Pancreatic cancer（PCA） is one of the most aggressive tumors with few effective treatment modalities. It is the 4th and 7th leading cause of cancer death in the United States and China, respectively. At the time of diagnosis, only 20% of cases present with a resectable tumor, and about 40% with a locally advanced tumor that is considered unresectable. Even resected patients still have a poor prognosis, with an incidence of local recurrence ranging from 20% to 60%. It is also reported that up to 30% of PCA patients die from locally obstructive disease with few or no distant metastases. These findings have highlighted the importance of local radiation therapy in the treatment of PCA. As the role of conventional chemoradiotherapy remains controversial, the dawn of the pancreas stereotactic body radiation therapy（SBRT） era represents a potential paradigm shift in the management of PCA. SBRT delivers a higher biological effective dose to the tumor with sharp dose escalation in a shorter treatment time course. Pancreas SBRT is a novel therapeutic option to achieve local tumor control with minimal toxicity. Herein, we review the advancement of SBRT for PCA patients with different stages of pancreatic adenocarcinoma.

Carbon-mediated persulfate advanced oxidation processes(PS-AOPs)are appealing in contaminant remediation.For the first time,S,B-co-doped carbon-based persulfate activators were synthesized through direct carbonization of sodium lignosulfonate and boric acid.By degrading sulfamethoxazole(SMX),CSB-750 obtained 98.7%removal and 81.4%mineralization within 30 min.In comparison with solo S or B doping,S and B co-doped carbon showed the coupling effect for enhanced catalysis.The rate constant(kobs)of 0.1679 min^(-1)was 22.38-and 279.83-fold higher than those of CS-750(0.0075 min^(-1))and CB-750(0.0006 min^(-1)),respectively.The degradation was efficient at strong acidic and weak basic conditions(pH 3-9).Substantial inhibition effect was presented at strong basic condition(pH 10.95)and in presence of CO_(3)^(2-).The CO_(3)^(2-)-caused inhibition was the combined result of the cooperation of pH and quenching O_(2)^(·-).Thiophene sulfur,BC_(3),BC_(2)O,and structural defects were identified as the active sites for PS activation.Radical and nonradical pathways were both involved in the CSB-750/PS/SMX system,where^(1)O_(2)dominated the degradation,SO_(4)^(·-),·OH and direct electron transfer played the subordinate role,and O_(2)^(·-)served as a precursor for the formation of partial^(1)O_(2).The toxicity of degradation system,the effect of real water matrix,and the reusability of carbocatalysts were comprehensively analyzed.Nine possible degradation pathways were proposed.This work focuses on the catalytic performance improvement through the coupling effect of S,B co-doping,and develops an advanced heteroatom doping system to fabricate carbonaceous persulfate activators.

γδT cells are a conserved population of innate lymphocytes with diverse structural and functional heterogeneity that participate in various immune responses during tumor progression. γδT cells perform potent immunosurveillance by exerting direct cytotoxicity, strong cytokine production and indirect antitumor immune responses. However, certain γδT-cell subsets also contribute to tumor progression by facilitating cancer-related inflammation and immunosuppression. Here, we review recent observations regarding the antitumor and protumor roles of major structural and functional subsets of human γδT cells, describing how these subsets are activated and polarized, and how these events relate to subsequent function in tumor immunity. These studies provide insights into the manipulation of γδT-cell function to facilitate more targeted approaches for tumor therapy.

As one of the four major means of cancer treatment including surgery,radiotherapy(RT),chemotherapy,immunotherapy,RT can be applied to various cancers as both a radical cancer treatment and an adjuvant treatment before or after surgery.Although RT is an important modality for cancer treatment,the consequential changes caused by RT in the tumor microenvironment(TME)have not yet been fully elucidated.RT-induced damage to cancer cells leads to different outcomes,such as survival,senescence,or death.During RT,alterations in signaling pathways result in changes in the local immune microenvironment.However,some immune cells are immunosuppressive or transform into immunosuppressive phenotypes under specific conditions,leading to the development of radioresistance.Patients who are radioresistant respond poorly to RT and may experience cancer progression.Given that the emergence of radioresistance is inevitable,new radiosensitization treatments are urgently needed.In this review,we discuss the changes in irradiated cancer cells and immune cells in the TME under different RT regimens and describe existing and potential molecules that could be targeted to improve the therapeutic effects of RT.Overall,this review highlights the possibilities of synergistic therapy by building on existing research.

硼中子俘获治疗(boron neutron capture therapy,BNCT)具有精准肿瘤靶向性、生物自适性强、对正常组织损伤小和治疗次数少等优点.目前,BNCT的发展还面临研发硼(^(10)B)含量高和肿瘤细胞靶向性强的含硼药物、开发更为精确的硼剂量测量与治疗计划系统,以及建造安装在医院内的加速器中子源等问题.已经开展的BNCT临床研究主要包括新诊断胶质母细胞瘤、复发胶质瘤、复发头颈部肿瘤、脑膜瘤、恶性黑色素瘤、肝转移癌等病种.然而,高质量的BNCT临床试验尚欠缺.开展高水平临床试验是BNCT发展的必经之路,只有获得更多高级别临床研究证据,才能有效推动BNCT临床应用.胶质母细胞瘤的BNCT研究可以考虑从新诊断的初治患者开始,其他肿瘤从复发/转移患者的后线治疗做起.需要探索BNCT联合治疗模式,比如将BNCT与传统光子放疗、免疫治疗、靶向治疗和化疗等多种治疗方法有机结合,还可以探索多种给药途径来提升肿瘤对含硼药物的摄取.BNCT的发展需要多领域、多学科的协作,共同促进BNCT早日成为常规开展的临床治疗方式.

肝内胆管细胞癌根治术后辅助化疗的价值

目的探讨辅助化疗对肝内胆管细胞癌根治术后患者的生存影响。方法回顾性分析127例肝内胆管细胞癌根治性切除术后的患者。28例进行术后化疗(化疗组),99例术后未进行化疗。采用SPSS软件的倾向得分匹配功能,按照分期、肿瘤大小、分化程度、切缘、淋巴结转移、年龄和性别的优先级顺序匹配,从99例未化疗患者中找出28例(未化疗组)与接受辅助化疗的患者匹配。未化疗组患者肝内胆管细胞癌根治术后无特殊处理。比较两组患者治疗效果与生存时间。通过Kaplan Meier法分析术后辅助化疗和未辅助化疗患者中位无复发生存时间和中位生存时间。结果56例患者均获得随访,随访时间为2~56个月,中位随访时间为20个月。化疗组患者中位总生存时间为43个月(范围12.0~74.0个月),1、2、3和4年总生存率分别为75.0%、35.7%、17.9%和14.3%;无进展中位生存时间为11.7个月(范围4.0~19.3个月),1、2、3和4年无进展生存率分别为50.0%、14.3%、10.7%和10.7%。未化疗组患者中位总生存时间为15.8个月(范围7.9~23.7个月),1、2、3和4年总生存率分别为53.6%、25.0%、10.7%和3.6%;无进展中位生存时间为10个月(范围2.2~17.7个月),1、2、3和4年无进展生存率分别为39.3%、14.3%、10.7%和3.6%。化疗组和未化疗组的中位总生存时间和中位无进展生存时间比较有延长的趋势,但是差异均无统计学意义(均P>0.05)。结论化疗对肝内胆管癌术后患者显示出一定的生存获益趋势,值得临床进一步研究。

A wide variety of studies have estimated the magnitude of global terrestrial net primary production （NPP）, but its variations, both spatially and temporally, still remain uncertain. By using an improved process-based terrestrial ecosystem model （DLEM, Dynamic Land Ecosystem Model）, we provide an estimate of global terrestrial NPP induced by multiple environmental factors and examine the response of terrestrial NPP to climate variability at biome and global levels and along latitudes throughout the first decade of the 21st century. The model simulation estimates an average global terrestrial NPP of 54.6 Pg C yr-1 during 2000-2009, varying from 52.8 Pg C yr-1 in the dry year of 2002 to 56.4 Pg C yr-1 in the wet year of 2008. In wet years, a large increase in terrestrial NPP compared to the decadal mean was prevalent in Amazonia, Africa and Australia. In dry years, however, we found a 3.2% reduction in global terrestrial NPP compared to the decadal mean, primarily due to limited moisture supply in tropical regions. At a global level, precipitation explained approximately 63% of the variation in terrestrial NPP, while the rest was attributed to changes in temperature and other environmental factors. Precipitation was the major factor determining inter-annual variation in terrestrial NPP in low-latitude regions. However, in midand high-latitude regions, temperature variability largely controlled the magnitude of terrestrial NPP. Our results imply that pro- jected climate warming and increasing climate extreme events would alter the magnitude and spatiotemporal patterns of global terrestrial NPP.
