moving fluid exerts on a surface affects many processes including those relating to vascular function. WSS plays an important role in normal physiology （e.g. angiogenesis） and affects the microvasculature＇s primary function of molecular transport. Points of fluctuating WSS show abnormalities in a number of diseases; however, there is no established technique for measuring WSS directly in physiological systems. All current methods rely on estimates obtained from measured velocity gradients in bulk flow data. In this work, we report a nanosensor that can directly measure WSS in microfluidic chambers with sub-micron spatial resolution by using a specific type of virus, the bacteriophage M13, which has been fluorescently labeled and anchored to a surface. It is demonstrated that the nanosensor can be calibrated and adapted for biological tissue, revealing WSS in micro-domains of cells that cannot be calculated accurately from bulk flow measurements. This method lends itself to a platform applicable to many applications in biology and microfluidics.

目的:普那布林靶向作用于肿瘤新生和原生血管,并通过RAS-JNK通道诱导肿瘤凋亡。本研究对普那布林和多西他赛联合用药(dN)与多西他赛单独用药(d)进行比较。方法:之前经历标准化疗的晚期NSCLC患者,被随机分至dN组和d组,21天治疗周期中,第1天多西他赛(75 mg/m,静注),dN组:第1和8天还给普那布林(静注,剂量:30或20 mg/m,简称30或20剂量组)。经历1次或2次既往化疗的ⅢB/Ⅳ期NSCLC患者入组,既往给d或血管阻断剂不入组。主要终点是总生存期(OS)。结果:172名患者被随机分为2个剂量组,其中163名进行治疗:30剂量组(dN:50;d:55)和20剂量组(dN:40;d:18)。30剂量组:dN组OS(月,90%CI)为8.7(6.6,12.6),d组7.5(6.3,10.5);客观缓解率:dN:14.0%对d:14.5%);dOR(月,90%CI):dN:12.7(4.0,13.9)对d:1.5(1.1,3.1),P=0.049。20剂量dN组的这些参数没30剂量组好。事后亚组分析:dN组经历1次既往化疗并有大肺肿瘤(>3 cm)患者,生存期可能更长。该亚组OS(月,95%CI)为dN组11.5(7.1,15.1)对d组7.8(4.1,17.4)。最常见不良反应为恶心,乏力,腹泻,便秘和厌食。30剂量组中dN组多西他赛初始剂量降低10%,而d组降低18.2%,所以dN组耐受性可能比d组更好。dN组中性粒细胞减少症发病率显著低于d组(30剂量组:8.0%对36.4%,PdN组OS比d组长,这发现与普那布林的靶向血管作用机制一致。计划在大肺肿瘤患者中进行Ⅲ期临床。

Infection after hip prostheses is a potentially devastating complication, and a serious medical and surgical challenge, especially when associated with Paprosky type III femoral bone loosening. Treatment is difficult and options are limited. We report on a 2-stage revision of 15 patients undergoing femur reconstruction with massive allografts. Materials and methods: This was a prospective study which included 15 patients (10 men and 5 women) with infected hip prosthesis, associated with Paprosky type III femoral proximal massive bone loss. The median age of patients was 64 years with a preoperative functional status score of 6. The average number of procedures to the same hip after the first arthroplasty was 6. All patients benefited from a 2-step surgery with massive allografts and locking prosthesis. The average follow-up time was 36 months. Results: Infection was monomicrobial in 14 cases;and was polymicrobial in 4 cases. Methicillin-resistant Staphylococcus epidermidis was the main bacteria (n = 10). The average C-reactive protein level before the second procedure was 2.3 ± 3.4. It was normalized in 8 cases. We recorded 13 cases of primary consolidation without another surgery, 3 cases of relapse, 2 traumatic dislocations and 2 fractures of the allograft. Conclusion: Hip prosthesis infection is a potentially catastrophic complication with significant negative ramifications for both the patient and the healthcare system. Massive allografts use in Paprosky III femoral defect remains very attractive for bone stock restoration and hip function improvement.

[背景]冬季逆温:温度随着海拔而升高的空气层,会吸引污染物,导致污染物浓度增加。以前的研究已经评估了污染物和哮喘急诊科(Ed)就诊之间的相关性,但从未将逆温作为哮喘Ed就诊的独立危险因素进行研究。[目的]探讨犹他州盐湖城冬季逆温与哮喘Ed就诊的相关性。[方法]收集犹他州盐湖城2003—2004年至2007—2008年冬季因哮喘Ed就诊的电子记录以及逆温、气象和空气污染的数据。确定3425名初诊为哮喘的Ed哮喘病例,采用时间分层病例交叉设计和条件logistic回归模型计算优势比(ORs)及其95%可信区间(CIs),估计在4d滞后期及长期逆温中哮喘Ed就诊率与逆温的相关性;并评估逆温、气象、污染物三者之间的交互作用。[结果]校正露点和平均气温后,与没有逆温的滞后期相比,逆温之前0～3d的哮喘就诊率OR值为1.14(95%CI:1.00～1.30);滞后期逆温天数每增加1d,OR值为1.03(95%CI:1.00～1.07)。只有当PM10、最大值和平均气温高于中位数水平时,才显示出相关性。[结论]冬季逆温与哮喘Ed就诊率增加相关。
