Programmed death-ligand-1(PD-L1)plays a crucial role in the suppression of the antitumor immune response upon interaction with programmed cell death protein-1(PD-1)on cytotoxic T ***-L1 is constitutively expressed on antigen-presenting cells or tumor ***,PD-L1 has been detected by immunohistochemistry in a variety of tumors,including malignant pleural mesothelioma(MPM),in which it has emerged as a predictive biomarker for PD-1/PD-L1 immune checkpoint blockers(ICBs).1,2 PD-L1 expression on tumor cells can be upregulated by several factors including the interferon gamma(IFN-γ)cytokine produced by tumor-infiltrating lymphocytes(TILs).3 In MPM cells,it has been reported that IFN-γupregulates PD-L1 mRNA expression due to the activation of the interferon regulatory factor 1(IRF1)transcription factor,4 but no data have been shown regarding cell surface PD-L1 that is functionally relevant for its contact with PD-1-positive cells.

In the present study we report on changes in irritable bowel syndrome-severity scoring system (IBS-SSS) and irritable bowel syndrome-quality of life (IBS-QoL) in 19 IBS patients, aged 18 to 74 years (F/M: 14/5), during 12 wk registering their symptoms on the web-application (***). During a control period of the first 6-wk patients were asked to register their IBS-SSS and IBS-QoL on the web-application weekly without receiving any intervention. Thereafter, low fermentable oligo-, di-, mono-saccharides and polyols (FODMAP) diet (LFD) was introduced for the next 6 wk while continuing the registration. Though a small sample size a significant improvement in disease activity (IBS-SSS) was observed during both the control period, median: 278 (range: 122-377), P = 0.02, and subsequently during the LFD period, median: 151 (range: 29-334), P < 0.01. The IBS-QoL solely changed significantly during the LFD period, median: 67 (37-120), P < 0.01. The significant reduction in disease activity during the control period shows a positive effect of the web-application on IBS symptoms when presented as a “traffic light”. However adding the diet reduced IBS-SSS to < 150, inactive to mild symptoms. In the future results from larger scale trials are awaited.

·AIM:Toevaluatethevisualfunctionevolutionofretinal pigment epithelial(RPE) tears in patients with age-related macular degeneration(AMD) according to type of occurrence [spontaneous or secondary to anti-vascular endothelial growth factor(anti-VEGF) injection] and the topographic location of the tear after a two-year followup period.·METHODS: A total of 15 eyes of 14 patients with RPE tears in exudative AMD were analyzed retrospectively at the University Eye Clinic of Trieste. Inclusion criteria were: patient age of 50 or older with AMD and RPE tears both spontaneous occurring or post anti-VEGF treatment. Screening included: careful medical history,complete ophthalmological examination, fluorescein angiography(FA), indocyanine green angiography(ICG),autofluorescence and infrared imaging and optical coherence tomography(OCT). Patients were evaluated every month for visual acuity(VA), fundus examination and OCT. Other data reported were: presence of PED,number of injections before the tear, location of the lesion.·RESULTS:Meanfollow-up was24wk(SD±4wk). Atotal of 15 eyes were studied for RPE tear. In 6 cases(40%),the RPE tears occurred within two years of anti-VEGF injections the others occurred spontaneously. In 13cases(86.6%), the RPE tear was associated with pigment epithelial detachment(PED). In 7 cases(46.6%), the RPE tear occurred in the central area of the retina and involved the fovea. Two lesions were found in the parafoveal region, six in the extra-macular area. In all cases visual acuity decreased at the end of the follow-up period(P <0.01) independently of the type or the topographical location of the lesion.·CONCLUSION: RPE tear occurs in exudative AMD as a spontaneous complication or in relation to anti-VEGF injections. Visual acuity decreased significantly and gradually in the follow-up period in all cases. No correlation was found between visual loss and the type of onset or the topographic location of the tears.

For the medicolegal evaluation of victims of survived strangulation,a neck-magnetic resonance imaging(MRI)can be performed for assessing lesions in the inner soft tissues(fat,muscles or lymph nodes,for example).In our institute,such MRI examinations have been performed for a test period of 4 years with the aim of evaluating the use of this tool by forensic pathologists and identifying medicolegal indicators for the performance of neck-MRI in surviving victims of *** retrospectively reviewed medicolegal reports from all victims examined during the test *** extracted objective lesions(***,bruising and abrasions)and reported clinical symptoms(*** disorder,dysphasia)from the *** findings were compared to those reported from the *** total,112 victims were clinically examined after suspected *** of these victims underwent an MRI examination of the ***-four of the victims presented objective lesions during the clinical examination,with eight showing signs of both petechiae and ***-MRI was performed in four of these eight victims and three of them showed lesions visible in *** 76 victims with bruising as the only objective finding,66 victims described clinical *** those 66 victims,seven were examined by MRI and two demonstrated lesions in *** MRI was performed,relevant findings were detected in 45%of the *** leads to the suspicion that many more findings could have been detected in the other victims,if an MRI had been performed in those *** results lead us to the conclusion that an MRI examination of victims of suspected strangulation is useful,and strict indications for its application should be established.

Background： Bacillus spp. seem to be an alternative to antimicrobial growth promoters for improving animals＇ health and performance. However, there is little information on the effect of Bacillus spp. in combination with different dietary crude protein（CP） levels on the ileal digestibility and microbiota composition. Therefore, the objective of this study was to determine the effect of Bacillus spp. supplementation to low-（LP） and high-protein diets（HP） on ileal CP and amino acid（AA） digestibility and intestinal microbiota ***： Eight ileally cannulated pigs with an initial body weight of 28.5 kg were randomly allocated to a rowcolumn design with 8 pigs and 3 periods of 16 d each. The assay diets were based on wheat-barley-soybean meal with two protein levels： LP（14% CP, as-fed） and HP diet（18% CP, as-fed）. The LP and HP diets were supplemented with or without Bacil us spp. at a level of 0.04%（as-fed）. The apparent ileal digestibility（AID） and standardized ileal digestibility（SID） of CP and AA was determined. Bacterial community composition from ileal digesta was analyzed by Illumina amplicon sequencing and quantitative real-time PCR. Data were analyzed as a 2 × 2 factorial design using the GLIMMIX procedures of ***： The supplementation with Bacillus spp. did not affect both AID and SID of CP and AA in growing ***, there was no difference in AID of CP and AA between HP and LP diets, but SID of cystine, glutamic acid,glycine, and proline was lower（P 〈 0.05） in pigs fed the HP diets. AIM To investigate the association of seven single nucleotide polymorphisms(SNPs) of the IL23 R gene with the clinical picture of ulcerative colitis(UC). METHODS Genomic DNA samples of 131 patients (66 males, 65 females, mean age 55.4 ± 15.8 years) with Caucasian origin, diagnosed with UC were investigated. The diagnosis of UC was based on the established clinical, endoscopic, radiological, and histopathological guidelines. DNA was extracted from peripheral blood leukocytes by routine salting out method. Polymerase chain reaction and restriction fragment length polymorphism were used to identify the alleles of seven SNPs of IL23 R gene(rs11209026, rs10889677, rs1004819, rs2201841, rs7517847, rs10489629, rs7530511).RESULTS Four out of seven analyzed SNPs had statistically significant influence on the clinical picture of UC. Two SNPs were associated with greater colonic extension(rs2201841 P = 0.0084; rs10489629 P = 0.0405). For two of the SNPs, there was more frequently need for operations (rs2201841 P = 0.0348, OR = 8.0; rs10889677 P = 0.0347, OR = 8.0). The rs2201841 showed to be a risk factor for the development of iron deficiency (P = 0.0388, OR = 6.1837). For patients with the rs10889677, a therapy with azathioprine was more frequently necessary(P = 0.0116, OR = 6.1707). Patients with rs10489629 SNP had a lower risk for weight loss(P = 0.0169, OR = 0.3394). Carriers of the heterozygous variant had a higher risk for an extended disease (P = 0.0284). The rs7517847 showed a protective character leading to mild bowel movements. Three SNPs demonstrated no statistically significant influence on any examined clinical features of *** We demonstrated susceptible or protective character of the investigated IL23 R SNPs on the phenotype of UC, confirming the genetic association.
