Inflammatory bowel diseases(IBD) such as Crohn's disease and ulcerative colitis are chronic-remittent inflammatory disorders of the gastrointestinal tract still evoking challenging clinical diagnostic and therapeutic situations. Murine models of experimental colitis are a vital component of research into human IBD concerning questions of its complex pathogenesis or the evaluation of potential new drugs. To monitor the course of colitis, to the present day, classical parameters like histological tissue alterations or analysis of mucosal cytokine/chemokine expression often require euthanasia of animals. Recent advances mean revolutionary noninvasive imaging techniques for in vivo murine colitis diagnostics are increasingly available. These novel and emerging imaging techniques not only allow direct visualization of intestinal inflammation, but also enable molecular imaging and targeting of specific alterations of the inflamed murine mucosa. For the first time, in vivo imaging techniques allow for longitudinal examinations and evaluation of intra-individual therapeutic response. This review discusses the latest developments in the different fields of ultrasound, molecularly targeted contrast agent ultrasound, fluorescence endoscopy, confocal laser endomicroscopy as well as tomographic imaging with magnetic resonance imaging, computed tomography and fluorescence-mediated tomography,discussing their individual limitations and potential future diagnostic applications in the management of human patients with IBD.

Artificial intelligence for closed-loop ventilation therapy with hemodynamic control using the open lung concept

Purpose–The purpose of this paper is to develop an automatic control system for mechanical ventilation therapy based on the open lung concept(OLC)using artificial *** addition,mean arterial blood pressure(MAP)is stabilized by means of a decoupling controller with automated noradrenaline(NA)dosage to ensure adequate systemic perfusion during ventilation therapy for patients with acute respiratory distress syndrome(ARDS).Design/methodology/approach–The aim is to develop an automatic control system for mechanical ventilation therapy based on the OLC using artificial *** addition,MAP is stabilized by means of a decoupling controller with automated NA dosage to ensure adequate systemic perfusion during ventilation therapy for patients with ***–Thisinnovativeclosed-loop mechanicalventilation system leadsto a significant improvement in oxygenation,regulates end-tidal carbon dioxide for appropriate gas exchange and stabilizes MAP to guarantee proper systemic perfusion during the ventilation *** limitations/implications–Currently,this automatic ventilation system based on the OLC can only be applied in animal trials;for clinical use,such a system generally requires a mechanical ventilator and sensors with medical approval for *** implications–For implementation of a closed-loop ventilation system,reliable signals from the sensors are a prerequisite for successful ***/value–Theexperiment with porcine dynamics demonstrates thefeasibility and usefulness of this automatic closed-loop ventilation therapy,with hemodynamic control for severe ***,this pilot study validated a new algorithm for implementation of the OLC,whereby all control objectives are fulfilled during the ventilation therapy with adequate hemodynamic control of patients with ARDS.

Lattice Boltzmann Simulations in the Slip and Transition Flow Regime with the Peano Framework

We present simulation results of flows in the finite Knudsen range, that is in the slip and transition flow regime. Our implementations are based on the Lattice Boltzmann method and are accomplished within the Peano framework. We validate our code by solving two- and three-dimensional channel flow problems and compare our results with respective experiments from other research groups. We further apply our Lattice Boltzmann solver to the geometrical setup of a microreactor consisting of differently sized channels and a reactor chamber. Here, we apply static adaptive grids to further reduce computational costs. We further investigate the influence of using a simple BGK collision kernel in coarse grid regions which are further away from the slip boundaries. Our results are in good agreement with theory and non-adaptive simulations, demonstrating the validity and the capabilities of our adaptive simulation software for flow problems at finite Knudsen numbers.

Structural Glass Fiber Reinforced Concrete for Slabs on Ground

This paper aims to contribute to the classification and specification of glass fiber reinforced concrete (GFRC) and to deal with the question if structural glass fiber reinforced concrete as a special kind of glass fiber reinforced concrete is suited for use in load-bearing members. Despite excellent material properties, the use of glass fibers in a concrete matrix is carried out so far only in non- structural elements or as a modification for the prevention of shrinkage cracks. The aim of re- search at the University of Applied Sciences in Leipzig is the use of alkali-resistant macro glass fibers as concrete reinforcement in structural elements as an alternative to steel fiber reinforcement. Slabs on ground, as an example for structural members, provide a sensible application for the new material because they can be casted as load bearing and non-load bearing and are mostly made of steel fiber reinforced concrete. In the future, structural glass fiber reinforced concrete shall provide a simple and visually appealing alternative to conventional steel bar or steel fiber reinforced concrete. The glass fibers can also be used in combination with conventional reinforcing bars or mat reinforcements. Initial investigations have announced some potential.

Repairing massive rotator cuff tendon defects remains a challenge due to the high retear rate after surgical intervention.3D printing has emerged as a promising technique that enables the fabrication of engineered tissues with heterogeneous structures and mechanical properties,as well as controllable microenvironments for tendon *** this study,we developed a new strategy for rotator cuff tendon repair by combining a 3D printed scaffold of polylactic-co-glycolic acid(PLGA)with cell-laden collagen-fibrin *** designed and fabricated two types of scaffolds:one featuring a separate layer-by-layer structure and another with a tri-layered structure as a *** tensile tests showed that both types of scaffolds had improved mechanical properties compared to single-layered PLGA *** printed scaffold with collagen-fibrin hydrogels effectively supported the growth,proliferation,and tenogenic differentiation of human adipose-derived mesenchymal stem *** implantation of the multilayered scaffolds demonstrated their excellent in vivo *** study demonstrates the feasibility of 3D printing multilayered scaffolds for application in rotator cuff tendon regeneration.

Objectives:To test for differences in overall and recurrence-free survival between laparoscopic and open surgical approaches in patients undergoing radical nephroureterectomy(RNU)for upper tract urothelial carcinoma(UTUC).Materials and methods:We retrospectively identified patients treated for UTUC from 2010 to 2020 from our institutional *** undergoing laparoscopic or open RNU with no suspicion of metastasis(cM0)were for the current study *** with suspected metastases at diagnosis(cM1)or those undergoing other surgical treatments were *** was performed according to the laparoscopic versus open surgical ***-Meier plots were used to test for differences in overall and recurrence-free survival with regard to the surgical ***,separate Kaplan-Meier plots were used to test the effect of preoperative ureterorenoscopy on overall and recurrence-free survival within the overall study ***:Of the 59 patients who underwent nephroureterectomy,29%(n=17)underwent laparoscopic nephroureterectomy,whereas 71%(n=42)underwent open *** and tumor characteristics were comparable between groups(p≥0.2).The median overall survival was 93 and 73 months in the laparoscopic nephroureterectomy group compared to the open nephroureterectomy group(p=0.5),*** median recurrence-free survival did not differ between open and laparoscopic nephroureterectomies(73 months for both groups;p=0.9).Furthermore,the median overall and recurrence-free survival rates did not differ between patients treated with and without preoperative ***:The results of this retrospective,single-center institution showed that overall and recurrence-free survival rates did not differ between patients with UTUC treated with laparoscopic and open ***,preoperative ureterorenoscopy before RNU was not associated with higher overall or recurrence-free survival rates.

欧洲钢铁工业的环境保护已经达到了很高的水平.20世纪60年代初,保护环境一般认为只是局限于限制工业对人们的影响.现在,尤其是在罗马协议(Club of Rome)的努力之下,环保的范围从节约资源延伸到到整个自然环境的保护,而后产生了目前应用中的工业过程和产品生产周期.整体来说,保护环境的所有途径都被视为是未来平衡发展的先决条件.欧盟环境保护技术的情况可以以德国为例,更准确地说是杜伊斯堡的蒂森克虏伯钢厂(ThyssenKruppStahl AG),其粗钢产量约占德国总产量的35%.钢铁工业环境保护的重点仍然是维护空气质量,在此目的下,进行除尘处理.由烟囱处取样获得目前的除尘水平来看,今后的除尘主要是控制粉尘扩散和减少细小微粒的排放.钢铁工业环境保护的关键策略就是循环利用,它能起到与环境协调一致的作用,能够解决相关工艺中需要的大量的原料问题.德国的蒂森克虏伯钢厂(ThyssenKrupp Stahl AG)现在炉渣的综合回收率水平已接近95%,其中冶金渣的回收利用率尤为突出.在德国,高炉渣利用率达到了100%,同时炼钢渣的利用率也超过了90%,每吨粗钢的耗水量也控制在在100 m3的范围内.大量的努力不仅保证了有效的回收和再生利用工序,而且冷却水的利用也保证了新水的补充率降低到每吨粗钢15 m3左右.污染控制除跟主要环境即空气、水和土壤有关外,已扩展到回收过程再利用的水平,更多地重点渐渐地放到相关产品的环境保护.产品和原料却日益受到生产商和消费者的重点检验,有点与环境协调性标准相反.组织机构的环境保护手段包括环境管理系统的认可、生态平衡、政府部门的自愿协议以及提高公众的信息、教育水平.欧洲的钢铁工业,当然也包括德国,都已经意识到环境保护的重要性同工业研究与开发的重要性处于同等地位.一个典型的例子就是超薄板汽车用钢项目,这个与产品相关的研究引入了35家合作厂商共同研究一个比目前传统的汽车车身更结实、更轻的项目.欧洲钢铁厂家的联合合作已经成功研究出应用于烧结厂的减少二恶英(dioxin)排放物的吸收工艺.用褐煤焦尘作为吸附剂,已能够平均减少排放物大约70%.很可能2003年在杜伊斯堡的ThyssenKrupp Stahl AG将被授权建全世界最先进的焦化厂.在这个项目过程中研究出的主要部分成果,例如低尘低烟煤湿熄法和减少PAH排放物的单室压力控制系统,会恰当的应用于新的工厂.另一个主要进步是ThyssenKrupp Stahl AG研制的Hamborn OxiCup竖炉,它有湿法冶炼和排除含铁残渣的功能.此技术预计成为回收利用铁水和钢产品中产生的粉尘和泥浆这一难题的解决方案.德国的局势尤其显示出环境法规的落实正开始对经济发展和劳动力市场起到不利影响.盛行的经济技术条件需要合法的体制治理环境保护的合理化.将来任何进一步的措施都要考虑这一事实,既要获得可持续发展,又要考虑到经济发展和社会需求.

Endoscopic ultrasound-fine needle biopsies of pancreatic lesions: Prospective study of histology quality using Franseen needle

BACKGROUND The introduction of fine needle biopsies(FNB)to clinical practice presents a changing trend towards histology in the endoscopic ultrasound-guided tissue acquisition(EUS-TA).AIM To evaluate the clinical performance of a new FNB needle,the 22-gauge(22G)Franseen needle,when sampling pancreatic solid *** Consecutive patients with an indication for EUS-TA for the assessment of pancreatic solid lesions were included in this prospective,single-center,singlearm *** patient underwent a puncture of the lesion two times using the 22G Franseen needle and the obtained samples were directly placed into formalin for histological *** primary study endpoint was the rate of high-quality obtained *** endpoints included the length and diameter of the core specimen,the diagnostic accuracy and the complication *** From June 2017 to December 2018,forty patients with pancreatic solid lesions(22 females;mean age 67.2 years)were *** acquisition was achieved in all ***-quality histology,rated with Payne score 3,was obtained in 37/40 cases(92.5%)after two needle *** mean size of the acquired histological core tissue was 1.54 mm×0.39 *** diagnostic accuracy for the correct diagnosis was 85%(34/40).Only one adverse event was occurred,consisting of a self-limiting bleeding in the puncture *** The 22G Franseen needle achieved according to our standardized protocol a high rate of histological core procurement,and a high diagnostic accuracy,with one minor adverse event reported.
