Autoimmune hepatitis(AIH)is a T-cell mediated,inflammatory liver disease affecting all ages and characterized by female preponderance,elevated serum transaminase and immunoglobulin G levels,positive circulating autoantibodies,and presence of interface hepatitis at liver *** type 1,affecting both adults and children,is defined by positive anti-nuclear and/or antismooth muscle antibodies,while type 2 AIH,affecting mostly children,is defined by positive anti-liver-kidney microsomal type 1 and/or anti-liver cytosol type 1 *** the autoantigens of type 2 AIH are well defined,being the cytochrome P4502D6(CYP2D6)and the formiminotransferase cyclodeaminase(FTCD),in type 1 AIH they remain to be ***-1 predisposition is conferred by possession of the MHC class II HLA DrB1*03 at all ages,while DrB1*04 predisposes to late onset disease;AIH-2 is associated with possession of DrB1*07 and DrB1**** majority of patients responds well to standard immunosuppressive treatment,based on steroid and azathioprine;second-and third-line drugs should be considered in case of intolerance or insufficient *** review offers a comprehensive overview of pathophysiological and clinical aspects of AIH.

Gastroesophageal reflux disease (GErD) is a condition that develops when the reflux of gastric contents into the esophagus leads to troublesome symptoms and/or complications. Heartburn is the cardinal symptom, often associated with regurgitation. In patients with endoscopy-negative heartburn refractory to proton pump inhibitor (PPI) therapy and when the diagnosis of GErD is in question, direct reflux testing by impedance-pH monitoring is warranted. Laparoscopic fundoplication is the standard surgical treatment for GErD. It is highly effective in curing GErD with a 80% success rate at 20-year follow-up. The Nissen fundoplication, consisting of a total (360°) wrap, is the most commonly performed antireflux operation. To reduce postoperative dysphagia and gas bloating, partial fundoplications are also used, including the posterior (Toupet) fundoplication, and the anterior (Dor) fundoplication. Currently, there is consensus to advise laparoscopic fundoplication in PPI-responsive GErD only for those patients who develop untoward side-effects or complications from PPI therapy. PPI resistance is the real challenge in GErD. There is consensus that carefully selected GErD patients refractory to PPI therapy are eligible for laparoscopic fundoplication, provided that objective evidence of reflux as the cause of ongoing symptoms has been obtained. For this purpose, impedance-pH monitoring is regarded as the diagnostic gold standard.

Autoimmune hepatitis is a rare chronic inflammatory liver disease,affecting all ages,characterised by elevated transaminase and immunoglobulin G levels,positive autoantibodies,interface hepatitis at liver histology and good response to immunosuppressive treatment. If untreated,it has a poor prognosis. The aim of this review is to summarize the evidence for standard treatment and to provide a systematic review on alternative treatments for adults and children. Standard treatment is based on steroids and azathioprine,and leads to disease remission in 80%-90% of patients. Alternative first line treatment has been attempted with budesonide or cyclosporine,but their superiority compared to standard treatment remains to be demonstrated. Second-line treatments are needed for patients not responding or intolerant to standard treatment. No randomized controlled trials have been performed for second-line options. Mycophenolate mofetil is the most widely used second-line drug,and has good efficacy particularly for patients intolerant to azathioprine,but has the major disadvantage of being teratogenic. Only few and heterogeneous data on cyclosporine,tacrolimus,everolimus and sirolimus are available. More recently,experience with the anti-tumour necrosis factoralpha infliximab and the anti-CD20 rituximab has been published,with ambivalent results; these agents may have severe side-effects and their use should be restricted to specialized centres. Clinical trials with new therapeutic options are ongoing.

Hepatitis C virus(HCV)is the major reason for liver transplantation and the main cause of liver-related morbidity and mortality in a great number of *** for the other viruses,this pathogen interferes in more than one process and in more than one way with host cell biology.A mounting body of evidence points,in particular,toward the drastic alterations of mitochondrial physiology and functions that virus is able to induce,albeit the mechanisms have partly remained *** of the mitochondria in immunity and in quality control systems,as autophagy,as well as the strategies that HCV has evolved to evade and even to manipulate mitochondrial surveillance for its benefit,highlights the importance of deepening the mechanisms that modulate this virus-mitochondrion interaction,not only to intensify our knowledge of the HCV infection pathogenesis but also to design efficient antiviral strategies.

This paper is devoted to reviewing the results achieved so far in the application of the single-pixel imaging technique to terahertz(THz)*** use of THz radiation for imaging purposes has been largely explored in the last twenty years,due to the unique capabilities of this kind of radiation in interrogating material ***,THz imaging systems are still limited by the long acquisition time required to reconstruct the object image and significant efforts have been recently directed to overcome this *** of the most promising approaches in this sense is the so-called“single-pixel”imaging,which in general enables image reconstruction by patterning the beam probing the object and measuring the total transmission(or reflection)with a single-pixel detector(i.e.,with no spatial resolution).The main advantages of such technique are that i)no bulky moving parts are required to raster-scan the object and ii)compressed sensing(CS)algorithms,which allow an appropriate reconstruction of the image with an incomplete set of measurements,can be successfully ***,this can result in a reduction of the acquisition *** this review,we cover the experimental solutions proposed to implement such imaging technique at THz frequencies,as well as some practical uses for typical THz applications.

＜正＞ Objectives: To evaluate the effect of cirrhosis on the inhibition by fluvoxamine of theophylline metabolism, to assess whether liver dysfunction has any influence of drug interaction involving CYP1A2. Methods: The study was carried out in 10 healthy volunteers, 10 patients with Child A, and 10 with Child C cirrhosis, according to a randomized, double - blind, 2 - phase, crossover design. results: Fluvoxamine - induced inhibition of theophylline clearance decreased from 62 %. in controls, to 52% and 12% in Child A and C cirrhotics, respectively. CYP1A2 - mediated formations of 3 - methylxanthine and 1 - methyluric acid were totally inhibited in controls, but reduced by only one third in Child C cirrhotics . Inhibition of 1,3 - dimethyluric acid formation decreased from 58% , in controls, to 43% and 7% in patients with Child grade A and C cirrhosis, respectively. Conclusions: Two mechanisms are proposed to explain the attenuating effect of cirrhosis on CYP1A2 inhibition: decreased sensitivity to fluvoxamine of CYP1A2 - mediated biotransformations. probably due to reduced uptake of the inhibitory drug by the cirrhotic liver;reduced hepatic expression of CYP1A2, which makes its inhibition less important.
