Splenic hamartomas(SHs)are uncommon,benign vascular lesions of unclear etiology and are mostly found incidentally on abdominal images,at surgery,or at autopsy.Since the first case description,in 1861,less than 50 pediatric SH cases have been reported in the literature.In this article,we have performed an analysis of all SH cases in children published in the literature to date and presented our case of an 8-year-old male with SH.These lesions in children were shown to cause symptoms more often than in the adult population.The observed SH sizes in children ranged from a few millimeters to 18 cm,and the symptomatic lesions were mostly larger or multiple.The most common clinical finding was splenomegaly.Signs of hypersplenism were present in children with a single SH larger than 4.5 cm(diameter range:4.5-18.0 cm)and in those with multiple hamartomas,ranging from a few millimeters to 5 cm.Eighty percent of patients with available laboratory findings had hematological abnormalities such as anemia,thrombocytopenia,or pancytopenia.Other symptoms and signs included abdominal pain,recurrent infections,fever,night sweats,lethargy,growth retardation,and weight loss.The use of multiple imaging modalities may suggest the preoperative diagnosis of a splenic mass in children and determine the therapeutic approach.However,the final diagnosis of SH relies on histopathological evaluation.Surgery,including total or partial splenectomy(PS),is the mainstay of SH management.Milickovic M et al.Splenomas in children WJCC https://*** 1910 April 16,2024 Volume 12 Issue 11 Although total splenectomy carries a greater risk of overwhelming post-splenectomy infection than PS it has remained the most performed surgical procedure in children with SH.In the majority of pediatric patients with symptomatic SH,resolution of symptoms and resolution or improvement of cytopenias occurred after surgical treatment.

Metabolic syndrome and the urinary microbiome of patients undergoing percutaneous nephrolithotomy

Objective:To identify possible stone-promoting microbes,we compared the profiles of microbes grown from stones of patients with and without metabolic syndrome(MetS).The association between MetS and urinary stone disease is well established,but the exact pathophysiologic relationship remains unknown.Recent evidence suggests urinary tract dysbiosis may lead to increased nephrolithiasis risk.Methods:At the time of percutaneous nephrolithotomy,bladder urine and stone fragments were collected from patients with and without MetS.Both sample types were subjected to expanded quantitative urine culture(EQUC)and 16 S ribosomal RNA gene sequencing.Results:Fifty-seven patients included 12 controls(21.1%)and 45 MetS patients(78.9%).Both cohorts were similar with respect to demographics and non-MetS comorbidities.No controls had uric acid stone composition.By EQUC,bacteria were detected more frequently in MetS stones(42.2%)compared to controls(8.3%)(p=0.041).Bacteria also were more abundant in stones of MetS patients compared to controls.To validate our EQUC results,we performed 16 S ribosomal RNA gene sequencing.In 12/16(75.0%)sequence-positive stones,EQUC reliably isolated at least one species of the sequenced genera.Bacteria were detected in both“infectious”and“non-infectious”stone compositions.Conclusion:Bacteria are more common and more abundant in MetS stones than control stones.Our findings support a role for bacteria in urinary stone disease for patients with MetS regardless of stone composition.

After decades of research and development, the WSR-88 D(NEXRAD) network in the United States was upgraded with dual-polarization capability, providing polarimetric radar data(PRD) that have the potential to improve weather observations,quantification, forecasting, and warnings. The weather radar networks in China and other countries are also being upgraded with dual-polarization capability. Now, with radar polarimetry technology having matured, and PRD available both nationally and globally, it is important to understand the current status and future challenges and opportunities. The potential impact of PRD has been limited by their oftentimes subjective and empirical use. More importantly, the community has not begun to regularly derive from PRD the state parameters, such as water mixing ratios and number concentrations, used in numerical weather prediction(NWP) models.In this review, we summarize the current status of weather radar polarimetry, discuss the issues and limitations of PRD usage, and explore potential approaches to more efficiently use PRD for quantitative precipitation estimation and forecasting based on statistical retrieval with physical constraints where prior information is used and observation error is included. This approach aligns the observation-based retrievals favored by the radar meteorology community with the model-based analysis of the NWP community. We also examine the challenges and opportunities of polarimetric phased array radar research and development for future weather observation.

Preparation and Characterization of Thermoplastic Starch from Sugar Palm (Arenga pinnata) by Extrusion Method

Sugar palm(Arenga pinnata)starch is considered an important renewable,biodegradable,and eco-friendly polymer,which is derived from agricultural by-products and residues,with great potential for the development of biocomposite materials.This research was aimed at investigating the development of TPS biocomposites from A.pinnata palm starch using an extrusion process.Palm starch,glycerol,and stearic acid were extruded in a twin-screw extruder.Scanning electron microscopy(SEM)analysis of TPS showed that the starch granules were damaged and gelatinized in the extrusion process.The density of TPS was 1.3695 g/mL,lower than that of palm starch,and the addition of stearic acid resulted in increased TPS density.X-ray diffraction(XRD)results showed that palm starch had a C-type pattern crystalline structure.The tensile strength,elongation at break,and modulus of elasticity of TPS were 7.19 MPa,33.95%,and 0.56 GPa,respectively.The addition of stearic acid reduced the tensile strength,elongation at break and modulus of elasticity of TPS.The rheological properties,i.e.,melt flow rate(MFR)and viscosity of TPS,were 7.13 g/10 min and 2482.19 Pa.s,respectively.The presence of stearic acid in TPS resulted in increased MFR and decreased viscosity values.The peak gelatinization temperature of A.pinnata palm starch was 70°C,while Tg of TPS was 65°C.The addition of stearic acid reduced the Tg of TPS.The thermogravimetric analysis(TGA)analysis showed that the addition of glycerol and stearic acid decreased the thermal stability,but extended the temperature range of thermal degradation.TPS derived from A.pinnata palm starch by extrusion method has the potential to be applied in industrial practice as a promising raw material for manufacturing bio-based packaging as a sustainable and green alternative to petroleum-based plastics.

The etiology of upper gastrointestinal bleeding(UGIB)varies by age,from newborns to adolescents,with some of the causes overlapping between age groups.While particular causes such as vitamin K deficiency and cow’s milk protein allergy are limited to specific age groups,occurring only in neonates and infants,others such as erosive esophagitis and gastritis may be identified at all ages.Furthermore,the incidence of UGIB is variable throughout the world and in different hospital settings.In North America and Europe,most UGIBs are nonvariceal,associated with erosive esophagitis,gastritis,and gastric and duodenal ulcers.In recent years,the most common causes in some Middle Eastern and Far Eastern countries are becoming similar to those in Western countries.However,variceal bleeding still predominates in certain parts of the world,especially in South Asia.The most severe hemorrhage arises from variceal bleeding,peptic ulceration,and disseminated intravascular coagulation.Hematemesis is a credible indicator of a UGI source of bleeding in the majority of patients.Being familiar with the most likely UGIB causes in specific ages and geographic areas is especially important for adequate orientation in clinical settings,the use of proper diagnostic tests, and rapid initiation of the therapy. The fundamental approach to the management of UGIBincludes an immediate assessment of severity, detecting possible causes, and providing hemodynamic stability,followed by early endoscopy. Unusual UGIB causes must always be considered when establishing a diagnosis inthe pediatric population because some of them are unique to children. Endoscopic techniques are of significantdiagnostic value, and combined with medicaments, may be used for the management of acute bleeding. Finally,surgical treatment is reserved for the most severe bleeding.

Interpretable discovery of semiconductors with machine learning

Machine learning models of material properties accelerate materials discovery,reproducing density functional theory calculated results at a fraction of the cost1–6.To bridge the gap between theory and experiments,machine learning predictions need to be distilled in the form of interpretable chemical rules that can be used by experimentalists.Here we develop a framework to address this gap by combining evolutionary algorithm-powered search with machine-learning surrogate models.We then couple the search results with supervised learning and statistical testing.This strategy enables the efficient search of a materials space while providing interpretable design rules.We demonstrate its effectiveness by developing rules for the design of direct bandgap materials,stable UV emitters,and IR perovskite emitters.Finally,we conclusively show how DARWIN-generated rules are statistically more robust and applicable to a wide range of applications including the design of UV halide perovskites.

Black liquor is obtained as a by-product of the pulping process,which is used to convert biomass into pulp by removing lignin,hemicelluloses and other extractives from wood to free cellulose fibers.Lignin represents a major constituent in black liquor,with quantities varying from 20%to 30%,of which a very low share is used for manufacturing value-added products,while the rest is mainly burned for energy purposes,thus underestimating its great potential as a raw material.Therefore,it is essential to establish new isolation and extraction methods to increase lignin valorization in the development of bio-based chemicals.The aim of this research work was to determine the effect of KOH or ethanol concentration as an isolation agent on lignin yields and the chemical characteristics of lignin isolated from formacell black liquor of oil palm empty fruit bunch(OPEFB).Isolation of lignin was carried out using KOH with various concentrations ranging from 5%to 15%(w/v).Ethanol was also used to precipitate lignin from black liquor at concentrations varying from 5%to 30%(v/v).The results obtained showed that the addition of KOH solution at 12.5%and 15%concentrations resulted in better lignin yield and chemical properties of lignin,i.e.,pH values of 3.86 and 4.27,lignin yield of 12.78%and 14.95%,methoxyl content of 11.33%and 10.13%,and lignin equivalent weights of 476.25 and 427.03,respectively.Due to its phenolic structure and rich functional groups that are favorable for modifications,lignin has the potential to be used as a green additive in the development of advanced biocomposite products in various applications to replace current fossil fuel-based material,ranging from fillers,fire retardants,formaldehyde scavengers,carbon fibers,aerogels,and wood adhesives.

Decision Support for Integrated Management of Local-Level Adaptation to Climate Changes: The Case of Serbia

Projected climate changes will additionally increase the already significant risk of natural hazard-related disasters in Serbia and the west Balkan region as a whole.Serbia is about to introduce the strategy for climate change adaptation and mitigation.However,a national decision-support system for implementation of the climate change law and strategy is yet to be developed.This study contributes to the implementation of adaptation policies at subnational levels by development of a decision-support model for local-level management of the climate change adaptation process.The study explores the potential for synergetic application of multicriteria decision making analysis and probabilistic reasoning methods by focusing on Bayesian networks,analytical hierarchy processes,and geographic information systems for selection of priority adaptation measures.The study was based on the formation of causal chains,which enable linking management decisions and socioeconomic or biophysical consequences into articulated sequences of conditional relationships.A model was tested in the forestry sector,and it clearly pointed out development of an early warning system and planning of water intake basins as priority adaptation measures.Since the results are shown as a probability distribution for each alternative solutions,the model can assist decision makers with prompt evaluation of various scenarios.
