To report an extended multivisceral transplantation （mVTx） including right kidney and ascending colon in a patient with complicated Crohn＇s disease （CD）. A 36-year old female suffering from short bowel syndrome and frozen abdomen due to fistulizing CD after multiple abdominal operations underwent mVTx of eight organs including stomach, pancreatoduodenal complex, liver, intestine, ascending colon, right kidney, right adrenal gland, and greater omentum in November 2003. Immunosuppression consisted of alemtuzumab, tacrolimus and steroids. The patient was off parenteral nutrition by postoperative wk 3. She experienced one episode of pneumonia. The patient recovered completely and discharged 2.5 mo and was doing well 30 mo after mVTx. This is one of the very rare cases in which a complete mulitivisceral graft of eight abdominal organs was transplanted orthotopically.

microsurgical epididymal sperm aspiration （mESA） refers to retrieval of sperm-containing fluid from optimal areas of the epididymis that are selected and sampled using high-power optical magnification provided by an operating microscope. Retrieved sperm are subsequently used for intracytoplasmic sperm injection （ICSI） to induce fertilization and pregnancy. mESA is considered by many experts to be the gold standard technique for sperm retrieval in men with obstructive azoospermia given its high yield of quality sperm, excellent reported fertilization and pregnancy rates, and low risk of complications. However, mESA must be performed in an operating room, requires microsurgical skills and is only useful for reproduction using ICSI. Herein we present an overview of the evaluation of candidate patients for mESA, the technical performance of the procedure and the outcomes that have been reported.

In this review, we address the question of how the tip-growing pollen tube achieves its rapid rate of elongation while maintaining an intact cell wall. Although turgor is essential for growth to occur, the local expansion rate is controlled by local changes in the viscosity of the apical wall. We focus on several different structures and underly- ing processes that are thought to be major participants including exocytosis, the organization and activity of the actin cytoskeleton, calcium and proton physiology, and cellular energetics. We think that the actin cytoskeleton, in particular the apical cortical actin fringe, directs the flow of vesicles to the apical domain, where they fuse with the plasma mem- brane and contribute their contents to the expanding cell wall. While pH gradients, as generated by a proton-ATPase located on the plasma membrane along the side of the clear zone, may regulate rapid actin turnover and new polymeri- zation in the fringe, the tip-focused calcium gradient biases secretion towards the polar axis. The recent data showing that exocytosis of new wall material precedes and predicts the process of cell elongation provide support for the idea that the intussusception of newly secreted pectin contributes to decreases in apical wall viscosity and to cell expansion. Other prime factors will be the localization and activity of the enzyme pectin methyl-esterase, and the chelation of calcium by pectic acids. Finally, we acknowledge a role for reactive oxygen species in the control of wall viscosity.

Characterization of colonic dendritic cells in normal and colitic mice

AIm： Recent studies demonstrating the direct involvement of dendritic cells （DC） in the activation of pathogenic T cells in animal models of inflammatory bowel disease identify DC as important antigen presenting cells in the colon. However, very little is known about the properties of colonic DC. mETHODS： Using immunohistochemistry, electron microscopy and flow cytometry we have characterized and compared colonic DC in the colon of healthy animals and interleukin-2-deficient （IL2-/-） mice that develop colitis. RESULTS： In the healthy colon, DC resided within the lamina propria and in close association with the basement membrane of colonic villi. Type i myeloid （CD11c^＋, CD11b^＋, B220, CD8） DC made up the largest （40-45%） population and all DC expressed low levels of CD80, CD86, and CD40, and had high endooltic activity consistent with an immature phenotype. In colitic IL2-/- mice, colonic DC numbers increased four- to five-fold and were localized within the epithelial layer and within aggregates of T and B cells. They were also many more DC in mesenteric lymph nodes （mLN）. The majority （〉85%） of DC in the colon and mLN of IL2./ mice were type 1 myeloid, and expressed high levels of mHC class II, CD80, CD86, CD40, DEC 205, and CCR5 molecules and were of low endocytic activity consistent with mature DC. CONCLUSION： These findings demonstrate striking changes in the number, distribution and phenotype of DC in the inflamed colon. Their intimate association with lymphocytes in the colon and draining lymph nodes suggest that they may contribute directly to the ongoing inflammation in the colon.

Celiac disease has been associated with some autoimmune disorders. A 40-year-old competitive strongman with celiac disease responded to a gluten-free diet, but developed profound and generalized motor weakness with acetylcholine receptor antibody positive myasthenia gravis, a disorder reported to occur in about 1 in 5000. This possible relationship between myasthenia gravis and celiac disease was further explored in serological studies. Frozen stored serum samples from 23 acetylcholine receptor antibody positive myasthenia gravis patients with no intestinal symptoms were used to screen for celiac disease. Both endomysial and tissue transglutaminase antibodies were examined. One of 23 (or, about 4.3%) was positive for both IgA-endomysial and IgA tissue transglutaminase antibodies. Endoscopic studies subsequently showed duodenal mucosal scalloping and biopsies confirmed the histopathological changes of celiac disease. Celiac disease and myasthenia gravis may occur together more often than is currently appreciated. The presence of motor weakness in celiac disease may be a clue to occult myasthenia gravis, even in the absence of intestinal symptoms.

Ejaculatory dysfunction is a highly prevalent clinical condition that may be classified along a continuum that ranges from premature ejaculation （PE）, through retarded or delayed ejaculation （DE）, to complete anejaculation （AE）. Retrograde ejaculation （RE） represents a distinct entity in which ejaculate is expelled either partially or completely into the bladder. While DE and PE are significant sources of sexual dissatisfaction among men and their partners, patients with these disorders retain normal fertility in most cases. Conversely, men with AE and RE are unable to deliver sperm into the female genital tract and are therefore rendered subfertile. Therefore, in reviewing ejaculatory disorders as they relate to fertility, this paper will primarily focus on the diagnosis and management of AE and RE. Physiology, diagnostic strategies, pharmacological treatments, and procedural interventions relevant to AE and RE are discussed.

A challenging task when applying high-order digital modulation schemes is the complexity of the detector. Particularly, the complexity of the optimal a posteriori probability （APP） detector increases exponentially with respect to the number of bits per data symbol. This statement is also true for the max-Log-APP detector, which is a common simplification of the APP detector. Thus it is important to design new detection algorithms which combine a sufficient performance with low complexity. In this contribution, a detection algorithm for two- dimensional digital modulation schemes which cannot be split-up into real and imaginary parts （like phase shift keying and phase-shifted snperposition modulation （PSm）） is proposed with emphasis on PSm with equal power allocation. This algorithm exploits the relationship between max-Log-APP detection and a Voronoi diagram to determine planar surfaces of the soft outputs over the entire range of detector input values. As opposed to state-of-the-art detectors based on Voronoi surfaces, a priori information is taken into account, enabling iterative processing. Since the algorithm achieves max-Log-APP performance, even in the presence of a priori information, this implies a great potential for complexity reduction compared to the classical APP detection.

Considering high-order digital modulation schemes, the bottleneck in consumer products is the detector rather than the modulator. The complexity of the optimal a posteriori probability （APP） detector increases exponentially with respect to the number of modulated bits per data symbol. Thus, it is necessary to develop low-complexity detection algorithms with an APP-like performance, especially when performing iterative detection, for example in conjunction with bit interleaved coded modulation. We show that a special case of superposition modulation, dubbed Direct Superposition modulation （DSm）, is particularly suitable for complexity reduction at the receiver side. As opposed to square QAm, DSm achieves capacity without active signal shaping. The main contribution is a low-cost detection algorithm for DSm, which enables iterative detection by taking a priori information into account. This algorithm exploits the approximate piecewise linear behavior of the soft outputs of an APP detector over the entire range of detector input values. A theoretical analysis and simulation results demonstrate that at least max-log APP performance can be reached, while the complexity is significantly reduced compared to classical APP detection.

The present article concerns the continuum modelling of the mechanical behaviour and equilibrium shapes of two types of nano-scale objects： fluid lipid bilayer membranes and carbon nanostructures. A unified continuum model is used to handle four different ease studies. Two of them consist in representing in analytic form cylindrical and axisymmetric equilibrium configurations of single-wall carbon nanotubes and fluid lipid bilayer membranes subjected to uniform hydrostatic pressure. The third one is concerned with determination of possible shapes of junctions between a single-wall carbon nanotube and a fiat graphene sheet or another single-wall carbon nanotube. The last one deals with the mechanical behaviour of closed fluid lipid bilayer membranes （vesicles） adhering onto a fiat homogeneous rigid substrate subjected to micro-injection and uniform hydrostatic pressure.
