Obscure gastrointestinal bleeding is still a clinical challenge for gastroenterologists. The recent development of novel technologies for the diagnosis and treatment of different bleeding causes has allowed a better management of patients, but it also determines the need of a deeper comprehension of pathophysiology and the analysis of local expertise in order to develop a rational management algorithm. Obscure gastrointestinal bleeding can be divided in occult, when a positive occult blood fecal test is the main manifestation, and overt, when external sings of bleeding are visible. In this paper we are going to focus on overt gastrointestinal bleeding, describing the physiopathology of the most usual causes, analyzing the diagnostic procedures available, from the most classical to the novel ones, and establishing a standard algorithm which can be adapted depending on the local expertise or availability. Finally, we will review the main therapeutic options for this complex and not so uncommon clinical problem.

AIM： To evaluate the safety and efficacy of a dexamethasone（deX） intravitreal implant for diabetic macular edema（DME）.METHODS： Totally 113 eyes of 84 patients were divided in three subgroups： naive patients（n=11）, pseudophakic patients（n=72） and phakic patients（n=30）. Inclusive criterion comprised adult diabetic patients with central fovea thickening and impaired visual acuity resulting from DME for whom previous standard treatments showed no improvement in both central macular thickness（CMT） and best corrected visual acuity（BCVA） after at least 3 mo of treatment. Outcome data were obtained from patient visits at baseline and at months 1, 3, 5, 9 and 12 after the first deX implant injection. At each of these visits, patients underwent measurement of BCVA, a complete eye examination and measurement of CMT and macular volume（MV） carried out with optical coherence tomography（OCT） images. RESULTS： Seventy-three eyes（64.5%） received a single implant, 30（26.5%） received two implants and 10（9%） received three implants. At baseline, average in BCVA, CMT and MV were 43.5±20.8, 462.8±145 and 12.6±2.5 respectively. These values improved significantly at 1 mo（BCVA： 47.2±19.5, CMT： 339.6±120, MV： 11.11±1.4） and 3 mo（BCVA： 53.2±18.1, CMT： 353.8±141, MV： 11.3±1.3）（P≤0.05）. At 5 mo（BCVA： 50.9±19.8, CMT： 425±150, MV： 12.27±2.3）, 9 mo（BCVA： 48.4±17.6, CMT： 445.5±170, MV： 12.5±2.3） and 12 mo（BCVA： 47.7±18.8, CMT： 413.2±149, MV： 12.03±2.5）, improvements in the three parameters were no longer statistically significant and decreased progressively but did not reach baseline values. There were no clinical differences between subgroups. A novel porous and crystalline two-dimensional(2D)electrochemically active covalent organic framework(COF)based on ortho-quinone units has been prepared as an innovative approach towards the development of organic cathode materials with multiple redox sites as an efficient electrocatalyst for the oxygen reduction reaction(ORR).In contrast with most of the previously reported COFs as electrocatalysts for the ORR,the electrocatalytic application of this material towards ORR has been investigated without adding any metal or conductive supporting material and avoiding any additional carbonization ***,the electrochemical properties of the COF material have been compared with two analogue amorphous frameworks with similar chemical composition,which points out the important role of the enhanced crystallinity and porosity of the COF network in its superior performance as an electrocatalyst towards ORR.

Urban cemeteries are increasingly surrounded by areas of high residential density as urbanization continues world-wide. With increasing rates of mortality caused by the novel coronavirus, SARS-CoV-2,urban vertical cemeteries are experiencing interments at an unprecedented rate. Corpses interred in the 3rd to 5th layer of vertical urban cemeteries have the potential to contaminate large adjacent *** general objective of this manuscript is to analyze the reflectance of altimetry, normalized difference vegetation index(NDVI) and land surface temperature(LST) in the urban cemeteries and neighbouring areas of the City of Passo Fundo, Rio Grande do Sul, Brazil. It is assumed that the population residing in the vicinity of these cemeteries may be exposed to SARS-CoV-2 contamination through the displacement of microparticles carried by the wind as a corpse is placed in the burial niche or during the first several days of subsequent fluid and gas release through the process of decomposition. The reflectance analyses were performed utilizing landsat 8 satellite images applied to altimetry, NDVI and LST, for hypothetical examination of possible displacement, transport and subsequent deposition of the SARS-CoV-2 virus. The results showed that two cemeteries within the city, cemeteries A and B could potentially transport SARS-CoV-2 of nanometric structure to neighboring residential areas through wind *** two cemeteries are located at high relative altitudes in more densely populated regions of the *** NDVI, which has been shown to control the proliferation of contaminants, proved to be insufficient in these areas, contributing to high LST values. Based on the results of this study, the formation and implementation of public policies that monitor urban cemeteries is suggested in areas that utilize vertical urban cemeteries in order to reduce the further spread of the SARS-CoV-2 virus.

AIM To determine the number of mutations in the NS5 A region of the hepatitis C virus(HCV) and its relationship to the response to antiviral therapy in patients with chronic hepatitis C genotype 1 who are non-responders to two or more treatments. METHODS Sequences within HCV NS5 A [PKR binding domain(PKRBD) and the interferon-sensitivity-determining region(ISDR)] were analysed via direct sequencing in a selected cohort of 72 patients, with a total of 201 treatments [interferon-alpha(IFN-α), n = 49; IFN-α + ribavirin(RBV), n = 75; pegylated(peg) IFN-α + RBV, n = 47; first-generation direct-acting antivirals(DAAs), n = 13; and second-generation DAAs, n = 17]. Of these, 48/201 achieved a sustained virological response(SVR) and 153/201 achieved no virological response(NVR).RESULTS For both regions, treatments resulting in SVR were associated with more baseline mutations than were treatments resulting in NVR(SVR vs NVR; PKRBD: 5.82 ± 3 vs 4.86 ± 2 mutations, P = 0.045; ISDR: 2.65 ± 2 vs 1.51 ± 1.7 mutations, P = 0.005). A decrease or no change in the number of mutations over time between treatments in the PKRBD or ISDR, as shown by sequencing, was associated with patients who usually failed to respond to treatment(PKRBD, P = 0.02; ISDR, P = 0.001). Moreover, patients showing a post-treatment baseline viral load > 600000 IU/m L and increased ISDR mutations with respect to the previous treatment were 9.21 times more likely to achieve SVR(P = 0.001). CONCLUSION The obtained results show that among patients who have shown no response to two or more antiviral treatments, the likelihood of achieving SVR increases with the genetic variability in the ISDR region(≥ 2 mutations or number of substitutions from the HCV-J and HCV-1 prototype), especially when the viral load is greater than 600000 IU/m L.

TTie Ultisols in the Rana de Canamero area in Southwest Spain showed aluminum(Al)phytotoxicity,and the clearance of natural vegetation and decades of intensive conventional agriculture caused the deplation of soil organic matter(SOM).Therefore,we studied the long-term effects of no tillage and liming using sugar beet foam(SF)and red gypsum(RG),alone or in combination,on the restoration of Ultisols affected by acidification,Al phytotoxicity,and SOM *** measured the main soil chemical properties,soil microbial biomass,soil enzyme activities involved in carbon,nitrogen,phosphorus,and sulfur mineralization,and the emission of volatile organic compounds(VOCs).The results indicated that liming effectively neutralized the soil acidity in the long term and,in combination with no tillage,significantly increased soil microbial biomass and enzyme ***-three VOCs were detected using the proton transfer reaction-time of flight(PTR-ToF)technique,and both liming and tillage changed the VOC emission *** greatest difference in VOC emission pattern was observed between no-tilled un-amended soils and tilled lime-amended soils,suggesting the activation of different metabolic pathways within the microbial communities of soils under different *** in VOC emission patterns could be attributed to the decomposition of carbohydrates,which were also sustained by the higher enzyme activities in the lime-amended soils.
